Abstract

In the past decades, Chimeric Antigen Receptor (CAR)-T cell therapy has achieved remarkable success, leading to the approval of six therapeutic products for haematological malignancies. Recently, the therapeutic potential of this therapy has also been demonstrated in non-tumoral diseases. Currently, the manufacturing process to produce clinical-grade CAR-T cells is complex, time-consuming, and highly expensive. It involves multiple steps, including the collection of T cells from patients or healthy donors, in vitro engineering and expansion, and finally reinfusion into patients. Therefore, despite the impressive clinical outcomes, ex vivo manufacturing process makes CAR-T cells out of reach for many cancer patients. Direct in vivo engineering of T cells could be a more rapid solution able to circumvent both the complexity and the costs associated with ex vivo manufactured CAR-T cells. This novel approach allows to completely eliminate ex vivo cell manipulation and expansion while producing therapeutic cell populations directly in vivo. To date, several studies have demonstrated the feasibility of in vivo T cell reprogramming, by employing injectable viral- or nanocarrier-based delivery platforms in tumour animal models. Additionally, in vivo production of CAR-T cells might reduce the incidence, or at least the severity, of systemic toxicities frequently occurring with ex vivo produced CAR-T cells, such as cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome. In this review, we highlight the challenges associated with the current ex vivo manufacturing protocols and review the latest progresses in the emerging field of in vivo CAR-T therapy, by comparing the various platforms so far investigated. Moreover, we offer an overview of the advantages deriving from in vivo reprogramming of other immune cell types, such as Natural Killer and macrophages, with CAR constructs. [ABSTRACT FROM AUTHOR]

After 4 weeks of treatment with a daily oral dose of the new food supplement BL, AM or placebo, plus diet, the main outcome was the decrease of LDL-C, total cholesterol (TC), and triglyceride levels.Findings: The absolute changes of LDL-C and TC levels from baseline, at week 4 were: −39.1 mg/dL ±17.76 and −45.9 mg/dL ±21.54, respectively in the BL group; 5.7 mg/dL ±14.98 and 2.4 mg/dL ±18.43, respectively in the placebo group. Results were statistically significant. In terms of mean percentage, BL was shown to be more effective in lowering LDL-C levels as compared to placebo and the active comparator (AM), with a reduction of −26.3%, +4.2%, −18.3%, respectively. Five adverse events (AEs) were reported by five patients after the initiation of the study treatment: two in the BL group (influence and insomnia), two in the AM group (ear pain and rash), and one in the placebo group (back pain). All AEs were mild in intensity, except for back pain (severe). The case of insomnia in the BL group and the case of rash in the AM group were judged as treatment related. The safety review of the laboratory (blood and urine) analyses, vital signs and physical findings did not show any clinical effect of the study products on any of the parameters.Implications: BL showed a good efficacy and safety profile and, for this reason, it can be considered an alternative to pharmacological treatment, for patients with mild-to-moderate hypercholesterolemia. Abstract

The COVID-19 pandemic and the need for additional safe, effective, and affordable vaccines gave new impetus into development of vaccine genetic platforms. Here we report the findings from the phase 1, first-in-human, dose-escalation study of COVID-eVax, a DNA vaccine encoding the receptor binding domain (RBD) of the SARS-CoV-2 spike protein. Sixty-eight healthy adults received two doses of 0.5, 1, or 2 mg 28 days apart, or a single 2-mg dose, via intramuscular injection followed by electroporation, and they were monitored for 6 months. All participants completed the primary safety and immunogenicity assessments after 8 weeks. COVID-eVax was well tolerated, with mainly mild to moderate solicited adverse events (tenderness, pain, bruising, headache, and malaise/fatigue), less frequent after the second dose, and it induced an immune response (binding antibodies and/or T cells) at all prime-boost doses tested in up to 90% of the volunteers at the highest dose. However, the vaccine did not induce neutralizing antibodies, while particularly relevant was the T cell-mediated immunity, with a robust Th1 response. This T cell-skewed immunological response adds significant information to the DNA vaccine platform and should be assessed in further studies for its protective capacity and potential usefulness also in other therapeutic areas, such as oncology.

