This article provides an in-depth analysis of digital transformation in European primary healthcare (PHC). It assesses the impact of digital technology on healthcare delivery and management, highlighting variations in digital maturity across Europe. It emphasizes the significance of digital tools, especially during the COVID-19 pandemic, in enhancing accessibility and efficiency in healthcare. It discusses the integration of telehealth, remote monitoring, and e-health solutions, showcasing their role in patient empowerment and proactive care. Examples are included from various countries, such as Greece's ePrescription system, Lithuania's adoption of remote consultations, Spain's use of risk stratification solutions, and the Netherlands’ advanced use of telemonitoring solutions, to illustrate the diverse implementation of digital solutions in PHC. The article offers insights into the challenges and opportunities of embedding digital technologies into a multidisciplinary healthcare framework, pointing towards future directions for PHC in Europe. Resumen: Este artículo proporciona un análisis profundo de la transformación digital en la atención primaria (AP) en Europa. Evalúa el impacto de la tecnología digital en la provisión y en la gestión de la atención sanitaria, destacando las variaciones en la madurez digital en el contexto europeo. Enfatiza la importancia de las herramientas digitales, especialmente durante la pandemia causada por la COVID-19, para mejorar la accesibilidad y la eficiencia en la atención sanitaria. Se discute la integración de la telemedicina, la monitorización remota y las soluciones de e-salud, mostrando su papel en el empoderamiento del paciente y el cuidado proactivo. Se incluyen ejemplos de varios países, como el sistema de prescripción electrónica de Grecia, la adopción de consultas remotas en Lituania, el uso de soluciones de estratificación de riesgo poblacional en España y el uso avanzado de soluciones de telemonitorización en los Países Bajos, para ilustrar la diversa implementación de soluciones digitales en AP. El artículo ofrece perspectivas sobre los desafíos y las oportunidades de incorporar tecnologías digitales en un marco de atención sanitaria multidisciplinario, apuntando hacia futuras direcciones para la AP en Europa.

3. Limited Use and Potential Implementation Hurdles of Telemedicine Tools for the Remote Management of Patients With Chronic Obstructive Pulmonary Disease Among Members of SEPAR

Introduction: Telemedicine (TM) can help in the management of chronic obstructive pulmonary disease (COPD). This study examines knowledge, current use and potential limitations for practical implementation of TM for the remoted management of COPD patients among members of the COPD area of SEPAR (n = 3118). Methods: An electronic survey was circulated three times to these 3118 health-care professionals. Their knowledge, current use and potential limitations for implementation of different forms of TM, including tele-monitoring, tele-education and self-care, tele-rehabilitation and mobile health, for the remote management of COPD patients were tabulated and described. Results: Only 120 health-care professionals responded to the survey (3.9%). The rate of response varied greatly across different Autonomous Communities (AACC); 99.2% of responders declared being aware of TM, but only 60.5% knew about the different TM alternatives investigated here, and only 40.3% actually used some form of TM for their current management of patients with COPD. Of those using TM, 47.1% referred being satisfied with its use. Main identified barriers for implementation of TM in their institutions were technological limitations and data security. Conclusions: The potential of TM for the clinical management of COPD is well known among interviewed health-care professionals, but only less than half used it currently. The potential for growth is therefore clear. We propose that SEPAR analyze critically this potential and promotes measures to achieve it for the benefit of COPD patients. Resumen: Introducción: La telemedicina (TM) puede ayudar en el tratamiento de la enfermedad pulmonar obstructiva crónica (EPOC). Este estudio examina el conocimiento, el uso actual y las posibles limitaciones para la implementación práctica de la TM para el tratamiento remoto de pacientes con EPOC entre los miembros del área de EPOC de la SEPAR (n = 3.118). Métodos: Se distribuyó 3 veces una encuesta electrónica entre estos 3.118 profesionales de la salud. Se tabularon y describieron sus conocimientos, el uso actual y las limitaciones potenciales para la implementación de diferentes formas de la TM, incluida la telemonitorización, la teleeducación y el autocuidado, la telerrehabilitación y la salud móvil, para el tratamiento remoto de los pacientes con EPOC. Resultados: Solo 120 profesionales sanitarios respondieron a la encuesta (3,9%). La tasa de respuesta varió mucho entre las distintas comunidades autónomas (CC. AA.); el 99,2% de los encuestados declaró conocer la TM, pero solo el 60,5% conocía las diferentes alternativas de la TM investigadas aquí, y solo el 40,3% realmente utilizó alguna forma de TM para el manejo actual de los pacientes con EPOC. De quienes utilizan la TM, el 47,1% refirió estar satisfecho con su uso. Las principales barreras identificadas para la implementación de la TM en sus instituciones fueron las limitaciones tecnológicas y la seguridad de los datos. Conclusiones: El potencial de la TM para el tratamiento clínico de la EPOC es bien conocido entre los profesionales sanitarios entrevistados, pero solo menos de la mitad la utiliza actualmente. Por tanto, el potencial de crecimiento es claro. Proponemos que la SEPAR analice críticamente este potencial y promueva medidas para alcanzarlo en beneficio de los pacientes con EPOC.

