- Abstract
Grazing represents the most extensive use of land worldwide. Yet its impacts on ecosystem services remain uncertain because pervasive interactions between grazing pressure, climate, soil properties, and biodiversity may occur but have never been addressed simultaneously. Using a standardized survey at 98 sites across six continents, we show that interactions between grazing pressure, climate, soil, and biodiversity are critical to explain the delivery of fundamental ecosystem services across drylands worldwide. Increasing grazing pressure reduced ecosystem service delivery in warmer and species-poor drylands, whereas positive effects of grazing were observed in colder and species-rich areas. Considering interactions between grazing and local abiotic and biotic factors is key for understanding the fate of dryland ecosystems under climate change and increasing human pressure. La pandemia COVID-19 llegó para reforzar la perspectiva del profesional de salud sobre la importancia de las medidas de bioseguridad y justificar su implementación en pro de la preservación de la salud. En el presente estudio se evaluó la adecuación de las medidas de bioseguridad aplicadas por profesionales de la odontología en Quito Ecuador durante el desarrollo de la pandemia COVID-19. A través de una investigación descriptiva con un diseño observacional de campo, de tipo transeccional contemporáneo, inicialmente se realizó un censo de las clínicas odontológicas de algunas parroquias del Distrito Metropolitano de Quito, para luego diseñar un instrumento digital de recolección de datos, validado por expertos, se aplicó mediante plataformas electrónicas (Correo electrónico y WhatsApp) otorgando un periodo de 14 días para obtener respuesta. Los resultados se analizaron cuantitativamente, se tabularon en frecuencia y porcentajes utilizando SPSS versión 19.0. De 100 odontólogos encuestados solo 66% respondió la encuesta, prevaleciendo el género femenino. Un alto porcentaje de los profesionales encuestados mostró estar actualizado y manejar las normas de bioseguridad, así como la disposición para la implementación de nuevos protocolos derivados de la pandemia. Sin embargo, se hace necesario ampliar los espacios geográficos para obtener una visión más global del comportamiento del odontólogo respecto a la bioseguridad. The COVID-19 pandemic came to reinforce the health professional’s perspective on the importance of biosecurity measures and to justify their implementation for the preservation of health. The present study evaluated the adequacy of biosecurity measures applied by dental professionals in Quito, Ecuador during the COVID-19 pandemic. Through a descriptive research with a field observational design, of a contemporary transectional. Initially, a census of the dental clinics of some parishes of the Metropolitan District of Quito was carried out, and then a digital data collection instrument was designed, validated by experts, and applied through electronic platforms (e-mail and WhatsApp) giving a period of 14 days to obtain a response. The results were analyzed quantitatively and tabulated in frequency and percentages using SPSS version 19.0. Of the 100 dentists surveyed, only 66% responded to the survey, with the female gender prevailing. A high percentage of the professionals surveyed showed that they were up to date and familiar with biosecurity norms, as well as willing to implement new protocols derived from the pandemic. La estomatitis subprotésica (ESP) asociada a Candida ha sido reportada en diversos estudios clínico-microbiológicos. La ESP grado II y III según Newton; supone el empleo de la terapia antifúngica, cuyo efecto puede verse afectado por interac ciones medicamentosas o la disminución del efecto asociado a la creciente resistencia antimicótica. De allí, que la presente revisión ofrece un acercamiento al estado del arte sobre el uso de lactobacilos probióticos como terapias coadyuvantes en el tratamiento de lesiones de ESP asociada a C. albicans. Se realizó una revisión de alcance empleando fuentes de in formación electrónica: PubMed, Scielo, LILACS y Google aca démico, para la búsqueda de estudios clínicos publicados en inglés y español durante los últimos 5 años, que describieran la acción de