12. Hysterectomy for benign conditions in a university hospital in Saudi Arabia

Abstract

Background and Objective: Hysterectomy is a common surgical procedure among women with a lifett time prevalence of 10%. The indications and complications of this procedure have not been previously reported from a teaching institution in Saudi Arabia. We examined the indications for hysterectomy and the surgical morbidity for women undergoing hysterectomy at a university hospital in Saudi Arabia. Patients and Methods: We reviewed the records of women who underwent hysterectomies for benign gynecological conditions between January 1990 and December 2002, at King Abdulaziz University Hospital (KAUH), Jeddah, Saudi Arabia, comparing patient characteristics, indications for hysterectomy and the rate of complications in women undergoing abdominal hysterectomy (AH) versus vaginal hysterectomy (VH). Results: Of 251 women, 199 (79%) underwent AH and 52 (21%) underwent VH. An estimated blood loss of 2500 mL occurred in 104 patients (52.3%) in the AH group and in 20 patients (38.5%) in the VH group (differtt ence not statistically significant). The most common indications for hysterectomy were uterine fibroids (n=107, 41.6%) and dysfunctional uterine bleeding (n=68, 27.1%). The most common indication for VH was uterine prott lapse (n=45, 86.5%). The overall complication rates were 33.5%, 15.4% and 30.4% in women who underwent AH, VH and both, respectively. Intraoperative and postoperative complications occurred in 24 (9.7%) patients in the AH group and in 51 patients in the VH group (20.3%). Postoperative infection occurred in 42/199 (21.6%) in the AH group and 5/52 (9.6%) in the VH group (difference not statistically significant). Conclusions: We describe a large series of hysterectomies, which provides information for surgeons on the expected rate of complications following hysterectomy for benign conditions. We found that the rate of complitt cations was not significantly higher than other centers internationally.

15. Growing teratoma syndrome with porta hepatis involvement – a case report

Abstract

Growing teratoma syndrome is a rare entity of tumors, it arises seldomly from ovarian and testicular carcinoma. It presents with disseminating masses of mature teratoma during or following chemotherapy of malignant germ cell tumors. We are reporting a 19-year old presented with recurrent left ovarian mass and supra renal large mass close to the porta hepatis was seen on magnetic resonance imaging. This patient was treated 3 years ago for stage I immature teratoma with left ovarian cystectomy and chemotherapy. Surgical excision of the left ovary and the abdominal mass required meticulous dissection, and the mass was shaved off the porta hepatis with no intraoperative or postoperative complications. Pathology showed mature teratoma. She has no recurrent 5 years after treatment. To the best of our knowledge, this is the first case report describing close relation of growing teratoma syndrome to the porta hepatis, no such case report like this has been reported in our region.

27. Prognostic value of matrix metalloproteinase 2 protein expression in ovarian cancer is age- and stage-dependent.

Abstract

Objective: Matrix metalloproteinase 2 (MMP2) has been associated with tumor development and invasion, however, the information available regarding its prognostic value in ovarian cancer (OC), especially in the Arabian Peninsula, is limited. The aim of this retrospective study was to analyze MMP2 protein expression and assess its prognostic value. Methods: In total, 245 formalin-fixed and paraffin-embedded (FFPE) primary OC tissue samples were randomly collected from patients with available clinicopathological data, including disease of all stages and all histological subtypes. MMP2 protein expression was measured using automated tissue microarray and immunohistochemistry techniques. Statistical analyses were performed using SPSS, with p < 0.05 considered statistically significant. Results: Cytoplasmic MMP2 protein expression patterns were higher in 53% of all tumor samples. The MMP2 expression profile was not significantly correlated with most clinicopathological features including age, tumor site, size, grade, and lymph node status (p > 0.05). However, when adjusted according to the disease stage or patient age, MMP2 overexpression showed a significant indication of a poor outcome and recurrence as evaluated using univariate Kaplan--Meier analysis for disease-free survival (DFS) (p = 0.04 and p = 0.03, respectively, log-rank test), but not for disease-specific survival (DSS) (p > 0.05, log-rank test). Conclusion: This study showed that MMP2 protein overexpression was a negative prognosticator in Saudi OC patients with advanced stage and/or young age. 31. Five-year survival and prognostic factors in women treated for cervix cancer at a tertiary care center

