Abstract

BACKGROUND: Bicuspid aortic valve (BAV) is often associated with a concomitant aortopathy. However, few studies have evaluated the effect of the aortic valve (AV) phenotype on the rate of dilation of the aorta. This study aimed to compare the progression rate of aorta dimensions according to AV phenotype (BAV vs tricuspid AV (TAV)), fusion type and sex in patients with aortic stenosis (AS). METHODS: 310 patients with AS (224 TAV and 86 BAV) recruited in the Metabolic Determinants of the Progression of Aortic Stenosis study (PROGRESSA, NCT01679431) were included in this analysis. Doppler echocardiography was performed annually to assess AS severity and measure ascending aorta (AA) dimensions. Baseline and last follow-up visit measurements were used to assess the annualised change. RESULTS: Median AA annualised change was larger in BAV versus TAV (0.33±0.65 mm/year vs 0.21±0.56 mm/year, p=0.04). In the whole cohort, BAV phenotype and higher low-density lipoprotein (LDL) levels were significantly associated with fast progression of AA dilation in univariate analysis (OR 1.80, 95% CI 1.08 to 2.98, p=0.02; 1.37, 95% CI 1.04 to 1.80, p=0.03, respectively). AA dilation rate did not vary according to the BAV subtype (p=0.142). Abstract

Beyond 5G wireless technology Cell-Free Massive MIMO (CFmMIMO) downlink relies on carefully designed precoders and power control to attain uniformly high rate coverage. Many such power control problems can be calculated via second order cone programming (SOCP). In practice, several order of magnitude faster numerical procedure is required because power control has to be rapidly updated to adapt to changing channel conditions. We propose a Graph Neural Network (GNN) based solution to replace SOCP. Specifically, we develop a GNN to obtain downlink max-min power control for a CFmMIMO with maximum ratio transmission (MRT) beamforming. We construct a graph representation of the problem that properly captures the dominant dependence relationship between access points (APs) and user equipments (UEs). We exploit a symmetry property, called permutation equivariance, to attain training simplicity and efficiency. Simulation results show the superiority of our approach in terms of computational complexity, scalability and generalizability for different system sizes and deployment scenarios.

Abstract

Summary: Nociceptor neurons play a crucial role in maintaining the body’s homeostasis by detecting and responding to potential environmental dangers. However, this function can be detrimental during allergic reactions, as vagal nociceptors contribute to immune cell infiltration, bronchial hypersensitivity, and mucus imbalance in addition to causing pain and coughing. Despite this, the specific mechanisms by which nociceptors acquire pro-inflammatory characteristics during allergic reactions are not yet fully understood. In this study, we investigate the changes in the molecular profile of airway nociceptor neurons during allergic airway inflammation and identify the signals driving such reprogramming. Using retrograde tracing and lineage reporting, we identify a specific class of inflammatory vagal nociceptor neurons that exclusively innervate the airways. In the ovalbumin mouse model of allergic airway inflammation, these neurons undergo significant reprogramming characterized by the upregulation of the neuropeptide Y (NPY) receptor Npy1r. A screening of cytokines and neurotrophins reveals that interleukin 1β (IL-1β), IL-13, and brain-derived neurotrophic factor (BDNF) drive part of this reprogramming. IL-13 triggers Npy1r overexpression in nociceptors via the JAK/STAT6 pathway. In parallel, NPY is released into the bronchoalveolar fluid of asthmatic mice, which limits the excitability of nociceptor neurons. Single-cell RNA sequencing of lung immune cells reveals that a cell-specific knockout of NPY1R in nociceptor neurons in asthmatic mice altered T cell infiltration. Opposite findings are observed in asthmatic mice in which nociceptor neurons are chemically ablated. In summary, allergic airway inflammation reprograms airway nociceptor neurons to acquire a pro-inflammatory phenotype, while a compensatory mechanism involving NPY1R limits the activity of nociceptor neurons.

