Congenital diaphragmatic hernia is a common cause of neonatal respiratory distress. The right or left diaphragmatic cupola hernia is most often involved. Rarely, a retro-costo-xiphoidian or Larrey or Morgagni hernia is diagnosed as part of respiratory distress. We present a case of congenital retro-costo-xiphoid diaphragmatic hernia in a midline situation, involving the sternal portion and extending to the diaphragmatic domes on each side. The discovery was made antenatal by obstetric ultrasound from 21 weeks. At birth, there was minor respiratory distress. The management consisted of a reduction and placement of a polypropylene plate in the natal post by abdominal approach.

Background/Purpose: Esophageal atresia (EA) is a congenital defect in the thoracic esophagus associated or not with a tracheoesophageal fistula (TEF). It is associated with high mortality in low-income countries such as Burkina Faso, Madagascar and Ghana. The purpose of this study was to present the recent management of newborns with EA/TEF in Benin and to identify the needs of families whose children have survived. Method: Over a period of 10 years, 54 newborns with EA/TEF operated on in the 2 largest university hospitals in Benin were included. Two groups were identified. Group A (n=33) included newborns in whom a primary repair had been performed. Group B (n=21) involved newborns who underwent staged repair. Staged repair involved cervical oesophagostomy + gastrostomy (n= 7) or upper oesophageal pouch suction + gastrostomy (n=2) or cervical esophagostomy + stoma of the lower esophageal pouch (n=12). Ten parents from group B were interviewed for the needs survey. The significance level was defined as p < 0.05. Results: There were 31 male newborns. The median for gestational age was 37 gestational weeks (range: 35 - 38). Polyhydramnios was observed in 4 cases. The mean birth weight was 2365 g. (range: 1000 g - 3500 g). The mean age at diagnosis was 3.48 days (range: 24 hours - 19 days). Surgery was performed at day 5 on average with extremes from day 2 to day 17. The surgery lasted an average of 2h12 min and the overall mortality rate was 74.07%. Mortality rate in group A was 100% and 33.33% in group B. There was a strong statistically significant difference in survival between the two groups. (p=19.10-8). Sepsis was the most common cause of death. Stress, fear and anxiety of losing the child were the difficulties frequently encountered at bedtime and every day. Four parents had to stop their activities to be available to take care of their child. All the parents had expressed the need for psychological support. Conclusion: Pending an improvement of the technical platform, staged repair remains an essential option for the survival of patients. The creation of a framework for discussion with families authorizing social actions and quality medical support is desired.

Recessive and de novo mutations in the TRIOgene are associated with intellectual deficiency (ID), autism spectrum disorder (ASD) and developmental epileptic encephalopathies (DEE). TRIO is a dual guanine nucleotide exchange factor (GEF) that activates Rac1, Cdc42 and RhoA. Trio has been extensively studied in excitatory neurons, and has recently been found to regulate the switch from tangential to radial migration in GABAergic interneurons (INs) through GEFD1-Rac1-dependent SDF1α/CXCR4 signaling. Given the central role of Rho-GTPases during neuronal migration and the implication of IN pathologies in ASD and DEE, we investigated the relative roles of both Trio’s GEF domains in regulating the dynamics of INs tangential migration. In Trio–/–mice, we observed reduced numbers of tangentially migrating INs, with intact progenitor proliferation. Further, we noted increased growth cone collapse in developing INs, suggesting altered cytoskeleton dynamics. To bypass the embryonic mortality of Trio–/–mice, we generated Dlx5/6Cre;Trioc/cconditional mutant mice (TriocKO), which develop spontaneous seizures and behavioral deficits reminiscent of ASD and ID. These phenotypes are associated with reduced cortical IN density and functional cortical inhibition. Mechanistically, this reduction of cortical IN numbers reflects a premature switch to radial migration, with an aberrant early entry in the cortical plate, as well as major deficits in cytoskeletal dynamics, including enhanced leading neurite branching and slower nucleokinesis reflecting reduced actin filament condensation and turnover as well as a loss of response to the motogenic effect of EphA4/ephrin A2 reverse signaling. Further, we show that both Trio GEFD1 and GEFD2 domains are required for proper IN migration, with a dominant role of the RhoA-activating GEFD2 domain. Altogether, our data show a critical role of the DEE/ASD-associated Triogene in the establishment of cortical inhibition and the requirement of both GEF domains in regulating IN migration dynamics.

