Abstract: The trustworthiness of qualitative data has been debatable, yet it has strong support from its supporters. However, the importance and worth of qualitative data cannot be undermined. This paper presents a critical review of the trustworthiness of the qualitative data. The degree of trustworthiness of qualitative research can be measured by ensuring the credibility, transferability, dependability, and conformability of research design, process, and action. The guarantee of trustworthiness in qualitative research is more complex than in quantitative research due to its subjective nature. Many researchers and experts denied the generalizability of qualitative research. However, few researchers, like Guba (1985), developed a widely accepted model and strategies to ensure the trustworthiness and generalizability of qualitative research. Credibility is like internal validity in quantitative analysis and provides the actual data about the phenomenon. Transferability shows the degree of application of the research finding in other exact natures of context, people, groups, and settings. If the findings of one study are replicated in a similar population, condition, or context, then the findings are dependable. Neutrality is the degree of fairness of results consisting of purity of initial responses and bias-free.

2. Online survey of university students’ perception, awareness and adherence to COVID-19 prevention measures

Abstract: Abstract Background Determining factors correlated with protective measures against COVID-19 is important to improve public health response. This study describes student opinions related to university COVID-19 preventive measures. Methods In fall 2020, 643 US university students completed an online survey on perception, awareness, and adherence to COVID-19 preventive measures. Outcomes included protocol effectiveness (self or others), protocol adherence (self or others), consequences of protocol violation, knowledge of violations, and level of concern for COVID-19. Multiple linear regression models determined correlates of outcome variables. Covariates included gender, race, residence, area of study, class, and knowledge of someone with a positive COVID-19 test. Results Overall, students agreed with protective measures (equivalent to higher scores). 3. Identifying and analyzing sepsis states: A retrospective study on patients with sepsis in ICUs.

Abstract: Sepsis accounts for more than 50% of hospital deaths, and the associated cost ranks the highest among hospital admissions in the US. Improved understanding of disease states, progression, severity, and clinical markers has the potential to significantly improve patient outcomes and reduce cost. We develop a computational framework that identifies disease states in sepsis and models disease progression using clinical variables and samples in the MIMIC-III database. We identify six distinct patient states in sepsis, each associated with different manifestations of organ dysfunction. We find that patients in different sepsis states are statistically significantly composed of distinct populations with disparate demographic and comorbidity profiles. Our progression model accurately characterizes the severity level of each pathological trajectory and identifies significant changes in clinical variables and treatment actions during sepsis state transitions. Collectively, our framework provides a holistic view of sepsis, and our findings provide the basis for future development of clinical trials, prevention, and therapeutic strategies for sepsis.

4. Optimization of production and partial characterization of cellulase and protease enzymes from Aeromonas hydrophila ASM-S32

Abstract: Microbial hydrolytic enzymes, especially cellulase and protease, are widely used in industrial processes due to their low cost, large productivity, chemical stability, environmental protection, plasticity and vast availability. But existing cellulase and protease enzymes are not capable enough to fulfil the industrial demands due to the lack of adapting operational conditions. So, the present study focused on extracting enzymes from microbial sources to overcome the problems. 260 isolated bacterial strains from different areas of Sylhet, Bangladesh, were screened for their cellulolytic and proteolytic activities using Carboxymethylcellulose (CMC) and skim milk agar respectively. Among them 6 cellulolytic and 15 proteolytic bacteria were initially identified. Based on the capability to degrade CMC and skim milk, strain S32 was found to be the most potential among the isolates. Biochemical tests and molecular identification revealed that S32 is a strain of Aeromonas hydrophila and was later named as Aeromonas hydrophila ASM-S32. Maximum cellulase production by the strain A. hydrophila ASM-S32 was obtained after 18 h of incubation in a fermentation medium with an initial pH of 6.5 at 37°C and that was 4-fold higher as compared to unoptimized conditions. Maximum cellulase activity was observed at 60°C with a pH of 6.5 in presence of Ca2+ metal ion. In case of protease, optimum enzyme production was observed after 24 h of incubation with an initial pH of 8.5 at 37°C and protease production was increased by 1.2-folds when optimized conditions were used. Maximum protease activity resulted from pH 6.0, at 70°C and in presence of Cu2+ ion. [ J Adv Biotechnol Exp Ther 2019; 2(3.000): 103-113]

5. Association of Age with Thyroid Hormone Status and Ischaemic Heart Disease (IHD)

Abstract: Background: Aging is associated with increased prevalence of thyroid function abnormalities including hypothyroidism. A meta-analysis showed an increased prevalence and incidence of cardiovascular mortality only in a relatively younger population. Objective: To compare the thyroid function status in IHD patients of different age groups. Methods: This cross sectional study was carried out on 31 IHD subjects aged 35-59 years (Group B1) and 19 IHD subjects aged 60-85 years (Group B2) in the Department of Physiology, Dhaka Medical College, Dhaka from July 2009 to June 2010. For comparison 24 healthy subjects aged 35-59 years (Group A1) and 26 with age 60-85 years (Group A2) were studied. The IHD subjects were selected from coronary care unit of cardiology department and OPD of Dhaka Medical College Hospital, Dhaka. Serum FT3, FT4 and TSH of all subjects were measured by radioimmunoassay. Statistical analysis was done by unpaired Student’s ‘t’ test. Results: The mean ± SD of FT3 and FT4 were significantly lower and TSH was significantly higher in Group B1 IHD patients than that of Group A1 healthy subjects, but no difference was found between Group A2 and Group B2 and between Group B1 and Group B2. Conclusion: Thyroid hormone levels are significantly lower in younger IHD population (age 35-59 years) than the age-matched normal controls.

