In contrast with extensive information on the management of deep vein thrombosis of the lower extremities, little is known on the most appropriate treatment of the superficial vein thrombosis (SVT). In a multicenter, prospective, controlled, double-blind, double-dummy clinical trial, 164 consecutive patients with acute SVT of the great saphenous vein were randomized to receive the s.c. administration of either fixed prophylactic doses (2850 a-Xa IU) or body-weight adjusted therapeutic doses of nadroparin once daily for 1 month. The main study outcome was to compare the rate of asymptomatic and symptomatic extension of SVT and/or venous thromboembolic (VTE) complications during a 3-month follow-up period. Of the 81 patients randomized to the prophylactic doses, seven [8.6%; 95% confidence interval (CI), 3.5-17.0] developed SVT progression or VTE complications as compared with six of the 83 (7.2%; 95% CI, 2.8-15.1) allocated to the treatment group (absolute difference, 1.4; 96% CI, -6.9 to 9.7; P = 0.74). No patient in either group developed major bleeding. Our findings suggest that therapeutic doses of low-molecular-weight heparin, administered for 1 month in patients with SVT of the greater saphenous vein do not improve results obtained by prophylactic doses, administered for the same period, over a 3-month follow-up period.

Recycling costs of mass products depend on the technology adopted, size of plant, quantity being recycled, constraints imposed by environmental legislation on process and output (materials, components and energy recovered). The value of output depends instead on market factors. To obtain a profitable recycling process the relationship between: products to be recycled, technology, plant size, output, legislation and market must be known. Consequently, providing a tool embedding such a relationship would enable the various partners - from legislators to companies, to research people - to act in order to efficiently and effectively implement the cycle. The aim of this paper is to present the research work carried out by ITIA, in cooperation with DEEI, to develop a tool responding to the needs mentioned above. To this end an Inverse Production Design Tool (IPDTool) has been conceived and developed. It is based on: - the modelling of entities such as in/out product, market and regulations; - an IDEFO model of the Inverse Production Plant; - a mathematical model to provide appropriate input for the simulation; - a simulation model based on the above. The IPDTool developed has been validated and used to assess the economic viability of an Inverse Production Plant in Italy. The results obtained show the usefulness of the IPDTool developed for Inverse Process designers and managers, product designers, legislators "designing" ecological laws and norms, scientists aiming at developing new Inverse Processes and related enabling technologies. The research work presented is a part of a research programme on Sustainable Production and Inverse Production Life Cycle Design in progress at ITIA-CNR. Paradigms developed for production will be studied in connection with Inverse Production to respond to respond to changes in market, ecological constraints, technology.

Despite growing information, questions still surround various aspects of post-stroke depression (PSD). The Italian multicenter observational study Destro was designed to help clarify in a large sample the frequency and clinical impact of PSD. A total of 53 centers consecutively admitted 1064 patients with ischemic or hemorrhagic stroke, assessing them periodically in the first 9 months after the event. Patients with depression were followed for two years. Depression was diagnosed on clinical examination, verbal (Beck Depression Inventory) and non-verbal rating systems (Visual Analog Mood Scale), identifying the nosographic condition attributable to the mental state. The patient's clinical history, residual independence, and post-ictus quality of life were also taken into account. PSD was detected in 383 patients (36 %), most of whom had minor depression (80.17 %), with dysthymia, rather than major depression and adaptation disorder. About 80% developed depression within three months of the stroke. Cases with later onset tended to have less severe symptoms. Risk factors were a history of depression, severe disability, previous stroke and female sex, but not the type and site of the vascular lesion. PSD was not correlated with any increase in mortality or cerebrovascular recurrences, but these patients had lower autonomy and quality of life ratings. In conclusion, patients should be close observed in the first few weeks after a stroke in order to check for depression,which is more likely in those with clear risk factors and may spoil their quality of life.