Due to the COVID-19 pandemic, telemedicine has seen a significant boom in employment and an undeniable impact, on healthcare. The objective of the present review was to assess the role that telemedicine and digital health play in comprehensive medical care. The authors relied on hermeneutics to analyze and evaluate the systematic review carried out with the PRISMA 2020 methodology on telemedicine and digital health. Finally, 9 articles were reviewed in the Pubmed database corresponding to the period from 2019 to 2023. The results highlight the growing importance of information and communications technologies in the context of medical care, mainly in the field of the COVID-19 pandemic. This pandemic has driven the adoption of telemedicine and technologies in healthcare, but efforts are required to ensure equity in their implementation.

Artificial intelligence (AI) can be a valuable tool for primary care (PC), as, among other things, it can help healthcare professionals improve diagnostic accuracy, chronic disease management and the overall efficiency of the care they provide. It is important to emphasise that AI should not be seen as a replacement tool, but as an aid to PC professionals. Although AI is capable of processing large amounts of data and generating accurate predictions, it cannot replace the skill and expertise of professionals in clinical decision making. AI still requires the interpretation and clinical judgement of a trained healthcare professional and cannot provide the empathy and emotional support often required in healthcare. Resumen: La inteligencia artificial (IA) puede ser una herramienta de gran valor para la atención primaria (AP), ya que, entre otras cosas, puede ayudar a los profesionales de la salud a mejorar la precisión en los diagnósticos, la gestión de enfermedades crónicas y la eficiencia general del cuidado que proporcionan. Es importante subrayar que la IA no debe ser vista como una herramienta de sustitución, sino como una ayuda para los profesionales de la AP. Aunque la IA es capaz de procesar grandes volúmenes de datos y generar predicciones precisas, no puede reemplazar la destreza y la experiencia de los profesionales en la toma de decisiones clínicas. La IA todavía requiere la interpretación y el juicio clínico de un profesional de la salud capacitado, y no puede ofrecer la empatía y el apoyo emocional que a menudo se requiere en el ámbito sanitario.