6. Application of Autoregressive Distributed Lag Model to Assess the Environmental Sustainability in Bangladesh

Abstract: This research inspects the associations among several macroeconomic factors in Bangladesh using yearly time series data from 1971 to 2019. It also aimed to determine if the environmental Kuznets curve (EKC) hypothesis exists in Bangladesh or not. The findings from the stationarity tests reveal that all variables are ordered I (1) integrated. The ARDL model has been used to explore the longrun and short-run connections between variables in the model. In the long term, the projected findings established the existence of the Kuznets curve hypothesis in the GDP-energy nexus. According to the study, there is a U-shaped association between economic expansion and greenhouse gas emissions. Food production and energy consumption increase emissions, however using biomass does not assist the country to attain sustainability, as an increase in biomass increases greenhouse gas emissions by 1.12%. As a result, our government should explore additional renewable energy sources to restrict emissions and take the required actions in all areas to achieve environmental sustainability. Dhaka Univ. J. 9. Fetomaternal outcome of pregnancy with COVID-19: a prospective study in a COVID dedicated hospital

Abstract: Background: Given physiological changes in immunological, pulmonary, cardiac, and coagulation systems during pregnancy, pregnant women have long been recognized as susceptible demographic during infectious disease pandemics. Learning about pregnancy outcomes, possible issues, and neonatal health issues in babies delivered to COVID-19-infected mothers is crucial. Study aimed to analyze fetomaternal outcome of pregnancy with COVID-19. Methods: This was a prospective longitudinal study done on COVID dedicated to Sylhet Shahid Shamsuddin Ahmed Hospital, from January 2021 to January 2022. A total of 54 pregnant women admitted with confirmed COVID-19 were included in this study as per inclusion criteria. Results: In this study, most patients were in the 26-33 age group (37.03%), followed by 18-25 age group (33.33%) and 24-40 age group (29.62%). 50% were primigravida. SARS COV-2 infection occurred in 51.85% of pregnant women in the 3rd trimester, 35.18% in the 2nd trimester, and 12.96% in the 1st trimester, 53.7% of patients had a mild case, 9.26% had moderate, and 37.04% had severe symptoms. 12.96% of patients needed ventilation, while 5.56% were in ICU without ventilation but on HFNC. Two patients had 1st-trimester abortion, and three had pre-term delivery. There was one IUFD, ongoing pregnancy in five patients, vaginal delivery in nine patients, and cesarean section in 25 patients. Sepsis occurred in 3.7% of patients, and post-partum hemorrhage in 1.85%. Oxygen was needed for 37.04% of patients with severe pneumonia. Out of 54 patients, 88.88% recovered, and 11.11% died in the hospital. The number of healthy babies was 74.07%, while premature babies were 5.56%, and there were two perinatal deaths. Conclusions: This study concluded that pregnant women with COVID-19 infection are at increased risk of adverse pregnancy and birth outcomes however, a low risk of congenital transmission, and the neonatal outcome was good. The availability of ICU in critical conditions is needed for better pregnancy outcomes.

10. Analysis of Assessment Design in English Language Curriculum 2006 Of Pakistan: A Qualitative Study

Abstract: Assessment is an essential component of the curriculum. The curriculum is a policy document that guides assessment mechanisms before, during, and after instruction. The curriculum envisioned assessment in standards, benchmarks, and students' learning outcomes (SLOs) by providing action words. assessment is an integral part of the learning process. Language skill development depends on the quality of assessment design and practice during and after instruction. This research study critically analyzes the language skills assessment design intended in Pakistan's language curriculum, 2006. The study aimed to explore the assessment design intended in standards, benchmarks, and students’ learning outcomes SLOs, in the language curriculum of Pakistan 2006 of secondary classes. The study's objective was to investigate the language assessment scheme of LNC, 2006, as well as understand the extent of the assess ment design of the national curriculum, 2006. It is also aimed at to what extent this design supports language assessment and responds to language assessment theories. The document analysis model of "Bowen (2009)" was adopted for this study. The study findings show that the assessment design intended in LNC, 2006 of Pakistan could be more supportive in developing language skills. The level of action verbs used for standards, benchmarks, and SLOs, is very low, almost from Bloom's Taxonomy first three-stage of the cognitive domain. Listening and speaking skills are almost neglected in assessment design. The findings of this study will inform policy developers and curriculum workers to improve the language assessment design. Additionally, the results will guide teachers in understanding language assessment design and implementation accordingly.

