- Abstract
Background: The outcome of patients with metastatic tumors who discontinued immune checkpoint inhibitors (ICIs) not for progressive disease (PD) has been poorly explored. We performed a meta-analysis of all studies reporting the clinical outcome of patients who discontinued ICIs for reasons other than PD. Methods: We searched PubMed, Embase and Scopus databases, from the inception of each database to December 2023, for clinical trials (randomized or not) and observational studies assessing PD-(L)1 and CTLA-4 inhibitors in patients with metastatic solid tumors who discontinued treatment for reasons other than PD. Each study had to provide swimmer plots or Kaplan–Meier survival curves enabling the reconstruction of individual patient-level data on progression-free survival (PFS) following the discontinuation of immunotherapy. The primary endpoint was PFS from the date of treatment discontinuation overall and according to tumor histotype, type of treatment and reason of discontinuation. The Combersure's method was used to estimate meta-analytical non-parametric summary survival curves assuming random effects at study level. Findings: Thirty-six studies (2180 patients) were included. The pooled median PFS (mPFS) was 24.7 months (95% CI, 18.8–30.6) and the PFS-rate at 12, 24, and 36 months was respectively 69.8% (95% CI, 63.1–77.3), 51.0% (95% CI, 43.4–59.8) and 34.0% (95% CI, 27.0–42.9). Univariable analysis showed that the mPFS was significantly longer for patients with melanoma (43.0 months), as compared with non-small cell lung cancer (NSCLC, 13.5 months) and renal cell carcinoma (RCC, 10.0 months; between-strata comparison test p-value < 0.001); for patients treated with anti-PD-(L)1 + anti-CTLA-4 as compared with anti-PD-(L)1 monotherapy (44.6 versus 19.9 months; p-value < 0.001), and in NSCLC when the reason of treatment discontinuation was elective as compared with toxicity onset (19.6 versus 4.8 months; p-value = 0.003). - Abstract
Purpose: Data on the prognostic impact of the micropapillary component in breast cancer are limited. The purpose of this study was to investigate the clinicopathological characteristics and long-term outcomes of pure and mixed invasive micropapillary breast cancer (IMPC) patients compared to invasive ductal cancer (IDC) patients. Methods: This retrospective study analysed all IMPC and IDC patients treated at the European Institute of Oncology (IEO) between 1997 and 2019. The overall cohort of IMPC patients was divided in two groups, pure and mixed IMPC. Each patient with mixed or pure IMPC was matched with one patient with IDC, based on year of surgery, age, pT, pN, and molecular subtype. Results: A total of 30,115 IDC, 120 pure IMPC and 150 mixed IMPC patients were considered eligible. Compared to IDC, pure and mixed IMPC patients presented a higher rate of locally advanced disease (pT2-T3, pN2-N3), vascular invasion, and Luminal B subtype. After matching, pure and mixed IMPC showed a significant higher rate of vascular invasion compared to IDC patients (p < 0.001). Invasive disease-free survival was better in IDC compared to pure IMPC patients (p = 0.11). Long-term overall survival was significantly worse in pure IMPC group compared to IDC group (p = 0.004), being instead similar between mixed IMPC vs matched IDC (p = 0.07). Conclusion: These real-world data reported the worse prognosis of pure IMPC compared to IDC, highlighting the peculiar prognostic value of the micropapillary subtype itself in the decision-making process of IMPC management. An accurate pre-surgical diagnostic evaluation and a multidisciplinary approach are pivotal to best personalize its treatment.

- Abstract
Few studies have been published on the long-term outcomes of patients with gastric neuroendocrine tumors (gNETs). We analyzed their management over a two-decade period, focusing on endoscopic and clinical outcomes. Clinical, laboratory, endoscopic, surgical, and histopathological data from Types 1 and 3 gNETs histologically diagnosed between March 2000 and December 2021 at the European Institute of Oncology (IEO, Milan) were retrospectively collected. Sixty-nine patients were included (60 Type 1, 9 Type 3): 53 (77%) were treated endoscopically, 6 (9%) surgically, and 10 (14%) did not receive any treatment. Overall, 293 lesions were removed endoscopically: 74% by forceps, 20% by endoscopic mucosal resection (EMR), and 5% by endoscopic submucosal dissection (ESD). No differences were observed between EMR and ESD in terms of complete resection rate (p value = .50) and complications rate (p value = .084). The median follow-up period was 5.8 years (range: 0.3-20.5), during which no gNET-related deaths were observed. Metachronous gNETs developed in 60% of patients with Type 1 gNET. Six patients with lymph node metastases (LNM) were younger (p value = .006) and had larger lesions (p value <.001) than patients without LNM. Most Type 1 gNETs were successfully excised using forceps, with EMR and ESD being equally effective. The presence of incomplete resection was not associated with a worse prognosis, which remains excellent in this highly recurrent disease. Younger age and a size ≥10 mm were associated with an increased risk of LNM. CLINICAL TRIAL REGISTRATION: Project code UID 2854.