The findings indicate that mentoring and participation in Google Classroom significantly improve knowledge and skills retention, as well as the utilization of data for decision-making. Bangladesh received the United Nations Award for DigitalHealth Development in 2011. Inspired by the recognition, the country has launched several digital health initiatives over the last decade. However, despite achievements in expanding information and communication technologies, recent studies show that current digital health services are difficult to use, fail to meet user needs, are subject to threats in data privacy and security, lack a robust digital infrastructure, and do not benefit from a cohesive policy and regulatory environment. As the country currently deals with the post‐COVID‐19 consequences, it is high time for its healthcare policymakers to address these challenges and take the necessary actions to establish a people‐centered framework for digital health service design and operations. In this commentary, we proposed a design framework for digital health services that address these ongoing challenges in the digital health services of Bangladesh. We believe the proposed framework stands to help Bangladesh and other coutnries attain sustainability. Key Points: Most digital health services in Bangladesh lack usability, user‐centricity, data privacy and security, digital infrastructure, technical and resource expertise, digital literacy, motivation, and trust of all healthcare stakeholders.It is time for Bangladesh's health policymakers to address the ongoing challenges that demand a people‐centred digital health service design and operations framework.This policy perspective proposes a design framework for digital health services that addresses the ongoing challenges in the digital health services of Bangladesh. Introduction: there is a wide variety of enteral nutrition and infant formulas preparations. when there is a need to find infomation on a product, only the infomation from industy is available. Comparison amomg products becomes then ardous. Objective: to describe the development of NEmecum as the first website that allows a directed and independent search for enteral nutrition products and infant formulas, currently available in Spain, and to evaluate the initial use of NEmecum. Methods: the structure of a database that unifies the parameters of all formulas was developed, and the nutritional composition of all formulas was analyzed. Subsequently, the main search algorithms were selected and the digital tool was codified. Intensive dissemination was performed and the impact was evaluated. The profile of users and registered centers and the use of the tool were analyzed, and its usability was evaluated using the System Usability Scale (SUS) questionnaire. Results: a free-access responsive website (http://nemecum.com) that allows searches based on pre-established search filters was obtained. This tool allows for a detailed analysis available formulas in Spain by observing a wide variety of formulas with similar characteristics. The dissemination campaign managed to increase its use exponentially, currently reaching 1,370 users and 79 registered centers. Usability was rated as excellent. Conclusion: the development of the NEmecum represents a valuable tool in the search and consultation of the characteristics of enteral nutrition formulas and infant preparations. In this text, I propose the concept of "metapresentiality" as a fundamental element for a Critical Theory of Digital Health. First, I present the concepts of technique, technology, and technical object, central to the theories of Álvaro Vieira Pinto and Milton Santos. Secondly, based on Luciano Floridi's philosophy of information, I question the relevance of the dichotomy real-material-concrete vs. digital-virtual-informational as an ontological foundation for concepts of reality, place, and presence, highlighting the notions of virtual reality and extended reality. Thirdly, I introduce an etymological-historical critique of the series presence-telepresence-metapresence, focusing on the emerging notion of meta-presentiality in the form of proto-concept and its eventual formalization as a conceptual foundation for a socio-technical appropriation and technosocial integration of digital technologies. Finally, I discuss Digital Health as a field of knowledge, techniques and practices and evaluate epistemological and pragmatic advantages of metapresentiality as a concept in the fields of computing, education and health. Objective: To describe digital health and the clinical practice in nursing in the Brazilian context. Methods: Theoretical study about the operational development of digital health in nursing clinical practice. Results: Digital health in Brazil involves a still maturing set of concepts, including several different nursing technologies. There are federal initiatives to institute digital health modalities in public and private health sectors, associated with the science, technology, innovation, and telecommunication sectors. In the scope of nursing clinical practice, this is an incipient operationalization process when it comes to daily work. Nonetheless, resolutions have been published to regulate this modality in nursing in a broader context, that is, beyond that of the pandemic, for several specialties, with public consultations to deal with specific issues. Final Considerations: Digital health has been widely disseminated in health services, with relevant implications to the working category of Brazilian nursing field, on the clinic practice context, being attention deserved. A questionnaire of 8 questions was designed and applied to 32 participants randomly selected from adult patients at the Amistad Peru Korea Bellavista Health Center, Callao, who usually use cell phones with internet access and who use mobile applications. Results. The dichotomous items of the knowledge dimension had a KR20 of 0.72, in the appraisal dimension a Cronbach’s alpha coefficient of 0.74 was obtained and the usability dimension obtained a Cronbach’s alpha coefficient of 0.75. Conclusions. The instrument demonstrated high reliability in the three dimensions and has internal validity and adequate reliability to be applied., Introducción. El uso de herramientas digitales para la salud es una práctica extendida. Objetivo. Determinar la validez de contenido y confiabilidad de un instrumento de medición del conocimiento, valoración y usabilidad de un aplicativo móvil de salud y nutrición con base de datos de alimentos nativos del Perú. Métodos. Se diseñó un cuestionario de 8 preguntas que se aplicó en 32 participantes seleccionados aleatoriamente entre los pacientes adultos del Centro de Salud Amistad Perú Corea Bellavista, Callao, que habitualmente utilizan celular con acceso a internet y que usan aplicativos móviles. Resultados: Los ítems dicotómicos de la dimensión conocimiento tuvieron un KR20 de 0,72, en la dimensión valoración se obtuvo un coeficiente alfa de Cronbach de 0,74 y la dimensión usabilidad obtuvo un coeficiente alfa de Cronbach de 0,75. Conclusión. El instrumento demostró una alta confiabilidad en las tres dimensiones, con validez interna y confiabilidad adecuada para su aplicación.

Los problemas del sistema de salud mexicano son de saturación, deterioro y carencia de infraestructura física y digital. Sin embargo, la salud digital (SD) funciona como tamiz para evitarlos. Este trabajo analiza el efecto de la SD para reducir el número de contagios y su influencia en la mitigación de la pandemia de covid-19 en Ciudad de México. Mediante un índice de componentes principales, se localizó a la población que carece de recursos digitales disponibles, de acceso a la salud, educación, de vivienda y bienes. Se examinó el efecto de no utilizar la SD para trazar los traslados de la población que salió fuera de su residencia para atenderse de covid-19. Finalmente, se georreferenció a la población usuaria de esta modalidad de salud con algún síntoma de dicha enfermedad. La estimación del efecto de la SD es que es una forma inmediata de atender a los pacientes covid.