In this work, we developed an analytical bioinformatics framework to reveal COVID-19 comorbidities, their genomic associations, and molecular mechanisms accomplishing transcriptomic analyses of the RNA-seq datasets provided by the Gene Expression Omnibus (GEO) database, where normal and infected tissues were evaluated. Using the framework, we identified 27 COVID-19 correlated diseases out of 7,092 collected diseases. Analyzing clinical and epidemiological research, we noticed that our identified 27 diseases are associated with COVID-19, where hypertension, diabetes, obesity, and lung cancer are observed several times in COVID-19 patients. Therefore, we selected the above four diseases and performed assorted analyses to demonstrate the association between COVID-19 and hypertension, diabetes, obesity, and lung cancer as comorbidities. We investigated genomic associations with the cross-comparative analysis and Jaccard’s similarity index, identifying shared differentially expressed genes (DEGs) and linking DEGs of COVID-19 and the comorbidities, in which we identified hypertension as the most associated illness. We also revealed molecular mechanisms by identifying statistically significant ten pathways and ten ontologies. Moreover, to understand cellular physiology, we did protein-protein interaction (PPI) analyses among the comorbidities and COVID-19. We also used the degree centrality method and identified ten biomarker hub proteins (IL1B, CXCL8, FN1, MMP9, CXCL10, IL1A, IRF7, VWF, CXCL9, and ISG15) that associate COVID-19 with the comorbidities. Finally, we validated our findings by searching the published literature. Thus, our analytical approach elicited interconnections between COVID-19 and the aforementioned comorbidities in terms of remarkable DEGs, pathways, ontologies, PPI, and biomarker hub proteins.

12. Fetomaternal Outcome of Patients with Multiple Pregnancy: A Single Centre Experience

Abstract: Background: Multiple pregnancies are prone to be associated with adverse maternal and perinatal outcome. The incidence of multiple pregnancies has shown a significant increase over the last decades. Aim of the Study: This study aimed to describe the maternal and perinatal outcomes in multiple pregnancies delivered in a tertiary care hospital, Bangladesh. Methods: This was a prospective observational study; 23 patients were enrolled and analyzed. The study conducted with 23 women with twin pregnancies, over 2 year’s months from January 2020 December 2021 in the department of Gynecology and Obstetrics, Chittagong Medical College and Hospital, Chittagong, Bangladesh. Result: Out of 23 cases, 8 patients had intrapartum complications like PPH, mal-presentation, cord prolapse, low-lying placenta, and placental abruption. In this, 4(23.53%) had mal-presentation, and only one had PPH. These intrapartum complications were also categorized according to their chronicity. Mal presentations were seen in 23.53% of DCDA and 33.3% of MCDA with p=0.47. Both were not statistically significant, as shown in Table 4. These twin-specific complications were noted according to their chronicity. Discordant twin was seen in 11.76% of DCDA and 16.67% MCDA. Single IUD in 7.9% of DCDA. The complications and outcomes of the study population; more than 65% of patients needed NICU. In our study, we noted 4 perinatal death caused by intrauterine death, twin-to-twin transfusion syndrome, birth asphyxia, and respiratory distress. Conclusion: Majority of the multiple pregnancy is high risk one. So, all multiple pregnancies need early diagnosis, adequate antenatal, intra-natal and post-partum care to improve the outcome and should have mandatory hospital delivery.

13. Clinical Characteristics of Patients Underwent Frozen Section Biopsy of Ovarian Masses in BSMMU, Dhaka, Bangladesh

Abstract: Background: Women with suspected early-stage ovarian cancer need surgical staging which involves taking samples from areas within the abdominal cavity and retroperitoneal lymph nodes in order to inform further treatment. Frozen section is an important and helpful adjunct in the intraoperative diagnosis of ovarian tumors. The aim of the study was to evaluate the clinical characteristics of patients undergoing frozen section biopsy in the diagnosis of ovarian malignancies. Materials and methods: This prospective observational study included 30 cases of ovarian masses from the Department of Obstetrics and Gynecology, Bangabandhu Sheikh Mujib Medical University (BSMMU) from March 2014 to August 2014. Clinical feature, sonographic evaluation, CA-125, CA-19-9 and frozen section biopsy report were collected. Results: The mean age was 35.6±12.4 years with range from 14 to 70 years. Frozen section biopsy was true positive in 6 cases and no false positive, 1 false negative and true negative in 23 cases compared to histopathology. Frozen section biopsy has sensitivity, specificity, accuracy, positive predictive and negative predictive values were respectively, 85.7%, 100.0%, 96.7%, 100.0% and 95.8% in evaluation of ovarian masses. Conclusion: The histopathological diagnosis of the present study significantly correlated well with frozen section biopsy findings and the validity tests are almost identical as observed by many investigators. 14. An Econometric Analysis to Forecast the Food Grain Production in Bangladesh by Using ARIMA and VAR Models

