Un toxidrome est le regroupement des symptômes cliniques et biologiques et/ou des perturbations à l’électrocardiogramme qui caractérisent une intoxication par une classe spécifique de xénobiotiques. En cas d’exposition à un agent du risque chimique agressif, tel qu’en cas d’attaque terroriste, l’étude des toxidromes permettrait d’évoquer une classe d’agents responsable, d’assurer la sécurité des équipes soignantes (port éventuel d’équipement de protection individuelle) et de mettre en œuvre une réponse individuelle (décontamination d’urgence, administration des traitements antidotiques et symptomatiques) et collective (décontamination médicale approfondie éventuelle des victimes) précoce qui pourra secondairement être adaptée selon les résultats d’analyses d’identification du toxique. Dans le contexte d’un évènement lié à un agent du risque chimique agressif, cinq grandes classes d’agents chimiques seraient susceptibles d’être mises en œuvre : les inhibiteurs des cholinestérases, les vésicants, les cyanés, les suffocants et les opioïdes de synthèse. Si certains symptômes (irritation des muqueuses ou toux) sont communs à plusieurs classes, d’autres sont plus spécifiques pour discriminer les différentes classes d’agents (fasciculations, hypersécrétions, crise comitiale rapide, myosis ou mydriase, dépression respiratoire). Plusieurs outils reposant sur l’approche toxidromique sont disponibles dans la littérature scientifique et seront présentés dans cet article. Leur utilisation doit être testée régulièrement lors d’exercices sur table (table top exercice) ou opérationnels (drill, exercice fonctionnel, exercice à échelle réelle) afin de s’assurer de leur maîtrise par les équipes soignantes.

La nitrendipine est un inhibiteur calcique utilisé dans le traitement de l'hypertension. Nous présentons dans ce travail une procédure analytique, effectuée en routine au laboratoire, permettant l'analyse toxicologique de la nitrendipine. Nous rapportons ensuite les résultats obtenus dans un cas mortel d'intoxication polymédicamenteuse où la nitrendipine était impliquée. Des échantillons autopsiques de sang, d'urine et de contenu gastrique, prélevés sur la victime ont été analysés par chromatographie en phase gazeuse couplée à la spectrométrie de masse (CPG-SM) et par chromatographie en phase liquide à haute performance avec détecteur à barrette de diodes (CLHP-DBD). Le dosage sanguin de la nitrendipine a été réalisé par CLHP-DBD. La méthode de dosage a été validée et la stabilité du produit a été vérifiée. La dégradation in vitro et in vivo de la nitrendipine a été discutée. La concentration de la nitrendipine dans le sang périphérique (0,750 mg/L) était 15 fois plus élevée que la valeur extrême de la fourchette thérapeutique. Les concentrations sanguines des autres composés mis en évidence (paracétamol et triazolam) étaient également toxiques. L'association de ces trois composés était vraisemblablement à l'origine du décès.

La venlafaxine est un antidépresseur inhibiteur de la recapture de la sérotonine et de la noradrénaline. En cas d’intoxication, elle peut être à l’origine d’un syndrome sérotoninergique sévère avec état de choc. Elle subit une forte métabolisation hépatique par le CYP2D6 en son métabolite actif, la O-desméthylvenlafaxine. Nous rapportons le cas d’une intoxication volontaire chez une femme caucasienne de 41ans, ayant ingéré 1200mg d’alimémazine, 11 700mg d’oxazepam et 13 500mg de venlafaxine. À j2, la patiente présente un syndrome sérotoninergique sévère avec état de choc. Des dosages de la venlafaxine et de l’O-desméthylvenlafaxine ont été réalisés par LC-MS/MS et retrouvent respectivement dans le sang à j4 5842 et 386ng/mL, à j11 2118 et 275ng/mL, et dans les urines à j11 30 000 et 51 650ng/mL, suggérant un défaut de métabolisation de la venlafaxine. Une analyse génétique réalisée chez cette patiente a permis de confirmer un polymorphisme du CYP2D6 à l’origine d’une perturbation du métabolisme de la venlafaxine. Approximativement 7 % des sujets de type caucasien présentent un génotype correspondant à une enzyme non fonctionnelle du CYP2D6, et présentent un phénotype de métaboliseurs lents. Ces sujets sont de ce fait susceptibles de présenter des intoxications particulièrement graves, ce qui était le cas de notre patiente. Dans ce contexte un syndrome sérotoninergique doit être soigneusement recherché car les signes sont aspécifiques et trompeurs et peuvent par ailleurs être masqués et/ou aggravés par la prise concomitante de médicaments neuroleptiques dont le tableau clinique est ressemblant.

