Aim: This nationwide survey study aimed to determine the initial and definitive treatment strategies for pilonidal abscess (PA) that are used by general surgeons in Turkey. Method: Surgeons working at centers in Turkey were sent an electronic questionnaire focusing on diagnostic, perioperative, and postoperative management options for PA. The questions were prepared based on an extensive assessment of the literature and were evaluated for usability prior to distribution. A survey consisting of 20 questions was sent to surgeons and surgical residents via email. The survey link was kept active for 1 month to give the surgeons enough time to complete it. Results: Of the 520 participants, 64% defined themselves as general surgeons and 9.5% as colorectal surgeons. The most preferred surgical approaches among the participants were as follows: day surgery unit (75.2%), local anesthesia (82.8%), and drainage through the most fluctuant location (65.1%). Irrigation of the cavity was applied by 70% of the participants (38.8% with saline and 32.3% with hydrogen peroxide). The majority (82.5%) prescribed oral antibiotics following PA drainage. Definitive treatment was scheduled within a timeframe of 4-8 weeks by 45.6%. Aim: Despite the increasing popularity of minimally invasive surgery (MIS) in recent years, its efficacy in treating T4 colon cancer remains a subject of ongoing debate. This study aimed to assess the perioperative and oncological outcomes of MIS for T4 colon cancer in comparison with open surgery (OP). Method: We conducted a retrospective cohort analysis on 181 consecutive patients who underwent a T4 colon cancer resection through either MIS or OP between December 2014 and September 2021. Converted patients were evaluated in the MIS group according to the intention-to-treat principle. Propensity score matching (PSM) was employed based on age, gender, American Society of Anesthesiologists score, and the T-stage subgroup (T4a and T4b) to control for potentially confounding factors. Demographics short-term and long-term oncological outcomes were evaluated and compared between the two groups. Results: Post-PSM resulted in 49 patients in each group. BackgroundAnastomotic leakage is a major complication in colorectal surgery, resulting in significant morbidity and mortality rates. Despite substantial progress in surgical technique, anastomotic leakage rates remain stable. An early diagnosis of anastomotic leaks was proven to reduce adverse outcomes and improve survival.ObjectiveThis study aims to find a novel scoring system for detecting anastomotic leaks using inflammatory and nutritional indicators after colorectal surgery. Our purpose was to analyze the diagnostic accuracy of leak scores ((CRPPOD3)(CRPPOD1)∗preoperativealbuminlevel) in predicting postoperative complications.DesignThe study included colorectal cancer patients who underwent curative surgery at Koc University Hospital between 2014 and 2018. Patients were categorized into two groups depending on the presence of anastomotic leaks and compared in terms of preoperative albumin levels, CRP levels in postoperative days 1 and 3, anastomotic leakage rates, length of hospital stay, and CRP quotient, which was calculated by dividing POD 3 CRP level to POD 1 CRP level. The bedside leak score is calculated by dividing the CRP quotient by the preoperative albumin level. The predictive value of bedside leak score, CRP quotient, and preoperative albumin levels in estimating anastomotic leakage was analyzed, and a cutoff value for the leak score was calculated.ResultsA total of 184 patients were included in the study. The leak score, CRP POD 3–1 ratio, and preoperative albumin levels were found to successfully detect anastomotic leakage. The area under the curve for the leak score was calculated as 0.78. The optimal cutoff value was found to be 50.3 for the bedside leak score, which shows 90.9% sensitivity and 59.3% specificity.ConclusionThe leak score may represent a valuable diagnostic tool for detecting patients at risk for anastomotic leakage after colorectal surgery and planning a better strategy to reduce morbidity and mortality rates and associated costs. However, further multicenter studies with large cohorts are necessary to confirm these results.

A new perspective presents how the interaction between the material stiffness of both matrix and fibre took place to form the laminate response. Thin square simply supported balanced laminated composite plate of different ply arrangements subjected to abrupt heat flux is utilized in this investigation. The effect of ply arrangement on the elapse time for vibrations to vanish and the maximum response amplitude is also investigated. The finite element analysis results show that the response is an amplitude modulated signal that represents frequencies of both fibre and matrix. The analysis shows that the ratio of the carrier frequency to the modulating frequency matches the ratio of the fibre stiffness to ply lateral stiffness. Through the investigation, the maximum static deformation and the dynamic amplitude are illustrated for different ply arrangements. Furthermore, it is found that almost all laminates have a similar elapsed time for vibrations to vanish.

Wind Funnel Concentrator System captures wind flow from any direction, funnels it down using tapered pipes leading to a concentrator that ends in Venturi section where the turbine should be placed. This Wind Concentrator system is called INVELOX machine. Parametric design analysis is carried out to obtain the optimum design parameters of the Wind Concentrator to maximize the wind speed at the venturi section. The parameters considered are the intake shape and size, guided vanes number and their length, elbow size, venturi length, and wind direction. The design and simulation is carried out using ANSYS software package. Results show that the intake area is directly proportional to venturi air speed, and is inversely proportional to the size of the venturi cross sectional area as well as the venturi cross section length. The highest venturi air speed is obtained when the system body orientation is aligned with the wind direction within a tolerance of +/-45°. The best performance achieved when the final shape of Wind Concentrator has five guided vanes extended to the end of the funnel length. The Velocity of wind at the Venturi throat is found to increase by more than four times of the intake speed.