In early 2009, Guadeloupe experienced a 44-day strike against the high cost of living and against the illegitimate profits some actors realise in the Island’s economy. Some of these dominant actors being heirs of settlers families, high prices are the starting point for a radical political critique. This article analyses the links between violence and the use of numbers in the course of the conflict. The mobilisation was a time of violence, clashes, and intimidation; but the denunciation of abuse also ascribes a central role to quantification, in order to estimate profit. This article shows how the figures comprise an instrument of mediation being used as a substitute for, or in combination with, multifaceted violent actions. It also shows that the figures may be themselves coercive techniques, playing a part in violent relationships. Quantification can therefore be combined, in a plurality of ways, with the transition to violence, not only by avoiding or replacing it, but sometimes overlapping with it or being its instrument

The ingestion of caustic soda is frequently a source of complications. Esophageal stenosis remains among them the most frequent complication and by far the most formidable. Pyloric stenosis remains rare following ingestion of soda. We report through this clinical case, a cicatricial pyloric stenosis after ingestion of caustic soda associated with a stenosis of the esophagus having caused vomiting with the onset of severe malnutrition. The diagnosis was confirmed by the oeso-gastro-duodenal transit (OGDT) and the child underwent under general anesthesia an enlargement pyloroplasty according to Mikulicz initially and an esophageal dilation secondly to the candle of SAVARY. The postoperative follow-up was favorable with an improvement of the signs and a progressive postoperative weight gain after a follow-up of 06 months. The evolution was favorable after the monthly dilation sessions with a good resumption of transit.

Introduction In Africa, frequent delays in seeking medical attention due to cultural considerations and lack of financial support could explain some particular clinical presentations. An enteroscrotal fistula following an incarcerated inguinal hernia is a rare condition in neonates. Presentation of case A 21-day-old infant was brought to our consultation for persistent stool discharge from the right scrotum for three days. The parents reported an inguinoscrotal swelling that had been evolving for ten days before admission and became painful two days later. He was diagnosed with a case of enteroscrotal fistula. After correction of electrolytes disorders, a laparotomy was performed through a right lower transverse incision. Operative findings were strangulated inguinal hernia with ileal perforation. Resection and end-to-end ileo-ileal anastomosis was performed. He was discharged from the hospital on the 6th post-operative day. No recurrence within the first year of follow-up. Discussion Thirteen cases have been reported in the literature until now. Most of them were from developing countries, including ours. Conclusion Early seeking medical attention of incarcerated inguinal hernias is necessary to reduce their morbidity and mortality. Paediatricians should emphasize providing adequate information, education and communication during routine examinations of neonates. We advocate the popularization of universal health insurance to facilitate health care., Highlights • Enteroscrotal fistula is an uncommon condition following incarcerated inguinal hernia in neonate. • Thirteen cases have been reported in literature until now. • Most of cases were reported in countries with limited resources. • Delay in seeking medical attention is thought to be the main cause of enteroscrotal fistula.

