Abstract

We consider the energy functional on the space of sections of a sphere bundle over a Riemannian manifold (M, <,>) equipped with the Sasaki metric and we discuss the characterising condition for critical points. Likewise, we provide a useful method for computing the tension field in some particular situations. Such a method is shown to be adequate for many tensor fields defined on manifolds M equipped with a G-structure compatible with <,>. This leads to the construction of a lot of new examples of differential forms which are harmonic sections or determine a harmonic map from (M,<,>) into its sphere bundle., Comment: 22 pages

Abstract

In this article we apply a Bochner type formula to show that on a compact conformally flat riemannian manifold (or half-conformally flat in dimension 4) certain types of orthogonal almost-complex structures, if they exist, give the absolute minimum for the energy functional. We give a few examples when such minimizers exist, and in particular, we prove that the standard almost-complex structure on the round S^6 gives the absolute minimum for the energy. We also discuss the uniqueness of this minimum and the extension of these results to other orthogonal G-structures.

Abstract

Up till now, only one study providing practically complete information on acute pancreatitis in Italy has been published. The aim of this prospective study was to evaluate the clinical characteristics, in terms of diagnostic assessment and outcome, of a large series of patients affected by acute pancreatitis in Italy.The study involved 56 Italian centres, homogeneously distributed throughout the entire national territory. Each participating centre was furnished with an ad hoc software including 530 items along with subsequent collection, tabulation and quality control of the data.One thousand five hundred and forty case report forms of patients affected by acute pancreatitis were collected but 367 of them (24%) were subsequently eliminated from the final analysis. Therefore, 1173 patients (581 females and 592 males) were recruited. Mean age of patients was 62.0+/-18.2 years (95% confidence interval, 60.9-63.0). On the basis of Atlanta classification, 1006 patients (85.8%) were defined as mild and 167 (14.2%) as severe pancreatitis. Biliary forms represented the most frequent aetiological category (813 cases, 69.3%) while alcoholic forms only 6.6% (77 cases); the remaining aetiologies accounted for 7.1% (83 cases) while 200 cases (17.1%) remained without a definite aetiological factor. Complete recovery was achieved in 1016 patients (86.6%) whereas morphological sequelae were found in 121 patients (10.3%) and mortality in 36 patients (3.1%; 0.4% in mild and 19.2% in severe acute pancreatitis). Ultrasonography was largely utilised as a first line diagnostic tool in all patients, with valuable visualisation of the pancreas in 85% of patients. Computer tomography scan was also widely used, with 66.7% of exams in mild and 33.3% in severe pancreatitis. Patients affected by biliary pancreatitis presented more severe (p=0.004) and necrotizing forms (p=0.021). Mortality was significantly related (p0.001) with the extension of pancreatic necrosis and with an age of over 70 years. Body mass index presented significantly greater values in severe than in mild forms (p0.001).Association of creatinine serum level over 2mg/dl with an abnormal chest X-ray showed a high significant correlation with a more severe outcome in terms of morphological sequelae and mortality (p=0.0001). Acute pancreatitis in Italy more commonly presents biliary aetiology and favourable outcome with low rate of complications and mortality. From a cost-effectiveness standpoint, diagnostic approach to this disease needs to be better standardised.

Abstract

The Italian Association for the Study of the Pancreas released a diagnostic and therapeutic algorithm for acute pancreatitis in 1999.This study focused on the analysis of the therapeutic approach for the treatment of acute pancreatitis in Italy.One thousand, one hundred and seventy-three patients were recruited: 1006 patients (85.8%) had mild acute pancreatitis (MAP) and 167 (14.2%) had the severe acute pancreatitis (SAP); 161 patients showed pancreatic necrosis at computed tomography; 121 patients (10.3%) had sequelae and 36 (3.1%) died.Non-steroidal anti-inflammatory drugs and tramadol were used more frequently in patients with the MAP whereas opioids and the association schedules were used more frequently in patients with the SAP (P0.001). Gabexate mesilate was utilised in 831 out of 1173 patients (70.8%); in particular, gabexate mesilate was used in 70.6% patients with MAP and in 73.1% of those with SAP (P=0.521). The duration of the drug administration was significantly shorter in those having MAP than in those having the SAP (P0.001). The antibiotics most frequently used for the prophylaxis against infection from pancreatic necrosis (43.1%) were carbapenems. Only a small number of patients received enteral nutrition (4.7%). Endoscopic retrograde cholangiopancreatography was carried out in 344 of the 1173 patients (29.3%). Surgery was performed in 48 with SAP (19 had elective biliary surgery and 29 had pancreatic surgery).The results of this survey indicate a lack of compliance with the guidelines which regard the indications mainly for interventional endoscopy and surgery.