Among these diseases, heart failure (HF) and acute myocardial infarction (AMI) are the most common causes of hospitalization. Therefore, readmission for HF and AMI is receiving increasing attention. Several socioeconomic factors could affect readmissions in this target group, and thus, a systematic review was conducted to identify the effect of socioeconomic factors on the risk for readmission in people aged 65 years and older with HF or AMI.Methods: The search was carried out by querying an electronic database and hand searching. Studies with an association between the risk for readmission and at least one socioeconomic factor in patients aged 65 years or older who are affected by HF or AMI were included. A quality assessment was conducted independently by two reviewers. The agreement was quantified by Cohen’s Kappa statistic. The outcomes of studies were categorized in the short-term and the long-term, according to the follow-up period of readmission. A positive association was reported if an increase in the risk for readmission among disadvantaged patients was found. A cumulative effect of socioeconomic factors was computed by considering the association for each study and the number of available studies.Results: A total of eleven articles were included in the review. They were mainly published in the United States. All the articles analyzed patients who were hospitalized for HF, and four of them also analyzed patients with AMI. Seven studies (63.6%) were found for the short-term outcome, and four studies (36.4%) were found for the long-term outcome. For the short-term outcome, race/ethnicity and marital status showed a positive cumulative effect on the risk for readmission. Regarding the educational level of a patient, no effect was found.Conclusion: Among the socioeconomic factors, mainly race/ethnicity and marital status affect the risk for readmission in elderly people with HF or AMI. Multidisciplinary hospital-based quality initiatives, disease management, and care transition programs are a priority for health care systems to achieve better coordination. Abstract

Increased attention has been given to particulate matter (PM) that, as well as worsening air quality, is responsible for chronic and acute respiratory or cardiovascular diseases. Currently, most of the studies are focused on the capacity of plants and other biological media to adsorb PM, whereas few works explore the functional damage due to PM on urban vegetation. The present study, considering Quercus ilex L. as target species for its wide distribution in the Mediterranean urban and natural areas, pointed out that PM accumulation, inferred from magnetic and chemical properties, has almost no effects on structural morpho-functional traits as Relative Water Content or Specific Leaf Area but can impair processes related to the first photochemical reactions suggesting shading effect on leaves. PSI functionality and thus, carbon assimilation related processes, are impaired to a lesser extent by the oxidative potential of PM. Our results showed that, although several oxidative stressors can simultaneously affect morpho-functional traits, the interdisciplinary approach tested here can be a key tool to enlarge the spatial scale of biomonitoring activities as much as possible, and highlight a functional indicator of PM injury. This is needed to enhance the knowledge about the complex processes that are implied in the dynamic relationship between air quality, vegetation functionality and ecosystem services provisioning in urban areas.

Questo dossier tematico vuole offrire un breve resoconto dei risultati del workshop tenuto su piattaforma virtuale, con particolare attenzione per quanto riguarda future iniziative di collaborazione e sperimentazione nel campo dell’elaborazione ed estrazione di informazioni da edizioni semi-strutturate di documenti archivistici. Funzionalità avanzate di questo tipo sono infatti essenziali per ampliare la fruizione delle edizioni digitali di fonti archivistiche e di altri documenti scritti, rendendole strumenti di ricerca più flessibili ed aperti alla lettura da parte di algoritmi. Abstract

Across the 28 EU member states there were nearly half a million premature deaths in 2015 as a result of exposure to PM2.5, O3 and NO2. To set the target for air quality levels and avoid negative impacts for human and ecosystems health, the National Emission Ceilings Directive (NECD, 2016/2284/EU) sets objectives for emission reduction for SO2, NOx, NMVOCs, NH3 and PM2.5 for each Member State as percentages of reduction to be reached in 2020 and 2030 compared to the emission levels into 2005. One of the innovations of NECD is Article 9, that mentions the issue of “monitoring air pollution impacts” on ecosystems. We provide a clear picture of what is available in term of monitoring network for air pollution impacts on Italian ecosystems, summarizing what has been done to control air pollution and its effects on different ecosystems in Italy. We provide an overview of the impacts of air pollution on health of the Italian population and evaluate opportunities and implementation of Article 9 in the Italian context, as a case study beneficial for all Member States. The results showed that SO42− deposition strongly decreased in all monitoring sites in Italy over the period 1999–2017, while NO3− and NH4+ decreased more slightly. As a consequence, most of the acid-sensitive sites which underwent acidification in the 1980s partially recovered. The O3 concentration at forest sites showed a decreasing trend. Consequently, AOT40 (the metric identified to protect vegetation from ozone pollution) showed a decrease, even if values were still above the limit for forest protection (5000 ppb h−1), while PODy (flux-based metric under discussion as new European legislative standard for forest protection) showed an increase. National scale studies pointed out that PM10 and NO2 induced about 58,000 premature deaths (year 2005), due to cardiovascular and respiratory diseases. Abstract