12. Surrogate endpoints for overall survival in randomized clinical trials testing immune checkpoint inhibitors: a systematic review and meta-analysis

Introduction: There is debate on which are the best surrogate endpoint and metric to capture treatment effect on overall survival (OS) in RCTs testing immune-checkpoint inhibitors (ICIs). Methods: We systematically searched for RCTs testing ICIs in patients with advanced solid tumors. Inclusion criteria were: RCTs i) assessing PD-(L)1 and CTLA-4 inhibitors either as monotherapy or in combination with another ICI, and/or targeted therapy, and/or chemotherapy, in patients with advanced solid tumors; ii) randomizing at least 100 patients. We performed a meta-analysis of RCTs to compare the surrogacy value of PFS and modified-PFS (mPFS) for OS in RCTs testing ICIs, when the treatment effect is measured by the hazard ratio (HR) for OS, and by the HR and the ratio of restricted mean survival time (rRMST) for PFS and mPFS. Results: 61 RCTs (67 treatment comparisons and 36,034 patients) were included in the analysis. In comparisons testing ICI plus chemotherapy, HRPFS and HRmPFS both had a strong surrogacy value (R2 = 0.74 and R2 = 0.81, respectively). In comparisons testing ICI as monotherapy, HRPFS was the best surrogate, although having a moderate correlation (R2 = 0.58). In comparisons testing ICI plus other treatment(s), the associations were very weak for all the surrogate endpoints and treatment effect measures, with R2 ranging from 0.01 to 0.22. Conclusion: In RCTs testing ICIs, the value of potential surrogates for HROS was strongly affected by the type of treatment(s) tested. The evidence available supports HRPFS as the best surrogate, and disproves the use of alternative endpoints, such as the mPFS, or treatment effect measures, such as the RMST.

13. Sodium glucose co-transporter 2 inhibitors and quality of life in patients with heart failure: a comprehensive systematic review and meta-analysis of randomized controlled trials

- Abstract
Background Sodium glucose co-transporter 2 inhibitors (SGLT2i) are one of the cornerstones of heart failure (HF) therapy. While benefits in terms of HF hospitalizations and death are well established, their impact on quality-of-life (QoL) has not been systematically investigated. Objective This systematic review and meta-analysis aims to evaluate the impact of SGLT2i treatment on QoL in patients with HF, by analysing data from randomized clinical trials (RCTs). Methods We identified a total of 23 RCTs that investigated the role of SGLT2i on quality of life in patients with HF, irrespective of their left ventricular ejection fraction (LVEF). RCTs that used Kansas City Cardiomyopathy Questionnaire overall summary score (KCCQ-OSS) to assess QoL and had a minimum follow-up of 3 months were included. The difference in mean change of the KCCQ-OSS between the SGLT2i group and the standard of care (SOC) group at 3 and 6 months from baseline was considered as the outcome measure. Findings Fourteen RCTs (21 737 patients) were included in the analysis. A significant improvement in KCCQ-OSS over time (p < 0.001) was observed in both patients receiving SOC and those receiving SGLT2i in addition. The pooled estimate showed a significant improvement of 1.94 points [95% confidence interval (CI), 1.41-2.46] in KCCQ-OSS mean change at 3 months and of 2.18 points (95% CI, 1.13-3.24) at 6 months from baseline, with SGLT2i compared to SOC alone, irrespective of LVEF. A greater improvement in KCCQ-OSS was observed among patients with a recent episode of worsening HF compared to those with chronic stable HF. Conclusions Among patients with HF, irrespective of their LVEF and clinical status, the addition of SGLT2i to SOC demonstrated a significant improvement in quality of life as early as at 3-month follow-up.

- Abstract
Introduction: To provide evidence explaining the poor association between pCR and patients’ long-term outcome at trial-level in neoadjuvant RCTs for breast cancer (BC), we performed a systematic-review and meta-analysis of all RCTs testing neoadjuvant treatments for early-BC and reporting the hazard ratio of DFS (HRDFS) for the intervention versus control arm stratified by pathological response type (i.e., pCR yes versus no). Methods: The objective was to explore differences of treatment effects on DFS across patients with and without pCR. We calculated the pooled HRDFS in the two strata of pathological response (i.e., pCR yes versus no) using a random-effects model, and assessed the difference between these two estimates using an interaction test. Results: Ten RCTs and 8496 patients were included in the analysis. Patients obtaining pCR in the intervention-arm had a higher, although not statistically significant, risk of DFS-event as compared with patients obtaining pCR in the control-arm: the pooled HRDFS for the experimental versus control arm was 1.23 (95%CI, 0.91–1.65). On the opposite, the risk of DFS-event was higher for control as compared with the intervention-arm in the stratum of patients without pCR: the pooled HRDFS was 0.86 (95%CI, 0.78–0.95). Treatment effect on DFS was significantly different according to pathological response type (interaction test p: 0.014). Conclusion: We reported new evidence that contributes to explaining the poor surrogacy value of pCR at trial-level in neoadjuvant RCTs for early-BC.