Digital health refers to the use of novel information communication technologies in healthcare. The use of these technologies could positively impact public health and health outcomes of populations by generating timely data, and facilitating the process of data collection, analysis, and knowledge translation. Using selected case studies, we aim to describe the opportunities and barriers in the use of technology applied to health-related research. We focus on three areas: strategies to generate new data using novel data collection methods, strategies to use and analyze existing data, and using digital health for health-related interventions. Exemplars from seven countries are provided to illustrate activity across these areas. Although the use of health-related technologies is increasing, challenges remain to support their adoption and scale-up -especially for under-served populations. Se presentan los resultados de una investigación cualitativa realizada en México, entre mayo y julio del 2021, con el propósito de identificar las necesidades de las y los cuidadores de pacientes con trastornos mentales o enfermedades neurodegenerativas como la demencia en sus diversas variantes: Enfermedad de Pick, Alzheimer, Parkinson, etc. Para ello se recurrió a un proceso de investigación centrado en el usuario, haciendo uso de encuestas, entrevistas etnográficas y la documentación del contexto actual de emprendimientos en el ámbito de salud mental y el uso de tecnología en el mismo. Con base en los resultados obtenidos, surge una propuesta de atención enfocada no en la persona enferma, sino en sus cuidadores: una plataforma que integre servicios como un directorio verificado de especialistas, con información de su geo-localización y costos; divulgación de contenido didáctico de fácil entendimiento, apoyo emocional para los cuidadores y un espacio para la creación de comunidad entre ellos, además de funcionalidades que les ayuden a mantener un registro actualizado del estado del paciente y organizar las tareas diarias de sus cuidados.

A partir del estudio de un programa de teleneurología implementado en Biobío, Chile, este artículo analiza las lógicas que articulan el trabajo moral de la medicina a distancia. Desde un enfoque pragmático informado por la semiótica material, la teoría de la subjetivación y la sociología de la moralidad, este artículo analiza tres lógicas que dan sentido a prácticas de valuación del programa de teleneurología. Ellas son desplegadas por actores que desempeñan roles distintos, generando puntos de tensión y encauzando la constante evolución de los entramados sociomateriales en que se realiza el cuidado de salud. Una lógica cívico industrial fundamenta discursos públicos que destacan la reducción de las listas de espera de especialidad como prueba de valor del programa de telemedicina. Desde una lógica de la seguridad, en tanto, personal médico prioriza la adopción de estándares que, orientados a controlar los riesgos implicados en la realización de medicina a distancia, son tenidos como válidos por una comunidad profesional. Por su parte, adoptando una lógica del cuidado, pacientes y médicos generales enfatizan el despliegue de prácticas orientadas a atender la situación particular que experimenta cada paciente. Se propone que reconocer las lógicas que distintos actores puedan adoptar para otorgar valor a las prácticas de cuidado de salud puede favorecer la comprensión, anticipación y resolución de las tensiones que emergen con la implementación de tecnologías de salud digital y telemedicina.

The health industry has experienced great innovation and will continue to do so in the coming years. The term innovation comes from "outside to inside" driven by the need for knowledge and research to truly translate into effective improvements (hence the sequence from Research and Development to Innovation: R+D+I); but it also comes from "bottom up" as a drive of the health organization (based, as few others, on knowledge as a fundamental asset) to give way to their creativity and their ability to find new solutions to old and new problems. The current health system must advance in the development of a more global and integrated philosophy of care, which allows dealing with the consequences of aging and the increase in chronic diseases and dependence, which represent an increase in the demand for care. In the mediumlong term, a care logic based on individual characteristics from the molecular perspective should be promoted, which is known as 5P medicine (personalized, preventive, predictive, participatory and population), also called personalized medicine, a paradigm that has already initiated its entry, slow and uneven, in health systems. And it must also adapt to a society with more informed and participatory people in the management of their own health, which increasingly use technologies whose development speed grows exponentially. Taking into account these characteristics and objectives, in this article we seek to define the fundamental features of the intersection between innovation and clinical nutrition. 36. Mechanisms of change in Internet- and mobile-based interventions for PTSD: a systematic review and meta-analysis.