Abstract: In this study, an Econometric analysis has been conducted to identify the important factors that affect the food grain productions inBangladesh. Here, we have considered time series data for the years from 1989- 1990 to 2019-2020. Vector Autoregressive (VAR)Model and Autoregressive Integrated Moving Average (ARIMA) model have been considered in this study. Both these modelshave been considered to forecast the productions of food grains in Bangladesh. The forecasting performances of these two modelshave been compared by using RMSE, MAE, and MAPE. It has been found that the VAR model is better than the ARIMA model toforecast the food grain production. On the other hand, it has been come out from the analysis that there is no significant impact ofchemical fertilizer on the food grain production, but irrigation area has significant impact on the food grain production. Abstract: Background: Distal radius fractures are one of the most common fractures patients experience. Although there are a variety of treatments, there is a lack of standardization as it relates to treatment of such fractures. Objective: In this study our main goal is to evaluate the management of distal radius fracture for adult patients in Bangladesh. Method: This observational prospective study was carried out at Chittagong Medical College Hospital and private hospitals in Chittagong from May 2015 to November 2020. Where a total of 30 Patients with unstable distal radial fracture attending at the emergency and outpatient department of Chittagong Medical College Hospital and Private Hospitals in Chittagong were included in this study. Results: During the study, according to mechanism of injury 60% patients fall on out stretched hand followed by 33.3% injuries responsible from MVA and 6.7% cases were occupational. most of the injuries belong to type II Fernandez classification, 33%. Followed by 30% injury cases belong to type II Fernandez classification, 23.3% cases belong to type I Fernandez classification, 6.7% cases belong to type IV and V classification. 86.7% cases need physiotherapy for 6 weeks. Also, according to distribution of overall assessment of anatomical outcome 80%cases were satisfactory and 20% cases unsatisfactory. Conclusion: Distal radius fractures are a common orthopedic injury. Advanced nursing practice providers either directly or indirectly participate in the care of these patients. Also, management of distal radius fracture for adult patients with volar locking plating system results in satisfactory results.

Abstract: Background: In order to improve the spontaneous Adverse Drug Reaction (ADR) reporting practice by the physicians, there is an obligatory need to investigate the current situation of ADR reporting by them. The study was conducted to observe the ADR reporting pattern among physicians in a tertiary medical college hospital. Materials and methods: This was a descriptive cross sectional study carried out in a tertiary medical college hospital during the study period of March 2020 to October 2020. Data were collected from 100 physicians working on different departments using self designed pretested questionnaire by convenient sampling technique. Results: Among the 100 respondents 70 provided response to questionnaire giving a response rate 70%. Most of the respondents (61.43%) had adequate knowledge on ADR reporting. A good number of respondents showed positive attitude but there is no practice of ADR reporting though most of the respondents (84.3%) had experienced it in last 1 year. Most of the respondents experienced ADR with antibiotic (81.35%), NSAIDs (33.89%) & anticonvulsants (15.25%) and maximum (44.06%) experienced ADR was with skin, 30.50% with GIT and 23.03% with respiratory system involvement. The cause of under reporting was mainly due to inaccessible ADR form, busy schedule, unaware of how and whom to report and lack of motivation to report. The respondents would be encouraged to report ADR if they were provided with simple & available reporting form, regular guideline & bulletin. Conclusion: There is an urgent need for educational training & seminar regarding our national online reporting system to emphasizing ADR reporting. Abstract: ObjectiveTo evaluate whether eculizumab helps patients with anti–acetylcholine receptor–positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension.MethodsPatients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study.ResultsA total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1–4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected.ConclusionEculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. Abstract: Background: Early pregnancy failure is a common complication of pregnancy. If women do not abort spontaneously, they will undergo medical or surgical treatment in order to remove the products of conception from the uterus. Curettage, although highly effective, is associated with a risk of complications; medical treatment with misoprostol is a safe and less expensive alternative. Unfortunately, after 1 week of expectant management in case of EPF, medical treatment with misoprostol has a complete evacuation rate of approximately 50%. Misoprostol treatment results may be improved by pre-treatment with mifepristone; its effectiveness has already been proven for other indications of pregnancy termination. Abstract: The purpose of this study was to understand the use of assessment formatively and its use for the improvement of instruction in the secondary school of District Quetta Balochistan. The study's major goal was to learn about teachers' perspectives on formative assessment. The other goal was to design a school-based assessment system that could be implemented. The investigator employed a parallel convergent mix design research strategy for this study. A questionnaire for quantitative data and an interview technique for qualitative data were devised and implemented. Quantitative data was examined using SPSS descriptive analysis, and qualitative data were analyzed using thematic analysis. The study discovered that teachers had a fundamental understanding of assessment, including types, strategies, instruments, and procedures. However, they did not understand and use every data formatively. The researcher made urgent recommendations for educators, investigators, curriculum developers, planners, and policymakers to incorporate formative use of assessment data to drive secondary school instruction appropriately

