14. Intraarterial Chemotherapy for Advanced Bladder Cancer

Abstract

The results of prolonged intra-arterial chemotherapy in 61 patients with locally advanced bladder cancer (T3–T4) are discussed. Regional internal iliac artery infusion of chemotherapeutic agents was done daily for 5 or 7 days at a rate of 1 to 3 ml/h over an 18-20-h period. The total dosages of each course were 60–120 mg/m5adriamycin, 2.5–3.5 g/m25-fluorouracil (5-FU), 20–40 mg/m2methotrexate or 30–50 mg/m2cisplatin. At 3 to 4 weeks after the completion of infusional chemotherapy the results were evaluated, based on data from control cystoscopy and sonography. One patient (1.6 %) was free of tumor; an objective response of greater than 50 % reduction in tumor size occurred in 26 (42.7 %) patients; 34 (55.7 %) demonstrated objective responses of less than 50 %. No further increase in tumor size during the management was detected. Hematuria, dysuria and urine infection were controlled and pain was relieved. The use of cytostatics and prolonged iliac artery catheterization did not incur any serious complications.

15. Post-Prandial Effects of Two Initial Insulin Strategies in Patients with Type 2 Diabetes (T2DM) Uncontrolled by Oral Agents (OAs).

Abstract

Control of post-prandial glucose (PPG) is of significant importance in achieving HbA1c targets. We assessed the effects on PPG of two frequently prescribed initial insulin regimens in T2DM patients who had failed OA therapy. Thirteen patients (8W/SM, mean age 61 ± 12 years, BMI 34 ±7 kg/m7²) with poorly controlled T2DM (HbA1C 9.7 ± 4%) on >1 OAs (2.3±0.5) were randomized in a cross-over design to either twice-daily 75%/25% neutral protamine lispro/lispro mix (LPM) vs. bedtime glargine (G). Patients continued OAs except secretagogues. Both insulins were initiated at a dose of 0.2 U/kg and titrated over 3 months to pre-meal plasma glucose (PG) of 90-110 mg/dL. At baseline and 3 months, patients underwent a 12-hr meal tolerance test (MTT) consisting of 3 mixed isocaloric meals at 8 AM, 12 PM, and 4 PM; PG was measured hourly. Over each treatment period, fasting PG reached the target and was similar between groups (LPM: 108 ± 22 vs. G: 110 ± 22; P=0.87) Despite this, overall PPG exposure (mean PPG after all 3 meals) was significantly lower with LPM than with G (153 ± 36 vs. 199 ± 49 mg/dl, P=0.001). Except after lunch, PPG excursions (highest PPG minus corresponding preprandial PG) were significantly smaller with LPM than with G (breakfast: 62 ± 32 vs. 94± 64, P=0.009; lunch: 53 ± 33 vs. 39 ± 37, P=0.174; dinner: 26 ± 21 vs. 61 ± 36 mg/dl, P=0.002). Peak PPG (the highest PG of the day) was also lower with LPM than with G (190 ± 39 vs. 239 ± 64 mg/dl, P=0.009). The terminal HbA1C for subjects on LPM was lower than those on G: 7.2±0.7 vs. 8.0±1.1 (P=0.009; no treatment period interaction). Median total daily insulin dose trended higher in the LMP group (43 U vs. 32 U, P=0.07). There was also a trend toward more frequent mild hypoglycemia (home PG <70 mg/dl) with LPM than G (3.2±6.2 vs. 0.9±1.7; P=0.096) per study period. We conclude that, in T2DM patients failing OAs, two commonly prescribed initial insulin strategies result in similar control of FPG but markedly different PPG control, which appears to influence HbA1c levels. The effects of these differences on long-term glycemic control and cardiovascular outcomes are not fully known. [ABSTRACT FROM AUTHOR]