Background: While Internet- and mobile-based interventions (IMIs) are potential options to increase the access to evidence-based therapies for post-traumatic stress disorder (PTSD), comprehensive knowledge on their working mechanisms is still scarce. Objective: We aimed to evaluate studies investigating the efficacy and mechanisms of change in IMIs for adults with PTSD. Method: In this systematic review and meta-analysis (PROSPERO CRD42019130314), five databases were consulted to identify relevant studies, complemented by forward (i.e. citation search) and backward (i.e. review of reference lists from included studies) searches. Randomized controlled trials (RCTs) investigating the efficacy of IMIs compared to active controls, as well as component and mediation studies were included. Two independent reviewers extracted the data and assessed the risk of bias and requirements for process research. Random-effects meta-analyses on PTSD symptom severity as primary outcome were conducted and further information was synthesized qualitatively. Results: In total, 33 RCTs were included (N = 5421). The meta-analysis comparing IMIs to non-bonafide active controls yielded a significant standardized mean difference (SMD) of −0.36 (95%CI −0.53 to −0.19) favouring IMIs. Although meta-analytic pooling was not possible for the component and mediation studies, evidence suggests no differential effects regarding PTSD symptom reduction between different levels of support and personalization and between different types of exposure. Moreover, mediation studies revealed significant intervening variable effects for self-efficacy beliefs, perceived physical impairment, social acknowledgement, and trauma disclosure. Conclusions: Results indicate that IMIs for PTSD are superior to active controls. Furthermore, findings may contribute to the development of new interventions by outlining important directions for future research (e.g. regarding requirements for process research) and highlighting potential mechanisms of change (i.e. self-efficacy, perceived physical impairment, social acknowledgement, and trauma disclosure). • Knowledge on mechanisms of change in psychothera-pies is crucial as it contributes to intervention development and improved outcomes. • This review suggests that digital interventions for PTSD are efficacious; yet, more studies are needed to enhance understanding on how they work. Teleconsultation is a remote health consultation using information and communication technologies. There are different modalities and specific practical and communication skills are required. Notwithstanding its prominence in Spain, there is little evidence on teleconsultation. This article explores the applicability, barriers, facilitators and future challenges of teleconsultation. While it has the potential to improve access to healthcare, as well as save time and costs for both patients and healthcare professionals, it faces a number of challenges such as the digital divide and resistance to change. To address new challenges and overcome obstacles, it is crucial to gain the trust of patients and professionals. Improving training in the skills required to optimize their use is also essential. El presente trabajo tiene el objetivo de explorar el desarrollo de las tecnologías de la información y comunicación (componentes de eSalud) en servicios de salud públicos de México, así como indagar sobre los facilitadores y barreras para su implementación. Se realizó un estudio de caso en el Instituto de Servicios de Salud del Estado de Aguascalientes utilizando un abordaje mixto de tipo concurrente y de predominancia cualitativa que incluyó observación no participante, entrevistas semiestructuradas y un cuestionario dirigido al personal médico. A través del estudio, se identifican diversas oportunidades de mejora en cuanto al acceso y desarrollo de las tecnologías de la información y comunicación; también se identifican diversas barreras para su implementación factibles de superarse. En cuanto a las limitaciones del estudio, se reconoce la exploración de perspectivas sólo desde la oferta de servicios. En lo que respecta a la originalidad, destaca la investigación integral de diferentes componentes de eSalud. Se concluye que es necesario fortalecer las políticas de las tecnologías de información y comunicación dirigidas al sector salud con el fin de optimizar su desarrollo e implementación.

In this text, I propose the concept of “metapresentiality” as a fundamental element for a Critical Theory of Digital Health. First, I present the concepts of technique, technology, and technical object, central to the theories of Álvaro Vieira Pinto and Milton Santos. Secondly, based on Luciano Floridi’s philosophy of information, I question the relevance of the dichotomy real-material-concrete vs. digital-virtual-informational as an ontological foundation for concepts of reality, place, and presence, highlighting the notions of virtual reality and extended reality. Thirdly, I introduce an etymological-historical critique of the series presence-telepresence-metapresence, focusing on the emerging notion of meta-presentiality in the form of proto-concept and its eventual formalization as a conceptual foundation for a socio-technical appropriation and technosocial integration of digital technologies. Finally, I discuss Digital Health as a field of knowledge, techniques and practices and evaluate epistemological and pragmatic advantages of metapresentiality as a concept in the fields of computing, education and health., En este texto, propongo el concepto de “metapresencialidad” como elemento fundante para una teoría crítica de la salud digital. En primer lugar, presento los conceptos de técnica, tecnología y objeto técnico, centrales en las teorías de Álvaro Vieira Pinto y Milton Santos. En segundo lugar, a partir de la filosofía de la información de Luciano Floridi, cuestiono la pertinencia de la dicotomía real-material-concreto versus digital-virtual-informacional como fundamento ontológico de los conceptos de realidad, lugar y presencia, destacando las nociones de realidad virtual y realidad extendida. En tercer lugar, introduzco una crítica etimológica e histórica de la serie presencia-telepresencia-metapresencia, enfocando la noción emergente de metapresencialidad en forma de protoconcepto y su eventual formalización como fundamento conceptual para una apropiación sociotécnica y una integración tecnosocial de las tecnologías digitales. Finalmente, discuto la salud digital como campo de saberes, técnicas y prácticas y evalúo las ventajas epistemológicas y pragmáticas de la metapresencialidad como concepto en los campos de la informática, la educación y la salud.