Abstract: In developing countries, there is a need for low-cost neurobehavioral (NB) test batteries for vulnerable populations, particularly for children exposed to environmental neurotoxicants. The objective of the current study was to assess the feasibility and test-retest reliability of the Behavioral Assessment and Research System (BARS) in children from a rural community in Bangladesh. Fifty healthy adolescents living in the Health Effects of Arsenic Longitudinal Study (HEALS) area in Araihazar, Bangladesh completed all six tests from the BARS in two test sessions scheduled two weeks apart. The BARS tests evaluated NB functions such as motor coordination, attention, memory, and information processing speed. The reliability assessment, evaluated by test-retest correlations demonstrated moderate to strong correlations (i.e., correlation coefficients ranged from 0.43 to 0.85), which were statistically significant (p < 0.05). Paired t-tests for comparing the test and retest outcomes indicated significant improvement in NB performance, highlighting learning and practice effects. NB performance improved with increasing age in most cases. Adolescent boys performed better than the girls in Finger Tapping, Digit Span, and Simple Reaction Time, whereas the girls performed better in Continuous Performance and Symbol Digit tests. The reliability scores (Pearson's correlations 0.43-0.85) were consistent with other children studies in different cultural settings. The effects of age and sex on NB tests were also consistent with findings reported in other countries. Overall, the findings of the study support the feasibility of using this computer-based test system to assess vulnerability of brain health due to environmental exposures among rural Bangladeshi children.

Abstract: Background: There is an unmet need for treatment options for generalised myasthenia gravis that are effective, targeted, well tolerated, and can be used in a broad population of patients. We aimed to assess the safety and efficacy of efgartigimod (ARGX-113), a human IgG1 antibody Fc fragment engineered to reduce pathogenic IgG autoantibody levels, in patients with generalised myasthenia gravis. Methods: ADAPT was a randomised, double-blind, placebo-controlled, phase 3 trial done at 56 neuromuscular academic and community centres in 15 countries in North America, Europe, and Japan. Patients aged at least 18 years with generalised myasthenia gravis were eligible to participate in the study, regardless of anti-acetylcholine receptor antibody status, if they had a Myasthenia Gravis Activities of Daily Living (MG-ADL) score of at least 5 (>50% non-ocular), and were on a stable dose of at least one treatment for generalised myasthenia gravis. Patients were randomly assigned by interactive response technology (1:1) to efgartigimod (10 mg/kg) or matching placebo, administered as four infusions per cycle (one infusion per week), repeated as needed depending on clinical response no sooner than 8 weeks after initiation of the previous cycle. Patients, investigators, and clinical site staff were all masked to treatment allocation. The primary endpoint was proportion of acetylcholine receptor antibody-positive patients who were MG-ADL responders (≥2-point MG-ADL improvement sustained for ≥4 weeks) in the first treatment cycle. The primary analysis was done in the modified intention-to-treat population of all acetylcholine receptor antibody-positive patients who had a valid baseline MG-ADL assessment and at least one post-baseline MG-ADL assessment. The safety analysis included all randomly assigned patients who received at least one dose or part dose of efgartigimod or placebo. This trial is registered at ClinicalTrials.gov (NCT03669588); an open-label extension is ongoing (ADAPT+, NCT03770403). Findings: Between Sept 5, 2018, and Nov 26, 2019, 167 patients (84 in the efgartigimod group and 83 in the placebo group) were enrolled, randomly assigned, and treated. 129 (77%) were acetylcholine receptor antibody-positive. Abstract: Background: In the phase III eculizumab for refractory generalized myasthenia gravis REGAIN study [ClinicalTrials.gov identifier: NCT01997229] and its open-label extension (OLE) [ClinicalTrials.gov identifier: NCT02301624], patients with treatment-refractory antiacetylcholine receptor antibody-positive generalized myasthenia gravis had clinically meaningful improvements with eculizumab versus placebo. This subgroup analysis evaluated data from patients with a recent history of chronic intravenous immunoglobulin (IVIg) use before study entry. Methods: The subgroup comprised patients who had received IVIg at least four times in 1 year, with at least one IVIg treatment cycle during the 6 months before the first REGAIN study dose. Data from REGAIN and the OLE were analyzed. Response to eculizumab versus placebo was assessed using four validated, disease-specific measures. Incidences of exacerbations and safety endpoints were recorded. Results: The subgroup had similar patient and disease characteristics as the overall REGAIN population. Clinical assessments showed sustained eculizumab efficacy during REGAIN and the OLE over 18 months. Patients receiving placebo in REGAIN experienced rapid improvements in assessment scores when treated with eculizumab in the OLE. There was a lower rate of disease exacerbations with eculizumab than with placebo during REGAIN, and eculizumab was well tolerated. Conclusion: Eculizumab treatment, compared with placebo, results in meaningful clinical improvements and fewer disease exacerbations for patients who previously received chronic IVIg. Abstract: Sepsis accounts for more than 50% of hospital deaths, and the associated cost ranks the highest among hospital admissions in the US. Improved understanding of disease states, progression, severity, and clinical markers has the potential to significantly improve patient outcomes and reduce cost. We develop a computational framework that identifies disease states in sepsis and models disease progression using clinical variables and samples in the MIMIC-III database. We identify six distinct patient states in sepsis, each associated with different manifestations of organ dysfunction. We find that patients in different sepsis states are statistically significantly composed of distinct populations with disparate demographic and comorbidity profiles. Our progression model accurately characterizes the severity level of each pathological trajectory and identifies significant changes in clinical variables and treatment actions during sepsis state transitions. Collectively, our framework provides a holistic view of sepsis, and our findings provide the basis for future development of clinical trials, prevention, and therapeutic strategies for sepsis.

