Acquired ileal diverticulum is an uncommon condition and diagnosis is often difficult when bleeding occurs from this source. Here we describe two cases of ileal diverticulum with massive bleeding. Both patients presented with anal bleeding, but upper and lower gastrointestinal endoscopy did not reveal the source. Selective visceral angiography finally detected bleeding lesions in the terminal ileum. Surgical resection was performed in both patients, confirming that the bleeding arose from diverticula less than 1 cm in size. In patients with obscure gastrointestinal bleeding, an ileal diverticulum should be considered, and selective visceral angiography should be performed for precise diagnosis. [ABSTRACT FROM AUTHOR]

The lysosomal membrane encloses numerous hydrolytic enzymes and prevents the cytoplasm from being damaged by these enzymes. It is possible that the fragility of this membrane may be implicated in the pathogenesis of gastric mucosal damage. We investigated the effects of 16,16-dimethyl prostaglandin E2 (dmPGE2), which is known to protect the gastric mucosa from various noxious agents, on lysosomal membrane stability in the rat stomach. Sodium taurocholate (TC) was used as the damaging agent. To assess lysosomal membrane stability in the gastric mucosa, we assayed acid phosphatase released from lysosomes isolated from a gastric mucosal homogenate. To assess lysosomal membrane stability in gastric surface epithelial cells, we used laser scanning confocal microscopy to observe the fading of red fluorescence in living cells vitally stained with acridine orange. Exogenous dmPGE2 enhanced lysosomal membrane stability in the gastric mucosa, whereas TC decreased it. In gastric surface epithelial cells, exogenous dmPGE2 protected the cells against TC-induced damage and prevented TC-induced decreased lysosomal membrane stability. It was concluded that a decrease in lysosomal membrane stability seemed to be closely involved in the pathogenesis of gastric mucosal damage. Moreover, it appears that stabilization of the lysosomal membrane by exogenous dmPGE2 may contribute to its protective effect in the gastric mucosa, both at the level of gastric surface epithelial cells and in regard to the entire gastric mucosa. Each hemisphere's ability to sustain attention over extended time periods was investigated in 14 schizophrenics and 17 controls using dichotic detection tasks. Schizophrenics produced significantly higher rates of omission errors, as compared to that of commission errors. The rates of omission errors for schizophrenics fluctuated markedly, while the rates of commission errors remained fairly constant. Primarily due to the fluctuation of omission errors, the detection index decreased progressively when schizophrenics were engaged in right-ear tasks. These results suggest that schizophrenics demonstrate a deficit concerning 'response set', especially in the left hemisphere. It is feasible that there may be a correlation with disturbances in integration mechanisms of both hemispheres in schizophrenics. [ABSTRACT FROM AUTHOR]

Although the spatial distribution of neural subsystems involved in music processing has been elucidated by recent neuroimaging studies, little is known about the temporal profile of such neural activities. In this study, spatiotemporal neuromagnetic activities during pitch processing in a musical context were investigated using a cross-modal pitch-matching task, which required subjects to find infrequent pitch errors implanted in heard performance while sight-reading its musical score. Eight right-handed musicians and eight right-handed non-musicians participated in this study. Neuromagnetic responses to each tone onset were recorded using VectorView™ (Neuromag, Helsinki, Finland). The source localization was estimated by minimum current estimates (MCE) algorithm and was verified by a multiple-current-dipole model. Although every subject showed magnetic components, M50, M100 and M200 in both erroneous and correct conditions, significant amplitude difference between these two conditions was detected only in musicians. Incongruent condition activated spatiotemporally distributed brain regions, including not only superior temporal gyrus but also other areas; the middle temporal, inferior temporal, inferior frontal, dorsolateral prefrontal cortex, inferior parietal lobule and sensorimotor cortex were activated in musicians. On the contrary, activated areas were limited in the vicinity of the auditory cortex in both conditions in non-musicians. Our findings suggest musicians'' multiple strategies of pitch processing. Introduction: