Les mucocèles buccales sont des pathologies tumorales bénignes des glandes salivaires accessoires de la muqueuse buccale. La localisation la plus fréquente de ces lésions est la muqueuse labiale. Etiologiquement, elles sont de deux types: le premier est dû à une rupture de l’épithélium de la glande déversant de la salive dans l’espace extra-glandulaire, et forment un pseudo kyste (mucocèle par extravasation); le second est causé par blocage de l’évacuation salivaire par prolifération épithéliale du conduit excréteur, réalisant un vrai kyste salivaire (kyste de rétention). Il existe diverses modalités thérapeutiques, l’exérèse chirurgicale conventionnelle reste la stratégie la plus efficace où la récidive est la moins fréquente. A travers une observation clinique, une mise au point sur cette lésion est faite, illustrée par un cas clinique pris en charge dans le Service d’Odontologie Chirurgicale, Centre de Consultation et de Traitement Dentaire (CCTD), Rabat, Maroc.

Ranula consists of a pathological process induced by ductal disruption of the minor salivary glands followed by extravasation of mucous material surrounding adjacent structures. A swelling causing breathing and feeding problems associated with tongue displacement is frequently observed. It is a disease that generally involves the younger age group. In newborns congenital ranula may occur, an uncommon variance that differs from common ranula by not relate to post-traumatic reactions. There are reports that indicate a salivary gland duct atresia as the main cause of this pathology. The aim of this study is to analyze the clinicopathological characteristics of congenital ranula by reporting a new case report of this salivary cyst and reviewing the case reports previously published in the English literature.

Introduction A number of papers exist about post-traumatic silaocele. Iatrogenic sialocele has been described as complication of parotid surgery but there is no specific data about the cases of sialocele and salivary duct surgical injury in condylar fracture management. Presentation of case A 36-year-old man was reported to the maxillofacial emergency department for mandibular sub-condylar fracture and internal fixation of the sub-condylar fracture was planned. Postoperatively he presented a salivary cyst which was managed by conservative treatment. Discussion Iatrogenic injury to the salivary duct and sialocele formation observed in the post-operative period of surgical treatment of a condylar fracture by sub-angulomandibular approach. Maxillofacial surgeons should be aware of this complication and the importance of adopting a conservative treatment whenever it is possible. Botulinum toxin injections can be performed as a first-line treatment for these complications. Conclusion A conservative management was adopted by pressure dressings and percutaneous needle aspiration of the swelling, which was useful to absorb cystic content and helped healing., Highlights • No specific data about the cases of sialocele and salivary duct surgical injury in condylar fracture management. • The iatrogenic lesion of the salivary gland is rarely observed in trauma surgery when the subcondylar fracture is revealed. • The principle of treatment is to dry up salivary secretion. • Maxillofacial surgeons should be aware of this complication and the importance of adopting a conservative treatment whenever it is possible.

A case of chronic, recurrent and asymptomatic facial swelling in a young male is presented. Swelling extended from lower midface to upper lateral neck and right commissure to anterior massetric border. History, clinical signs and symptoms and examination pointed towards the benign nature of the swelling. Fine-needle aspiration cytology tapered the diagnostic possibilities to a salivary cyst or pseudocyst. Ultrasonography identified the lesion to contain echogenic fluid with irregular borders. "Tail sign" was absent on contrast magnetic resonance imaging, excluding the involvement of the sublingual gland. Surgical excision of the lesion was done along with submandibular gland as both were in continuity via a bottle-neck tract. Final histopathological diagnosis was that of the submandibular gland extravasation phenomenon. As per the best of our knowledge, it is the first case report of a submandibular gland extravasation causing swelling in a retrograde direction onto the face.

Adenoid cystic carcinoma is a relatively rare tumor that usually arises in the parotid and submandibular salivary glands. The initial management is surgical, and this is often combined with post-operative radiotherapy, but local relapse is common and distant metastasis is not infrequent. We experienced the case of a 59 years old male who had been previously operated on for a primary submandibular salivary cyst, and he then had a distant pulmonary metastasis 9 years later. We operated on him with performing a wedge resection on the left lower lobe for the metastatic lesion, and he hasn’t had any evidence of tumor recurrence for 84 months after the second operation.

A ranula is a mucous extravasation cyst which appears as a swelling in the submental and submandibular regions. We describe a case of congenital ranula or retention salivary cyst diagnosed at 22 weeks' gestation. The fetal growth was normal, as was the amniotic fluid volume. Follow-up ultrasound scans revealed no changes in the size or the position of the cyst. Surgical treatment was performed two days after the delivery. There were no complications and no recurrences have occurred to date.

Summary Congenital parotid salivary fistulas are unusual entities that can arise from accessory parotid glands or, even more infrequently, from normal parotid glands via an aberrant Stensen's duct. A unique case of a congenital parotid salivary fistula presented in a four-week-old infant as polycystic swelling of the buccal region, with cyst contents draining to a cutaneous pit near the oral commissure. This patient offered an initial diagnostic challenge until it became evident that the drainage represented a salivary fistula. The case is used here to highlight the overlapping clinical and pathologic features shared between type I branchial cleft anomalies and congenital salivary cysts. This case is the second report of buccal salivary cysts in structural association with a congenital parotid salivary fistula and the first such case in which a normal Stensen's duct orifice has been preserved.