- Abstract
Spirometry is a pulmonary function test where correct interpretation of the results is crucial for accurate diagnosis of disease. There are online tools to assist in the interpretation of spirometry results; however, as yet none are validated. We evaluated the interpretation accuracy of the Espiro app using pulmonologist interpretations as the gold standard. This is an observational descriptive study in which 118 spirometry results were interpreted by the Espiro app, two pulmonologists, two primary care physicians, and two residents of a primary care training program. We determined the interpretation accuracy of the Espiro app and the concordance of the pattern and severity interpretation between the Espiro app and each of the observers using Cohen's kappa coefficient (k). We obtained a sensitivity and specificity for the Espiro app of 97.5% (95% confidence interval (CI): 86.8-99.9%) and 94.9% (95%CI: 87.4-98.6%) with pulmonologist 1 and 100% (95%CI: 91.6-100%) and 98.7% (95%CI: 92.9-99.9%) with pulmonologist 2. The concordance for the pattern interpretation was greater than k 0.907, representing almost perfect agreement. The concordance of the severity interpretation was greater than k 0.807, representing substantial to almost perfect agreement. We concluded that the Espiro app is a valid tool for spirometry interpretation.

Purpose: Neoadjuvant chemotherapy (NAC) significantly improved the prognosis of patients with locally advanced gastric cancer (LAGC). Several biomarkers, including HER2 and MMR/MSI are crucial for treatment decisions in the advanced stage but, currently, no biomarkers can guide the choice of NAC in clinical practice. Our aim was to evaluate the role of MSI and HER2 status on clinical outcomes. Methods: We retrospectively collected LAGC patients treated with NAC and surgery +/- adjuvant chemotherapy from 2006 to 2018. HER2 and MSI were assessed on endoscopic and surgical samples. Pathologic complete response (pCR) rate, overall survival (OS), and event‐free survival (EFS) were estimated and evaluated for association with downstaging and MSI. Results: We included 76 patients, 8% were classified as MSI-H, entirely consistent between endoscopic and surgical samples. Six percent of patients were HER2 positive on endoscopic and 4% on surgical samples. Tumor downstaging was observed in 52.5% of cases, with three pCR (5.1%), none in MSI-H cancers. According to MSI status, event-free survival (EFS) and overall survival (OS) were higher for MSI-H patients to MSS [EFS not reached vs 30.0 months, p = 0.08; OS not reached vs 39.6 months, p = 0.10]. Conclusion: Our work confirms the positive prognostic effect of MSI-H in the curative setting of LAGC, not correlated with pathologic tumor downstaging. Prospective ad-hoc trial and tumor molecular profiling are eagerly needed.

- Abstract
Background: Studies on SARS-CoV-2 conducted in confined settings for prolonged times allow researchers to assess how the coronavirus spreads. San Patrignano (SP), Italy, is the largest European drug rehabilitation facility. Methods: Between 15 October and 31 December 2020, all SP residents were tested for SARS-CoV-2. We analyzed the relationships between individual characteristics and being SARS-CoV-2-positive. Three selected predictive models were used to calculate the number of expected hospitalizations. For each model, we summed the estimated individual risks to obtain the expected number of hospitalizations in our sample, and we tested whether the observed and expected numbers differed. Results: Of 807 residents, 529 (65.6%) were SARS-CoV-2-positive. Of these 323 (61.1%) were symptomatic. A strong relationship was found between being positive and living connections (p-value < 0.001). No statistically significant relationship was found with age, sex, smoking history, or comorbidities. Although 9 to 17 hospitalizations were expected, no hospitalizations were observed (p-value < 0.001). No one died of COVID-19. Conclusions: The peculiar characteristics of SP residents or the SP environment might at least partially explain the null hospitalization rates. Despite the extreme uniqueness of our population and despite the protected environment and all precautions that were taken, the fact that the virus was able to circulate and infect a large portion of the population highlights the fundamental role of social interactions in the spread of the disease.