L’acide gras synthase, codé par le gène FASN, est une protéine multi-enzyme homodimérique responsable de la lipogenèse de novo à partir de l’acétyl-CoA et du malonyl- CoA. La finalité de cette lipogenèse est la production de l’acide palmitique, un acide gras simple, précurseur des acides gras à très longues chaînes. L’acide palmitique est impliqué dans plusieurs processus biologiques, dont la palmitoylation qui permet d’ancrer diverses protéines à la membrane cellulaire sous-tendant, entre autres, la transmission synaptique. Le rôle de l’acide gras synthase dans le développement embryonnaire est bien établi. En effet, il est exprimé de manière ubiquitaire dans l’embryon, principalement dans les tissus en pleine croissance et soumis à un remodelage, participant ainsi activement au développement cérébral. Par conséquent, des mutations du gène FASN ont été associées à plusieurs maladies, incluant divers types de cancers, les maladies cardiovasculaires, mais également, plus récemment, à certaines maladies du neurodéveloppement, incluant les troubles du spectre de l’autisme. Des données récentes des laboratoires Rossignol et Campeau, au CHU Ste-Justine, suggèrent un lien entre des mutations récessives ou de novo du gène FASN et des formes précoces d’épilepsie avec atteinte cognitive (encéphalopathies épileptogènes). Nous postulons que les mutations du gène FASN modifient la synthèse de l’acide palmitique et perturbent le développement des réseaux neuronaux, en altérant la migration, le développement morphologique, l’excitabilité et/ou la fonction synaptique de populations neuronales spécifiques, résultant en une hyperexcitabilité neuronale et à l’épilepsie. Pour explorer cette hypothèse, nous avons recueilli les informations cliniques de dix patients porteurs de mutations du gène FASN dans le cadre d’études génomiques en cours au CHU Ste- Justine et à travers le monde. Nous avons également généré un nouveau modèle murin de la maladie, exprimant une mutation retrouvée chez un membre de notre cohorte clinique, que nous avons caractérisé sur les plans histochimique et électrophysiologique. Nos données suggèrent que les mutations du gène FASN induisent chez l’humain un phénotype clinique de retard global du développement évoluant vers une déficience intellectuelle, s’accompagnant d’un éventail de signes neurologiques (déficit moteur, spasticité, réflexes ostéotendineux vifs, hypotonie et ataxie) et d’un risque accru d’épilepsie. De plus, notre modèle de souris knock-in Fasn.S154N révèle la fonction critique de ce gène dans le développement embryonnaire puisqu’une mutation homozygote entraîne une mortalité in utero. Par ailleurs, les souris porteuses de mutations hétérozygotes survivent et présentent un phénotype clinique rappelant celui observé chez les patients, incluant un comportement anxieux, une activité épileptique interictale à l’électroencéphalogramme ainsi qu’un abaissement du seuil convulsif lors d’une exposition au pentylenetetrazole (PTZ). Nous discutons certains mécanismes sous-jacents contribuant potentiellement au développement de l’épilepsie dans cette maladie, incluant une altération de l’activité de l’acide gras synthase au niveau du cortex préfrontal et de l’amygdale, une palmitoylation aberrante des protéines synaptiques, une plus grande vulnérabilité des cellules granulaires du gyrus denté, un dysfonctionnement des cellules souches neurales, une neurogénèse insuffisante, ainsi qu’une altération de la myélinisation et de la croissance axonale impactant la migration des interneurones. Ces mécanismes sont prédits pour altérer l’excitabilité neuronale et la transmission synaptique, perturbant la fonction des circuits. Des études subséquentes permettront d’élucider lesquels de ces divers mécanismes contribuent au phénotype clinique dans notre nouveau modèle murin de la maladie., Fatty Acid Synthase is a large protein complex encoded by the FASN gene, which is responsible for de novo lipogenesis from acetyl-CoA and malonyl-CoA in the presence of NADPH. The endpoint of this process is the production of palmitic acid. The roles of fatty acid synthase in embryonic development are well established: it is ubiquitously expressed in early embryos, particularly in tissues undergoing active proliferation, outgrowth, and remodelling, and it is thus essential for normal brain development and neuronal function. Consequently, FASN gene mutations have been associated with several neurodevelopmental conditions, including autism spectrum disorders (ASD). Recently, the laboratories of Drs. E. Rossignol and P. Campeau at the CHU Ste-Justine (Université de Montréal), with their international collaborators, have identified 10 patients with neurodevelopmental disorders (i.e., developmental delay, intellectual disability and/or epilepsy) carrying recessive or de novo mutations in the FASN gene, supporting a critical role of FASN in