cepas de lactobacilos probióticos como terapia en ESP asociada a C. albicans. De la búsqueda se encontraron 2.633 artículos, luego de descartar aquellos sin relación con el objetivo y de filtrar según los criterios de inclusión y exclusión se seleccionaron 46 artículos; descartándose 33 duplicados, obteniendo finalmente 12 artículos. En líneas generales, el empleo de lactobacilos probióticos como tratamiento de ESP asociada a C. albicans mostró resultados favorables. Sin embargo, la poca cantidad de ensayos clínicos disponibles no permiten ratificar, ni extrapolar estos efectos a una población mayor, observándose también metodologías muy heterogéneas y la presencia de variables intervinientes que afectan los resultados, lo que indica la necesidad de establecer ensayos clínicos con protocolos estandarizados que conduzcan a la obtención de resultados estadísticamente significativos. Denture stomatitis (DS) associated with Candida species has been reported in several clinical and microbiological studies. DS grade II and III according to Newton; it involves the use of antifungal therapy, the effect of this one may be affected by drug interactions or a decrease in the effect associated with increasing antifungal resistance. Therefore, the present review offers an approach to the state of the art on the use of probiotic lactobacilli as adjunctive therapies in the treatment of lesions of subprosthetic stomatitis associated with C. albicans. A scoping review was carried out using the databases PubMed, Scielo, LILACS and Google Scholar, in order to search for clinical studies published in English and Spanish during the last 5 years, which described the action of probiotic lactobacilli strains such as therapy in ESP associated with C. albicans. From this search, 2,633 articles were found, after discarding those unrelated to the objective and filtering according to the inclusion and exclusion criteria, 46 articles were selected; discarding 33 duplicates, finally obtaining 12 articles. In general, the use of probiotic lactobacilli as a treatment for ESP associated with C. albicans showed favorable results. El virus de inmunodeficiencia humana (VIH), pertenece a la familia Retroviridae que infecta a las células CD4 y CD8, afectando la respuesta inmune, desencadenando complicaciones asociadas a neoplasias o infecciones causadas por microorganismos patógenos, oportunistas o comensales y cuyo resultado final es el Síndrome de Inmuno Deficiencia Adquirida (SIDA). Las infecciones fúngicas son procesos patológicos comúnmente asociados al VIH/SIDA, siendo la Candidiasis, la más frecuente en cavidad bucal. Partiendo de la premisa de investigaciones que reportan fracasos en el tratamiento de la Candidiasis, así como el aislamiento de especies no albicans como agentes causales, el objetivo de esta investigación es identificar las especies de Candida en lesiones diagnosticadas clínicamente como candidiasis bucal en 46 pacientes VIH/SIDA, mediante un estudio descriptivo, experimental, con diseño de campo y transversal. Previo consentimiento del paciente y el aval de la Directiva del Hospital, se levantó la ficha clínica estomatológica y se recolectó la muestra mediante raspado de la lesión con paletas de madera. En el laboratorio se inocularon por duplicado placas de CHROMagar™ Candida e incubaron en aerobiosis a 37 °C durante 24 a 48 horas. Se seleccionaron colonias y se realizaron pruebas confirmatorias. Se observó un predominio del género masculino (80,4%), siendo Candidiasis pseudomembranosa la más frecuente. Se confirmó la presencia de Candida en el 67% de las muestras, siendo C. albicans la especie con mayor incidencia (86.6%); también, se identificaron especies no albicans de gran interés epidemiológico por presentar resistencias intrínsecas y secundarias a los antifúngicos. The human immunodeficiency virus (HIV) belongs to the Retroviridae family that infects CD4 and CD8 cells, affecting the immune response, which triggers complications associated with neoplasms or infections caused by pathogenic, opportunistic or commensal microorganisms and whose final result is Acquired Immune Deficiency Syndrome (AIDS). Fungal infections are pathological processes commonly associated with HIV/AIDS, with Candidiasis being the most