Abstract Objective This study aims to investigate the interrelationship of risk factors for suicidal behaviour and their influence on attempt severity in a sample of adolescents who have recently attempted suicide. For it a network analyse was performed. Method Data from a sample of adolescents from 12 to 17 years of age with documented suicide attempts were collected and analysed in the context of a larger study conducted in Spain called SURVIVE. Several factors were examined including age, sex, depression, trauma, impulsivity, and substance abuse. Graph analysis was performed to identify relationships and centrality measures among these factors. Results A total of 267 participants were enrolled: 233 females and 34 males with a mean age of 15.00 years (SD = 1.52). The results indicate that age and sex do not have a notable relationship with attempt severity in adolescents. Emotional and behavioural difficulties, measured by the Strengths and Difficulties Questionnaire (SDQ), have the greatest influence on other variables. Depression and childhood trauma show varying degrees of association with suicidal behaviour, as does motor impulsivity. Substance use does not appear to be strongly related to suicide attempt severity. The number of suicide attempts is strongly correlated with emotional and behavioural difficulties, depression, and childhood trauma. Lethality of suicide attempts and intensity of suicidal ideation do not show significant associations with the other variables. Conclusion This study identifies significant risk factors for adolescent suicide. Emotional and behavioural symptoms, depression, and childhood trauma are strongly linked to suicidal behaviour. However, age, sex, and substance abuse show minimal correlation. Assessing emotional difficulties and depressive symptoms using specific questionnaires could be crucial in evaluating suicidal behaviour in adolescents.

Masquerades dominated the culture of polite urban entertainment in eighteenth-century England, summoning London’s bon ton to engage in dancing, dining, role-playing, and conversation while in disguise. The anonymity and enticement provided by masks and costumes turned the masquerade into a frequent narrative frame for the depiction of amorous encounters and assignations in Georgian literary and visual culture, especially in relation to women’s behavior, morality, and presence in the public sphere during the nascent cult of sensibility. Satirical prints, widely consumed at this time, found this topic a particularly appealing one for the Georgian public’s taste, denouncing and disseminating masquerade’s moral perils for women. This article explores amorous encounters associated with the space of the masquerade as represented in Georgian print culture and how such visual narratives employed the figure of the female masquerader to define models of respectable or deviant femininity.

Background: Cancer-therapy-related cardiac dysfunction (CTRCD) is a growing concern for public health, with a growing incidence due to improved survival rates of patients with hematological malignancies due to diagnostic and therapeutic advances. The identification of patients at risk for CTRCD is vital to developing preventive strategies. Methods: A single-center retrospective cohort study was conducted between 1 January 2017 and 15 February 2023. Medical records of patients with lymphoma treated with first-line anthracyclines were reviewed. Demographic data, cardiovascular risk factors, biomarkers of myocardial damage, and echocardiographic information were collected. Results: A total of 200 patients were included. The incidence of CTRCD was 17.4% (35/200). Patients with CTRCD were older than those without CTRCD, with a mean age of 65.17 years vs. 56.77 (p = 0.008). Dyslipidemia (DL) (31.4% vs. 13.4% p = 0.017) and previous cardiovascular disease (40% vs. 13.3%; p < 0.001) were more frequent in the group who developed an event. Mean baseline NT-proBNP levels in the subgroup with cardiovascular events were 388.73 kg/L ± 101.02, and they were 251.518 kg/L ± 26.22 in those who did not (p = 0.004). Differences in Troponin I levels were identified during and after treatment without exceeding the laboratory’s upper reference limit. Patients were followed for a median of 51.83 months (0.76–73.49). The presence of a CTCRD event had a negative impact on overall mortality from any cause (HR = 2.23 (95% CI: 1.08–2.93); p = 0.031). Conclusions: Early identification of risk factors is crucial to manage patients at risk for CTRCD.