Abstract: The terminal complement inhibitor eculizumab was shown to improve myasthenia gravis-related symptoms in the 26-week, phase 3, randomized, double-blind, placebo-controlled REGAIN study (NCT01997229). In this 52-week sub-analysis of the open-label extension of REGAIN (NCT02301624), eculizumab's efficacy and safety were assessed in 11 Japanese and 88 Caucasian patients with anti-acetylcholine receptor antibody-positive refractory generalized myasthenia gravis. For patients who had received placebo during REGAIN, treatment with open-label eculizumab resulted in generally similar outcomes in the Japanese and Caucasian populations. Rapid improvements were maintained for 52 weeks, assessed by change in score from open-label extension baseline to week 52 (mean [standard error]) using the following scales (in Japanese and Caucasian patients, respectively): Myasthenia Gravis Activities of Daily Living (-2.4 [1.34] and - 3.3 [0.65]); Quantitative Myasthenia Gravis (-2.9 [1.98] and - 4.3 [0.79]); Myasthenia Gravis Composite (-4.5 [2.63] and - 4.9 [1.19]); and Myasthenia Gravis Quality of Life 15-item questionnaire (-8.6 [5.68] and - 6.5 [1.93]). Overall, the safety of eculizumab was consistent with its known safety profile. Abstract: Cerebral Venous Sinus Thrombosis (CVST) in pregnancy is an uncommon condition and affects about 5 women per million. Actual etiology is difficult to confirm, a hypercoagulable state along with protein Sand C deficiency, hyperhomocystinaemia are the contributing factors. MRV like neuro imaging is the main stay of diagnosis. Treatment with Low molecular weight heparin throughout the pregnancy and post partum period can save the life of both mother and child. Abstract: Background Complement is likely to have a role in refractory generalised myasthenia gravis, but no approved therapies specifically target this system. Results from a phase 2 study suggested that eculizumab, a terminal complement inhibitor, produced clinically meaningful improvements in patients with anti-acetylcholine receptor antibody-positive refractory generalised myasthenia gravis. We further assessed the efficacy and safety of eculizumab in this patient population in a phase 3 trial. Methods We did a phase 3, randomised, double-blind, placebo-controlled, multicentre study (REGAIN) in 76 hospitals and specialised clinics in 17 countries across North America, Latin America, Europe, and Asia. Eligible patients were aged at least 18 years, with a Myasthenia Gravis-Activities of Daily Living (MG-ADL) score of 6 or more, Myasthenia Gravis Foundation of America (MGFA) class II-IV disease, vaccination against Neisseria meningitides, and previous treatment with at least two immunosuppressive therapies or one immunosuppressive therapy and chronic intravenous immunoglobulin or plasma exchange for 12 months without symptom control. Patients with a history of thymoma or thymic neoplasms, thymectomy within 12 months before screening, or use of intravenous immunoglobulin or plasma exchange within 4 weeks before randomisation, or rituximab within 6 months before screening, were excluded. We randomly assigned participants (1:1) to either intravenous eculizumab or intravenous matched placebo for 26 weeks. Dosing for eculizumab was 900 mg on day 1 and at weeks 1, 2, and 3; 1200 mg at week 4; and 1200 mg given every second week thereafter as maintenance dosing. Randomisation was done centrally with an interactive voice or web-response system with patients stratified to one of four groups based on MGFA disease classification. Where possible, patients were maintained on existing myasthenia gravis therapies and rescue medication was allowed at the study physician's discretion. Patients, investigators, staff, and outcome assessors were masked to treatment assignment. The primary efficacy endpoint was the change from baseline to week 26 in MG-ADL total score measured by worst-rank ANCOVA. The efficacy population set was defined as all patients randomly assigned to treatment groups who received at least one dose of study drug, had a valid baseline MG-ADL assessment, and at least one post-baseline MG-ADL assessment. The safety analyses included all randomly assigned patients who received eculizumab or placebo. This trial is registered with ClinicalTrials.gov, number NCT01997229. Findings Between April 30, 2014, and Feb 19, 2016, we randomly assigned and treated 125 patients, 62 with eculizumab and 63 with placebo. The primary analysis showed no significant difference between eculizumab and placebo (least-squares mean rank 56·6 [SEM 4·5] vs 68·3 [4·5]; rank-based treatment difference -11·7, 95% CI -24·3 to 0·96; p=0·0698). No deaths or cases of meningococcal infection occurred during the study. The most common adverse events in both groups were headache and upper respiratory tract infection (ten [16%] for both events in the eculizumab group and 12 [19%] for both in the placebo group). Myasthenia gravis exacerbations were reported by six (10%) patients in the eculizumab group and 15 (24%) in the placebo group. Six (10%) patients in the eculizumab group and 12 (19%) in the placebo group required rescue therapy. Interpretation The change in the MG-ADL score was not statistically significant between eculizumab and placebo, as measured by the worst-rank analysis. Eculizumab was well tolerated. The use of a worst-rank analytical approach proved to be an important limitation of this study since the secondary and sensitivity analyses results were inconsistent with the primary endpoint result; further research into the role of complement is needed. Abstract: Objectives: To evaluate perinatal outcome associated with meconium stained amniotic fluid in pregnant women.Methods: It was a prospective cross sectional study, conducted in the Department of Obstetrics and Gynecology in Bangabandhu Sheikh Mujib Medical University, Dhaka, from January 2013 to December 2013. Total 50 pregnant women admitted in the labour ward for delivery with meconium stained amniotic fluid were the study population. Singleton pregnancy of more than 34 weeks duration was included and pregnancy with APH, breech presentation, congenital malformation of fetus, IUD were excluded from the study. Out of 50 patients two did not provide all the information needed to analyze the data and hence were excluded. Outcome Variables were gestational age, antenatal checkup, medical diseases of mother (HTN, Diabetes mellitus, Heart disease), obstetric complication (oligohydramnios, prolonged labour), mode of delivery, neonatal details (weight of the baby in kg, APGAR scoring at 1 min & 5 min), neonatal resuscitation, admission in neonatal ICU(NICU), neonatal complications (RDS, MAS, Neonatal death).Results: Over half (52.1%) of the neonates needed resuscitation and 54.2% admitted in ICU. About 90% of the neonates had normal birth weight and only 10.4% were of low birth weight. 14.6% of the neonates developed meconium aspiration syndrome and 10.4% respiratory distress syndrome. Neonatal jaundice and neonatal sepsis were observed in 4.2% neonates each. Four neonates (8.3%) died early in the neonatal life, while 1 (2.1 %) was still-born. Low APGAR score (