Objectives/Hypothesis: The differential diagnosis of midcheek masses include pathology arising from normal anatomic structures or from variations of normal accessory parotid gland tissue. Accessory parotid gland tissue has been described as salivary tissue adjacent to Stenson's duct that is separate from the main body of the parotid gland. We report our 10-year experience with the diagnosis and treatment of eight accessory parotid gland neoplasms that have been followed by the senior authors. Study Design: This is a retrospective review of our experience with eight accessory parotid gland neoplasms. Methods: A literature review and retrospective chart review of our experience with accessory parotid gland tumors over the past 10 years. The presentation, evaluation, management, treatment, and outcome were recorded. Results: Eight cases of accessory lobe parotid tumors were identified, which have been followed since the date of initial treatment. All of the patients presented with a slowly growing cheek mass. There was one case of carcinoma expleomorphic adenoma, one case of undifferentiated carcinoma (small cell carcinoma), one case of basal cell adenocarcinoma, one case of benign salivary cyst, two cases of pleomorphic adenoma, and two cases of monomorphic adenoma. A standard facelift approach or modified Blair incision was used to excise these tumors. Conclusions: Neoplasms of the accessory parotid gland are rare. Management of these tumors include a high index of suspicion, good understanding of the anatomy, and meticulous surgical approach.

CASE DESCRIPTION A 67-year-old woman presented with dry eyes, dry mouth, intermittent polyarticular nonerosive arthropathy, bronchial and bronchiolar involvement with bronchiectasis, peripheral sensory neuropathy, cryoglobulinemic vasculitis (IgM |gk paraprotein), pronounced hypergammaglobulinemia (|gg-globulin fraction, 28.6 g/L; reference interval, 8.0–13.5 g/L), and rheumatoid factor positivity (rheumatoid arthritis latex test, 728 IU/mL; reference interval, ≤40 IU/mL). 若齢牛における唾液腺嚢腫に遭遇し、外科的摘出を行ったところ良好な経過を示した。症例は元気、食欲は良好であったが、2週間前より左の顎が徐々に腫大してきたとの稟告のもとに来院。血液所見ならびに穿刺所見より唾液腺嚢腫と診断し、摘出手術を実施した。摘出嚢腫を病理組織学的に検索したところ下顎腺由来と確認され、また嚢包内膜は小葉間導管の上皮に類似していた。その後、本症例は良好な発育を遂げ、手術の6カ月後に売却された。

We retrospectively compared fine-needle samplings (FNS) from two recently individualized low-grade papillary salivary carcinomas, polymorphous low-grade adenocarcinoma (PLGAC) and papillary cystadenocarcinoma (PCAC), to define the cytologic criteria allowing the correct diagnosis. Twelve PLGACs in 10 patients and 5 PCACs in 4 patients were investigated by FNS preoperatively. In both entities, smears contained variable proportions of malignant cells, occasionally arranged in pseudopapillary formations. In contrast to PCAC, PLGAC showed stromal fragments and hyaline globules resembling pleomorphic adenoma or adenoid cystic carcinoma. Cytologic diagnoses of malignancy were established in 10 (83.3%) PLGACs and in 4 (80%) PCACs. Two (16.7%) PLGACs were misdiagnosed as pleomorphic adenoma, and 1 (20%) PCAC as a salivary cyst.

• Labial pits are usually described as blind epidermal invaginations of lip mucosa with occasional drainage of minor salivary gland secretions. A case of an aberrant parotid duct in communication with both a unilateral, commissural labial pit and an interposed salivary cyst is presented. A 7-year-old boy who had experienced lifelong clear drainage from a congenital pit at his right oral commissure developed fever, pain, and a submucosal mass of the right cheek over a 6-week period. A preoperative sialogram through the pit demonstrated a communication between the oral commissure fistula, an intervening salivary cyst, and a proximal tract from the cyst to the parotid gland. An en bloc resection of the pit, fistula tract, and cyst was performed through a transoral approach. The dissection was medial to the buccinator muscle. There has been no recurrence of the cyst after 1 year follow-up. A description of labial pits and parotid duct anomalies is presented; however, to our knowledge, this combination of anomalies has not been previously described. A case of a salivary cyst is reported. Among the various diagnostic imaging modalities used, computed tomography was the most useful in demonstrating not only the relationship between the mass and the submandibular gland but also its nature.

An eight-year-old neutered male domestic shorthair cat had a zygomatic salivary cyst with associated mucocele formation. A fluctuating swelling developed ventral to the right eye, causing bulging of the lower eyeild, and a corresponding swelling was present in the caudal vestibule of the oral cavity. Cytological examination of the fluid obtained from aspiration of the swelling was consistent with the appearance of saliva. A tentative diagnosis of zygomatic salivary mucocele was made. Surgical exploration and resection of the swelling demonstrated the presence of a multilobular cystic zygomatic salivary gland. The histopathological appearance of the resected tissue was interpreted as an inflammatory reaction to mucus derived from a multilocular salivary cyst.
