Abstract

This article discusses a case of widespread sebaceous hyperplasia on the face of a kidney transplant recipient after starting everolimus treatment. Sebaceous hyperplasia is a benign neoplasm of sebaceous glands that presents as yellow-colored papules on the face. The patient developed these lesions one month after starting everolimus and they gradually increased in size and number. While sebaceous hyperplasia is usually asymptomatic and does not require treatment, it has been associated with certain medications used in organ transplant patients. This case adds to the literature on the association between medications and sebaceous hyperplasia in kidney transplant recipients. [Extracted from the article]

Fingolimod has been used for about ten years to treat multiple recurrent sclerosis. It has been reported that Fingolimod causes an elevation in liver enzymes. In this case report, the clinical and laboratory parameters improved after discontinuation of the drug. However, there is no publication in the literature regarding acute liver failure and liver transplantation following Fingolimod treatment. Abstract

Aim: The most common causes of blunt trauma are traffic accidents and falls from height. Thoracic region complications may be seen in cardiopulmonary resuscitation (CPR). The aim of this study was to examine thoracic complications developing in cases applied with CPR following a fall from height with no direct chest trauma. Materials and Methods: The thoracic complications of CPR were examined in cases with CPR applied after a fall from height following the exclusion of cases where death occurred and an autopsy was performed. Results: Evaluation was made of 109 cases who met the study criteria, comprising 82 males and 27 females. Fractures of the sternum and costa were seen at a higher rate than reported in studies in literature. No statistically significant difference was determined between the genders in respect of fracture rates. As age increased, there was found to be a statistically significant increase in fractures. Conclusion: This is the first study to have examined CPR complications following a fall from height. Just as there are direct effects of trauma on the body in general, there are also known to be some indirect effects. The results of this study showed an in increase in thoracic region complications developing after CPR applied to cases who developed cardiac arrest following a fall from height. 26. Does the use of cannabinoids affect the ocular surface?

Abstract

Aim: To examine tear functions and ocular surface variables in patients with cannabis addiction. Materials and Methods: This clinical trial was planned as a prospective case-control study. In this study, 51 male patients with cannabis addiction (group 1) and 51 healthy volunteers (group 2) with similar demographic characteristics were included. Visual acuity, spherical equivalent, biomicroscopic examination, Schirmer test, tear break-up time (TBUT), and impression cytology (Nelson scores) results were recorded, and the 2 groups were compared with each other. Results: The mean Schirmer values were 9.68 ± 6.8 mm and 8.39 ± 4.08 mm in group 1 and group 2 (p = 0.97), respectively, whereas the mean TBUT values were 9.96±3.9 and 9.29±4.01s. (p = 0.35) and the mean Nelson scores were 1.49±1 and 1.25±0.97 in group 1 and group 2, respectively (p = 0.26). Conclusion: In this study, the effects of cannabis addiction on the ocular surface were evaluated using Schirmer, TBUT, and impression cytology methods. In the statistical analysis, no significant difference was found compared to the control group. This study showed that tear production, tear film stability, and impression cytology of patients with cannabis addiction were not different from the control group. [ABSTRACT FROM AUTHOR]

31. Histopathological Features of the Steatohepatitic Variant of Hepatocellular Carcinoma and Its Relationship with Fatty Liver Disease

Abstract

Hepatocellular carcinoma (HCC) is the most common primary malignant tumor of the liver in adults. Steatohepatitic HCC (SH-HCC) is a recently described, rarer variant of HCC and is associated with nonalcoholic fatty liver disease (NAFLD). The relationship between fatty liver disease and/or steatohepatitis and SH-HCC is now known. This subtype can be confused with lipid-containing nodules (such as cirrhotic nodules, regenerative nodules, focal nodular hyperplasia) clinically, radiologically and histopathologically. Here, the histopathological features of SH-HCC, its relationship with fatty liver disease and briefly its clinical features will be discussed. In addition, histopathological features of this specific variant, immunohistochemical staining of the tumor and diagnostic difficulties in tru-cut biopsies will also be discussed. Actually, I think this article will raise clinicopathological awareness about this rare variant.