regulating neuronal circuit development and function. However, the mechanisms by which mutations in the FASN gene result in epilepsy are unknown. We postulate that FASN mutations alter palmitic acid synthesis and disrupt neuronal network development, resulting in network hyperexcitability and epilepsy. In this study, we expand the phenotypic description of patients carrying FASN mutations, while generating a novel mouse model carrying a patient-derived FASN mutation to explore the underlying cellular and network mechanisms. Our data reveal that FASN mutations, in humans, generate neurodevelopmental disorders characterized by epilepsy, global developmental delay (GDD), intellectual disability (ID), and a broad range of neurological signs (motor deficit, spasticity, hyperreflexia, hypotony, and ataxia). In our knock-in FasnS154N mouse model, homozygous mutations resulted in prenatal lethality. In contrast, heterozygous mutations caused a clinical phenotype reminiscent of the patient phenotype, with anxiety-like behaviors, spontaneous interictal spikes on electroencephalograms (EEG), and a tendency to a reduced PTZ-induced seizure threshold. We discuss the potential underlying mechanisms, including an altered FAS activity within the prefrontal cortex and the amygdala, aberrant palmitoylation of postsynaptic density proteins, the vulnerability of dentate gyrus granules cell, altered neural stem cells activity and neurogenesis, improper axonal growth and myelination, resulting in altered neuronal excitability and synaptic function, aberrant network activities and epilepsy. These mechanisms will be explored in subsequent studies using our novel animal model.

- Abstract
Bowel malrotation is an abnormality in the rotation of the primary intestinal loop during embryonic life. It is a severe abnormality exposing the patient to fatal complications. We report here a series of five patients presenting digestive malrotation with various malformative associations. The first three cases are multiple intestinal atresia on incomplete common mesentery. They underwent an intestinal anastomosis: end-lateral duodeno-ileal for one and end-to-end jejuno-jejunal for the other two with Ladd procedure. In the latter two cases, the omphalocele was the associated malformation; complications such as rupture of the omphalocele with intestinal perforation for one and occlusion by strangulation at the level of the neck for the other were recorded. Ileal resection removing the perforation with end-to-end ileo-ileal anastomosis was performed for the first, while reduction of strangulation at the origin of the occlusion was sufficient for the second. The LADD procedure was the complementary gesture in all cases. The outcome was favorable in 3 cases; two of the newborns who had jejunal atresia died, one on day 5 in a picture of acute respiratory distress, the other on day 6 in a picture of disseminated intravascular coagulation.

Die neoliberale Wende und die Konsolidierung der transnationalen Bürokratie -- Diese Forschungsnotiz befasst sich mit der Entstehung eines Korps von Statistikern und Ökonomen im frankophonen Westafrika in den 1990er Jahren. Der Abwertung des westafrikanischen Franc (CFA) im Januar 1994 folgte eine Politik wirtschaftlicher Integration, deren vorrangiges Ziel eine makroökonomische Stabilisierung sowie die Einhaltung europäisch inspirierter Sparmassnahmen war. Die Etablierung von Afristat, die ihre Arbeit 1996 aufnahm, ermöglichte die multilaterale Beobachtung, indem sie harmonisierte statistische Instrumentarien bereitstellte. Sie erlaubte Frankreichs Rückzug von seinen bilateralen Beihilfen für die wirtschaftliche Verwaltung, doch sie eröffnete zugleich auch neue Räume der Sozialbilität für westafrikanische Statistiker und die Entwicklung ihrer Karrierestrategien, die zwischen der national und transnationalen Ebene angesiedelt waren, was hier anhand des Beispiels der Entwicklung neuer nationaler Rechnungswesen aufgezeigt wird. Die 1990er Jahre brachten zugleich die Afrikanisierung der Ausbildung der Statistiker und Ökonomen zum Abschluss, die im Gefolge der Unabhängigkeit eingesetzt hatte. Die regionalen Ausbildungstätten für Wirtschaft und Statistik in Abidjan und Dakar sind heute die Grundpfeiler der Ausbildung für höhere frankophone westafrikanische Verwaltungsbeamte. Die Statistiker und Ökonomen der Westafrikanischen Zentralbank stellen nicht zuletzt auch eine Verwaltungselite dar, die ihr know-how in den Dienst der Desinflationspolitik stellen und die zugleich ihren Einfluss spielen lassen, um sich in den nationalen und regionalen Politikfeldern zu positionieren. Eine biographische Analyse, für die sich diese Forschungsnotiz stark macht, würde es erlauben, darzustellen, wie die regionale Integration orthodoxen makroökonomischen Prinzipien gehorchte und so die Konstituierung eines regionalen Korps von Statistikern und Ökonomen