Abstract

We report an airborne gravity survey with an absolute gravimeter based on atom interferometry and two relative gravimeters: a classical LaCoste\&Romberg (L\&R) and a novel iMAR strap-down Inertial Measurement Unit (IMU). We estimated measurement errors for the quantum gravimeter ranging from 0.6 to 1.3 mGal depending on the flight conditions and the filtering used. Similar measurement errors are obtained with iMAR strapdown gravimeter but the long term stability is five times worse. The traditional L\&R platform gravimeter shows larger measurement errors (3 - 4 mGal). Airborne measurements have been compared to marine, land and altimetry derived gravity data. We obtain a good agreement for the quantum gravimeter with standard deviations and means on differences below or equal to 2 mGal. This study confirms the potential of quantum technology for absolute airborne gravimetry which is particularly interesting for mapping shallow water or mountainous areas and for linking ground and satellite measurements with homogeneous absolute referencing.

Abstract

IntroductionRespiratory syncytial virus (RSV) causes acute respiratory tract infection (ARTI) and reinfects adults throughout life, posing a risk for hospitalization in older adults (>60 years) with frailty and comorbidities.MethodsTo investigate serum and mucosal antibodies for protection against RSV infections, baseline serum samples were compared for RSV-pre- and -post-fusion (F) binding, and RSV-A2 neutralizing IgG antibodies between symptomatic RSV-ARTI (N = 30), non-RSV (RSV negative) ARTI (N = 386), and no ARTI (N = 338). Mucosal RSV-pre-F IgA and IgG levels, as well as serum RSV-G IgG antibodies, were analyzed to determine their association with protection from symptomatic RSV-ARTI in a subset study.ResultsUsing a receiver operating characteristic (ROC) analysis, we established thresholds of 1.4- to 1.6-fold change (FC) for RSV-pre-F and -post-F, and RSV-A2 neutralizing IgG antibodies, respectively, enabling the identification of asymptomatic RSV cases with high sensitivity and specificity (>80% and >90%, respectively). As a result, serum RSV-pre-F, RSV-G IgG, and mucosal pre-F binding IgA antibodies showed correlations with protection against symptomatic RSV infection. RSV-pre-F IgG antibodies were correlated with protection from RSV infections irrespective of the symptoms.DiscussionThis study provides insights into antibody-mediated protection for symptomatic RSV infection in a community-dwelling older-adult population and establishes a threshold to identify asymptomatic RSV infection using a data-driven approach.

Abstract

Cardiac troponin is extensively used as a biomarker in modern medicine due to its diagnostic capability for myocardial injury, as well as its predictive and prognostic value for cardiac diseases. However, heterophile antibodies, antitroponin antibodies, and macrotroponin complexes can be observed both in seemingly healthy individuals and patients with cardiac diseases, potentially leading to false positive or disproportionate elevation of cTn (cardiac troponin) assay results and introducing discrepancies in clinical interpretations with impact on medical management. In this review article, we describe the possible mechanisms of cTn release and the sources of variations in the assessment of circulating cTn levels. We also explore the pathophysiological mechanisms underlying antitroponin antibody development and discuss the influence exerted by macrotroponin complexes on the results of immunoassays. Additionally, we explore approaches to detect these complexes by presenting various clinical scenarios encountered in routine clinical practice. Finally, unsolved questions about the development, prevalence, and clinical significance of cardiac autoantibodies are discussed.

Abstract

Pulmonary optical endomicroscopy (POE) is an imaging technology in real time. It allows to examine pulmonary alveoli at a microscopic level. Acquired in clinical settings, a POE image sequence can have as much as 25% of the sequence being uninformative frames (i.e. pure-noise and motion artefacts). For future data analysis, these uninformative frames must be first removed from the sequence. Therefore, the objective of our work is to develop an automatic detection method of uninformative images in endomicroscopy images. We propose to take the detection problem as a classification one. Considering advantages of deep learning methods, a classifier based on CNN (Convolutional Neural Network) is designed with a new loss function based on Havrda-Charvat entropy which is a parametrical generalization of the Shannon entropy. We propose to use this formula to get a better hold on all sorts of data since it provides a model more stable than the Shannon entropy. Our method is tested on one POE dataset including 2947 distinct images, is showing better results than using Shannon entropy and behaves better with regard to the problem of overfitting. 24. Comparison of three diagnostic strategies for suspicion of pulmonary embolism: planar ventilation-perfusion scan (V/Q), CT pulmonary angiography (CTPA) and single photon emission CT ventilation-perfusion scan (SPECT V/Q): a protocol of a randomised controlled trial