- Abstract
Suicide is a public health problem. In 2020, 3,941 suicide deaths were recorded. In 2021, 4,003 suicides were reported, remaining the first External cause of death in Spain. It is estimated that 4.4% of deaths in Spain correspond to mental and behavioral disorders. The World Health Organization (WHO) encourages all countries to develop prevention strategies. However, no single factor is sufficient to explain a death by suicide, given the complexity of suicidal behaviors (SB). Therefore, some research indicates that suicide prevention will not be achieved with a single approach or strategy either. This research considers the strategies of the region, the available resources and the choice of some Information and Communication Technologies (ICT). This thesis aims to help prevent CS using ICT in the following aspects: (1) Determine in public hospitals in the CyL region, the mental disorders that most influence SB readmissions. (2) Evaluate a Digital Health Strategy (DHS) by patients with SB. Two components in the selection of attributes/variables were used to carry out the feature selection (FS) by ML, with the objective of identifying the mental disorders that influence the readmissions of patients with SB by hospitals in this region. The first component consisted of using classical statistics, which was evaluated by the Chi-Square (X2) distribution technique. The second component involves the use of ML techniques with three different approaches such as: entropy, probability and the linear relationship of the variables. Then, the evaluation was carried out by experts. The FS with ML has allowed the identification of mental disorders, which were included in the design of the proposed DHS. This technological proposal is based on a software ecosystem, to provide patient follow-up, help prevent SB and reduce the hospital burden in CyL considering the characteristics of the region. The main conclusions are: (1) ML techniques help to identify the mental disorders that most, El suicidio es un problema de salud pública. En 2020, se registraron 3 941 muertes por suicidio. En el 2021, se reportaron 4 003 suicidios, manteniéndose como la primera causa de muerte Externa en España. Se estima que el 4.4% de las defunciones en España corresponden a trastornos mentales y comportamiento. La Organización Mundial de la Salud (OMS) anima a todos los países a desarrollar estrategias de prevención. Sin embargo, ningún factor por sí sola es suficiente para explicar una muerte por suicidio, dada la complejidad de los comportamientos suicidas (CS). Por lo tanto, algunas investigaciones indican que tampoco se logrará la prevención del suicidio con un único enfoque o estrategia. Esta investigación toma en cuenta las estrategias de la región, los recursos disponibles y la elección de algunas Tecnologías de la Información y las Comunicaciones (TIC). Esta tesis tiene el objetivo de ayudar a la prevención del CS utilizando las TIC en los siguientes aspectos: (1) Determinar en los hospitales públicos de la región de CyL, los trastornos mentales que más influyen en los reingresos con CS. (2) Evaluar una Estrategia de Salud Digital (ESD) por parte de pacientes con CS. Se utilizaron dos componentes en la selección de atributos/variables para llevar a cabo la selección de características (FS) por ML, con el objetivo de identificar los trastornos mentales que influyen en los reingresos de pacientes con CS por hospitales en esta región. El primer componente consistió en usar la estadística clásica, la cual fue evaluada por la técnica de distribución Chi-Cuadrado(X2). El segundo componente implica el uso de las técnicas de ML con tres enfoques distintos como: la entropía, la probabilidad y la relación lineal de las variables. Luego, se realizó la evaluación por parte de expertos. La FS con ML ha permitido identificar los trastornos mentales, los cuales se incluyeron en el diseño de la ESD propuesta. - Abstract
[Resumen]: Introducción: existe una amplia variedad de fórmulas o preparados de nutrición enteral y fórmulas o preparados infantiles. La consulta de información relacionada debe hacerse en las herramientas propias de cada laboratorio, lo que dificulta la visión crítica y la comparativa entre las mismas. Objetivo: describir el desarrollo de NEmecum como la primera web que permite realizar una búsqueda dirigida e independiente de fórmulas de nutrición o preparados infantiles, analizar el abanico nutricional actual en España y evaluar los datos de uso de la herramienta. Métodos: se desarrolló la estructura de una base de datos que unifica los parámetros de todas las fórmulas y se analizó el abanico nutricional español. Posteriormente, se seleccionaron los principales algoritmos de búsqueda dirigida y se codificó la herramienta digital. Se llevó a cabo una intensa difusión y se evaluó el impacto obtenido. Se analizaron el perfil de usuarios y centros registrados y los datos de uso de la herramienta y se evaluó su usabilidad mediante el cuestionario System Usability Scale (SUS). Resultados: se obtuvo una web responsive de acceso gratuito (http://nemecum.com) que permite realizar búsquedas dirigidas en base a unos filtros de búsqueda preestablecidos. La herramienta permitió analizar detalladamente el abanico nutricional en España, observándose la gran variedad de fórmulas disponibles de similares características. La campaña de difusión consiguió incrementar su uso de forma exponencial y cuenta actualmente con 1.370 usuarios y 79 centros registrados. La usabilidad fue valorada como excelente. Conclusión: el desarrollo de NEmecum supone una herramienta valiosa en la búsqueda y consulta de datos de fórmulas o preparados de nutrición enteral y fórmulas o preparados infantiles., [Abstract]: Introduction: there is a wide variety of enteral nutrition and infant formulas preparations. When there is a need to find infomation on a product, only the infomation from industy is available. Comparison amomg products becomes then ardous. Objective: to describe the development of NEmecum as the first website that allows a directed and independent search for enteral nutrition products and infant formulas, currently available in Spain, and to evaluate the initial use of NEmecum. Methods: the structure of a database that unifies the parameters of all formulas was developed, and the nutritional composition of all formulas was analyzed. Subsequently, the main search algorithms were selected and the digital tool was codified. Intensive dissemination was performed and the impact was evaluated. The profile of users and registered centers and the use of the tool were analyzed, and its usability was evaluated using the System Usability Scale (SUS) questionnaire. Results: a free-access responsive website (http://nemecum.com) that allows searches based on pre-established search filters was obtained. This tool allows for a detailed analysis avalaible formulas in Spain by observing a wide variety of formulas with similar characteristics. The dissemination campaign managed to increase its use exponentially, currently reaching 1,370 users and 79 registered centers. Usability was rated as excellent. Conclusion: the development of the NEmecum represents a valuable tool in the search and consultation of the characteristics of enteral nutrition formulas and infant preparations.

