Abstract

Objective & Approach Routine optometry examinations, which may include retinal images, have been free to everyone in Scotland since 2006. Images and patient data are stored locally on non-interoperable systems, usually without unique identifiers. The Scottish Collaborative Optometry-Ophthalmology Network for e-research (SCONe) aimed to create a secure, longitudinal repository collating community-acquired images linked to national, routinely-collected healthcare data. We developed bulk export processes to retrieve images and patient data from current and archived systems and a series of deterministic and probabilistic algorithms to match images to patients. Results We successfully exported images and patient data from several commonly used systems. Exact matching of available data between image and clinical systems within practices linked 78% of images (IQR 71.2-84.1). Our algorithms increased match rate to 90% (IQR 83.5-98.6) of images, efficiently reducing risk of systemic bias in the cohort. Match method is included in the repository metadata to facilitate quality assurance checks after national linkage and pseudonymisation. To date over 600k images have been processed and delivered to the National Safe Haven for linkage to healthcare data. Conclusions & Implications Breakthrough research into early detection and pre-symptomatic disease can be facilitated by harnessing community-acquired retinal images which capture normal ageing and disease progression. Rather than await interoperability and widespread use of unique identifiers, this has been achieved through innovative solutions to real-world issues around time-pressured clinical data entry and non-standardised data storage. The SCONe repository continues to grow. It is currently supporting ocular disease prediction research and offers the potential for much more.

Abstract

Purpose Comparatively, men have poorer physical and mental health outcomes than women, with a significantly higher suicide rate. Contributory factors are thought to be social and biological, leading to reduced access to health-care services. The study aims to develop and implement community-based support to increase awareness of and access to men’s mental health support networks and groups. Design/methodology/approach The project involved three key work-packages discussed in this paper: raising awareness of men’s mental health needs in health care, educational and community settings; collaboration between National Health Services (NHS) and non-NHS health-care support organisations to build multi-sector partnership working; and developing a supported sports-based community intervention aimed at men living with mental health conditions. The acceptability and feasibility of these work-packages were pragmatically evaluated through mixed-methods surveys and qualitative content analysis. Findings Overall, both community events and sports groups successfully engaged men living with mental health problems. Organisations interested in men’s mental health are continuing to engage in a partnership initiative. Community events were well-attended and received positive feedback, particularly regarding the educative and real-life experiences approach promoted in the events. The sports intervention is feasible and well-accepted by participants, who described feeling supported with their physical and mental health needs, with increased mental well-being reported. Research limitations/implications The main limitations of this project are that the authors only evaluated a football group rather than all work areas. The project collected outcomes relating to participants’ demographics and qualitative reflections of participating in the football group along with a retrospective survey of perceived benefits, but the project did not undertake a pre- and post-comparison of well-being outcomes owing to low completion of these measures. Future work could focus on collecting more pre- and post-measures related to well-being, recovery and inclusion and compare these with men not involved in the football groups or public events. Practical implications This paper discusses the development and feasibility of setting up community-based men’s mental health support networks, involving public events, partnership working and targeted-sports interventions. All initiatives were well-received and successfully attended by men living with mental health conditions. Evaluation of the programme revealed the value placed on education about mental health and the role that community sports interventions may play in men’s mental health care. Social implications This project has demonstrated three different ways of supporting men’s mental health needs in the community. Community public events were held to raise awareness of men’s mental health needs and issues were well-attended and highlighted the need for health promotion and education in this area across all the communities. The men’s football group demonstrated the feasibility of moving mental health support out into a non-clinical and more community arena in a way that men engaged effectively. Finally, the creation of MensNet has bought together disparate multi-sector organisations successfully to lead public health mechanisms to support men’s mental health needs. Originality/value This paper describes a new multi-disciplined approach to supporting health-seeking challenges among men, in particular, how partnership working across NHS and non-NHS sectors can successfully support an identified public health need pragmatically using existing services and organisations.

