Abstract

Fibro-osseous benign lesions rarely affect the sinonasal tract and are divided into 3 different entities, namely osteoma, fibrous dysplasia and ossifying fibroma. They share several clinical, radiological and histological similarities, but have different behaviours. Ossifying fibroma, and in particular the "juvenile" histological subtype, may have a locally aggressive evolution and a high risk for recurrence if removal is incomplete. The purpose of the present study is to compare the clinical behaviour of ossifying fibroma with the other benign fibro-osseous lesions; highlight different behaviour between the histological subtypes; compare the advantages, limitations and outcomes of an endoscopic endonasal approach with reports in the literature. We retrospectively reviewed 11 patients treated for sinonasal ossifying fibroma at a tertiary care centre. All patients underwent CT scan, and MRI was performed in cases of cranial base involvement or recurrence. Pre-operative biopsy was performed in cases where it was possible to use an endoscopic approach. One patient underwent pre-operative embolisation with ipsilateral visual loss after the procedure. Depending on its location, removal of the tumour was performed using an endoscopic (n = 7), or an external (n = 3) or combined (n = 1) approach. Histopathologically, 5 patients presented the conventional type, 5 the juvenile psammomatoid variant, which was associated in 1 case with an aneurismal bone cyst, and 1 case presented the trabecular juvenile variant. Three patients affected by the juvenile psammomatoid histological variant presented invasion of the skull base and underwent a subtotal removal that subsequently required, due to the regrowth of the remnant, a transbasal approach. Clinical, radiological and histological findings should all be considered to establish differential diagnosis among fibrous osseous lesions. More studies are necessary to conclude if the localisation and extension of the disease at the time of diagnosis is more important than the histological variant. An endoscopic approach is the first choice in most of cases even if an external open approach may be necessary in selected patients.Le lesioni fibro-ossee benigne raramente colpiscono i seni paranasali e sono suddivise in 3 entità: osteoma, displasia fibrosa e fibroma ossificante. Questi presentano simili caratteristiche cliniche, radiologiche e istologiche ma hanno un comportamento diverso. Il fibroma ossificante, in particolare la variante istologica giovanile, può presentare un comportamento aggressivo con un alto rischio di recidiva se rimosso in modo incompleto. Lo scopo dello studio è quello di paragonare il comportamento clinico del fibroma ossificante con quello delle altre lesioni fibro-ossee; di evidenziare un eventuale comportamento differente tra i vari sottotipi istologici; di descrivere i vantaggi, i limiti e i risultati della chirurgia endoscopica endonasale rispetto ai dati presenti in letteratura. Abbiamo analizzato retrospettivamente 11 pazienti affetti da fibroma ossificante naso-sinusale e trattati in un centro ospedaliero di terzo livello. Tutti i pazienti sono stati sottoposti a TC. La RM è stata eseguita in caso di coinvolgimento del basicranio o in caso di recidiva. Una biopsia pre-operatoria è stata effettuata nei casi in cui la massa era raggiungibile per via endoscopica. Un paziente è stato sottoposto a embolizzazione pre-operatoria ed ha riportato una cecità monolaterale al termine della procedure. In base alla localizzazione, l'exeresi del tumore è stata eseguita con un approccio endoscopico (7 pazienti), esterno (3), combinato (1). Istologicamente 5 pazienti hanno riportato un sottotipo convenzionale, 5 la variante giovanile psammomatoide associata in un caso a cisti aneurismatica ossea, e un paziente la variante giovanile trabecolare. Tre pazienti affetti dalla variante istologica giovanile psammomatoide hanno presentato un'invasione del basicranio e sono stati sottoposti ad exeresi subtotale per via endoscopica che ha richiesto in seguito, a causa di un aumento di volume del residuo, un secondo intervento per via transbasale. I reperti clinici, radiologici e istologici dovrebbero essere considerati insieme per una accurata diagnosi differenziale tra le lesioni fibro-ossee. Ulteriori studi sono necessari per concludere se la localizzazione e l'estensione del fibroma ossificante al momento della diagnosi sono più importanti della variante istologica. L'approccio endoscopico è la prima opzione nella maggior parte dei casi anche se in alcuni selezionati pazienti l'approccio esterno risulta ancora necessario.

Abstract

The Atmosphere Explorer C data base of ion temperature and ion drift velocities has been used to study the long recognized principle that large scale convection electric fields are responsible for substantial enhancements in the ion temperature at high latitudes. As had been indicated by various authors previously, the present study shows that there is a clear relationship between large ion velocities and ion temperature enhancements. Furthermore, with the help of a more quantitative study, it is found that, since ion heating is sensitive to the relative motion between the ion and neutral gases, the ion temperature and drift measurements can be used to extract neutral wind and neutral temperature information along the satellite track. From a study of several examples, it is concluded that the present approach can be used with varying degrees of accuracy for the diagnostic of thermospheric dynamical properties above 300 km when those properties are not measured directly. When neutral dynamical properties are measured, the approach could be used for a detailed study of the ion energy balance at high latitudes.