Background: Controversy exists regarding the optimal duration of the extended adjuvant endocrine treatment (ET) in patients with early-stage breast-cancer (eBC). We performed a systematic review and trial-level meta-analysis of all randomized clinical trials (RCTs) comparing a “limited-extended” adjuvant ET (defined as more than 5 but less than 7.5 years of treatment overall) versus a “full-extended” adjuvant ET (defined as more than 7.5 years of treatment overall) in eBC. Methods: To be eligible, RCTs had to i) compare a “limited-extended” adjuvant ET versus a “full-extended” adjuvant ET in patients with eBC; and ii) report disease-free survival (DFS) hazard ratio (HR) according to the disease nodal-status [i.e., nodal-negative (N-ve) versus nodal-positive (N + ve)]. The primary endpoint was to assess the difference in efficacy of full-versus limited-extended ET, measured in terms of the difference in DFS log-HR, according to the disease nodal-status. Secondary endpoint was the difference in efficacy of full-versus limited-extended ET according to tumor size (i.e., pT1 vs pT2/3/4), histological grade (i.e., G1/G2 vs G3), patients’ age (i.e., ≤60 vs > 60 years) and previous type of ET (i.e., aromatase inhibitors vs tamoxifen vs switch strategy). Results: Three phase III RCTs fulfilled the inclusion criteria. A total of 6689 patients were included in the analysis, of which 3506 (53%) had N + ve disease. The full-extended ET provided no DFS-benefit as compared with the limited-extended ET in patients with N-ve disease (pooled DFS-HR = 1.04, 95%CI: 0.89 to 1.22; I2 = 18%). Conversely, in patients with N + ve disease the full-extended ET significantly improved DFS, with a pooled DFS-HR of 0.85 (95%CI: 0.74 to 0.97; I2 = 0%). There was a significant interaction between the disease nodal-status and the efficacy of the full-versus limited-extended ET (p-heterogeneity = 0.048). The assessment of bacterialiferous Asian citrus psyllid (ACP) frequency is important for (i) studies of bacteria acquisition and inoculation by ACP, (ii) disease detection in disease free areas but with ACP presence, (iii) efficiency evaluation of inoculum reduction strategies, (iv) evaluation of frequency of Candidatus Liberibacter asiaticus (Las)-positive ACP and the abundance of inoculum sources or putative new HLB infections relationships. Depending on the conditions and time of storage of collected psyllids, Las DNA in ACP could degrade and Las-false negative results might occur. Thus, this study was conducted to evaluate the detection of Las in ACP adults submitted to different storage methods and time of storage by real-time PCR (qPCR). Two 2x3x7 factorial experiments were conducted. Factors were ‘Ethanol’ (with or without 70% ethanol), ‘Temperature’ (-20°C, 4°C and 26°C) and ‘Time’ (0, 3, 7, 14, 21, 28 and 35 days). For each treatment, 20 samples with 3 ACP adults from nymphs reared on Las infected trees were tested for Las presence by qPCR. No significant differences in percentages of psyllids samples positive for Las were observed among the storage methods up to 35 days, except a slight trend of decline in Las detection in samples storage without ethanol at 26°C after 14 days of storage.

The assessment of bacterialiferous Asian citrus psyllid (ACP) frequency is important in epidemiological and management studies because it can be related with the abundance of inoculum sources and with putative new HLB infections. For that, ACP can be collected directly or on yellow sticky traps (YST) commonly used by Brazilian growers to monitor psyllid population. The YST are usually left in the field for 2 weeks after which time YST are visually evaluated for the ACP presence, and if present, the psyllids are removed from the YST and tested by real-time PCR (qPCR) for liberibacter presence. Previous studies in Florida showed that the incidence of Las-positive ACP declined with increasing time on the YST (Irey et al., 2011). Thus, the objective of this work was to determine if time ACP is keep on YST affects qPCR results for Las and if it was related to weather conditions during winter and summer of Araraquara-SP (Brazil). ACP adults from nymphs reared on Las infected trees were placed on YST (BUG-Agentes Biológicos) in the field and 20 samples with 3 individuals were tested after 0, 1, 3, 9, 12 and 15 days. The results were compared with samples directly collected without trap glue. Experiments were done in June, July and August (winter) and in January, February and March (summer). In contrast with previous report in Florida, no difference on the incidence of Las-positive ACP samples was observed up to 15 days on the YST in both seasons.

Pachacamac fue una de las principales y más respetadas deidades del antiguo Perú; su poder se expresaba en la convocatoria a peregrinos, que concurrían a su espacio sagrado a gestionar sus súplicas. El santuario de Pachacamac está ubicado frente al litoral del Océano Pacífico, en el departamento de Lima-Perú; en el conjunto de sus estructuras monumentales destacan, entre otros, el Templo Viejo, el Templo Pintado y el Templo del Sol. La conquista española no significó el abandono total del sitio de Pachacamac, pues continuó siendo un santuario y centro de peregrinación aunque se produjo un cambio que se documenta en el registro material recuperado en nuestras excavaciones en diferentes edificios. Este artículo se centra en las investigaciones del equipo del Museo Pachacamac y aborda el estado de la cuestión de la arqueología de la temprana colonia en el sitio y un análisis de los hallazgos recientes y un debate sobre su largo tiempo de continuidad en su ocupación y sacralidad.

