BackgroundDisparities in prostate cancer-specific mortality (PCSM) between African American and non-Hispanic White (White) patients have been attributed to biological and systemic factors. We evaluated drivers of these disparities in the Surveillance, Epidemiology, and End Results (SEER) national registry and an equal-access system, the Veterans Health Administration (VHA).MethodsWe identified African American and White patients diagnosed with prostate cancer between 2004 and 2015 in SEER (n = 311 691) and the VHA (n = 90 749). We analyzed the association between race and metastatic disease at presentation using multivariable logistic regression adjusting for sociodemographic factors and PCSM using sequential competing-risks regression adjusting for disease and sociodemographic factors.ResultsThe median follow-up was 5.3 years in SEER and 4.7 years in the VHA. African American men were more likely than White men to present with metastatic disease in SEER (adjusted odds ratio = 1.23, 95% confidence interval [CI] = 1.17 to 1.30) but not in the VHA (adjusted odds ratio = 1.07, 95% CI = 0.98 to 1.17). African American vs White race was associated with an increased risk of PCSM in SEER (subdistribution hazard ratio [SHR] = 1.32, 95% CI = 1.10 to 1.60) but not in the VHA (SHR = 1.00, 95% CI = 0.93 to 1.08). Adjusting for disease extent, prostate-specific antigen, and Gleason score eliminated the association between race and PCSM in SEER (aSHR = 1.04, 95% CI = 0.93 to 1.16).ConclusionsRacial disparities in PCSM were present in a nationally representative registry but not in an equal-access health-care system, because of differences in advanced disease at presentation. Strategies to increase health-care access may bridge the racial disparity in outcomes. Longer follow-up is needed to fully assess mortality outcomes.

Evidence supporting radical prostatectomy (RP) for men with clinically node-positive (cN+) prostate cancer (PC) is limited. In a US national database, we identified 741 men with cN+ nonmetastatic PC diagnosed during 2000-2015 who underwent definitive local therapy with RP (n=78), radiotherapy (RT) with neoadjuvant androgen deprivation therapy (ADT) (n=193), or nondefinitive therapy with ADT alone (n=445) or observation (n=25). We compared PC-specific mortality (PCSM) and all-cause mortality (ACM) using multivariable Fine-Gray competing risk regression and Cox regression, respectively. How do police respond to the presence of cameras and photographers? Many speculative theories have been proposed offering mixed and sometimes contradictory answers to this question. Some theories propose that cameras will deter police misconduct, others suggest that cameras might improve police accountability, others suggest that police might respond to cameras by engaging in a risk-averse style of policing. Unfortunately, little empirical data is available to assess these theories. Drawing on data from a participant-observation research study conducted in Edmonton, Alberta, Canada, this paper helps fill this gap in research and argues that police might be learning to adapt to cameras by engage in what I call camera-friendly policing. This style of policing involves efforts to control how the police are perceived by photographers, and how they will be perceived by viewers of any recorded footage. In this paper, I outline the basic elements of the police's camera-friendly tactics, and discuss the implications of these tactics for contemporary understandings of police visibility. Abstract: Purpose: The 6-MV flattening filter-free mode (6F) of the Varian TrueBeam (Varian Medical Systems, Palo Alto, CA) enables faster dose delivery and shortens treatment time, which are especially beneficial for stereotactic radiosurgery. This study is to evaluate the feasibility and advantages of using 6F in stereotactic radiosurgery treatment of multiple brain lesions in comparison with regular 6-MV mode (6X). Materials and Methods: Ten patients having 2-12 brain metastases treated by intensity modulated stereotactic radiosurgery were selected for this study. For each patient, 2 RapidArc (RA; Varian Medical Systems) plans were generated: one using the 6F mode with a dose rate of 1400 monitor units (MU)/minute and another using the regular 6X mode of 600 MU/minute for a Varian TrueBeam linac. For each patient, both plans employed the same beam arrangement and optimization process. Results: The dosimetric parameters of homogeneity, conformity, and gradient indices were calculated and found to be comparable in the 6F and 6X plans for each patient. The mean dose to the normal brain and maximal doses to brainstem, chiasm, eyes, and optical nerves were also comparable in both RA plans using either 6F or 6X. The total number of MUs in the RA plans using 6F was 10%-20% more than that in the RA plan using 6X, but the beam-on-time was much less if 6F was used for planning and dose delivery (50% less). Conclusions: The fast delivery of the 6F beam is not only beneficial in stereotactic radiosurgery of a single brain lesion, but also for treating multiple brain lesions (2-12 lesions in this study group). Due to the beam falloff away from the central axis for large field sizes, more MUs are needed for 6F beams as compared with 6X. However, for the 6F mode with 1400 MU/minute, the delivery times are still much shorter compared with the 6X mode, thus greatly shortening the treatment time. [Copyright &y& Elsevier]