Recent interpretive studies of electric field-driven ionospheric plasma convection data from the AE-C satellite are described, where the instruments employed include an ion drift meter and an ion-retarding potential analyzer. Electrostatic potential curves are derived from ion drift velocity measurements for high-latitude segments of the satellite's orbit. The potential curves are shown to be useful in determining the character of the global electrostatic potential pattern, with emphasis on the separation of convective cells. Results are given for six orbits, with attention to the mid-day auroral region.

Abstract

The rate coefficient for the reaction O(+) + N2 yields NO(+) + N is determined as a function of temperature from the photochemistry of NO(+) for both day and night conditions by using a large sample of simultaneous measurements of ion and neutral concentrations and temperatures made by the Atmosphere Explorer C satellite. The results cover the ion temperature range from 500 to 1200 K. Using recent flow-drift-tube results, the rate coefficient is calculated as a function of ion temperature and mean ion drift velocity for ionospheric conditions. The satellite and laboratory determinations are found to be in good agreement. Using this temperature dependence, an earlier determination of the dissociative recombination coefficient of NO(+) with electrons is refined.

An aneurysm of the right renal artery was discovered in a patient suffering from cerebral arterial angiodysplasia and arterial hypertension. The aneurysm was large necked, which made selective endovascular treatment very difficult. To perform the embolization of the aneurysm, a balloon remodelling technique was used. 48. Safety and efficacy compared for two doxorubicin loaded microspheres in liver VX2 model.

Abstract

Purpose: Hepasphere and DC Bead loaded with Doxorubicin differ in terms of size ranges, mechanical properties, drug/bead interactions, but were never compared in vivo. The present study has compared the safety and efficacy of the two microspheres in the model of rabbit liver VX2 tumor. Materials and Methods: VX2 tumors were grown in the liver of 20 rabbits for 11-12D then randomized to embolization with DC-DOX 100-300µm, HS-DOX 30/60µm, Bland-HS 30/60µm or non-treated (n=5/group). HS and DC were loaded with the same dose of drug (17mg/mL). DOX plasma concentration was assessed at 5/20/60/180min/24h/72h. Plasma biochemistry was assessed daily. Tumor and liver parenchyma necrosis were quantified on digitized histology sections at D3. Results: For the two DOX microspheres, drug plasma concentration was not significantly different at any time point with a peak at 5 min and not detectable after 3h. ALAT levels were highly elevated after embolization with a stronger increase in DOX vs Bland (p=0.0338), NS between HS-DOX and DC-DOX. Necrosis of peri-tumoral liver was increased for both HS-DOX and DC-DOX (23% and 34% liver surface respectively, p=0.465) compared to Bland-HS (5%) and non-treated (<1%). Tumor growth after embolization was <6% in the two DOX groups (p=0.361), not significantly different from Bland-HS (5%) but lower than non-treated (+148%, p=0.030). Tumor necrosis reached >99% tumor surface in the two DOX groups (p=0.523), significantly higher than in Bland-HS (80%) and non-treated group (32%) Conclusion: Hepasphere and DC Bead loaded with Doxorubicin demonstrated equivalent safety and efficacy in the present tumor model and higher efficacy compared to bland embolization. 49. Doxorubicin eluting microsphere: is there a size effect ? Comparison of two sizes in VX2 tumor model.

Abstract

Purpose: Small sizes of doxorubicin microspheres could act differently on tumors from large microspheres: 1) they could distribute differently inside/around the tumor 2) they may elute their drug load faster. We compared bead repartition, drug tissue distribution and antitumor effects for two sizes of doxorubicin-loaded microspheres in liver VX2 tumor model. Materials and Methods: Chemoembolization was randomly performed on VX2 liver tumors (age 12D, size 2.3±0.6cm) with a fixed volume of 50µL sediment HS-DOX 30/60µm or HS-DOX 50/100µm (n=5/group) loaded at same dose. Non-treated tumors (n=5) served as Control. DOX concentrations in bead, DOX tissue concentration profiles, bead distribution and tumor necrosis were assessed at D3. Results: HS-DOX 30/60 occluded vessels of smaller caliber than HS-DOX 50/100 (158µm vs 254µm, p=0.001). The majority of microspheres were located outside tumor for both sizes of HS-DOX (88% for small and 86% for large, p=0.835) though small HS-DOX were closer to tumor boundary than large HS-DOX (819µm vs 2800µm, p=0.059). The quantity of drug released by the microsphere at D3 was 55% of initial load for HS-DOX 30/60 and 70% for HS-DOX 50/100, not significantly different (p=0.331). For both sizes, DOX was detected in the tissue up to a radius of 800µm from the bead and above the drug IC50 in VX2 at a radius of 100µm. Tumor necrosis was >98% tumor surface for the two HS-DOX groups (p=0.784), significantly higher than for non-treated tumors (34%, p=0.004). Conclusion: The two sizes of HS-DOX were very similar in terms of repartition in embolized territory, elution properties and strong antitumor efficacy. [Copyright &y& Elsevier]