Les institucions de la Cerdanya, tant de la banda catalana com de la francesa, conscients de la importància estratègica que té el fet de conservar el seu paisatge i potenciar-ne la qualitat, l’any 2012 van sumar esforços a l’hora de pensar i d’establir plegats les bases de futur per posar-lo en valor, amb la complicitat dels principals agents del territori. El resultat n’és el Pla de Paisatge Transfronterer de la Cerdanya, una eina de coneixement, d’acció, de sensibilització i de compromís, que aplica els principis de cooperació europea establerts pel Conveni Europeu del Paisatge (CEP). La metodologia d’elaboració del pla, desenvolupada conjuntament pel Parc Natural Regional dels Pirineus Catalans i l’Observatori del Paisatge de Catalunya, es fonamenta en la concertació amb els principals actors del territori. El Pla intenta repensar el territori des de la unitat de paisatge, en un moment en què les administracions locals busquen fórmules alternatives i imaginatives d’ordenació i de gestió del paisatge.

- Abstract
Purpose: Despite extensive research on cancer and work-related outcomes, evidence from longitudinal cohort studies is limited, especially in young women with breast cancer (BC). We aimed to investigate employment trajectories in young BC survivors and to identify potential factors associated with changes in work activity. Methods: The HOHO European prospective multicenter cohort study enrolled 300 young women (≤ 40 years) with newly diagnosed BC. Women completed surveys at baseline and every 6 months for 3 years, then yearly for up to 10 years to assess, among other variables, employment status, sociodemographic, medical, and treatment data. Symptoms were assessed by the Breast Cancer Prevention Trial symptom scales and single items from the Cancer Rehabilitation Evaluation System. Univariable and multivariable multinomial logistic regression analyses identified factors associated with changes in employment status. Results: Among the 245 women included in this analysis, 85% were employed at the last individual post-baseline assessment (1 to 10 years). At 5 years, women had a 29.4% probability (95% CI: 23.6–35.5) of experiencing any reduction and a 14.9% probability (95% CI: 10.6–19.9) of experiencing any increase in work activities. Being enrolled in Switzerland (vs. Italy) and reporting more trouble in performing daily activities were significantly associated with work reduction. Conclusion: Our results suggest that most young BC survivors remain employed in the long-term. Implications for Cancer Survivors: Regular evaluation of symptoms which may interfere with daily life and identification of financial discomfort is critical in providing timely and individually tailored interventions and in limiting unwanted reductions in work activities.

- Abstract
Background: We retrospectively investigated the role of (neo)adjuvant chemotherapy in patients with primary, localized angiosarcoma. Methods: We selected all patients with primary, localized angiosarcoma, who had received radical surgery between January 2005 and December 2019 at 33 European sarcoma reference centers. The primary objective was to compare the outcome of patients who received (neo)adjuvant chemotherapy versus those who did not, in terms of overall survival (OS), disease-free survival (DFS) and distant metastasis-free survival (DMFS). To reduce the risk of confounding due to imbalance, a propensity-score matching(PSM) was performed. Finally, subgroups analysis was performed according to tumor site, tumor size (< 50 mm or ≥ 50 mm) and patients predicted 10-years OS according to the nomogram sarculator (two different cutoff-values were applied: ≤ 33% or > 33% and < 60% or ≥ 60%). Results: 362 patients were analyzed: 149 (41.2%; treated group) received (neo) adjuvant chemotherapy and 213 (58.6%; control group) did not. The median follow-up for the OS endpoint was 5.1 years (95% CI: 4.0–5.5). The OS-HR was 0.58 (95%CI: 0.40–0.83; p-value = 0.003) in the univariate analysis and 0.74 (95% CI: 0.38–1.43; p = 0.367) in the PSM analysis. The DFS-HR was 0.75 (95% CI: 0.57–0.98; p-value = 0.036) in the univariate analysis, and 0.91 (95% CI:0.56–1.48; p-value = 0.7) in the PSM analysis. The DMFS-HR was 0.75 (95% CI: 0.55–1.02; p-value = 0.065) in univariate analysis and 0.92 (95% CI: 0.53–1.61; p-value = 0.769) in the PSM analysis. Subgroup analysis revealed no heterogeneity of results in strata of tumor site. On the contrary, there was a trend for heterogeneity according to tumor size and patient's risk of death. For all the endpoints analyzed, patients with tumors smaller than 50 mm or at lower risk of death seem to have no benefit from chemotherapy, while patients with larger tumors or at higher risk of death at 10 years seem to derive substantial benef