This work quantifies water contamination in jet fuel (Jet A-1), using silica-based Bragg gratings. The optical sensor geometry exposes the evanescent optical field of a guided mode to enable refractometery. Quantitative analysis is made in addition to the observation of spectral features consistent with the emulsification of water droplets and Stokes’ settling. Abstract

Increased demand for renewable energy has led to growth in the use of land for electricity generation and associated infrastructure. Land-based wind farms are amongst the commonest generators of renewable energy. To date, most research on the effects of wind farms on wildlife have focussed on birds and bats, with very little work on terrestrial taxa. We hypothesised that widely reported negative effects of wind farms on predatory birds might benefit potential prey species. We focussed on reptiles due to concerns over worldwide declines in this group. We compared avian attack rates on clay model snakes at a site in Caithness UK within a wind farm relative to a control site of the same topography and habitat class, 1 km away, using life-sized clay models of adder Vipera berus, a widespread but declining Palaearctic species. Attack rates at the control site were comparable with similar studies elsewhere in Europe. However, we found that attack rates were lower within wind farm arrays, although several species of bird known to prey on reptiles were observed both within the wind farm and the control site. Therefore, given the high rate of loss to avian predators experienced in reptile reintroduction and reinforcement projects, wind farm sites may offer safe-havens, representing a neglected opportunity in reptile conservation. Grazing by sheep severely offset this benefit, presumably through removal of plant cover which was apparent in those areas of the wind farm where sheep were allowed access; grazing must thus be managed carefully for these benefits to be realised.

Abstract

Background: PROACTIVE is a psycho-educational support intervention for prostate cancer patients managed on Active Surveillance. PROACTIVE is comprised of two interdependent components: group workshops and internet delivered information modules. \ud \ud Aims: We conducted a feasibility study to determine the practicality of delivering PROACTIVE at two prostate cancer centres.\ud \ud Methods: The feasibility study was a mixed methods randomized parallel-group exploratory trial. Participants were randomised using a ratio of 3:1 PROACTIVE group to treatment as usual. Qualitative semi-structured interviews and quantitative measures were completed at baseline, intervention completion (week 6), and at 6-months follow-up. Interview transcripts were analysed thematically using Framework analysis. Descriptive statistics were used to examine recruitment and retention rates, and changing trends in outcome measures. \ud \ud Results: Most aspects of the research design and PROACTIVE intervention were acceptable to those participating in the study. In particular participants valued the opportunity to share and discuss experiences with other prostate cancer patients on Active Surveillance, and receive detailed authoritative information. However, three issues were identified: \ud 1. a low response rate (13 participants recruited, response rate 16%)\ud 2. low utilisation of internet delivered information modules\ud 3. self-perceived low levels of anxiety amongst participants with the majority perceiving their cancer as not impacting on their day-to-day life or causing anxiety. \ud \ud Conclusions: Due to these significant research design issues it is not recommended PROACTIVE be evaluated in a large scale randomised controlled trial. Further research is required to explore the impact of Active Surveillance on anxiety amongst men with localized prostate cancer managed by Active Surveillance.

This paper considers the role of Islamic scholars as non-statutory providers of support for individuals with issues of psychological distress and wellbeing. A qualitative phenomenological analysis of the experiences of six Imams (Islamic religious officials) who saw this type of support as being part of their role is presented. Four themes were identified: “Building a personal relationship with the ultimate authority”; “Treating the whole person”; “God's medicine” and “Duty bound to help”. Clinical implications for the current and potential relationship between this type of support and mental health and psychological services are presented and discussed.

Abstract

Obesity is set to become one of the biggest health problems facing governments in Europe. With a significant sequela of associated chronic conditions, treatment and prevention must become priorities in healthcare provision and policy. The cause and treatment of obesity is complex. However, evidence supports the patient-centred approach, which was the focus of discussion at a multidisciplinary educational event for healthcare students and patients held at the University of Southampton. The aim was to increase existing knowledge, promote shared learning and facilitate a better understanding of strategies needed to address the growing prevalence of obesity. The necessity for improved research strategies and a more integrated, multidisciplinary approach to treatment were echoed by the panel of expert researchers and practitioners who spoke. There was also a consensus that obesity needs to be seen as a chronic health concern by governments and health practitioners. In addition, the event provided a locus for inter-professional student groups to work collaboratively with obese (or formerly obese) patients to explore the patients’ perspective regarding obstacles to health improvement and possible solutions. Feedback indicated that both students and patients found this experience valuable and rewarding. Some patients were motivated to join other community health initiatives as a result, whilst students indicated that their understanding, future practice and research would be enhanced. Therefore it seems that this experiential approach to collaborative learning may provide a model for further educational events with a focus on patient-centred treatment of similar long term chronic health problems.