frequent in the oral cavity. Starting from the premise of investigations that report failures in the treatment of Candidiasis, as well as the isolation of non-albicans species as causative agents, the objective of this investigation is to identify the Candida species in lesions clinically diagnosed as oral candidiasis in 46 HIV patients/AIDS, through a qualitative, descriptive, experimental, non-probabilistic and cross-sectional study. With the consent of the patient and the endorsement of the Hospital Board, the stomatological clinical record was drawn up and the sample was collected by scraping the lesion with wooden pallets. In the laboratory, CHROMagar™ Candida plates were inoculated in duplicate and incubated aerobically at 37 °C for 24 to 48 hours. Colonies were selected and confirmatory tests were performed. A predominance of the male gender (80.4%) was observed, with pseudomembranous candidiasis being the most frequent. SARS-CoV-2 es el nuevo coronavirus identificado como agente causal de la enfermedad por Coronavirus COVID-19, quien fue descubierto en China y se ha responsabilizado de la pandemia más extensa en la actualidad. El tropismo del virus por las células de la lengua y del aparato respiratorio superior, hacen del ejercicio odontológico una de las profesiones de mayor riesgo, de allí que el objetivo fue describir el nivel de conocimiento que poseen los estudiantes de la FOULA sobre la enfermedad por Coronavirus (COVID-19) y las medidas de prevención en la atención odontológica. Se realizó un estudio descriptivo con un diseño observacional de campo, de tipo transeccional contemporáneo, utilizando para la recolección de datos una encuesta escrita tipo cuestionario digital auto administrado diseñado para tal fin y validado por expertos, obteniéndose una muestra de 112 estudiantes de una población aproximada de 340 estudiantes activos. El análisis estadístico de los resultados arrojó que el 64,3 % de la muestra presentó un conocimiento regular. Más del 80 % respondieron correctamente sobre el grupo más susceptible, formas de contagio, signos y síntomas y las medidas básicas de prevención indicadas por la Organización Mundial de la Salud. De igual manera señalaron conocer las medidas de prevención en la atención odontológica, aunque muestran inquietud al momento de reactivar las actividades clínicas y académicas. SARS-CoV-2 is the new coronavirus identified as the causal agent of the Coronavirus COVID-19 disease, which was discovered in China and has been responsible for the most extensive pandemic today. The tropism of the virus by the cells of the tongue and upper respiratory system, make the dental exercise one of the professions of greater risk; therefore, the objective was to describe the level of knowledge that students of the FOULA have about the disease caused by Coronavirus (COVID- 19) and prevention measures in the dental care. A descriptive study with a contemporary transectional field observational design was carried out, using for the data collection a written survey, a self-administered digital questionnaire designed for that purpose and validated by experts, obtaining a sample of 112 students from an approximate population of 340 active students. The statistical analysis of the results showed that 64,3% of the sample presented regular knowledge. More than 80% responded correctly about the most susceptible group, forms of contagion, signs and symptoms, and basic prevention measures indicated by Word Health Organization. Con el objetivo de establecer valores típicos de dosis en TC de cerebro en Clínica Bupa Reñaca, se estudió una muestra de 73 informes dosimétricos generados en un equipo TC Toshiba Aquilion 64, en términos de indicadores de dosis para TC: Índice de dosis en TC por volumen (CTDIvol) y Producto dosis longitud (DLP). Con los datos obtenidos, se estimó el valor del percentil 50 (p50) para cada indicador de dosis, y se determinaron los valores típicos de dosis en cada grupo estudiado según sexo, edad e indicación clínica. Se logró definir y establecer una metodología que permitió la obtención de los valores típicos de dosis para TC de cerebro, optimizando las dosis sin producir una disminución en la calidad de la imagen necesaria para nuestro propósito clínico.