Aims: Despite evidence of hemodynamic benefit of sodium nitroprusside (SNP) treatment for acute heart failure (AHF), there are limited data about its efficacy and safety. This study aimed to assess the effectiveness and safety of SNP treatment, to explore the impact of N-terminal pro-B natriuretic peptide (NT-proBNP) reduction on clinical endpoints and to identify possible predictors of clinical response. Methods and results: Multicenter retrospective cohort study of 200 patients consecutively admitted for AHF in 2 Italian Centers. Primary endpoint was the reduction of NT-proBNP levels ≥25% from baseline values within 48 h from the onset of SNP infusion. Secondary and safety endpoints included all-cause mortality, rehospitalization for HF at 1, 3 and 6 months, length of hospital stay (LOS) and severe hypotension. 131 (66%) patients experienced a NT-proBNP reduction ≥25% within 48 h from treatment onset, irrespective of initial systolic blood pressure (SBP). Left ventricular end diastolic diameter (LVEDD) was the only independent predictor of treatment efficacy. Patients who achieved the primary endpoint (i.e., ‘responders’) had lower LOS (median 15 [IQR:10–27] vs 19 [IQR:12–35] days, p-value = 0.033) and a lower incidence of all-cause mortality and rehospitalization for HF at 1 and 3 months compared to “non responders” (p-value <0.050). Severe hypotension was observed in 10 (5%) patients, without any adverse clinical consequence. Conclusion: SNP is a safe and effective treatment of AHF, particularly in patients with dilated left ventricle. Reduced NT-proBNP levels in response to SNP is associated to shorter LOS and lower risk of 1- and 3-month re-hospitalizations for HF. Clinical trial registration. http://www.clinicaltrials.gov. - Abstract
Background: Patients with advanced type B3 thymoma and thymic carcinoma resistant to chemotherapy have few treatment options. We report the efficacy and safety results of the combination of the anti-PD-L1 inhibitor avelumab with the anti-angiogenesis drug axitinib in patients with advanced type B3 thymoma and thymic carcinoma. Methods: CAVEATT was a single-arm, multicentre, phase 2 trial, conducted in two Italian centres (the European Instituteof Oncology and the Humanitas Institute, Milan) in patients with histologically confirmed type B3 thymoma or thymic carcinoma, with advanced stage of disease who had progressed after at least one line of platinum-based chemotherapy. Previous treatment with an anti-angiogenesis drug was allowed but not with immune checkpoint inhibitors. Other inclusion criteria were age 18 years or older, an Eastern Cooperative Oncology Group performance status of 0–2, progressive disease, and presence of measurable disease according to Response Evaluation Criteria In Solid Tumours (RECIST) version 1.1. Patients received avelumab 10 mg/kg intravenously every 2 weeks and axitinib 5 mg orally twice daily until disease progression or unacceptable toxicity. The primary endpoint was the centrally assessed overall response rate according to RECIST version 1.1. Patients who received at least one cycle of treatment and had at least one CT scan after treatment start at scheduled time point by protocol were judged assessable for response and were included in efficacy and safety analyses. This study is registered with EUDRACT, 2017–004048–38; enrolment is completed and follow-up is ongoing. Findings: Between April 22, 2019, and June 30, 2021, 32 patients were enrolled. 27 patients had a thymic carcinoma, three a type B3 thymoma, and two a mixed type B3 thymoma and thymic carcinoma. 29 (91%) of 32 patients had stage IVB disease and 13 (41%) of 32 had been pretreated with an anti-angiogenesis drug. 11 of 32 patients had an overall response; thus the overall

Aquest fet ha agreujat veloçment la salut mental de la població, en concret, entre les franges més joves. La present memòria cerca i proposa un seguit d’intervencions per millorar la salut mental dels ciutadans a través del Centre de Salut Mental de Barcelona. Els plantejaments que aquí es mostren parteixen de la base teòrica i projectual realitzada durant tot el màster habilitant, a través d’una arquitectura que pretén donanr nous significats al Parc del Laberint d’Horta i al Campus Mundet a través d’una visió contemporània, tractant qüestions d’estil, llenguatge, encaix urbanístic i preservació patrimonial., The issue of Mental Health has become one of the most important social affairs that we have to deal with and find solutions for today as a society. This fact has rapidly worsened the mental health of the population, specifically among the younger groups. This report seeks and proposes a series of interventions to improve the mental health of citizens through the Mental Health Center of Barcelona. The approaches that are shown here start from the theoretical and project basis carried out throughout the master’s degree, through an architecture that aims to give new meanings to the Parc del Laberint d’Horta and the Mundet Campus through a contemporary vision, dealing with issues of style, language, urban fit and heritage preservation.