Leaf dark respiration (RD) plays a key role in plant carbon balance and the knowledge about how environmental stresses affect the response of RD to temperature under field conditions, might improve the reliability of carbon budget models. This study analyzed the influence of air temperature and drought on the short-term temperature response of RD of three woody evergreen Mediterranean species. The field activities were carried out in two different coastal areas of Italy and for each site, two different areas located near the shoreline and inland were considered. The respiration–temperature response curves were carried out seasonally, together with leaf water potential and sclerophyll degree measurements. The effect of the average air temperature of the previous two days (T2m) and predawn water potentials on basal respiration measured at 18°C (R18) and temperature sensitivity (Q10), were tested with linear regression analysis. Both R18 and Q10 were weakly correlated with T2m, while a multifactor model including predawn water potentials yielded a better relation. These findings suggest that, besides air temperature, water availability and in general, plant energy balance, must be taken into account to predict acclimation of leaf dark respiration., Annali di Botanica, Vol 9 (2019)

Abstract

To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Abstract

The Division Models and Technologies for Risks Reduction of the ENEA Sustainability Department was established in July 2015 with the specific aim of assemble all the resources, both human and instrumental, that could propose reflections, analyzes and assessments about the interactions among energy strategies, emissions of pollutants and greenhouse gases, air quality, climate change and resulting impacts on health and ecosystems. The use of numerical models, at different spatial scales, to describe the physical and chemical processes affecting the atmosphere and the ocean is at the hearth of the history and activities of the research groups involved. Nevertheless, great effort and commitment was also dedicated to the development of integrated models to explore links and feedback between the different aspects of the same system. These unique platforms called IAM (Integrated Assessment Modelling) help and support policy-makers to develop and promote sustainable policies. With this vision we have been developing the national model MINNI (Integrated National Model in support to the International Negotiation on Air Pollution) since 2003. It includes the Integrated Assessment Model GAINS-Italia, which helped our Country: to negotiate revisions of European and International Directives and Protocols with technical data and national scenario simulations; to support technically the actions needed to deal with European infringement procedures; to help the Regional Authorities to fully understand and use the measurements until they have become totally autonomous; to support the early season of the Air Quality Regional Plans together with the assessment of the measures identified in them. A parallel and equally far-sighted vision has been offered by our colleagues of the actual Division who have been dealing for years with the “pollution effects”. Abstract

Quercus ilex L. seedlings were exposed in open-top chambers for one growing season to three levels of ozone (O3): charcoal filtered air, non-filtered air supplemented with +30% or +74% ambient air O3. Key functional parameters related to photosynthetic performance and stomatal density were measured to evaluate the response mechanisms of Q. ilex to chronic O3 exposure, clarifying how ecophysiological traits are modulated during the season in an ozone-enriched environment. Dark respiration showed an early response to O3 exposure, increasing approximately 45% relative to charcoal-filtered air in both O3 enriched treatments. However, at the end of the growing season, maximum rate of assimilation (Amax) and stomatal conductance (gs) showed a decline (-13% and -36%, for Amax and gs, respectively) only in plants under higher O3 levels. Photosystem I functionality supported the capacity of Q. ilex to cope with oxidative stress by adjusting the energy flow partitioning inside the photosystems. The response to O3 was also characterised by increased stomatal density in both O3 enriched treatments relative to controls. Our results suggest that in order to improve the reliability of metrics for O3 risk assessment, the seasonal changes in the response of gs and photosynthetic machinery to O3 stress should be considered.