Abstract

ABSTRACTObjectivesTo estimate the 5-year survival rate among patients treated for cervical cancer and to investigate the associated clinical, management- and outcome-related factors and predictors.MethodsA retrospective study including women with cervix cancer who were treated at the Gynecology Oncology Unit in King Abdulaziz University Hospital, Jeddah, and who achieved at least 60 months (5 years) of follow up. Baseline demographic and clinical data, tumor characteristics, treatment option and outcome including recurrence were collected and analyzed as factors of 5-year survival.ResultsA total 153 patients were included, mean (SD) age was 54.52 (13.47) years. Tumor characteristics showed FIGO stage (I [19.6%], II [50.3%], III [15.7%], IV [14.4%]), (grade I [17.6%], grade II [46.4%], grade III [35.3%]) and the most frequent histological type was squamous cell carcinoma (73.9%). Patients received initial radiotherapy with concurrent chemotherapy (51.6%), systemic CT (3.9%) and palliative care (5.9%). Abstract

Background. Gestational choriocarcinoma (GC) is a recognized clinicopathological subtype of gestational trophoblastic neoplasia that usually metastasizes hematogenously to highly vascular organs like the lung, liver, and brain. However, orbital metastasis to the choroid and lacrimal gland is a rare occurrence. Case Presentation. A 21-year-old female presented with headache and left orbital swelling one year after resection of a complete hydatidiform mole followed by adjuvant methotrexate chemotherapy. A metastatic imaging screening revealed multiple metastases in the lungs, brain, and adrenal gland, in addition to the choroid and lacrimal gland. Based on her modified WHO risk factors scoring she was started on chemotherapy and whole brain radiotherapy, which resulted in a complete response. At two-year follow-up, serum b-HCG level was with normal limits; imaging surveillance was uneventful. Conclusion. We present the first case of lacrimal gland metastasis in a young girl from GC relapse.

44. Early experience with the da Vinci® surgical system robot in gynecological surgery at King Abdulaziz University Hospital

Khalid H SaitObstetrics and Gynecology Department, Faculty of Medicine, Gynecology Oncology Unit, King Abdulaziz University Hospital, Jeddah, Saudi ArabiaBackground: The purpose of this study was to review our experience and the challenges of using the da Vinci® surgical system robot during gynecological surgery at King Abdulaziz University Hospital.Methods: A retrospective study was conducted to review all cases of robot-assisted gynecologic surgery performed at our institution between January 2008 and December 2010. The patients were reviewed for indications, complications, length of hospital stay, and conversion rate, as well as console and docking times.Results: Over the three-year period, we operated on 35 patients with benign or malignant conditions using the robot for a total of 62 surgical procedures. The docking times averaged seven minutes. The mean console times for simple hysterectomy, bilateral salpingo-oophorectomy, and bilateral pelvic lymphadenectomy were 125, 47, and 62 minutes, respectively. In four patients, laparoscopic procedures were converted to open procedures, giving a conversion rate of 6.5%. All of the conversions were among the first 15 procedures performed. The average hospital stay was 3 days. Complications occurred in five patients (14%), and none were directly related to the robotic system.Conclusion: Our early experience with the robot show that with proper training of the robotic team, technical difficulty with the robotic system is limited. There is definitely a learning curve that requires performance of gynecological surgical procedures using the robot.Keywords: da Vinci robot, gynecological surgery, laparoscopy

45. Molecular tests to detect human papillomavirus infection in patients with cervical dysplasia and invasive cervical cancer in Saudi Arabia

The subjects studied were women with a mean age of 48.18 years, who attended the hospital for cervical biopsy due to the suspected diagnosis of cervical dysplasia or an invasive disease, based on previous suspicious Pap smear. HPV DNA hybridization by HC2 was performed on the cervical biopsies of these patients, to detect HPV infection.Results: During the period of this study, 45 patients had cervical biopsies taken for HPV testing. Seven patients had a negative HC2 result and were found to have no cervical dysplasia on the final pathology review. Seventeen cases with cervical dysplasia and 21 patients with invasive disease were presented; the mean age was 48 years. HC2 testing for HPV were found to be positive in patients with cervical dysplasia, invasive carcinoma, and all in 5 (29.4%), 13 (61.9%) and 18 (47.4%), respectively. The sensitivity of the test is 47% and specificity is 100%.Conclusion: The use of molecular detection of HPV DNA by HC2 in biopsy is feasible and effective. These results confirm the finding that HPV contributes to the etiology of cervical cancer in Muslim society.Keywords: HPV, subtyping, cervical neoplasia