ABSTRACTMalaria remains a major global health problem. Parasite resistance to existing drugs makes development of new antimalarials an urgency. The protein synthesis machinery is an excellent target for the development of new anti-infectives, and aminoacyl-tRNA synthetases (aaRS) have been validated as antimalarial drug targets. However, avoiding the emergence of drug resistance and improving selectivity to target aaRS in apicomplexan parasites, such as Plasmodium falciparum, remain crucial challenges. Here we discuss such issues using examples of known inhibitors of P. falciparumaaRS, namely halofuginone, cladosporin and borrelidin (inhibitors of ProRS, LysRS and ThrRS, respectively). Encouraging recent results provide useful guidelines to facilitate the development of novel drug candidates which are more potent and selective against these essential enzymes.

Abstract:This study examines the beliefs of 378 first-year learners of French at University of Melbourne about the role of culture in the FL classroom and the teaching and learning of language as culture, rather than language and culture. The study suggests that it is our role as FL teachers to invite learners to explore language-learning through a postmodern lens, where the notion of culture is construed not as a static or a strictly synchronic utilitarian self-referential concept, but rather as a dynamic system in which FL learners are full and active participants in the process of meaning-making.

The stability of mammalian telomeres depends upon TRF2, which prevents inappropriate repair and checkpoint activation. By using a plasmid integration assay in yeasts carrying humanized telomeres, we demonstrated that TRF2 possesses the intrinsic property to both stimulate initial homologous recombination events and to prevent their resolution via its basic N-terminal domain. In human cells, we further showed that this TRF2 domain prevents telomere shortening mediated by the resolvase-associated protein SLX4 as well as GEN1 and MUS81, 2 different types of endonucleases with resolvase activities. We propose that various types of resolvase activities are kept in check by the basic N-terminal domain of TRF2 in order to favor an accurate repair of the stalled forks that occur during telomere replication.

Studies exploring the influence of obesity on septic shock remain limited and controversial. Pigs were chosen as a clinically relevant species, resembling to humans in various functions. We hypothesize obesity may impair porcine acute endotoxic shock. Four groups of five “Yucatan” minipigs were studied: lean and obese control groups, lean lipopolysaccharide (LPS) group receiving Escherichia coliendotoxin (LPS) and obese LPS group receiving the same endotoxin dose. We measured hemodynamic and oxygenation parameters, skin microvascular blood flow at rest and during reactive hyperemia, von Willebrand factor, tumor necrosis factor , and interleukin 6. All measurements were performed at baseline and at 30, 60, 90, 150, and 300 min. Results were given as median with 25th to 75th interquartile range. Control groups remained stable during the study period. In LPS groups, administration of endotoxin resulted in a typical hypokinetic shock. In obese LPS group at 300 min, we observed a significant impairment of cardiac index (1.2 1.06–1.45 vs. 1.7 1.57–1.97 Lmin per m2, P= 0.008) compared with the lean LPS group; moreover, pulmonary hypertension (mean arterial pressure: 42 39–47 vs. 32 28–34 mmHg, P= 0.008), hypoxemia (partial pressure of oxygen: 216 178–262 vs. 325 285–414 mmHg, P= 0.02), and lactate levels (5.8 4.2–6.8 vs. 3.9 2.2–5.5 mmolL, P= 0.04) were significantly higher compared with the lean LPS group. Throughout the study, rest flow and peak flow during reactive hyperemia were more decreased in the obese LPS group. Compared with the lean LPS group, tumor necrosis factor levels at 60 min (269 178–428 vs. 126 105–166 ngmL, P= 0.03) and interleukin 6 levels at 300 min (101 61–142 vs. 52 36–64 ngmL, P= 0.03) were significantly higher in the obese LPS group. In our model of endotoxic shock, obese pigs developed a more severe hemodynamic failure with pronounced microcirculatory dysfunction and proinflammatory response.

