Scope: Nutrients stimulate the secretion of glucagon-like peptide-1 (GLP-1), an incretin hormone, secreted from enteroendocrine L-cells which decreases food intake. Thus, GLP-1 analogs are approved for the treatment of obesity, yet cost and side effects limit their use. L-cells are mainly localized in the distal ileum and colon, which hinders the utilization of nutrients targeting GLP-1 secretion. This study proposes a controlled delivery system for nutrients, inducing a prolonged endogenous GLP-1 release which results in a decrease food intake. Methods and Results: α-Linolenic acid (αLA) was loaded into thermally hydrocarbonized porous silicon (THCPSi) particles. In vitro characterization and in vivo effects of αLA loaded particles on GLP-1 secretion and food intake were studied in mice. A total of 40.4 ± 3.2% of loaded αLA is released from particles into biorelevant buffer over 24 h, and αLA loaded THCPSi significantly increased in vitro GLP-1 secretion. Single-dose orally given αLA loaded mesoporous particles increased plasma active GLP-1 levels at 3 and 4 h and significantly reduced the area under the curve of 24 h food intake in mice. Conclusions: αLA loaded THCPSi particles could be used to endogenously stimulate sustain gastrointestinal hormone release and reduce food intake.

Using unique administrative register data, we investigate old-age retirement under the statutory pension scheme in Finland. The analysis is based on multi-outcome modelling of pensions and working lives together with a range of explanatory variables. An adaptive multi-outcome LAD-lasso regression method is applied to obtain estimates of earnings and socioeconomic factors affecting old-age retirement and to decide which of these variables should be included in our model. The proposed statistical technique produces robust and less biased regression coefficient estimates in the context of skewed outcome distributions and an excess number of zeros in some of the explanatory variables. The results underline the importance of late life course earnings and employment to the final amount of pension and reveal differences in pension outcomes across socioeconomic groups. We conclude that adaptive LAD-lasso regression is a promising statistical technique that could be usefully employed in studying various topics in the pension industry.

Fibrosing interstitial lung diseases (ILD) are a group of diseases in which healthy peripheral lung tissue is replaced by scar tissue and accumulation of extracellular matrix. The most common fibrosing ILD is idiopathic pulmonary fibrosis (IPF), a disease with a poor prognosis, with median survival of 3−5 years. In acute exacerbation of interstitial lung disease (AE-ILD), concurrently with the chronic fibrotic pattern of the lung tissue, a new change, namely diffuse alveolar damage develops. AE-ILD causes a rapidly progressive respiratory failure and often death. We searched hospital treatment periods caused by acute deterioration of respiratory symptoms in patients with fibrosing ILDs treated in Oulu University Hospital or Oulaskangas Hospital between 2008 and 2017. We also examined surgical lung tissue samples obtained from 47 IPF patients treated in our hospital. A total of 128 patients experienced an AE-ILD, of whom 62% had IPF. The median survival after AE-ILD was poorer in IPF compared with other ILDs, namely 2.6 months versus 21 months. ILD was the most common underlying cause of death of the study subjects. AE-ILD was the cause of the first respiratory-related hospital treatment period in 41% of the total of 237 patients. Other common causes were symptoms related to ILD in the absence of AE-ILD (19%), lower respiratory tract infections (22%), and cardiovascular diseases (7%). AE-ILD was associated with worse posthospitalization survival than lower respiratory tract infection. The non-IPF ILD patients whose treatment period was due to cardiovascular cause had a shorter survival than non-IPF patients with AE-ILD. We collected bronchoalveolar lavage (BAL) differential cell count data from all patients who had undergone bronchoscopy and BAL (n = 155). Occurrence of BAL basophils was associated with an increased risk for AE-ILDs and mortality. In patients who had undergone BAL during AE-ILD (n = 43), BAL neutrophils were associated with poor survival time and BAL lymphocytes with a more favorable prognosis. The mast cell density of fibrotic lung tissue areas was associated with gender, smoking, and lung diffusion capacity. The lung tissue mast cell density was significantly reduced in the patients suffering from AE-ILD. Our study provided important information on AE-ILDs in our area, which can be helpful for clinicians treating these seriously ill patients. Tiivistelmä Fibroottiset interstitiaaliset keuhkosairaudet, joita kutsutaan myös keuhkofibrooseiksi, ovat ryhmä sairauksia, joissa keuhkokudokseen muodostuu poikkeavalla tavalla sidekudosta. Yleisin näistä on idiopaattinen keuhkofibroosi (IPF), johon sairastuneenkeskimääräinen elossaoloaika on 3−5 vuotta. Keuhkofibroosin äkillisissä pahenemisvaiheissa fibroottisten muutosten rinnalle kehittyy äkillinen alveolivaurio. Tilaan liittyy nopeasti etenevä hengitysvajaus ja korkea kuolleisuus. Tutkimuksessamme etsittiin keuhkofibroosipotilaiden äkillisestä hengitystieoireiden vaikeutumisesta johtuvat ei-elektiiviset hoitojaksot Oulun yliopistollisessa sairaalassa ja Oulaskankaan aluesairaalassa vuosilta 2008–2017. Tutkimme lisäksi 47 IPF-potilaan keuhkokudosnäytteitä. 128 potilasta sai äkillisen pahenemisvaiheen. Heistä 79:llä oli IPF. IPF:n pahenemisvaiheeseen liittyi huonompi ennuste kuin muiden keuhkofibroosien pahenemisvaiheisiin, sillä mediaanielossaoloaika pahenemisvaiheen jälkeen oli 2,6 kuukautta IPF:ssä, kun se muissa keuhkofibrooseissa oli 21 kuukautta. Keuhkofibroosi oli aineistomme yleisin peruskuolinsyy. Keuhkofibroosin äkillinen pahenemisvaihe oli hoitojakson syy 41 prosentilla aineistomme 237 potilaasta. Muita syitä sairaalahoidolle olivat keuhkofibroosin vaikeutuneet oireet ilman äkillistä pahenemisvaihetta (19 %), alahengitystieinfektiot (22 %) ja sydän- ja verisuonisairaudet (7 %). Äkillisiin pahenemisvaiheisiin liittyi huonompi ennuste kuin alahengitystieinfektioihin. Sydän- ja verisuonisairauksiin liittyi suurempi kuolemanriski kuin äkillisiin pahenemisvaiheisiin niillä keuhkofibroosipotilailla, joilla oli muu kuin IPF. Tutkimuksessamme kerättiin tiedot 155 potilaan keuhkohuuhtelunestenäytteiden (BAL) solujakaumasta. BAL-huuhteen basofiilit olivat yhteydessä lisääntyneeseen äkillisten pahenemisvaiheiden riskiin ja kuolleisuuteen. Äkillisen pahenemisvaiheen aikana määritetyt BAL-huuhteen neutrofiilit olivat yhteydessä lisääntyneeseen kuolleisuuteen ja lymfosyytit taas parempaan ennusteeseen. Fibroottisten keuhkokudosalueiden syöttösolujen määrä oli yhteydessä sukupuoleen, tupakointiin ja keuhkojen diffuusiokapasiteettiin. Syöttösolujen määrä oli alentunut keuhkofibroosin äkillisessä pahenemisvaiheessa. Tutkimuksemme toi alueemme keuhkofibroosien äkillisistä pahenemisvaiheista tärkeää tietoa, jota voidaan jatkossa hyödyntää näitä potilaita hoidettaessa.