Abstract

Introduction Pulmonary embolism (PE) is a challenge to diagnose and when missed, exposes patients to potentially fatal recurrent events. Beyond CT pulmonary angiography (CTPA) and planar ventilation/perfusion (V/Q) scan, single photon emission CT (SPECT) V/Q emerged a new diagnostic modality of scintigraphic acquisition that has been reported to improve diagnostic performances. To date, no management outcome study or randomised trial evaluated an algorithm based on SPECT V/Q for PE diagnosis. We present the design of a randomised multicentre, international management study comparing SPECT V/Q with validated strategies.Material and methods We will include a total of 3672 patients with suspected PE requiring chest imaging, randomised into three different groups, each using a different diagnostic strategy based on SPECT V/Q, CTPA and planar V/Q scan. Randomisation will be unbalanced (2:1:1), with twice as many patients in SPECT V/Q arm (n=1836) as in CTPA and planar V/Q arms (n=918 in each). Our primary objective will be to determine whether a diagnostic strategy based on SPECT V/Q is non-inferior to previously validated strategies in terms of diagnostic exclusion safety as assessed by the 3-month risk of thromboembolism in patients with a negative diagnostic workup. Secondary outcomes will be the proportion of patients diagnosed with PE in each arm, patients requiring additional tests, the incidence of major and clinically relevant non-major bleeding and the incidence and cause of death in each arm.Ethics and dissemination This trial is funded by a grant from Brest University Hospital and by INVENT. The study protocol was approved by Biomedical Research Ethics Committee. The investigator or delegate will obtain signed informed consent from all patients prior to inclusion in the trial. Our results will inform future clinical practice guidelines and solve the current discrepancy between nuclear medicine guidelines and clinical scientific society guidelines.Trial registration number NCT02983760

Abstract

Immunocompromised individuals, particularly autologous hematopoietic stem cell transplant (auHSCT) recipients, are at high risk for herpes zoster (HZ). We provide an in-depth description of humoral and cell-mediated immune (CMI) responses by age (protocol-defined) or underlying disease (post-hoc) as well as efficacy by underlying disease (post-hoc) of the adjuvanted recombinant zoster vaccine (RZV) in a randomized observer-blind phase III trial (ZOE-HSCT, NCT01610414). 1846 adult auHSCT recipients were randomized to receive a first dose of either RZV or placebo 50-70 days post-auHSCT, followed by the second dose at 1-2 months (M) later. In cohorts of 114-1721 participants, at 1 M post-second vaccine dose: Anti-gE antibody geometric mean concentrations (GMCs) and median gE-specific CD4[2+] T-cell frequencies (CD4 T cells expressing ≥2 of four assessed activation markers) were similar between 18-49 and ≥50-year-olds. Despite lower anti-gE antibody GMCs in non-Hodgkin B-cell lymphoma (NHBCL) patients, CD4[2+] T-cell frequencies were similar between NHBCL and other underlying diseases. The proportion of polyfunctional CD4 T cells increased over time, accounting for 79.6% of gE-specific CD4 T cells at 24 M post-dose two. Vaccine efficacy against HZ ranged between 42.5% and 82.5% across underlying diseases and was statistically significant in NHBCL and multiple myeloma patients. In conclusion, two RZV doses administered early post-auHSCT induced robust, persistent, and polyfunctional gE-specific immune responses. Efficacy against HZ was also high in NHBCL patients despite the lower humoral response.