In ischaemic stroke, thrombolytic drugs speed the recanalisation of intracerebral arteries. The effects of aspirin are not known. A trial was conducted to determine whether, separately or together, streptokinase and aspirin have clinical benefits in acute ischaemic stroke similar to those in acute myocardial infarction. 622 patients with acute ischaemic stroke within 6 hours of symptom onset were randomised with a 2x2 factorial design to (i) a 1-hour intravenous infusion of 1 . 5 MU streptokinase, (ii) 300 mg/day buffered aspirin for 10 days, (iii) both active treatments, or (iv) neither. Early results raised a question whether the trial should be continued. Objective: The Italian multicenter observational study depression in stroke (DESTRO) aimed to identify risk factors for poststroke depression (PSD) and quantify the likelihood of it arising in various categories of patients. Method: Mood evaluation was performed in 1064 consecutive stroke patients by means of Beck Depression Inventory and Visual Analog Mood Scale. Depressive symptoms were classified using the DSM-IV and revised WHO criteria for depression in the course of a neurological disorder. Results: Poststroke depression was seen in 36% of the survivors, with dysthymia by far the predominant form (80.7%). Female sex, disability, previous cerebrovascular or depressive episodes were significantly associated with an increased risk of depression. Combinations of these factors raised the risk of PSD exponentially, from 24.3 to 89.1%. The site of the stroke did not come into the uni- or multivariate analysis. Conclusion.: At admission, it is possible to predict the likelihood of PSD and quantify the relative risk.

Context Patients with suspected deep vein thrombosis ( DVT) of the lower extremities are usually investigated with ultrasonography either by the proximal veins ( 2-point ultrasonography) or the entire deep vein system ( whole- leg ultrasonography). The latter approach is thought to be better based on its ability to detect isolated calf vein thrombosis; however, it requires skilled operators and is mainly available only during working hours. No randomized comparisons are yet available evaluating the relative values of these 2 strategies. Objective To assess if the 2 diagnostic strategies are equivalent for the management of symptomatic outpatients with suspected DVT of the lower extremities. Design, Setting, and Patients A prospective, randomized, multicenter study of consecutive symptomatic outpatients ( n= 2465) with a first episode of suspected DVT of the lower extremities who were randomized to undergo 2- point or whole- leg ultrasonography. Data were taken from ultrasound laboratories of 14 Italian universities or civic hospitals between January 1, 2003, and December 21, 2006. Patients with normal ultrasound findings were followed up for 3 months, with study completion on March 20, 2007. Main Outcome Measure Objectively confirmed 3- month incidence of symptomatic venous thromboembolism in patients with an initially normal diagnostic workup. Results Of 2465 eligible patients, 345 met 1 or more exclusion criteria and 22 refused to participate; therefore, 2098 patients were randomized to either 2- point ( n= 1045) or whole- leg ( n= 1053) ultrasonography. Symptomatic venous thromboembolism occurred in 7 of 801 patients ( incidence, 0.9%; 95% confidence interval [ CI], 0.3%1.8%) in the 2- point strategy group and in 9 of 763 patients ( incidence, 1.2%; 95% CI, 0.5%- 2.2%) in the whole- leg strategy group. This met the established equivalence criterion ( observed difference, 0.3%; 95% CI, - 1.4% to 0.8%). Conclusion The 2 diagnostic strategies are equivalent when used for the management of symptomatic outpatients with suspected DVT of the lower extremities. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a recently identified autosomal dominant cerebral arteriopathy characterized by the recurrence of subcortical infarcts leading to dementia. A genetic linkage analysis conducted in two large families recently allowed us to map the affected gene on chromosome 19 in a 12-cM interval bracketed by D19S221 and D19S215. In the present study, these first 2 families and 13 additional ones, including a total of 199 potentially informative meiosis, have been genotyped with eight polymorphic markers located between D19S221 and D19S215. All families were linked to chromosome 19. The highest combined lod score (Zmax = 37.24 at theta = .01) was obtained with marker D19S841, a new CAn microsatellite marker that we isolated from chromosome 19 cosmids. The recombinant events observed within these families were used to refine the genetic mapping of CADASIL within a 2-cM interval that is now bracketed by D19S226 and D19S199 on 19p13.1. These data strongly suggest the genetic homogeneity of this recently identified condition and establish the value of its clinical and neuroimaging diagnostic criteria. Besides their importance for the ongoing positional cloning of the CADASIL gene, these data help to refine the genetic mapping of CADASIL relative to familial hemiplegic migraine and hereditary paroxysmal cerebellar ataxia, conditions that we both mapped within the same chromosome 19 region.