Resumo Este artigo objetiva mapear a literatura sobre as aplicações e percepções acerca do uso de tecnologias digitais nas práticas de trabalhadores comunitários de saúde. Trata-se de uma revisão de escopo realizada na PubMed, Bireme, SciELO, Web of Science, Embase e Scopus. Foram incluídos 63 artigos que relatam o uso de tecnologias digitais por esses trabalhadores em 24 países. Como resultados, identificou-se que o suporte à saúde materno-infantil é a condição com maior predomínio das práticas. Os benefícios identificados envolvem ampliação do acesso, melhoria da gestão do trabalho, qualificação, diversificação, ampliação da formação e ganho de legitimidade da categoria. Os desafios se traduzem nas limitações em relação ao vínculo com a comunidade, longitudinalidade do cuidado, acesso à internet, energia elétrica e alfabetização digital. Como conclusão, corrobora-se com análises acerca da irreversibilidade do uso de tecnologias de informação e comunicação no mundo do trabalho, destacando-se a necessidade do seu uso racional dessas com a garantia do acesso de forma integral, universal e equitativa. Resumen Este artículo tiene por objeto mapear la literatura sobre las aplicaciones y percepciones acerca del uso de tecnologías digitales en las prácticas de los trabajadores comunitarios de la salud. Se trata de una revisión del alcance realizada en PubMed, Bireme, SciELO, Web of Science, Embase y Scopus. Se han incluido 63 artículos sobre el uso de tecnologías digitales por parte de estos trabajadores en 24 países. Como resultado, se ha identificado que el apoyo a la salud maternoinfantil es la condición con mayor predominio de las prácticas. Los beneficios identificados implican la ampliación del acceso, mejora de la gestión del trabajo, calificación, diversificación, ampliación de la formación y aumento de la legitimidad de la categoría. Los desafíos se traducen en limitaciones en relación con el vínculo con la comunidad, la longitudinalidad de la atención, el acceso a Internet, la energía eléctrica y la alfabetización digital. En conclusión, se corrobora con análisis sobre la irreversibilidad del uso de las tecnologías de la información y la comunicación en el mundo del trabajo, y se destaca la necesidad de su uso racional con la garantía del acceso de manera integral, universal y equitativa. Abstract This article aims to map the literature on the applications and perceptions regarding the use of digital technologies in the practices of community health workers. This is a scoping review conducted on PubMed, Bireme, SciELO, Web of Science, Embase, and Scopus. A total of 63 articles reporting the use of digital technologies by these workers in 24 countries were included. As a result, it was identified that support for maternal and child health is the most prevalent condition in these practices. The identified benefits involve increased access, improved work management, qualification, diversification, expanded training, and increased legitimacy of the profession. The challenges are reflected in limitations regarding community engagement, continuity of care, internet access, electricity, and digital literacy. In conclusion, it supports analyses regarding the irreversibility of the use of information and communication technologies in the world of work, emphasizing the need for their rational use while ensuring comprehensive, universal, and equitable access.