Abstract

Plastic pollution of the ocean is a major environmental threat. In this context, a better understanding of the microorganisms able to colonize and potentially degrade these pollutants is of interest. This study explores the colonization and biodegradation potential of fungal communities on foamed polystyrene and alternatives biodegradable plastics immersed in a marina environment over time, using the Brest marina (France) as a model site. The methodology involved a combination of high-throughput 18S rRNA gene amplicon sequencing to investigate fungal taxa associated with plastics compared to the surrounding seawater, and a culture-dependent approach to isolate environmentally relevant fungi to further assess their capabilities to utilize polymers as carbon sources. Metabarcoding results highlighted the significant diversity of fungal communities associated with both foamed polystyrene and biodegradable plastics, revealing a dynamic colonization process influenced by the type of polymer and immersion time. Notably, the research suggests a potential for certain fungal species to utilize polymers as a carbon source, emphasizing the need for further exploration of fungal biodegradation potential and mechanisms.

Abstract

Non-orthogonal multiple access (NOMA) has received significant attention for future wireless networks. NOMA outperforms orthogonal schemes, such as OFDMA, in terms of spectral efficiency and massive connectivity. The joint subcarrier and power allocation problem in NOMA is NP-hard to solve in general, due to complex impacts of signal superposition on each user's achievable data rates, as well as combinatorial constraints on the number of multiplexed users per sub-carrier to mitigate error propagation. In this family of problems, weighted sum-rate (WSR) is an important objective function as it can achieve different tradeoffs between sum-rate performance and user fairness. We propose a novel approach to solve the WSR maximization problem in multi-carrier NOMA with cellular power constraint. The problem is divided into two polynomial time solvable sub-problems. First, the multi-carrier power control (given a fixed subcarrier allocation) is non-convex. By taking advantage of its separability property, we design an optimal and low complexity algorithm (MCPC) based on projected gradient descent. Secondly, the single-carrier user selection is a non-convex mixed-integer problem that we solve using dynamic programming (SCUS). This work also aims to give an understanding on how each sub-problem's particular structure can facilitate the algorithm design. In that respect, the above MCPC and SCUS are basic building blocks that can be applied in a wide range of resource allocation problems. Furthermore, we propose an efficient heuristic to solve the general WSR maximization problem by combining MCPC and SCUS. Abstract

Non-orthogonal multiple access (NOMA) is a promising radio access technology for 5G. It allows several users to transmit on the same frequency and time resource by performing power-domain multiplexing. At the receiver side, successive interference cancellation (SIC) is applied to mitigate interference among the multiplexed signals. In this way, NOMA can outperform orthogonal multiple access schemes used in conventional cellular networks in terms of spectral efficiency and allows more simultaneous users. This paper investigates the computational complexity of joint subcarrier and power allocation problems in multi-carrier NOMA systems. We prove that these problems are strongly NP-hard for a large class of objective functions, namely the weighted generalized means of the individual data rates. This class covers the popular weighted sum-rate, proportional fairness, harmonic mean and max-min fairness utilities. Our results show that the optimal power and subcarrier allocation cannot be computed in polynomial time in the general case, unless P = NP. Nevertheless, we present some tractable special cases and we show that they can be solved efficiently.

Abstract

Abstract Background Despite therapeutic advances, lung cancer remains the first cause of death from cancer. The main objective of this study was to identify risk factors associated with death within 3‐months of the first hospitalization for lung cancer in France. Methods This analysis included patients with a first hospitalization for lung cancer (between January 1, 2016 and December 31, 2018) according to diagnosis‐related groups entered into the French national medical‐administrative database. Clinical and socioeconomic parameters and characteristics of that first hospitalization were analyzed. A model predictive of early mortality was developed based on those variables. Results The 144,087 included patients were 67% men; median age of 68 [interquartile range 60–76] years; 47% had metastatic disease at diagnosis; and 34% and 23%, respectively, had received systemic treatment or undergone curative surgery. The 3‐month mortality was 19%, and significantly higher for those ≥70 versus 30 days (OR, 1.58 1.49–1.68). In contrast, no socioeconomic characteristic was associated with early mortality. Conclusion Almost 1 in 5 patients diagnosed with lung cancer in France died within 3 months post‐diagnosis. Improving survival requires diagnosis at an earlier stage and better organization of diagnosis and specific care pathways.