Abstract: This paper shows a systematic & comprehensive demonstration of the methods available for analyzing production data, highlighting the advantages and limitations of each method. These methods comprise those techniques developed by Arps2, Fetkovich5, Blasingame2 and Agarwal-Gardner1 and a new method called Flowing Material Balance. Some methods yield recoverable reserves, while others give hydrocarbons-in-place. Traditional (Arps) decline analysis (exponential or hyperbolic) shows reasonable answers in many cases, but has its failings due to completely ignoring the flowing pressure data, one of the most important parameters. As a result, the reserves are underestimated or overestimated which are shown by tow examples. Abstract: Microbial hydrolytic enzymes, especially cellulase and protease, are widely used in industrial processes due to their low cost, large productivity, chemical stability, environmental protection, plasticity and vast availability. But existing cellulase and protease enzymes are not capable enough to fulfil the industrial demands due to the lack of adapting operational conditions. So, the present study focused on extracting enzymes from microbial sources to overcome the problems. 260 isolated bacterial strains from different areas of Sylhet, Bangladesh, were screened for their cellulolytic and proteolytic activities using Carboxymethylcellulose (CMC) and skim milk agar respectively. Among them 6 cellulolytic and 15 proteolytic bacteria were initially identified. Based on the capability to degrade CMC and skim milk, strain S32 was found to be the most potential among the isolates. Biochemical tests and molecular identification revealed that S32 is a strain of Aeromonas hydrophila and was later named as Aeromonas hydrophila ASM-S32. Maximum cellulase production by the strain A. hydrophila ASM-S32 was obtained after 18 h of incubation in a fermentation medium with an initial pH of 6.5 at 37°C and that was 4-fold higher as compared to unoptimized conditions. Maximum cellulase activity was observed at 60°C with a pH of 6.5 in presence of Ca2+ metal ion. In case of protease, optimum enzyme production was observed after 24 h of incubation with an initial pH of 8.5 at 37°C and protease production was increased by 1.2-folds when optimized conditions were used. Abstract: This chapter reviews research on leadership and management in three association types – grassroots-local, supralocal all volunteer, and paid-staff associations. Leaders within these associations may include board members and chairs, elected volunteer officers, committee Chairs, informal leaders, and other volunteer and paid-staff leaders. We review research pertaining to entry into leadership, leadership succession, leaders’ characteristics, leadership styles, leaders’ relationships to others (both within and outside the association), leader activities and management processes, and leader quality. Our key conclusion is that successful association leadership is usually very different from successful business, government, or nonprofit agency leadership and management. Associations must focus mainly on leading volunteers, not on managing paid staff, who have very different motivations and incentives.We close with usable knowledge, future trends, and research needed.