Background Right ventricular systolic dysfunction (RVSD) is a known risk factor for adverse outcome in surgical aortic valve replacement. Transcatheter aortic valve replacement (TAVR), on the other hand, has been shown to be either beneficial or have no effect on right ventricular systolic function. However, the prognostic significance of RVSD on TAVR has not been clearly determined. We conducted a systematic review and meta‐analysis to define the impact of RVSD on outcomes in terms of 1‐year mortality in patients with severe aortic stenosis undergoing TAVR. Methods and Results An extensive literature review was performed, with an aim to identify clinical studies that focused on the prognosis and short‐term mortality of patients with severe symptomatic aortic stenosis who underwent TAVR. A total of 3166 patients from 8 selected studies were included. RVSD, as assessed with tricuspid annular plane systolic excursion, fractional area change or ejection fraction, was found to be a predictor of adverse procedural outcome after TAVR (hazard ratio, 1.31; 95% CI, 1.1–1.55; P=0.002). Overall, we found that RVSD did affect post‐TAVR prognosis in 1‐year mortality rate. Conclusions Patients with severe, symptomatic aortic stenosis and concomitant severe RVSD have a poor 1‐year post‐TAVR prognosis when compared with patients without RVSD. Right ventricular dilation and severe tricuspid regurgitation were associated with increased 1‐year morality post‐TAVR and should be considered as independent risk factors. Further evaluations of long‐term morbidity, mortality, as well as sustained improvement in functional class and symptoms need to be conducted to determine the long‐term effects.

Background: Colorectal cancer is the third most common and second deadliest cancer globally. Increasingly, early‑onset colorectal cancer is diagnosed in those under 50 years, leading to some countries advising screenings from ages 40 or 45 years. The ’position of the tumor, right or left, impacts the characteristics and prognosis of early‑onset colorectal cancer. Objectives: Investigating the 5‑year disease‑free and overall survival and histopathologic nature of left‑ and right‑sided early‑onset colorectal cancer. Design: Retrospective study. Setting: Koc University and VKF American Hospital. Materials (Patients) and Methods: Our population includes patients under 50 years with invasive colon cancer treated in two tertiary Turkish hospitals from 2014 to 2019. Of 178 patients, 74.3% had left‑sided tumors. Sample Size: One hundred and seventy‑eight patients. Main Outcome Measures: Histopathological features of left‑ and right‑sided colorectal tumors, and the 5‑year disease‑free and overall survival rate in patients under 50 years of age. Results: The 5‑year disease‑free survival was higher in right‑sided tumors(78.2% vs. 53.1%), but overallsurvival wassimilar. No significant histopathological differences were noted between left‑ and right‑sided tumors, but right‑sided tumors had more mucinous adenocarcinoma and high microsatellite instability (MSI). Conclusion: Our data did not show a difference in survival between right‑ and left‑sided tumors in the age group <50 years contrary to other published literature. Limitations: Retrospective nature and inclusion of a single center, which usually serves Turkiye population with a higher socioeconomic status. The retrospective nature of the study makes it susceptible to selection bias. The small sample size precludes robust group matching and diminishes the statistical power, increasing the risk of Type II errors. [ABSTRACT FROM AUTHOR]

Aim: This study aimed to investigate the effects of ergothioneine and quercetin on the in vitro development of bovine embryos. Materials and Methods: Cumulus-oocyte complexes (COCs) were collected via ovary aspiration from a local abattoir and cultured in vitro for maturation. After maturation, in vitro fertilization was performed. The pronuclear embryos were divided into three groups: control, 10 ?M L-ergothioneine, and 10 ?M quercetin supplemented. After the addition of antioxidants to the CR1aa medium, in vitro culture of embryos were performed. The cleavage and morula rates were assessed on days 2 and 5, respectively. Blastocyst formation and quality were assessed on days 7-8. Results: Statistical analysis showed cleavage and morula rates were significantly higher in the ergothioneine group compared to the quercetin and control groups (P Conclusion: In conclusion, supplementation with 10 ?M ergothioneine enhanced the in vitro development of bovine oocytes. However, 10 ?M quercetin supplementation impaired development, and no blastocyst formation observed. Further studies utilizing different concentrations are warranted to better understand the effects. This study provides insights into modulating oxidative stress during in vitro embryonic production.

DC-DC converters used in LED driver applications are desired to operate with high efficiency at high switching frequencies. Class-E resonant converter, which is simple in structure and easy to implement, is especially suitable for high-frequency and low-power applications because the directly grounded switch can operate under soft switching conditions. However, when the input voltage and output power vary over a wide range in variable frequency control, the soft switching conditions are lost. To eliminate this drawback, the class-E resonant converter with synchronous rectifier is proposed as an LED driver. Although designed to operate at the fixed switching frequency, this converter structure is controlled using a hybrid method in which variable and fixed frequency switching techniques are used together because the efficiency decreases as the load becomes lighter. The converter incorporates all the advantages of conventional class-E resonant converter, while the hybrid control highlights the advantageous aspects of both variable and fixed frequency switching. To verify the validity of the driver, a prototype circuit that can operate in the 25–50 V input voltage range and whose load is a 20 W LED module is constructed. A control algorithm where the operating parameters such as switching frequency, duty ratio and synchronization angle of the converter are estimated using the polynomial regression technique is developed for closed-loop control of the driver. The driver is dynamically tested against varying input voltage and reference current across a wide range. Test results confirm the validity and feasibility of the driver, and it is observed that the polynomial regression-based control algorithm effectively and robustly follows the reference current. Furthermore, while power control of the LED module is effectively carried out throughout the entire input voltage range from 0 W to 20 W, the power switches are turned on under soft switching conditions.