Abstract

Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Abstract

Tropospheric ozone (O3) and Particulate Matter (PM) have become a major concern in most European cities. In particular, in Italy the O3 concentrations exceed the limits established for the protection of both human health and vegetation. More integrative studies are revealing that urban trees could concretely help in improving air quality, not only because of their well known aesthetic and recreational benefit, but also for their capability to reduce air temperature and to remove air pollutants. This reduction takes place both directly, by dry deposition to plant surfaces and uptake through stomata, and indirectly, by mitigation of the urban heat island intensity by canopy transpiration and building shading, that lowers the activity of chemical reactions that led to the formation of photochemical pollutants in air. This is of particular importance especially for those cities located in the Mediterranean Basin, whose urban vegetation is often characterized by VOC-emitting species that can contribute significantly to the O3 formation and destruction dynamics. The aim of this paper is to present a short review of ecological research performed on vegetation of the metropolitan area of Rome, at different spatial and temporal scale, in order to evaluate the functional role of urban green to monitor and improve urban air quality. In the frame of the project HEREPLUS (EU FP7), all of this information, opportunely integrated with climatic and pollutant data, will be implemented in a GIS and, by the use of geo-statistical methods, the ameliorating effect of urban vegetation will be quantified and mapped.

Abstract

The impact of headache on the person and society represents a public health issue. Recently a study evaluated 51% of headache's prevalence in Europe, of which 14% is affected by migraine. Besides, 4% of adult population is affected by chronic forms, which constitute therefore an even more relevant problem in terms of health and social policies. The International Classification of Headache Disorders, II version (ICHD-II) recognises 24 types of chronic headache and states primary episodic headaches as chronic when attacks appear for more than 15 days per month, for at least three months. Headache given by drugs overuse, defined by ICDH-II in 2004 (and revised in 2005) as Medication Overuse Headache (MOH), is associated with overuse of a combination of analgesics, barbiturates, opioids, ergot alkaloids, aspirin, AINS, caffeine and triptans. Patients affected by MOH present a reduced work performance and a significant alteration in the quality of life. Furthermore, some psychological and behavioural states seem particularly important in promoting and sustaining drug abuse. The management and rehabilitation of patients affected by CDH, abusing symptomatic drugs, consists in the withdrawal and/or gradual reduction of their assumption, because of tolerance and addiction possibilities.

Abstract

he responses to mild root zone salinity stress were investigated in two co-occurring Mediterranean woody evergreens, Quercus ilex L. and Arbutus unedo L., which differ in morpho-anatomical traits and strategies to cope with water deficit. The aim was to explore their strategies to allocate potentially toxic ions at organism level, and the consequential physiological and biochemical adjustments. Water and ionic relations, gas exchange and PSII performance, the concentration of photosynthetic pigments, and the activity of antioxidant defences, were measured. Q. ilex displayed a greater capacity to exclude Na + and Cl-from the leaf than A. unedo, in part as a consequence of greater reductions in transpiration rates. Salt-induced reductions in CO 2 assimilation resulted in Q. ilex suffering from excess of light to a greater extent than A. unedo. Consistently, in Q. ilex effective mechanisms of nonphotochemical quenching, also sustained by the lutein epoxide-lutein cycle, operated in response to salinity stress. Q. ilex also displayed a superior capacity to detoxify reactive oxygen species (ROS) than A. unedo. Our data suggest that the ability to exclude salt from actively growing shoot organs depends on the metabolic cost of sustaining leaf construction, i.e. species-specific leaf life-span, and the relative strategies to cope with salt-induced water stress. We discuss how contrasting abilities to restrict the entry and transport of salt in sensitive organs relates with species-specific salt tolerance.

Abstract

Two field exercises were carried out to compare chlorophyll a fluorescence measurements taken in the field by field teams working on the same project. In the first exercise (2007, Passo Pura, Ampezzo, Udine, Northern Italy) the operators took measurements on the same leaf areas (maintaining fixed leaf clips); in the second (2009, Monterotondo Marittimo, Grosseto, Central Italy) the teams worked independently, but addressing a common research question. The results of the first exercise showed that: (a) F(V)/F(M) was stable and had little variation among teams and instruments; (b) the results from the different teams correlated well; (c) the most suitable parameters of fast kinetics analysis are those measured on the normalized fluorescence transients. In the second exercise, when the teams worked independently, the results were much more variable and the correlations between measurements of different operators were weak. These results suggest that field chlorophyll a fluorescence measurements taken by different teams/operators can be comparable only if particular care is taken to the internal variability of the samples and a standardized sampling strategy is applied. A statistically sound representation of a population can be then reached.