Background: Acute respiratory illness (ARI) remains the leading cause of global morbidity. Its primary etiology is viral; nevertheless, viral pathogen identification is limited. Clinical information about Latin America's viral etiology, outcomes, and severity is unknown. This study aims to identify the clinical burden of respiratory viral infections, severity, and adult outcomes. Methods: This multicentric, population-based study was conducted through the Health Institute of Bogotá, Colombia, including adult patients diagnosed with ARI between 2013 and 2019. Data collection followed ARI public health surveillance program. Incidence, etiological pathogens, and mortality were calculated. Results: A total of 2304 patients were included in the study. ARI was most frequently reported in 2018 (23.3% [538/2304]). Incidence varies between years, maintaining a range between 3.5 and 8.4. The most frequent clinical diagnosis was pneumonia in 59.1%. Etiological viral detection was obtained in 21.5% of patients [495/2304], principally by Influenza A. Mortality was 21.8%, and ICU admission was 7.3%. The type of event did not predict the causative pathogen, disease severity, or mortality. Conclusions: ARI is a leading cause of morbidity and mortality in Colombia. ARI incidence varies per year and is caused mainly by Influenza A. The classification used in the surveillance program does not correlate with viral etiology, disease severity, and mortality.

Abstract Despite the biological and therapeutic relevance of CDK4/6 for the treatment of HR+, HER2- advanced breast cancer, the detailed mode of action of CDK4/6 inhibitors is not completely understood. Of particular interest, phosphorylation of CDK4 at T172 (pT172) is critical for generating the active conformation, yet no such crystal structure has been reported to date. We describe here the x-ray structure of active CDK4-cyclin D3 bound to the CDK4/6 inhibitor abemaciclib and discuss the key aspects of the catalytically-competent complex. Furthermore, the effect of CDK4/6 inhibitors on CDK4 T172 phosphorylation has not been explored, despite its role as a potential biomarker of CDK4/6 inhibitor response. We show mechanistically that CDK4/6i stabilize primed (pT172) CDK4-cyclin D complex and selectively displace p21 in responsive tumor cells. Stabilization of active CDK4-cyclin D1 complex can lead to pathway reactivation following alternate dosing regimen. Consequently, sustained binding of abemaciclib to CDK4 leads to potent cell cycle inhibition in breast cancer cell lines and prevents rebound activation of downstream signaling. Overall, our study provides key insights demonstrating that prolonged treatment with CDK4/6 inhibitors and composition of the CDK4/6-cyclin D complex are both critical determinants of abemaciclib efficacy, with implications for this class of anticancer therapy.

Introduction/Background: Patients with Coronavirus Disease 2019 (COVID-19) present with a spectrum of respiratory symptoms. There are no studies describing respiratory system involvement adjusted for other organ systems, oxygen saturation nadir, hospitalization days until respiratory involvement, proportion of days of respiratory system involvement, and persistent respiratory involvement at discharge in COVID-19 patients. We studied these parameters in COVID-19 patients that received respiratory therapy interventions and their association with mortality and length of stay (LOS). Methods: A single-center cross-sectional retrospective study of 738 COVID-19 patients with respiratory involvement at a hospital in the New York metropolitan area. Results: COVID-19 patients with respiratory involvement had increased mortality with oxygen requirement of FiO2 >55% (OR:39.02, 95% CI:1.59–960.51, P < 0.05) and mechanical ventilation (OR:236.64, 95% CI:8.24–6798.93, P < 0.01). Respiratory system involvement adjusted for other organ system involvement was associated with increased mortality (OR:1.60, 95% CI:1.20, 2.14, P < 0.01) and LOS (B = 0.02, SE = 0.01, P < 0.01). Oxygen saturation nadir of 70%–89% was significantly associated with increased LOS (B = 0.07, SE = 0.03, P < 0.05), whereas oxygen saturation nadir of 1 was associated with increased LOS. Respiratory involvement persistent at discharge was associated with increased mortality (OR:56.82, 95%CI:18.51–174.43, P < 0.001). Conclusions: Among all respiratory parameters, high oxygen requirements and low oxygen saturation nadir are the most predictive of COVID-19 prognosis.

This study explores the redefinition of the professional actress’s identity as a «lady of the stage» in the context of Spanish romantic theatre, focusing on the role played by visual culture in such a process. In the context of the different social circumstances and gender discourses that contributed to the actress’s new social identity, we shall explore how both print culture and portraiture acted as vehicles for the definition and dissemination of the new ideas surrounding this figure.