12. Depression and anxiety in ovarian cancer: a systematic review and meta-analysis of prevalence rates

Abstract

Objectives To systematically review the literature pertaining to the prevalence of depression and anxiety in patients with ovarian cancer as a function of treatment stage. Design Systematic review and meta-analysis. Participants 3623 patients with ovarian cancer from primary research investigations. Primary outcome measure The prevalence of depression and anxiety in patients with ovarian cancer as a function of treatment stage. Results We identified 24 full journal articles that met the inclusion criteria for entry into the meta-analysis resulting in a pooled sample size of 3623 patients. The meta-analysis of prevalence rates identified pretreatment, on-treatment and post-treatment depression prevalences of 25.34% (CI 22.79% to 28.07%), 22.99% (CI 19.85% to 26.46%) and 12.71% (CI 10.14% to 15.79%), respectively. Pretreatment, on-treatment and post-treatment anxiety prevalences were 19.12% (CI 17.11% to 21.30%), 26.23% (CI 22.30% to 30.56%) and 27.09% (CI 23.10% to 31.49%). Conclusions Our findings suggest that the prevalence of depression and anxiety in women with ovarian cancer, across the treatment spectrum, is significantly greater than in the healthy female population. With the growing emphasis on improving the management of survivorship and quality of life, we conclude that further research is warranted to ensure psychological distress in ovarian cancer is not underdiagnosed and undertreated.

Abstract

Objective: To quantitatively determine the prevalence of anxiety and depression in men on active surveillance (AS).Design: Cross-sectional questionnaire survey.Setting: Secondary care prostate cancer (PCa) clinics across South, Central and Western England.Participants: 313 men from a total sample of 426 with a histological diagnosis of PCa currently managed with AS were identified from seven UK urology departments. The mean age of respondents was 70 (51–86) years with the majority (76%) being married or in civil partnerships. 94% of responders were of white British ethnicity.Primary outcome measures: The prevalence of clinically meaningful depression and anxiety as assessed by the Hospital Anxiety and Depression Scale (HADS; score ?8/21).Secondary outcome measures: Patient demographic data (age, employment, relationship, ethnic and educational status). Each demographic variable was cross-tabulated against patients identified as depressed or anxious to allow for the identification of variables that were significantly associated with depression and anxiety. In order to determine predictors for depression and anxiety among the demographic variables, logistic regression analyses were conducted, with pResults: The prevalence of clinical anxiety and depression as determined via the HADS (HADS ?8) was 23% (n=73) and 12.5% (n=39), respectively. Published data from men in the general population of similar age has shown prevalence rates of 8% and 6%, respectively, indicating a twofold increase in depression and a threefold increase in anxiety among AS patients. Our findings also suggest that AS patients experience substantially greater levels of anxiety than patients with PCa treated radically. The only demographic predictor for anxiety or depression was divorce.Conclusions: Patients with PCa managed with AS experienced substantially higher rates of anxiety and depression than that expected in the general population. Strategies to address this are needed to improve the management of this population and their quality of life.

Pretreatment, on-treatment and post-treatment anxiety prevalences were 27.04% (95% CI 24.26% to 30.01%), 15.09% (95% CI 12.15% to 18.60%) and 18.49% (95% CI 13.81% to 24.31%), respectively.ConclusionsOur findings suggest that the prevalence of depression and anxiety in men with prostate cancer, across the treatment spectrum, is relatively high. In light of the growing emphasis placed on cancer survivorship, we consider that further research within this area is warranted to ensure that psychological distress in patients with prostate cancer is not underdiagnosed and undertreated.

The long term photosensitivity locking obtained should enable the development of all UV-written devices limited to date by the rapid deuterium outdiffusion during fabrication.
