Abstract

ObjectiveTo compare secondary patient reported outcomes of perceptions of treatment success and function for patients treated for appendicitis with appendectomy vs. antibiotics at 30 days.Summary background dataThe Comparison of Outcomes of antibiotic Drugs and Appendectomy trial found antibiotics noninferior to appendectomy based on 30-day health status. To address questions about outcomes among participants with lower socioeconomic status, we explored the relationship of sociodemographic and clinical factors and outcomes.MethodsWe focused on 4 patient reported outcomes at 30 days: high decisional regret, dissatisfaction with treatment, problems performing usual activities, and missing >10 days of work. The randomized (RCT) and observational cohorts were pooled for exploration of baseline factors. The RCT cohort alone was used for comparison of treatments. Logistic regression was used to assess associations.ResultsThe pooled cohort contained 2062 participants; 1552 from the RCT. Overall, regret and dissatisfaction were low whereas problems with usual activities and prolonged missed work occurred more frequently. In the RCT, those assigned to antibiotics had more regret (Odd ratios (OR) 2.97, 95% Confidence intervals (CI) 2.05-4.31) and dissatisfaction (OR 1.98, 95%CI 1.25-3.12), and reported less missed work (OR 0.39, 95%CI 0.27-0.56). Factors associated with function outcomes included sociodemographic and clinical variables for both treatment arms. Fewer factors were associated with dissatisfaction and regret.ConclusionsOverall, participants reported high satisfaction, low regret, and were frequently able to resume usual activities and return to work. When comparing treatments for appendicitis, no single measure defines success or failure for all people. The reported data may inform discussions regarding the most appropriate treatment for individuals.Trial registrationClinicaltrials.gov Identifier: NCT02800785.

Abstract

The coupled evolution of the magnetic field and the flow at the Earth's core mantle boundary is modeled within the 1900.0-2014.0 time period. To constraint the dynamical behavior of the system with a core field model deriving from direct measurements of the Earth's magnetic field we used an Ensemble Kalman filter algorithm. By simulating an ensemble of possible states, access to the complete statistical properties of the considered fields is available. Furthermore, the method enables to provide predictions and to assess their reliability. In this study, we could highlight the cohabitation of two distinct flow regimes. One associated with the large scale part of the eccentric gyre, which evolves slowly in time and posses a very long memory of its past, and a faster one associated with the small scale velocity field. We show that the latter can exhibit rapid variations in localized areas. The combination of the two regimes can predict quite well the decadal variations in length of day, but it can also explain the discrepancies between the physically predicted and the observed trend in these variations. Hindcast tests demonstrate that the model is well balanced and that it can provide accurate short term predictions of a mean state and its associated uncertainties. However, magnetic field predictions are limited by the high randomization rate of the different velocity field scales, and after approximately 2000 years of forecast, no reliable information on the core field can be recovered., Comment: 14 pages, 9 figures, 2tables

Abstract

The Aux/IAA proteins are auxin-sensitive repressors that mediate diverse physiological and developmental processes in plants [1, 2]. There are 29 Aux/IAA genes in Arabidopsis that exhibit unique but partially overlapping patterns of expression [3]. Although some studies have suggested that individual Aux/IAA genes have specialized function, genetic analyses of the family have been limited by the scarcity of loss-of-function phenotypes [4]. Furthermore, with a few exceptions, our knowledge of the factors that regulate Aux/IAA expression is limited [1, 5]. We hypothesize that transcriptional control of Aux/IAA genes plays a central role in the establishment of the auxin-signaling pathways that regulate organogenesis, growth, and environmental response. Here, we describe a screen for transcription factors (TFs) that regulate the Aux/IAA genes. We identify TFs from 38 families, including 26 members of the DREB/CBF family. Several DREB/CBF TFs directly promote transcription of the IAA5 and IAA19 genes in response to abiotic stress. Recessive mutations in these IAA genes result in decreased tolerance to stress conditions, demonstrating a role for auxin in abiotic stress. Our results demonstrate that stress pathways interact with the auxin gene regulatory network (GRN) through transcription of the Aux/IAA genes. We propose that the Aux/IAA genes function as hubs that integrate genetic and environmental information to achieve the appropriate developmental or physiological outcome.

48. A mean-field Babcock-Leighton solar dynamo model with long-term variability

Abstract

Dynamo models relying on the Babcock-Leighton mechanism are successful in reproducing most of the solar magnetic field dynamical characteristics. However, considering that such models operate only above a lower magnetic field threshold, they do not provide an appropriate magnetic field regeneration process characterizing a self-sustainable dynamo. In this work we consider the existence of an additional \alpha-effect to the Babcock-Leighton scenario in a mean-field axisymmetric kinematic numerical model. Both poloidal field regeneration mechanisms are treated with two different strength-limiting factors. Apart from the solar anti-symmetric parity behavior, the main solar features are reproduced: cyclic polarity reversals, mid-latitudinal equatorward migration of strong toroidal field, poleward migration of polar surface radial fields, and the quadrature phase shift between both. Long-term variability of the solutions exhibits lengthy periods of minimum activity followed by posterior recovery, akin to the observed Maunder Minimum. Based on the analysis of the residual activity during periods of minimum activity, we suggest that these are caused by a predominance of the \alpha-effect over the Babcock-Leighton mechanism in regenerating the poloidal field., Comment: 21 pages, 6 figures

Abstract

Space weather is a matter of practical importance in our modern society. Predictions of forecoming solar cycles mean amplitude and duration are currently being made based on flux-transport numerical models of the solar dynamo. Interested in the forecast horizon of such studies, we quantify the predictability window of a representative, advection-dominated, flux-transport dynamo model by investigating its sensitivity to initial conditions and control parameters through a perturbation analysis. We measure the rate associated with the exponential growth of an initial perturbation of the model trajectory, which yields a characteristic time scale known as the e-folding time $\tau_e$. The e-folding time is shown to decrease with the strength of the $\alpha$-effect, and to increase with the magnitude of the imposed meridional circulation. Comparing the e-folding time with the solar cycle periodicity, we obtain an average estimate for $\tau_e$ equal to 2.76 solar cycle durations. From a practical point of view, the perturbations analysed in this work can be interpreted as uncertainties affecting either the observations or the physical model itself. 50. Between Extraordinary and Marginalized: Negotiating Tensions in Becoming Special Education-Certified Teachers

Abstract

The field of special education is in flux, with high shortages and attrition and increasing requirements around quality teachers and teaching contexts. In this study, we explore how preservice teachers develop identities as special education-certified teachers within this context. Data include focus groups, admissions essays, and class assignments from 3 years of a masters-level preparation program. Analysis began with open coding, and initial themes included positioning, power, and marginalization. Iterative analysis led to findings around negotiation--how preservice teachers negotiated what it meant to be a teacher in diverse teaching contexts while both maintaining a view of special education-certified teachers as extraordinary and also experiencing marginalization within their roles. Implications include supporting teacher candidates in managing these tensions, as well as a need for programs to partner with schools as a whole and not just individual cooperating teachers.