ermöglichte.; The neoliberal turn and the consolidation of a transnational bureaucracy --This research note analyses the prominence taken by a corps of statisticians and economists in Francophone West African since the 1990s. The devaluation of the CFA franc in January 1994 was followed by a policy of economic integration with the primary aim, influenced by European policies, of sustaining macroeconomic stability and budgetary rigor. The creation of Afristat, which became operational in 1996, contributed to the actualization of a multilateral control, through the deployment of harmonized statistical tools. While this enabled France to reduce its bilateral assistance to economic administrations, it also carved out spaces of socialization for West African statisticians, by allowing them to develop career strategies across the national and the international – a dynamic illustrated with the production process of new systems of national accounting. It was also fostered by the completion, during the 1990s, of the africanisation of the training scheme of statisticians and economists set up at the turn of the Independence era. The regional schools of economics and statistics of Abidjan and Dakar are now positioned as cores for the training of the high-level officers of Francophone Western African economic administrations. Finally, the statisticians and economists of the Central Bank of West African states constitute a high administrative elite which puts its expertise at the service of disinflationary policies, while capitalizing on its influence within national and regional political fields. This research note builds on a biographical study of this group of individuals as an entry-point to assess the impact of regional integration around orthodox macroeconomic principles on the constitution of a regional corps of statisticians and economists.; Giro neo-liberal y consolidación de la burocracia transnacional -- Esta nota de investigación se centra en el desarrollo de un cuerpo de estadísticos y economistas en África occidental de habla francesa a partir de la década de 1990. La devaluación del Franco de la comunidad financiera africana (Franco CFA) en enero de 1994 fue seguida por una política de integración con el objetivo prioritario de defender la estabilidad macroeconómica y cumplir criterios estrictos inspirados en políticas europeas de rigor presupuestario. La puesta en marcha de Afristat, que comenzó a funcionar en 1996, contribuye a la vigilancia multilateral, mediante el despliegue de herramientas estadísticas armonizadas. Este organismo permite que Francia reduzca su asistencia bilateral a las administraciones económicas ; pero, además, abre espacios de sociabilidad para los estadísticos de África Occidental, al permitirles desarrollar estrategias de carrera transversas, entre lo nacional y lo transnacional – lo cual queda ilustrado por los diferentes proyectos que impulsan nuevas contabilidades nacionales. La década de 1990 también vio el final de la africanización del sistema de capacitación para estadísticos y economistas establecido después de la independencia. Las escuelas regionales de economía y estadística en Abidjan (Costa de Marfil) y Dakar (Senegal) son hoy pilares en la formación de altos ejecutivos de las administraciones económicas francófonas en África occidental. Finalmente, los estadísticos y economistas del Banco Central de los Estados de África Occidental constituyen una élite administrativa particularmente elevada que utiliza su conocimiento al servicio de las políticas de desinflación, mientras utiliza su influencia para posicionarse en los campos políticos nacionales y regionales. La mise en place d’Afristat, qui démarre ses activités en 1996, contribue à rendre possible la surveillance multilatérale en déployant des outils statistiques harmonisés. Si elle permet à la France de diminuer son assistance bilatérale aux administrations économiques, elle ouvre aussi des lieux de sociabilité aux statisticiens ouest-africains, en leur permettant de développer des stratégies de carrière à la jonction entre le national et le transnational – ce qui est illustré à partir du chantier d’élaboration de nouvelles comptabilités nationales. Congenital diaphragmatic hernia is a common cause of neonatal respiratory distress. The right or left diaphragmatic cupola hernia is most often involved. Rarely, a retro-costo-xiphoidian or Larrey or Morgagni hernia is diagnosed as part of respiratory distress. We present a case of congenital retro-costo-xiphoid diaphragmatic hernia in a midline situation, involving the sternal portion and extending to the diaphragmatic domes on each side. The discovery was made antenatal by obstetric ultrasound from 21 weeks. At birth, there was minor respiratory distress. The management consisted of a reduction and placement of a polypropylene plate in the natal post by abdominal approach.