Background: Invasive pneumococcal disease (IPD) is the leading cause of infectious death worldwide. This study aimed to describe the epidemiology of IPD and the impact of pneumococcal conjugate vaccine-10 (PCV-10) over a 10-year period in Bogotá, Colombia. Methods: This was a laboratory-based surveillance study of Streptococcus pneumoniae isolated from patients with IPD from 82 hospitals over 10 years in Bogotá, Colombia. Data were compared between two periods: 2007–2011 (before the introduction of PCV-10) and 2012–2017 (after the introduction of PCV-10). Results: In total, 1670 patients with IPD were included in the study between 2007 and 2017. Between 2007 and 2011, the most common serotypes were 14, 1, 6B, 6A and 3. Between 2012 and 2017, the most common serotypes were 19A, 3, 14 and 1. A decrease in the incidence of IPD, particularly in children aged 0–4 years, was noted after the introduction of PCV-10. Importantly, this reduction in incidence was not observed in patients aged ≥50 years. Conclusions: The IPD burden in Bogotá remained stable between 2007 and 2017. The incidence of IPD decreased in children but not in older adults. The introduction of PCV-10 led to a change in the most prevalent serotypes to serotypes that are not included in PCV-10.

Eye diseases have been a severe problem worldwide, especially in developing countries where technology and finance are limited. Today, the problem is being resolved thanks to the task of classification that is part of pattern recognition. Its primary goal is to group standard features from any entity, object, phenomenon, or event belonging to the real or abstract world. Convolutional Neural Networks are a type of Artificial Neural Network used in intelligent pattern classification, Machine Learning, and Data Mining. Also, medicine and ophthalmology used these algorithms for detecting diseases in the human body. This work presents a novel intelligent pattern classification algorithm based on a Convolutional Neural network, which is validated through the K-Fold Cross Validation test. Two different groups of retinography images are given: Glaucoma and Diabetic Retinopathy. The result of accuracy percentage was 99.89%. Numerical metrics: Accuracy, Recall, Specificity Precision, and F1 score with values close to 1, and ROC curves support the suitable performance of the proposed classifier.

Phthalates are endocrine disrupting compounds (EDCs) used as plasticizers in a wide array of daily-use products, from flooring and automotive parts to medical devices and are even present in the children´s toys. Since these compounds are not covalently bound other molecules, they leach from these synthetic products, causing a high level of human exposure to them. EDCs exert several endocrine effects, most typically, reduced biosynthesis of the male hormone, testosterone and disturbances in estrogen, androgen, PPAR-gamma and AhR that control complex immunoendocrine regulatory networks. Besides impacting the developmental processes and long-term adverse effects, since cells of the immune system express endocrine receptors, and synthetize and respond to several hormones and other endocrine ligands, phthalates also cause dysregulation of immune system.

The availability of natural gas in Lima — Peru, from Camisea, in several urban areas of Lima, for technical reasons there are no natural gas facilities especially in peripheral urban areas in the highlands or hills. This is compensated by FISE government social programs providing subsidized liquefied petroleum gas balls. In this context, a proposal has been developed to provide natural gas to these elevated areas, in particular to Cerro San Cristóbal, located in the boundaries of the districts of Rímac and San Juan de Lurigancho — Lima. This proposal is based on the principles of Lean Logistic and linear programming of mixed burials. The results will imply a better supply of natural gas to peripheral areas and energy poverty, the inherent security problems must also be addressed. Nowadays, malware has become an epidemic problem. Among the attacks exploiting the computer resources of victims, one that has become usual is related to the massive amounts of computational resources needed for digital currency cryptomining. Cybercriminals steal computer resources from victims, associating these resources to the crypto-currency mining pools they benefit from. This research work focuses on offering a solution for detecting such abusive cryptomining activity, just by means of passive network monitoring. To this end, we identify a new set of highly relevant network flow features to be used jointly with a rich set of machine and deep-learning models for real-time cryptomining flow detection. We deployed a complex and realistic cryptomining scenario for training and testing machine and deep learning models, in which clients interact with real servers across the Internet and use encrypted connections. A complete set of experiments were carried out to demonstrate that, using a combination of these highly informative features with complex machine learning models, cryptomining attacks can be detected on the wire with telco-grade precision and accuracy, even if the traffic is encrypted.