Internal waves are commonly observed in oceans and lakes where high stratification exists. In the present study in the Strait of Gibraltar, we analyse internal soliton signals recorded in different locations in their eastward propagation from their release point (Camarinal Sill) to the continental slope of the northwestern Alboran Sea. Moreover, the effect of subinertial forcing on the release of solitons is also explored. The internal soliton activity was assessed from different approaches: (i) in-situ data (i.e., current and temperature measurements or High-Frequency Radar), (ii) numerical modelling, and (iii) an analytical approach. The arrival of solitons over the continental slope of the north-western Alboran Sea showed fortnightly variability in both number (occurrence) and amplitude during spring tides when compared with during neap tides. The observed arrival times of the solitons oscillated between 14 (spring tides) and 20 h (neap tides). Nevertheless, to provide a comprehensive explanation for the fluctuations in travel times, it is necessary to consider the subinertial variability driven by atmospheric forcing, which impacts both the flow in the Strait of Gibraltar and the mesoscale patterns in the Alboran Sea (e.g., the Coastal Cyclonic Gyre).

- Abstract
Background: First responder programs were developed to speed up access to cardiopulmonary resuscitation and defibrillation for out-of-hospital cardiac arrest (OHCA) victims. Little is known about the factors influencing the efficiency of the first responders arriving before the EMS and, therefore, effectively contributing to the chain of survival. Objectives: The primary objective of this retrospective observational study was to identify the factors associated with first responders' arrival before EMS in the context of a regional first responder program arranged to deliver automated external defibrillators on suspected OHCA scenes. Methods: Eight hundred ninety-six dispatches where FRs intervened were collected from 2018 to 2022. A robust Poisson regression was performed to estimate the role of the time of day, the immediate availability of a defibrillator, the type of first responder, distances between the responder, the event and the dispatched vehicle, and the nearest available defibrillator on the probability of responder arriving before EMS. Moreover, a geospatial logistic regression model was built. Results: Responders arrived before EMS in 13.4% of dispatches and delivered a shock in 0.9%. The immediate availability of a defibrillator for the responder (OR = 3.24) and special categories such as taxi drivers and police (OR = 1.74) were factors significantly associated with the responder arriving before EMS. Moreover, a geospatial effect suggested that first responder programs may have a greater impact in rural areas. Conclusions: When dispatched to OHCA scenes, responders already carrying defibrillators could more probably reach the scene before EMS. Special first responder categories are more competitive and should be further investigated.

Tengo el honor de prologar un libro que pone de relieve el interés creciente que está despertando el paisaje en los últimos años entre instituciones académicas y gubernamentales, entidades culturales y actores sociales en América Latina. En la última década he tenido la oportunidad de conocer en este continente una amplia y apasionante galería de organizaciones, grupos de investigación, asociaciones, redes, iniciativas y profesionales con una nueva manera de acercarse, de entender, de pensar, y de interactuar con el paisaje, y que están desarrollando una tarea tenaz y encomiable en sus respectivos países, a veces incluso nadando a contracorriente en los contextos que les rodean. Hoy el paisaje, en su sentido más amplio, es percibido cada vez más como un bien común, como un activo patrimonial, como un motor para el desarrollo de los territorios, y como una vía para incrementar la calidad de vida de las personas. Coordinada y editada de manera excelente por los arquitectos y urbanistas Carolina Fiallo y Joaquim Sabaté, la presente obra hace emerger doce paisajes culturales chilenos de gran interés. Paisajes donde la singular y excepcional combinación de elementos naturales y culturales -materiales o intangibles- se traduce en una riqueza de valores patrimoniales, estéticos, productivos, simbólicos e identitarios que merecen ser preservados y destacados mediante una gestión y ordenación adaptadas a la contemporaneidad, evitando su fosil- ización y que contribuyan a la concienciación colectiva y al desarrollo local, sin caer en la creación de parques temáticos o de paisajes escaparate. Para referirme a los paisajes culturales seleccionados en el libro he añadido muy conscientemente en el párrafo anterior el calificativo “de gran interés”, utilizado muy oportunamente por Eugenio Garcés en su introducción, ya que si solo calificamos algunos paisajes culturales como “de interés” -y esto pasa a menudo- existe el riesgo de inducir a pensar que el resto de los paisajes no lo son. El matiz me parece relevante, ya que partimos de la idea de que todo es paisaje, y de que todos los paisajes son culturales, en tanto que son el resultado de una actuación humana milenaria y de la traducción directa sobre el territorio de una determi- nada cultura. Con todo, si bien esto es cierto, también lo es que algunos paisajes muestran con más intensidad que otros los valores que emanan de la relación histórica entre una sociedad y el territorio que construyen. Los paisajes culturales que nos descubre detalladamente el libro son los de Cabo de Hornos, Tierra del Fuego, Chilloé, Mapuche, del carbón en Lota, del Valle Central, del cobre en Sewell, de Santiago, de Rapa Nui, de Valparaíso, de las oficinas salitreras y de la Ruta de la Sierra, y de cada uno ellos se resalta su carácter, es decir, sus rasgos propios y distintivos, que les dan identidad y personalidad. Todos estos paisajes, heredados de la ocupación indígena, la colonización española, los asentamientos históricos, las condiciones de insularidad, la agricultura, la pesca, la minería, o las actividades extractivas tradicionales, reclaman y reivindican una gran variedad de valores que se van desgranando en cada capítulo y que las políticas públicas deberían tener cada vez más presentes.