45. AVIP Project (Prosthetic Virtual Friend): Study of clinical-functional outcomes and satisfaction with a mobile application in the perioperative management and follow-up of hip arthroplasty.

- Abstract
Objective: To analyze the clinical, quality of life, and healthcare quality outcomes obtained in a series of patients undergoing total hip arthroplasty (THA), who were empowered and monitored using the AVIP application. These results will be compared with a control group followed through a standard protocol., Material and Method: Randomized clinical trial with parallel groups involving patients with an indication for THA. Clinical variables were measured and compared using the WOMAC and mHHS, pain assessed by the VAS, quality of life with the SF-12 test. Walking capabilities were analyzed using the Functional Gait Assessment Scale, along with satisfaction levels assessed through the SUCE questionnaire, and perceived anxiety levels related to the process., Results: A total of 68 patients were evaluated, with 31 patients in the AVIP group and 33 in the Control group completing the follow-up. Both groups demonstrated improvement in clinical outcomes based on the WOMAC and mHHS hip tests, a reduction in perceived pain, and an enhancement in quality of life according to the SF-12 test. Patients in the AVIP study group exhibited non-inferiority in clinical outcomes and satisfaction compared to the control group, as well as lower anxiety levels and improved walking capabilities after the first month of follow-up. Notably, 82.25% of the follow-up visits for this group were conducted remotely., Conclusion: The implementation of a mHealth application like AVIP can be safely offered to selected patients undergoing hip arthroplasty, enabling effective monitoring and providing continuous information and training., (Copyright © 2024 SECOT. Artificial intelligence (AI) can be a valuable tool for primary care (PC), as, among other things, it can help healthcare professionals improve diagnostic accuracy, chronic disease management and the overall efficiency of the care they provide. It is important to emphasise that AI should not be seen as a replacement tool, but as an aid to PC professionals. Although AI is capable of processing large amounts of data and generating accurate predictions, it cannot replace the skill and expertise of professionals in clinical decision making. - Abstract
Introduction: Kidney stone disease (KSD) is a common urological condition that often requires long-term care. Mobile health (mHealth) and eHealth technologies have the potential to enhance chronic disease management and behavioral change. To assess opportunities to apply these tools to improve KSD treatment and prevention, we aimed to assess current evidence on the use, benefits, and limitations of mHealth and eHealth in KSD., Methods: We performed a systematic review of primary research studies of mHealth and eHealth in the evaluation and management of KSD. Two independent researchers screened citations by title and abstract for relevance, then full-text review was performed for descriptive summary of the studies., Results: A total of 37 articles were included for analysis. Primary domains of evidence included: 1) "smart" water bottles and mobile-device apps for tracking fluid consumption, which showed increased intake in most studies; 2) ureteral stent tracking platforms, which improved the rate of long-term retained stents; 3) virtual stone clinics, which have been suggested to increase access, lower costs, and have satisfactory outcomes; 4) smartphone-based endoscopy platforms, which offered cost-effective image quality in resource-limited settings; 5) patient information about KSD online, which was typically characterized as poor quality and/or accuracy, particularly on YouTube. Most studies were proof-of-concept or single-arm intervention designs, with limited assessment of effectiveness or long-term clinical outcomes., Conclusions: Mobile and eHealth technologies have significant real-world applications to KSD prevention, intervention, and patient education.