Coronavirus diseases 2019 (COVID-19) emerged in China in December 2019 and it rapidly widespread around the world. As of May, 2020, there had been 3.618.325 confirmed cases of COVID-19, 253.381deaths and 1.184.145 recovered persons reported globally. In this review we present the evolution of the infection in Costa Rica and analyze preventive measurements taken to contain a potential explosive dissemination of COVID-19 infection. In Costa Rica, the accumulated incidence per 100.000 habitants of COVID-19 was 14.5 and the death rate 0.81 per 100.000 habitants. By May 3, the number of recovered patients were identical to those with active disease. So far, the health care system has not collapsed as most of the COVID-19 patients have being managed at home. However, the battle is not over yet. Due to the fact that re-opening of the country gradually will take and the number of new cases of undocumented immigrants, children and youngsters is steadily increased, unfavorable consequences in the following weeks might be anticipated. The Costa Rican health authorities are vigilant and diligently managing the COVID-19 pandemic.

Resumen Las fístulas anorrectales complejas son un desafío para el coloproctólogo. Son una patología frecuente que afecta la calidad de vida de los pacientes. La patogénesis aún no está clara, estarían involucradas citoquinas y el proceso de transición de epitelio a mesénquima. El gold standard para su estudio es la resonancia nuclear magnética, su uso por sí mismo disminuye la recurrencia. El objetivo del tratamiento es lograr la curación sin afectar la función del esfínter evitando las recidivas. Existen múltiples técnicas, siendo la de mayor aceptación la ligadura interesfinteriana del trayecto fistuloso, con tasa de curación sobre el 70%, con mínimo impacto en continencia. Esta revisión incluye otras técnicas como el colgajo endorrectal de avance, uso de sellante, permacol, células madres, Anal fistula plug, Video asisted anal fistula treatment, Over the scope clip y fistula laser closure. Complex anal fistula are a challenge for colorectal surgeons. It is a common pathology in population. Pathogenesis is still unclear, it would be involved citokines and the process of epitelial to eesenchymal transition. The gold standard for study is MRI, its use reduces recurrences. The goal of treatment is heal the fistula without damaging the function of the sphincter and avoid recurrences. There are multiple techniques, the most accepted is ligation of intersphincteric fistula tract with cure rate over 70%, with minimal impact in continence. This review includes other techniques like rectal advancement flap, fibrin glue, permacol, stem cells, anal fistula plug, video asisted anal fistula treatment, over the scope clip and fistula laser closure.

Abstract: Due to the increase in the use of Computed Tomography (CT), and consequently, the probability of generating a progressive increase in the dose received by the patient and its relationship in the potential risk of the effects of ionizing radiation, it is important to implement the use of diagnostic reference levels (DRLs) in CT, as a fundamental tool within a quality control program that allows the evaluation and optimization of the doses delivered to patients according to the desired clinical task. In order to establish typical dose values in brain CT at Bupa Reñaca Clinic, a sample of 73 dosimetric reports generated on a Toshiba Aquilion 64 CT unit was studied, in terms of dose indicators for CT: Dose rate on CT by volume (CTDIvol) and Dose Length Product (DLP). With the data obtained, the value of the 50th percentile (p50) for each dose indicator was estimated, and typical dose values were determined in each group studied according to sex, age and clinical indication. It was possible to define and establish a methodology that allowed obtaining typical dose values for brain CT, optimizing the doses without producing a decrease in the image quality necessary for our clinical purpose.