- Abstract
In 1990 we studied the prevalence and determinants of carotid atherosclerosis in an Italian general asymptomatic population (630 males and 718 females aged 18-99 years) living in the San Daniele district of the Friuli-Venezia Giulia Region. The global prevalence of subclinical carotid atherosclerosis was 25.4% in men and 26.4% in women. We considered intima-media thickness, non-stenotic plaque (< 40%), and stenotic plaque (> 40%). In the multiple logistic regression, the cross-sectional analysis of subjects aged 40 years showed a positive significant association between plaques/stenosis and age (p < 0.001), systolic blood pressure (p < 0.01), cigarette smoking (p < 0.0001), and the protective effect of high-density lipoprotein cholesterol (p < 0.037). In 2002 we decided to re-examine the initial cohort with the following objectives: prospectively evaluating the modifications of the previous carotid findings, their relationship with known and less documented cardiovascular risk factors and the predictive power of those variations on incident coronary and cerebrovascular events. We plan to evaluate the association of carotid plaque and carotid intima-media thickness with the genetic polymorphisms involved in atherosclerosis in survivors and finally to study the incidence and the determinants of atrial fibrillation in a general population. In this paper, we will describe the methodology of the screening and the cohort population characteristics. We have compared the San Daniele Project survivor cohort's characteristics to the current general population of the same age living in the Friuli-Venezia Giulia Region so as to extend the study's conclusions to the whole regional population.

- Abstract
Cardiac complications, including focal myocytolysis, electrocardiographic changes, arrhythmias and left ventricular wall motion abnormalities, frequently occur following stroke and contribute to worsen the prognosis. Their clinical spectrum seems to be related to the type of cerebrovascular disease and its localization. Thus, the incidence of arrhythmias and pulmonary edema is significantly higher in subarachnoid hemorrhage than in ischemic stroke, and the lesions in the right insular cortex are a major risk for complex arrhythmias and sudden death. Elevated plasma norepinephrine levels are frequently associated with these events and strongly suggest an underlying sympathetically mediated mechanism. The autonomic and cardiovascular effects of stroke, however, are modulated by concomitant factors such as pre-existent cardiac diseases, electrolyte disorders and, probably, by genetic alterations in the ionic control of myocyte repolarization. Although beta-blockers have been reported to prevent myocardial damage following stroke, adequate clinical trials are lacking, and the widespread use of these drugs in acute cerebrovascular disease is not supported by evidence.

- Abstract
Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a recently identified autosomal dominant cerebral arteriopathy characterized by the recurrence of subcortical infarcts leading to dementia. A genetic linkage analysis conducted in two large families recently allowed us to map the affected gene on chromosome 19 in a 12-cM interval bracketed by D19S221 and D19S215. In the present study, these first 2 families and 13 additional ones, including a total of 199 potentially informative meiosis, have been genotyped with eight polymorphic markers located between D19S221 and D19S215. All families were linked to chromosome 19. The highest combined lod score (Zmax = 37.24 at theta = .01) was obtained with marker D19S841, a new CAn microsatellite marker that we isolated from chromosome 19 cosmids. The recombinant events observed within these families were used to refine the genetic mapping of CADASIL within a 2-cM interval that is now bracketed by D19S226 and D19S199 on 19p13.1. These data strongly suggest the genetic homogeneity of this recently identified condition and establish the value of its clinical and neuroimaging diagnostic criteria. - Abstract
A double-blind closed sequential scheme study was carried out in 82 patients with insomnia to compare the hypnotic effectiveness of 15 mg midazolam and 1 mg lorazepam. Pairs of randomized patients were treated on one and the same night with midazolam or lorazepam, respectively. Eight parameters were recorded for each member of the pair. The time taken to fall asleep, the duration of sleep and the overall assessment of response showed that midazolam was significantly superior to lorazepam: the number of awakenings and evaluation by the patient verged on statistical significance. Both drugs were equal for the quality of sleep, dreams and the patient's condition on awakening. No side-effects were observed in any of the patients.