Tiivistelmä. Tämä kandidaatintyö koostuu teoriaosasta sekä kokeellisesta osiosta. Työn koostuessa sekä teoriaosasta että kokeellisesta osiosta, lukijalle tarjotaan teoriaosiossa pintaraapaisu tiedonlouhinnan sekä keinotekoisten neuroverkkojen pariin. Kerrottaessa tiedonlouhinnasta esitellään tiedonlouhinnan merkitystä nykypäivän yhteiskunnalle, jossa kerättävän ja varastoitavan datan määrä kasvaa lähes eksponentiaalisella vauhdilla. Tiedonlouhinnan yhteydessä esitellään kolme tyypillistä tiedonlouhinnan tuottamaa mallia sekä esitellään tiedonlouhinnassa käytettävä CRISP-DM-toimintamalli, jonka avulla tiedonlouhintaprosessit saavat kronologisesti etenevät suuntaviivat. Tiedonlouhinnan lisäksi työssä esitellään keinotekoisten neuroverkkojen keskeisimpiä periaatteita perehtymällä siihen, miten keinotekoiset neuroverkot todellisuudessa rakentuvat, oppivat ja vaikuttavat eri sovellusten kautta ihmisten elämään. Keinotekoisten neuroverkkojen luokittelukykyä havainnollistetaan työn kokeellisessa osiossa, jota varten työhön on etsitty luokittelua vaativa aineisto. Kokeellisessa osiossa esitellään yksi mahdollisista neuroverkkoluokittimen muodostamistavoista hyödyntäen numeerisen laskennan tietokoneohjelmisto MATLAB®:ia. MATLAB® tarjoaa suoraviivaisen menettelytavan keinotekoisten neuroverkkojen opettamiseen tietyn luokitteluongelman ratkaisemiseksi. Kokeellisen osion lopussa neuroverkkoluokittimen avulla saatua lopputulosta arvioidaan, jonka perusteella laaditaan yhteenveto keinotekoisten neuroverkkojen käytettävyydestä erilaisten luokitteluongelmien yhteydessä.