Cardiopulmonary resuscitation (CPR) is an emergency procedure for maintaining blood circulation and oxygenation during cardiac arrest. In this study, we aimed to evaluate the association of CPR with IPFE in autopsy cases.Methods: A total 402 cases among 4,118 autopsies were diagnosed with IPFE, and the medical background of these cases was retrospectively evaluated. Diagnosis of FE and FE grading were performed with histopathological examinations of postmortem tissue samples, and injury-severity scores of traumatic cases were assessed. Data of traumatic and atraumatic cases were statistically compared.Results: Of the IPFE cases, 298 (741%) were male and 104 (25.9%) female, with overall mean age 53.7 (2–99) years. Causes of death of studied subjects were traumatic for 302 (75.1%) and atraumatic reasons for 100 (24.9%) cases. CPR was performed for 277 cases of which 177 (63.9%) were traumatic and 100 (36.1%) were non-traumatic. . This study aimed to investigate the potential protective effects of Mol in Mtx-induced liver toxicity in rats.Materials and methods: Forty Wistar albino rats were equally divided into five groups: control, Mol, Mtx, Mol–Mtx, and Mtx–Mol. Following treatment, the animals were sacrificed, and liver tissue samples were histopathologically evaluated using Roening grading and Bcl-2 antibody staining. Tissue oxidants, antioxidants, and serum transaminases were measured and statistically compared across all groups.Results: No hepatic fibrosis or steatosis was observed in any of the groups. In the Mtx group, grade 2 liver injury and score 2 Bcl-2 antibody staining were observed; however, in the Mol–Mtx group, these were lower (grade 1, score 1). There were no statistically significant differences in serum transaminase levels among groups. Malondialdehyde levels were higher in all rats that received Mtx, but no differences in myeloperoxidase levels were observed among the groups. Levels of tissue antioxidants, including superoxide dismutase, glutathione (GSH) peroxidase (GSH-Px), and reduced GSH, were significantly higher in the Mol-treated and Mol pre-treated groups. Avrupa’da ‘Spice’, ABD’de ‘K2’, Türkiye’de ‘Bonzai’ ya da ‘Jamaika’ olarak adlandırılan sentetik kannabinoid (SK)’lerin içerikleri sabit olmadığı gibi; ülkeden ülkeye değişiklik gösteren yaklaşık 400 kadar kimyasal bileşik içerdiği bilinmektedir. SK’lar vücut hücrelerinde kannabinoid-1 (CB1) ve kannabinoid2 (CB2) reseptörleri ile etkileşerek Hint keneviri ya da esrarın içindeki majör etken madde olan Δ9-tetrahidrokannabinol (THC)'ün etkisini taklit ederler. Her ne kadar yeni SK’lar sentezlenip, yeni türevlerin sayıları gün geçtikçe artsa da; bu türevler arasında JWH-018, hem CB1 ve CB2 reseptörlerine tam agonist hem de 2 saat gibi kısa etki süreli olması nedeniyle en sık kullanılan SK’dır. SK’ların farmakokinetik ve farmakodinamik etkileri hakkında literatürde oldukça az veri bulunmaktadır ve mevcut verilerin çoğu da in vitro deneylere dayanmaktadır. Dünya Sağlık Örgütü (WHO)'nün 16-20 Haziran 2014 tarihinde Cenevre'de gerçekleştirdiği Uyuşturucu Bağımlılığı Uzman Komitesinin 36. Toplantısında sunulan JWH-018 raporunda, SK’lara bağlı intoksikasyonda belirgin kalp hızı artışının oldukça sık görüldüğü belirtilmesine rağmen; kardiyovasküler, solunum, gastrointestinal, genitoüriner sistem, karaciğer ve böbrek dokuları üzerine olan etkileri hakkında yeterli veriye ulaşılmadığı belirtilmiştir.Bu derlemedeki amaç, SK’lar için prototip olarak kabul edilen JW-018’in kardiyovasküler sistem üzerindeki etkilerini ortaya koyarak yapılacak yeni çalışmalara zemin hazırlamaktır, The content of synthetic cannabinoids (SC) which are known in Europe as ‘Spice’, in the U.S.A. as K2’, and in Turkey as ‘Bonsai’ or ‘Jamaica’ is not standardized. Also SC are known to contain approximately 400 chemical compounds which vary from country to country. SC mimics the effect of Δ9tetrahydrocannabinol (THC) which is the major effective content of Cannabis or Marijuana by cannabinoid-1 (CB1) and cannabinoid-2 (CB2) receptors in the body cell. Although the new SCs are synthesized and the number of new derivatives is increased in every day; JWH-018 among these derivatives exerts full agonist effect on both CB1 and CB2 receptors also it has a short-term effect as 2 hours. For this reason, JWH-018 is the most commonly used as SC. There are a few data in the literature regarding to the pharmacokinetic and pharmacodynamic effects of SC. Furthermore, most of the existing data are based on in vitro experiments. Abstract