This book, composed of articles first published in the journal Politique africaine, proposes an original interpretation of neoliberalism in Africa. Instead of seeing neoliberal reforms as intrinsically destructive of the post-colonial state, the authors, who include some of Africa’s best-known social scientists, focus on the resilience and adaptability of African state structures, economic systems, and social survival mechanisms and examine the diversity of responses to neoliberalism in what the editors call the “everyday politics of the state.” In essays that range from diverse theoretical or historical discussions to close studies of the dynamics of specific reforms in particular places, they argue against univocal interpretations of the effects of neoliberalism. They show that the African state, far from disappearing, is adapting and reconfiguring itself in fascinating new social realities “co-constructed” by state action as well as by the improvisations of communities and other private actors. Elle situe son approche à l’intersection de l’anthropologie, de la sociologie de la quantification et de la sociologie historique du politique.Dans les trois cas étudiés, l’observation fine du calcul macroéconomique montre que les ethos techniciens sont déterminants pour saisir l’exercice du pouvoir. Les luttes sociales et politiques autour de la vie chère ou de l’éducation, ou encore les débats sur les faux semblants du réformisme mettent les chiffres au centre des rapports sociaux et politiques. Sur les scènes politiques en ébullition de la Mauritanie, de la Guadeloupe et du Burkina Faso, les objets de la gestion économique opèrent, guident les actions des individus, suscitent la contestation. La macroéconomie n’est pas seulement dans les bureaux, elle constitue un répertoire populaire du politique. Elle est ancrée dans l’histoire propre de ces sociétés et dans les logiques autonomes de la technique et des procédures. It is rooted both in the specific history of these societies and in the autonomous logics of technique and procedure.

This thesis examines the exercise of power and its transformations, based on the observation of concrete operations of macroeconomic management in two African countries, Burkina Faso and Mauritania, and one Caribbean territory, Guadeloupe. Its approach involves a combination of anthropology, the sociology of quantification and the historical sociology of the political.The exercise of power rests on practices of economic management that are caught up in a network of relations. There are those who dangle the prospect of steering the economy in accordance with an instrumental logic; there are those turn formal procedures into tools for legitimizing current regimes; and there are those who turn macroeconomic management into the site of a struggle for access to resources. Macroeconomics has many meanings and lies at the centre of a wide-ranging ‘technocratic compromise’. In all three cases studied, the detailed observation of macroeconomic calculation shows that the technocratic ethos is crucial for an understanding of the exercise of power. Social and political struggles over education and the high cost of living, and debates on the illusory claims of reformism, place the numerical approach at the very heart of social and political relations. La prise en considération des principales statistiques démographiques, économiques et financières disponibles confirme que l’Afrique n’est décidément pas le continent de l’« immobilité » ou de la « tradition », n’en déplaise à Hegel (et à Nicolas Sarkozy). Elle est en pleine transformation sociale. Bien que la projection de ces données sous forme de prévision soit toujours sujette à caution, elle permet de s’interroger sur les formes d’organisation possibles de la région dans le domaine politique, à un horizon d’une cinquantaine d’années (...).
