Abstract

Background: Ankylosing spondylitis (AS) and Behçet's disease (BD) are distinct inflammatory disorders, but their coexistence is a rare clinical entity. This case sheds light on managing this complex scenario with Janus kinase (JAK) inhibitors. Case Presentation: A 42‐year‐old woman presented with a decade‐long history of lower back pain, nocturnal spinal discomfort, recurrent eye issues, oral and genital ulcers, hearing loss, pus formation in the left eye, and abdominal pain. Multidisciplinary consultations and diagnostic tests confirmed AS (HLA‐B27 positivity and sacroiliitis) and BD (HLA‐B51). Elevated acute‐phase markers were observed. Conclusion: This case fulfills diagnostic criteria for both AS and BD, emphasizing their coexistence. Notably, treatment with upadacitinib exhibited promising efficacy, underscoring its potential as a therapeutic option in patients with contraindications for conventional treatments. Our findings illuminate the intricate management of patients presenting with these two diverse systemic conditions and advocate for further exploration of JAK inhibitors in similar cases. 14. Честота и медико-социални аспекти на предтерминното раждане

Abstract

Premature birth (PB) is a serious problem, both globally and nationally gauge. The importance of the problem is determined both by the persistent frequency and the effects on the fetus and the newborn. Premature birth is the cause of up to 75% of prenatal and 85% of neonatal infant mortality, as well as a high incidence of permanent injuries to various organs and systems in surviving children - a moment of high personal and social significance. Controversial data from the scientific literature provoked us to carry out our research, which we tried to clarify some of the medical and social aspects of premature birth, and to help solve this medical and social dilemma. The results of this study are designed to inform the problem and to contribute to the preparation of measures to prevent premature birth. The aim is to stimulate the creation of targeted intervention programs that reduce the burden of the problem. It is necessary to ensure equal access to complex prenatal care and high quality of emergency obstetric care. It is of particular importance that prenatal care includes early detection of women at high risk of premature birth., ПТР се явява сериозен проблем, както в световен, така и в национален мащаб. Важността на проблема се определя, както от устойчивата честота, така и от последиците за плода и новороденото. Преждевременното раждане е причина за до 75% от пренаталната и 85% от неонаталната детска смъртност, както и за немалък процент трайни увреждания на различни органи и системи при преживелите деца – момент с висока степен на лична и обществена значимост. Противоречивите данни от научната литературата ни провокираха да осъществим нашето проучване, с което се опитахме да изясним част от медико-социалните аспекти на ПТР и с това да подпомогнем решаването на тази медицинска и социална дилема. Резултатите от това проучване са предназначени да информират за проблема и да допринесат за изготвянето на мерки за предотвратяване на преждевременното раждане. Целта е да се стимулира създаването на целенасочени програми за интервенции, водещи до намаляване тежестта на проблема. Необходимо е осигуряване на равен достъп до комплексни пренатални грижи и високо качество на спешните акушерски грижи. От особена важност е пренаталните грижи да включват своевременно откриване на жени с висок риск от преждевременно раждане.