INTRODUCTION: There is no consensus on duration of the nasal splint after nasal septum surgeries. The pressure of nasal splint on the mucosa may cause tissue necrosis and nasal septum perforation.OBJECTIVES: To investigate the histopathological changes of the nasal mucosa caused by nasal splints in a rabbit model.METHODS: No splint was used in group A. Bilateral silicone nasal splints were placed for five, ten, and 15 days in groups B, C, and D, respectively. Biopsy of the nasal mucosa was performed after removal of splint. Histopathologic evaluations were performed. The severity and depth of the inflammation were scored.RESULTS: Group A had a normal histological appearance. Comparison of the results of groups B, C, and D with group A demonstrated statistically significant differences with regards to the severity of histopathological findings. There was no statistically significant difference between groups B and C. There were statistically significant differences between the groups B and D, and also between groups C and D.CONCLUSIONS: Longer duration of nasal splint had a higher risk for septal perforation. Therefore, removal of the splint as soon as possible may be helpful for preventing potential perforations. INTRODUÇÃO: Não existe consenso acerca do tempo de permanência de splintsnasais no pós-operatório de cirurgias no septo. A pressão causada pelos mesmos na mucosa nasal pode causar necrose e perfurações septais.OBJETIVOS: Investigar mudanças histopatológicas da mucosa nasal causadas por splints nasais em coelhos.MÉTODO: Nenhum splint foi utilizado no grupo A. Splintsde silicone foram utilizados por 5, 10 e 15 dias nos grupos B, C e D, respectivamente. Biópsia da mucosa nasal foi realizada após a remoção dos mesmos. Avaliações histopatológicas foram realizadas, e a gravidade e a profundidade do processo inflamatório foram medidas.RESULTADOS: Grupo A apresentou uma aparência histológica normal. Comparações de resultados entre os grupos B, C e D com o grupo A demonstraram diferenças estatísticas relevantes na gravidade histopatológica. Não houve diferenças estatísticas relevantes entre os grupos B e D, assim como entre os grupos C e D.CONCLUSÃO: De acordo com os resultados, quanto maior a duração no uso de splints nasais maior o risco de perfuração septal. Portanto, a remoção de splints nasais deve ser realizada assim que possível, prevenindo potenciais perfurações.

Abstract

Background: Breast cancer is the most prevalent cancer among females, and metastatic disease is not curable and is treated palliatively. Members of the ErbB family have an important role in the development and progression of breast cancer. The aim of this study was to determine the relationship between epidermal growth factor receptor (EGFR) gene mutation, human epidermal growth factor 2 (HER2) expression, hormone receptor statuses, and clinicopathological parameters in liver metastases from breast cancer. Materials and Methods: This study included 41 patients diagnosed with liver metastasis from breast carcinoma, based on morphological and immunohistochemical findings, in our pathology laboratory between 2011 and 2018. EGFR gene mutations were analyzed by polymerase chain reaction (PCR) in these cases. Results: EGFR gene mutation analysis was performed by PCR, and no mutations were detected. HER2 and ER statuses of the primary breast tumor were available in 23 cases. HER2 status conversions were present in 9 cases (39.1%); however, this was not statistically significant (P = 0.197). Estrogen receptor (ER) conversions were present in 4 cases (17.4%); however, this was not statistically significant (P = 1.000). Progesterone receptor (PR) conversions were detected in 10 cases (45.5%). There were 10 (45.5%) cases with PR-positive primary tumors and PR-negative liver metastases. No cases with a PR-negative primary tumor developed a PR-positive liver metastasis (P = 0.02). Conclusions: No EGFR gene mutations were detected in any of our cases by PCR. There was no statistically significant relationship between clinicopathological parameters and EGFR mutation. The comparison of ER, PR, and HER2 expression between the primary tumor and metastases revealed status conversions in some cases. However, only PR conversion was statistically significant. Studies on EGFR gene mutations that include larger series are warranted to identify the candidates who can benefit from targeted therapies. [ABSTRACT FROM AUTHOR]

50. Protective effects of molsidomine against doxorubicin-induced renal damage in rats

Abstract

Purpose: The purpose of this study was to investigate the therapeutic and protective effects of molsidomine (MLS) against doxorubicin (DOX)-induced renal damage in rats. Methods: Forty rats were randomly divided into five groups (control, MLS, DOX, DOX+MLS and MLS+DOX groups). Thiobarbituric acid reactive substance (TBARS), reduced glutathione (GSH), superoxide dismutase (SOD), catalase (CAT), nitric oxide (NO) and glutathione peroxidase (GPx) levels were determined from kidney tissues and blood urea nitrogen (BUN), creatinine (Cr) and albumin (Alb) levels also determined. Results: DOX treatment caused a significant increase in TBARS levels and a significant decrease in the GSH and CAT levels compared with the control group. In comparison, MLS administration before DOX injection caused a significant decrease in TBARS levels and also increases in GSH and CAT levels, whereas treatment of MLS after DOX injection did not show any beneficial effect on these parameters. All groups showed a significant increase in NO levels compared to the control group. There were no significant differences among the all groups for BUN and Cr levels. Serum level of Alb decreased in the DOX-treated groups when compared with control and MLS groups. The histopathological findings were in accordance with the biochemical results. MLS treatment reversed the DOX-induced kidney damage in group 4. MLS treatment before DOX injection exerted a protective effect against DOX-induced kidney damage. Conclusions: MLS shows promise as a possible therapeutic intervention for the prevention of kidney injury associated with DOX treatment. Additional studies are warranted.