Abstract: Background: Aging is associated with increased prevalence of thyroid function abnormalities including hypothyroidism . A meta-analysis showed an increased prevalence and incidence of cardiovascular mortality only in a relatively younger population. Objective: To compare the thyroid function status in IHD patients of different age groups. Methods: This cross sectional study was carried out on 31 IHD subjects aged 35-59 years (Group B 1 ) and 19 IHD subjects aged 60-85 years (Group B 2 ) in the Department of Physiology, Dhaka Medical College, Dhaka from July 2009 to June 2010. For comparison 24 healthy subjects aged 35-59 years (Group A 1 ) and 26 with age 60-85 years (Group A 2 ) were studied. The IHD subjects were selected from coronary care unit of cardiology department and OPD of Dhaka Medical College Hospital, Dhaka. Serum FT 3 , FT 4 and TSH of all subjects were measured by radioimmunoassay. Statistical analysis was done by unpaired Student’s ‘t’ test. Results: Th e mean ± SD of FT 3 and FT 4 were significantly lower and TSH was significantly higher in Group B 1 IHD patients than that of Group A 1 healthy subjects, but no difference was found between Group A 2 and Group B 2 and between Group B 1 and Group B 2 . Conclusion: Thyroid hormone levels are significantly lower in younger IHD population (age 35-59 years) than the age-matched normal controls. Abstract: available in the course of the development of a gas field. Volumetric, Material Balance and Traditional Decline curve analysis are commonly used methods to estimate the reservoirs. After discovery, during initial delineation, volumetric method is used. When adequate production data are available, material balance is used to estimate the reserve. As depletion proceeds decline curve analysis is used to determine the remnant amount of reserve. However, modern estimation practice also uses type curves such as Blasingame, Fetkovich, Agarwal-Gardner as important tool to get a lucid idea about the reserve. They provide a more clear signal from the production data and quite handy to use and understand.

Abstract: Objectives: The objective of the study was to find out the incidence, indications, risk factors and outcomes of cases requiring relaparotomy following caesarean delivery during the puerperium. Material and Methods: This was a retrospective descriptive study set in a tertiary referral and teaching hospital ( Dhaka Medical College Hospital) in Dhaka, capital of Bangladesh. Results: Over a period of one year from January 1st to 31st December 2007, there were 3830 caesarean deliveries (48.43%) out of a total of 7909 deliveries. Relaparotomy was done in 24 patients (0.63%) of the caesarean sections. The indications of repeat laparotomy were secondary postpartum haemorrhage (PPH) in 11 cases(45.8%), primary PPH due to uterine atony in 8 cases (33%), uterine sepsis with haemorrhage in 1 case (4.17%), rectus sheath haematoma in 2 cases (8.3%), internal haemorrhage after caesarean section in 1 case (4.17%) and abdominal wound dehiscence in 1 case (4.17%). Of these 24 cases, in 4 cases, primary caesarean section was done in this institution while 20 had caesarean delivery at other hospitals and clinics at (5 cases) and outside Dhaka (15 cases). Main surgeries performed at relaparotomy were subtotal hysterectomy in 12 cases, total hysterectomy in 5 cases, drainage of haematoma and peritoneal toileting in 2 cases, resuturing of uterine incisions in 1 case, ligation of uterine vessels in 2 cases and internal iliac arteries in 1 case and others. More than one procedure was often performed in one case. There were 6 maternal deaths following relaparotomy caused by shock following PPH, septicaemia and internal haemorrhage. Conclusion: Repeat laparotomy within six weeks of caesarean delivery was required 1 in 200 cases in this institute. Case fatality rate was high (25%). Near miss fatalities were also common. Majority of these were preventable. Identification of risk factors, adequate attention during primary surgery, expert decision, prompt intervention and proper case management during relaparotomy will improve the outcome. 46. Serum Immunoglobulin-G Level In Children With Respiratory Tract Infection

Abstract: Introduction- Acute respiratory tract infection is one of the important cause of death in children under 5 years of age in developing countries. Perhaps they have a reduced immunologic capacity. Objectives- To observe the serum IgG level in the children suffering from respiratory tract infection in order to evaluate the immunity status in this group of population. Study design- This cross-sectional study was conducted in the Department of Physiology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Shahbagh, Dhaka from January to December 2001.Total 60 children age ranged from 3-5 years of both sexes were included in this study. Of them 30 apparently healthy children were considered as control and 30 children suffering from respiratory tract infection were considered as study group and further subdivided into 2 subgroups on the basis of presence of type of RTI. Group B1 consisted of 15 children with acute attack and B2 with recurrent attack. Method: Serum imunoglobulin G level of all the children were measured by Radial Immuno diffusion Method. Datawere analyzed by unpaired t test. Result- The mean (Â±SE) of serum IgG levels were 10.17Â±0.37, 10.05Â± 0.40 and 10.01Â± 0.52 g/l in control group and two study subgroups respectively. The data were almost similar in all the groups and no statistically significant (p > 0.05) differences were observed. Conclusion - This study reveals that there was no involvement of the IgG status with acute and recurrent respiratory tract infections in the children.