Coronavirus disease 2019 (COVID-19) is spreading throughout the world. Limited data are available for the dynamics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral load (VL) in immunocompromised pediatric patients. Here, we report the clinical characteristics and the dynamics of SARS-CoV-2 VL of a pediatric patient with acute myeloid leukemia who developed a hyperinflammatory status mimicked MIS-C. The clinical course was characterized by the late onset of fever, GI symptoms, rash, and respiratory distress, including oxygen requirement with sustained VL of SARS-CoV-2 around 7 log10 RNA copies/mL for 6 weeks. It is important to note that the hyperinflammatory status developed early at the third week of hospitalization—in a context of high VL and immunocompromised status. All these characteristics make this clinical case unique. On the other hand, while many reports have characterized the dynamics of SARS-CoV-2 VL in adults and immunocompetent hosts, it remains unreported in pediatrics—even less in immunosuppressed children.

The complexity of cloud datacenter scenarios poses new challenges in infrastructure management processes such as the impracticality of collecting specific service level events from inside the datacenter infrastructure, and the scalability issues that can appear during event monitoring when thousands of virtual machines have to be polled at a granularity of seconds. Therefore, it would be desirable to provide mechanisms for obtaining these types of events without incurring in the previously described problems. To this end, we propose a generic and scalable method based on the application of deep neural network architectures for predicting service level events using only a reduced number of generic datacenter infrastructure statistics that can be monitored in a scalable way. We demonstrate in a controlled scenario of a real datacenter and using only three variables from a physical machine that it is possible to predict events in real-time and with decent accuracy, without needing to deploy any meter in the end-user equipment. Specifically, we demonstrate this over two service-level events: i) the so-called Noisy Neighbors effect, a harmful situation that appears in physical machines due to the interferences created by the interaction of virtual machines running on them; and ii) the jitter values of a multimedia call running in a virtual machine. We set up a testbed in a real datacenter deploying physical and virtual machines, running a large amount of different experiments for 1000 hours and collecting samples at a 10 seconds granularity in a dataset of 260,000 records. Two different scenarios, in which training and testing data sets contain significant statistical differences, are deployed to demonstrate a better generalization ability of deep models in changing scenarios when compared with traditional Machine Learning techniques. A set of different deep architectures are proposed for both use cases and approximately 4,000 deep models were trained and tested. In both use cases, the best deep models show a good performance when predicting service level events, even if the inputs do not exactly follow the statistical patterns of the data used during training.

Murine cysticercosis by Taenia crassiceps is a model for human neurocysticercosis. Genetic and/or immune differences may underlie the higher susceptibility to infection in BALB/cAnN with respect to C57BL/6 mice. T regulatory cells (Tregs) could mediate the escape of T. crassiceps from the host immunity. This study is aimed to investigate the role of Tregs in T. crassiceps establishment in susceptible and non-susceptible mouse strains. Treg and effector cells were quantified in lymphoid organs before infection and 5, 30, 90, and 130 days post-infection. The proliferative response post-infection was characterized in vitro. The expression of regulatory and inflammatory molecules was assessed on days 5 and 30 post-infection. Depletion assays were performed to assess Treg functionality. Significantly higher Treg percentages were observed in BALB/cAnN mice, while increased percentages of activated CD127+ cells were found in C57BL/6 mice. The proliferative response was suppressed in susceptible mice, and Treg proliferation occurred only in susceptible mice. Treg-mediated suppression mechanisms may include IL-10 and TGFβ secretion, granzyme- and perforin-mediated cytolysis, metabolic disruption, and cell-to-cell contact. Tregs are functional in BALB/cAnN mice. Therefore Tregs could be allowing parasite establishment and survival in susceptible mice but could play a homeostatic role in non-susceptible strains.