Cold saline application through an irrigation catheter may induce reversible inhibition of submyocardial excitation, which may be used to identify in advance an ideal site for radiofrequency (RF) energy delivery around delicate areas., Methods: An open irrigation catheter was positioned vertically or parallel with 10-g contact on coronary perfusing porcine hearts and the contacted myocardium was cooled by cold saline at 4°C (20 mL/min). A temperature electrode was inserted 2 mm below the myocardial surface (intra-myocardial temperature at approximately 2 mm below the surface [Temp-BS]). Pacing threshold inside the ventricular wall was measured using an eight-pole electrode plunge needle inserted 5-8 mm close to the ablation catheter, and percent increase of the pacing threshold by the cold saline application (%increase-PT) was calculated., Results: During cooling, pacing at 10 V from the myocardial surface interrupted constant capture in 7/10 experiments in vertical and in 9/10 experiments in parallel ablation catheter contact. Minimum Temp-BS was not different in both catheter contact positions (25.9 ± 4.0°C in vertical vs. 25.4 ± 2.6°C in parallel). Large % increase-PT on the surface myocardium decreased as the myocardial depth became deeper, and > 150% increase was at a depth approximately 2-3 mm from the surface and > 120% increase around 6-7 mm from the surface. After cessation of cold saline application, the increased %increase-PT recovered to the pre-cooling values., Conclusions: Cold saline application through an irrigation catheter reversibly inhibited submyocardial excitation. Introduction: Unipolar radiofrequency (RF) ablation using half-normal saline irrigation (UNIP-HNS) and bipolar RF ablation using normal saline irrigation (BIP-NS) are effective to treat arrhythmias from inside thick myocardium. However, differences between these two ablations when using a long application time had not fully been studied., Methods: UNIP-HNS, BIP-NS and unipolar RF ablation using normal saline irrigation (UNIP-NS) were applied for 120 s (30 W and 20-g contact) to porcine endocardial wall (≧15.0 mm thickness)., Results: All ablations (30 applications each in UNIP-HNS and BIP-NS, and 20 applications in UNIP-NS) were successfully accomplished without steam-pop. Total impedance decline was largest in BIP-NS followed by UNIP-HNS and UNIP-NS. UNIP-HNS created larger surface lesions and greater maximum lesion width under the surface than those by UNIP-NS and BIP-NS. Endocardial lesion depth in UNIP-HNS and BIP-NS were deeper than that in UNIP-NS, but with no difference between UNIP-HNS and BIP-NS, when selecting non-transmural lesions. Similar results were obtained when all lesions (non-transmural and transmural) were included and endocardial lesion depth of the transmural lesions (13/30 applications of BIP-NS) was estimated as 50 % of the myocardial thickness. Lesion length in the transverse myocardial wall (endocardial plus epicardial lesions) was greatest in BIP-NS., Conclusions: Longer application time ablation (30 W) targeting the thick myocardium was performable in UNIP-HNS and BIP-NS. Introduction: Platinum-based chemotherapy is the mainstay of first-line therapy for advanced-stage non-small cell lung cancer (NSCLC). Although carboplatin-induced hypersensitivity reactions (HSRs) commonly occur following multiple cycles of therapy, they are rarely observed during the first cycle of the treatment., Case Report: Here, we report the case of a 70-year-old man with advanced-stage NSCLC who developed HSR possibly caused by carboplatin during the first cycle of induction with platinum-doublet chemotherapy plus pembrolizumab. The patient presented with bronchial obstruction due to a centrally located tumor. No driver mutations were detected, and the programmed death-ligand 1 expression ranged from 1% to 24%. Consequently, the patient was treated with pembrolizumab combined with carboplatin and paclitaxel. However, immediately after the start of carboplatin, the blood pressure and oxygen levels of the patient dropped and he began exhibiting an altered level of consciousness. These findings indicated carboplatin-induced anaphylaxis. Hypotension and oxygen desaturation improved following carboplatin discontinuation and normal saline administration., Management and Outcome: The basophil activation test for both carboplatin and cisplatin was negative. Thus, the risk of anaphylaxis owing to both drugs was ruled out, and carboplatin was believed to have induced grade 3 HSR. Subsequently, carboplatin-based chemotherapy was switched to cisplatin-based chemotherapy.