The aim of this research was to retard the onset of maturation and breeding using Methallibure (I.e.I. 33,828), thereby allowing more time for somatic growth. The process of development of the gonads was studied, laboratory experiments were performed upon a breeding stock of Tilapia in an attempt to postpone spawning, and the results of these investigations were tested, on fish in ponds at the Fish Culture Research Station in Israel.

El resultado principal es el teorema (13)probado por el matemático alemán Kronecker. Este teorema es sumamente importante, en particular para la construcción de ejemplos en distintas ramas de la matemática.

Abstract

We report a case of chronic bullous dermatosis of children, a benign dermatologic disease of infancy of unknown etiology, characterized, on IIF, by linear deposits of immunoglobulins in the Basement Membrane Zone of cutis, and clinically by a ring shaped disposition of bullae (rosette-like). This work concerns a ten year old girl admitted to the pediatric ward at E. Agnelli Hospital of Pinerolo (Turin) about one year ago, who started having perioral bullous eruption, subsequently spread all over the body skin, with general symptoms of fever and itching. Round erythematous plaques, crusts, and a ring disposition of sausage-shaped subepidermal bullae were suggestive of CBDC. Diagnosis was confirmed by cutaneous biopsy and DIF. Corticosteroids, given for about six mths., and DDS in the last four mths. of treatment, quickly changed the course of the disease, which has not recurred for about one year. Differential diagnosis with other bullous dermatoses of 2nd and 3rd childhood has been addressed to bacterial and viral diseases as well as to toxico-allergic and autoimmune diseases, the latter rarely seen by Pediatricians. Among those we discuss on clinical and anatomopathological aspects of Dermatitis herpetiformis, Erythema multiforme, Pemphigus and Pemphigoid, with regard to different anatomic sites of bullous lesions and to pathogenesis involved immunoglobulins and skin antigens.

Abstract

We report a case of secondary skin tuberculosis due to endogenous secondary infection in a 27-year-old subject affected by ulcerative colitis. The clinical appearance the lesion was atypical and its classification uncertain. The morphology of the lesion and the fact that the primary tubercular complex, at pulmonary level, was masked by a simultaneous candidiasis infection were probably due to cell-mediated immunodeficiency consequent to the ulcerative colitis and on-going therapy (Salazopyrin and prednisone). Rapid remission of cutaneous and pulmonary lesions was achieved following specific therapy (rifampicin, isoniazid, ethambutol).

Abstract

The Authors describe two cases of Sporotrichoid infection characterized by the onset, above previous skin accidental injuries, of a papulo-pustular lesion, with further development of centripetal satellite lesions similar to the first one. A precise etiological definition was possible only after the cultural,l mycological and bacteriological examination of biopsy material. The very similar clinical features of the two cases were due to different etiological pathogens: Sporothrix schenckii in one case and Mycobacterium marinum in the other case. A complete healing of the skin lesion was obtained with Itraconazole in the first case and Rifampin in the second case.

Abstract

The Authors describe two cases of notalgia paresthesica, a disorder with localized pruritus of the back and hypesthesia for touch possibly due to a sensory neuropathy involving the posterior primary rami of thoracic nerve T2 through T6. The condition is not rare, but underdiagnosed for the poor clinical relevance. The possible X-linked hereditary transmission and other pathogenetic hypotheses are discussed.