Actualmente se reconoce la transmisión vertical de la microbiota habitual de la madre al hijo durante la gestación; sin embargo, desde tiempos remotos se aceptó que durante el paso por el canal de parto el feto tenía su primer contacto microbiológico y era inoculado con la microbiota vaginal de la madre. Dentro del universo microbiano vaginal coexisten cerca de 300 especies de bacterias, siendo Lactobacillus el género predominante, por el cual cumplen funciones protectoras probióticas. El objetivo del presente trabajo fue comprobar la presencia de Lactobacillus spp. en muestras bucales de 17 neonatos nacidos por parto natural en la Unidad de Ginecología-Obstetricia del Hospital Sor Juana Inés de la Cruz Mérida-Venezuela durante el tercer trimestre del año 2015. Previo consentimiento de los representantes legales y el aval de la Directiva del Hospital se recolectó por hisopado una muestra de los carrillos, encías y piso de la boca, se inoculó en caldo MRS e incubó a 37 °C durante 48 horas en microaerobiosis. Posteriormente se efectuó el aislamiento en agar MRS bajo las mismas condiciones de incubación. Se seleccionaron macroscópicamente colonias típicas, se evaluaron las características microscópicas, así como las pruebas preliminares para el género Lactobacillus obteniéndose 49 aislados. Se seleccionaron 6 aislados para la identificación bioquímica resultando 2 Lactobacillus acidophilus, 1 Lactobacillus plantarum, 2 Lactobacillus paracaseissp. paracasei y 1 Lactobacillus fermentum especies ampliamente reconocidas como probióticas. Los hallazgos obtenidos demuestran la presencia de Lactobacilos en la cavidad bucal de neonatos y contribuyen a los estudios relacionados con transmisión vertical madre hijo. Recently, the vertical transmission of the usual microbiota from mother to child during pregnancy has been stablished. However, since ancient times it has been accepted that during the passage through the birth canal the fetus had its first microbiological contact and was inoculated with the vaginal microbiota of the mother. Within the vaginal microbial universe coexist about 300 species of bacteria, Lactobacillus being the predominant genus, which performs protective probiotic functions. The objective of the present research consisted in verifying the presence of Lactobacillus spp. in oral samples of 17 neonates born by natural birth in the Gynecology-Obstetrics Unit of Hospital Sor Juana Ines de la Cruz, Merida-Venezuela during the third quarter of 2015. Prior consent of the parents and endorsement of the Hospital Directive, the sample was collected by swab of the cheeks, gums and floor of the mouth, inoculated in MRS broth and incubated at 37 °C for 48 hours in microaerobiosis. Subsequently, isolation was carried out on MRS agar under the same incubation conditions. Typical colonies were selected macroscopically, microscopic characteristics were evaluated, as well as preliminary tests for the genus Lactobacillus, obtaining 49 isolates. Six isolates were selected by biochemical identification resulting in 2 Lactobacillus acidophilus, 1 Lactobacillus plantarum, 2 Lactobacillus paracaseissp. paracasei and 1 Lactobacillus fermentum species widely recognized as probiotics. BACKGROUND: Smoking has been linked to mucinous ovarian cancer, but its effects on other ovarian cancer subtypes and on overall ovarian cancer risk are unclear, and the findings from most studies with relevant data are unpublished. To assess these associations, we review the published and unpublished evidence. METHODS: Eligible epidemiological studies were identified by electronic searches, review articles, and discussions with colleagues. Individual participant data for 28,114 women with and 94,942 without ovarian cancer from 51 epidemiological studies were analysed centrally, yielding adjusted relative risks (RRs) of ovarian cancer in smokers compared with never smokers. FINDINGS: After exclusion of studies with hospital controls, in which smoking could have affected recruitment, overall ovarian cancer incidence was only slightly increased in current smokers compared with women who had never smoked (RR 1·06, 95% CI 1·01-1·11, p=0·01). Of 17,641 epithelial cancers with specified histology, 2314 (13%) were mucinous, 2360 (13%) endometrioid, 969 (5%) clear-cell, and 9086 (52%) serous. SummaryBackgroundHalf the epidemiological studies with information about menopausal hormone therapy and ovarian cancer risk remain unpublished, and some retrospective studies could have been biased by selective participation or recall. We aimed to assess with minimal bias the effects of hormone therapy on ovarian cancer risk.MethodsIndividual participant datasets from 52 epidemiological studies were analysed centrally. The principal analyses involved the prospective studies (with last hormone therapy use extrapolated forwards for up to 4 years). Sensitivity analyses included the retrospective studies. Adjusted Poisson regressions yielded relative risks (RRs) versus never-use.FindingsDuring prospective follow-up, 12 110 postmenopausal women, 55% (6601) of whom had used hormone therapy, developed ovarian cancer.