The Spanish Constitution of 1812 established the provincial council as an elective institution, and endowed it with various pow-ers taxation, public works, education, welfare, justice, territorial de-marcation, directives to the deputies of the Cortes, overseas missions, etc. With respect to the case of Peru (1812-1814 and 1820-1824), the following will be analysed: the electoral processes, the prosopogra-phy and governing actions of the deputies, and their relationships with other state organisms and their influence on regional political dynam-ics. A partir de 1820 y tras la invasión de San Martín, el sistema judicial peruano tuvo que adecuarse a la desaparición de facto de la Audiencia de Lima y al modelo diseñado por la Constitución de 1812 y el Reglamento de las Audiencias y Juzgados de primera instancia del 9 de octubre de 1812. El artículo analiza tal proceso y las tensiones generadas cuando las Audiencias Nacionales de Cusco y Charcas hubieron de resolver problemas de procedimiento, pero sobre todo se ahonda en los debates y actores, abogados fundamentalmente, que conformaron la cultura jurídica hispana durante el Trienio Liberal en el virreinato del Perú, asumiendo que tal realidad trascendió en la construcción nacional del Perú y Bolivia.

42. «Quedarán ya para el polvo y el olvido» : las elecciones a diputados a las Cortes españolas en el Perú, 1810-1824

- Abstract
La influencia del liberalismo hispano en la construcción del republicanismo peruano ha sido abordada por diversos historiadores (Peralta, 2008), si bien carecemos aún de un balance global, pero sobre todo sigue pendiente reconstruir su alcance durante el Trienio Liberal. Uno de los aspectos de cierta centralidad fue la representación peruana en las Cortes, y si bien existe un cierto consenso sobre el papel poco relevante de los diputados peruanos en el arco parlamentario doceañista, se conoce menos la trascendencia de las elecciones y de los dictados constitucionales en los dominios coloniales andinos o la actuación de los diputados en defensa de sus provincias. Nuestra propuesta busca aportar una reflexión sobre el eco de lo que Paniagua dio en llamar los orígenes del régimen representativo peruano (Paniagua, 2003) durante la etapa abierta por el virrey La Serna al trasladar la capital al Cuzco a finales de 1821, que se finiquitaría con el retorno al absolutismo a principios de 1824. En ese período, en las provincias de Arequipa, Cuzco, Huamanga, Huancavelica, Puno y Tarma, que aún seguían bajo el dominio español, se aplicó la Constitución de 1812 y las sucesivas leyes y decretos de Cortes emitidos hasta al menos mediados de 1822. Posteriormente, el bloqueo naval impidió que en el Perú se conociera y se implementara la política de la fase exaltada del Trienio Liberal. El trabajo trata de rescatar lo que podría considerarse en cierto sentido el eslabón perdido del segundo liberalismo hispano, una coyuntura que coincidió en el tiempo con el gobierno del general San Martín en el Perú independiente y con el período en que el Congreso Constituyente debatiría y promulgaría la Constitución de 1823. En las siguientes páginas abordamos el proceso de elección de diputados a Cortes, lo que nos servirá para reconstruir tanto su dinámica como la prosopografía de los electores de partido y de los diputados electos.

The temporal evolution of the physical and biogeochemical properties of the Atlantic Jet (AJ) along the first 75 m of the water column during a 4-day journey was analysed by following the trajectory of a drifter dragged by the jet from the Strait of Gibraltar towards the Alborán Sea. Three stages were differentiated based on the evolution of several variables (e.g., velocity, temperature, nutrients, fluorescence). (i) Within the Strait of Gibraltar, the water column was primarily influenced by the tidal cycle, leading to a nutrient-enrichment of surface waters. However, due to the short residence time, the phytoplankton community that was mainly dominated by diatoms, did not demonstrate significant changes. (ii) Once outside the Strait, the drifter trajectory was mainly influenced by the frontal dynamics associated with the AJ. The drifter moved forward along the jet but also laterally across it and was continuously attracted to the mainstream (maximum current speed) or detached to its southern edge (minimum current speed). Due to the associated upwelling processes induced by the intensification of the current along the mainstream, the water column was characterized by colder, nutrient-richer water and lower fluorescence values. Conversely, along the southern edge of the jet, the water column was characterized by higher temperature, low nutrient concentration, and higher fluorescence. Along the first stations of this stage, diatom total abundance and biovolume continuously increased, reaching values 12-times higher than the initial concentrations. (iii) In the last stage, the water parcel was still influenced by the frontal dynamics but with less intensity. Additionally, the colder and denser water of the AJ and the associated phytoplankton community subducted progressively as it moved into the region surrounded by warmer waters.