The flatworm Taenia solium causes human and pig cysticercosis. When cysticerci are established in the human central nervous system, they cause neurocysticercosis, a potentially fatal disease. Neurocysticercosis is a persisting public health problem in rural regions of Mexico and other developing countries of Latin America, Asia, and Africa, where the infection is endemic. The great variability observed in the phenotypic and genotypic traits of cysticerci result in a great heterogeneity in the patterns of molecules secreted by them within their host. This work is aimed to identify and characterize cysticercal secretion proteins of T. solium cysticerci obtained from 5 naturally infected pigs from Guerrero, Mexico, using 2D-PAGE proteomic analysis. The isoelectric point (IP) and molecular weight (MW) of the spots were identified using the software ImageMaster 2D Platinum v.7.0. Since most secreted proteins are impossible to identify by mass spectrometry (MS) due to their low concentration in the sample, a novel strategy to predict their sequence was applied. In total, 108 conserved and 186 differential proteins were identified in five cysticercus cultures. Interestingly, we predicted the sequence of 14 proteins that were common in four out of five cysticercus cultures, which could be used to design vaccines or diagnostic methods for neurocysticercosis. A functional characterization of all sequences was performed using the algorithms SecretomeP, SignalP, and BlastKOALA. We found a possible link between signal transduction pathways in parasite cells and human cancer due to deregulation in signal transduction pathways. Bioinformatics analysis also demonstrated that the parasite release proteins by an exosome-like mechanism, which could be of biological interest.

Colorectal cancer (CRC) is the third leading cause of cancer and second leading cause of cancer death in the United States. Recent evidence has linked a high fat and animal protein diet and microbial metabolism of host bile acids as environmental risk factors for CRC development. We hypothesize that the primary bile salt taurocholic acid (TCA) is a key, diet-controlled metabolite whose use by bacteria yields a carcinogen and tumor-promoter, respectively. The work is motivated by our published data indicating hydrogen sulfide (H2S) and secondary bile acid production by colonic bacteria, serve as environmental insults contributing to CRC risk. The central aim of this study is to test whether a diet high in animal protein and saturated fat increases abundance of bacteria that generate H2S and pro-inflammatory secondary bile acids in African Americans (AAs) at high risk for CRC. Our prospective, randomized, crossover feeding trial will examine two microbial mechanisms by which an animal-based diet may support the growth of TCA metabolizing bacteria. Each subject will receive two diets in a crossover design― an animal-based diet, rich in taurine and saturated fat, and a plant-based diet, low in taurine and saturated fat. A mediation model will be used to determine the extent to which diet (independent variable) and mucosal markers of CRC risk and DNA damage (dependent variables) are explained by colonic bacteria and their functions (mediator variables). This research will generate novel information targeted to develop effective dietary interventions that may reduce the unequal CRC burden in AAs.

T regulatory cells play a key role in the control of the immune response, both in health and during illness. While the mechanisms through which T regulatory cells exert their function have been extensively described, their molecular effects on effector cells have received little attention. Thus, this revision is aimed at summarizing our current knowledge on those regulation mechanisms on the target cells from a molecular perspective.

IntroductionThe effect of varying portion size in a natural environment on the self-regulation of 24-hour energy intake has not been evaluated. We studied students at 16 preschools in Chicago to determine the effect of meals with variable energy and macronutrients on the amount consumed over a 24-hour period (ie, at preschool and outside of preschool).MethodsThe food items and portion sizes served at 16 preschools in the Chicago Public Schools early childhood education programs were observed for 3 to 8 days. Children were assessed for total energy and selected macronutrients consumed at preschool and outside of preschool for a 24-hour period; their data were pooled and reported by school.ResultsThe students were predominantly African American (96%); on average, 32 students (range, 21-38) participated at each of the 16 preschools, and the age range of students was 4.0 to 4.5 years. The energy served at preschools ranged from 48% to 90% of the daily recommended energy allowance (REA). The mean energy intake at school was significantly higher (39% of REA) at 2 preschools, including 1 that served 90% of the REA. Mean energy consumption outside of preschool and total 24-hour energy consumption did not differ by preschool, adjusting for body mass index z score and sex.ConclusionThe preschools served meals that widely varied by portion size and energy; however, this variation did not result in differences in mean 24-hour nutrient intakes for the students attending these schools.