Purpose: Cryotherapy is an option for the primary treatment of localized prostate cancer, along with radical prostatectomy, external beam radiation therapy, and brachytherapy. Although it is known that local recurrence can occur in >20% of patients treated with primary cryotherapy, unfortunately there is a paucity of data on later salvage treatments. The use of external beam radiation therapy is an attractive option after cryotherapy failure, but there is little data on its efficacy and toxicity. We evaluated the biochemical control and complication rates of salvage dose-escalated image guided intensity modulated radiation therapy (IG-IMRT) after cryotherapy failure., Methods and Materials: Patients who were treated at our institution from 2005 to 2016 were reviewed for those who underwent cryotherapy as initial treatment followed by salvage IGRT. Patients were treated with dose-escalated IG-IMRT using standard treatment margins of 3 mm posterior and 7 mm in all other directions and daily cone beam computed tomography or kv imaging to implanted fiducial markers. Biochemical progression was defined in accordance with the Phoenix consensus conference definition., Results: Eight patients were identified as having received post-cryotherapy salvage radiation within the study period. The median total dose was 77.7 Gy (range, 75.6-81.0 Gy). Median follow-up was 55 months (range, 6-88 months). Six patients remained biochemically controlled at the latest follow-up. One patient developed distant metastases after 22 months and one experienced biochemical failure at 30 months with no evidence of distant metastases. No patients experienced acute gastrointestinal toxicities of grade 2 or higher. There were no cases of late gastrointestinal or genitourinary toxicity., Conclusions: High-dose IG-IMRT results in high rates of salvage and extremely low rates of serious late toxicity for patients with locally recurrent prostate cancer after cryotherapy. Although the results are encouraging, given the small number of patients in this and other series, we remain cautious with regard to this treatment and believe the use of salvage radiation therapy after cryotherapy warrants further study.

Background: The lung is a common site of extracranial metastases. Frameless Stereotactic Body Radiation Therapy (SBRT) is a promising new therapy for non surgically resectable neoplastic lung lesions used at our institution., Methods: A retrospective study of 21 patients and 33 lesions treated with SBRT was done. Local control (LC), distant control (DC), progression-free survival (PFS) and overall survival (OS) were analyzed using the Kaplan-Meier method. Potential prognostic factors were analyzed using the log-rank test and Cox regression. Toxicities were also reported., Results: Actuarial local control rates by lesions were 88% (95% confidence interval [CI], 77-99%) and 76% (95% CI, 59-92%) at 12-and 24-months, respectively. Actuarial local control rates by patients was 80% (95% CI, 62-98%) and 71% (95% CI, 43-100%) for 12- and 24-months, respectively. DC rates were 52% (95% CI 31-74%) and 38% (95% CI, 15-61%), at 12- and 24-months respectively. PFS rates were 52% (95% CI 31-74%) and 32% (95% CI 9-55%) at 12- and 24-months, respectively. Overall survival rates were 90% (95% CI 77-100%) and 78% (95% CI 59-97%) at 12- and 24-months, respectively. Single metastasis was associated with better PFS (p=0.023). No toxicities greater than CTCAE grade 3 were observed., Conclusions: Frameless SBRT achieves acceptable control in pulmonary metastatic lesions with an excellent toxicity profile.