The beaches of Haute-Normandie, of which Quiberville and Pourville are the subject of this study, are characterised by (1) an accumulation of coarse sediment with a steep slope (10%) and, at the foot of this incline, a sandy foreshore with a low slope (1%) that is more dissipative, (2) a macrotidal environment and (3) a seawall, which stops them from moving inland. According to existing literature these beaches experience a surface gravel drift, while their internal structure includes an interstitial sand fraction. The internal structure of these mixed beaches is often quite heterogeneous andmay contain up to 20% of siliceous splinters resulting from gravel attrition. At the end of periods of high energy, some splinters are found close to the low foreshore, suggesting that sand exchanges take place across the beach from the gravel accumulation towards the lowest part of the beach. Hydrodynamic measurements and the monitoring of fluorescent tracers confirmed displacement of sand from the low beach towards the gravel beach during moderate swells, and in the opposite direction during storm events. These sand transfers seem to be part of the system of adaptation of the gravel beaches to changing hydrodynamic conditions, determining the infiltration potential of the mixed sediment beach and the reflective or dissipative nature of the whole beach. The results also suggest that understanding the morpho-sedimentary behaviour of these gravel or composite beaches (Shulmeister & Kirk 1993) requires, to the same extent as for sand beaches, an appreciation of the sediment exchanges from the lower beach or even from the infra-tidal zone. French Les plages de Haute-Normandie, dont celles de Quiberville et de Pourville spécifiquement étudiées, se caractérisent par l'existence, (1) d'une accumulation de sédiments grossiers de haut estran à forte pente (10%), au pied de laquelle se développe un estran sableux à pente douce (1%) plus dissipatif, (2) d'un environnement macrotidal, (3) d'un perrés qui bloquent leur évolution vers l'intérieur des terres. Dans la littérature, ces formes ont souvent été envisagées sous l'angle unique de la mobilité des cailloutis, alors que leur structure interne est constituée majoritairement par une fraction sableuse interstitielle. Contrairement aux sables de bas estran, ceux de la structure interne des cordons sont mal classés, hétérogènes, et surtout, contiennent 20% d'éclats siliceux provenant de l'attrition des galets. A l'issue de périodes de forte agitation, des éclats sont retrouvés sur le bas estran, suggérant l'existence d'échanges sableux transversaux depuis le cordon vers la basse plage. Les mesures hydrodynamiques et le suivi de traceurs fluorescents ont confirmé ces déplacements de la basse plage vers le cordon de galets lors de houles modérées, et dans le sens contraire lors de tempêtes. Finalement, ces transferts sableux semblent faire partie du système d'adaptation des plages de galets aux conditions hydrodynamiques, en déterminant le potentiel d'infiltration du cordon, voire l'aspect réflectif et dissipatif de l'ensemble de la plage. Ces résultats suggèrent également que la compréhension du fonction nement morpho-sédimentaire des accumulations de galets dites composites nécessite, au même titre que les côtes sableuses, d'intégrer les flux de sédiments (sables) de la basse plage, voire de la zone infratidale.

To complete our panorama in structure–activity relationships (SARs) of sandalwood-like alcohols derived from analogues of α-campholenal (= (1R)-2,2,3-trimethylcyclopent-3-ene-1-acetaldehyde), we isomerized the epoxy-isopropyl-apopinene (−)-2d to the corresponding unreported α-campholenal analogue (+)-4d (Scheme 4;1). Derived from the known 3-demethyl-α-campholenal (+)-4a, we prepared the saturated analogue (+)-5a by hydrogenation, while the heterocyclic aldehyde (+)-5b was obtained via a Bayer-Villiger reaction from the known methyl ketone (+)-6. Oxidative hydroboration of the known α-campholenal acetal (−)-8b allowed, after subsequent oxidation of alcohol (+)-9b to ketone (+)-10, and appropriate alkyl Grignard reaction, access to the 3,4-disubstituted analogues (+)-4f,g following dehydration and deprotection. (Scheme 4;2). Epoxidation of either (+)-4b or its methyl ketone (+)-4h, afforded stereoselectively the trans-epoxy derivatives 11a,b, while the minor cis-stereoisomer (+)-12a was isolated by chromatography (trans/cis of the epoxy moiety relative to the C2 or C3 side chain). Alternatively, the corresponding trans-epoxy alcohol or acetate 13a,b was obtained either by reduction/esterification from trans-epoxy aldehyde (+)-11a or by stereoselective epoxidation of the α-campholenol (+)-15a or of its acetate (−)-15b, respectively. Their cis-analogues were prepared starting from (+)-12a. Either (+)-4h or (−)-11b, was submitted to a Bayer-Villiger oxidation to afford acetate (−)-16a. Since isomerizations of (−)-16 lead preferentially to β-campholene isomers, we followed a known procedure for the isomerization of (−)-epoxyverbenone (−)-2e to the norcampholenal analogue (+)-19a. Reduction and subsequent protection afforded the silyl ether (−)-19c, which was stereoselectively hydroborated under oxidative condition to afford the secondary alcohol (+)-20c. Further oxidation and epimerization furnished the trans-ketone (−)-17a, a known intermediate of either (+)-β-necrodol (= (+)-(1S,3S)-2,2,3-trimethyl-4-methylenecyclopentanemethanol; 17c) or (+)-(Z)-lancifolol (= (1S,3R,4Z)-2,2,3-trimethyl-4-(4-methylpent-3-enylidene)cyclopentanemethanol). Finally, hydrogenation of (+)-4b gave the saturated cis-aldehyde (+)-21, readily reduced to its corresponding alcohol (+)-22a. Similarly, hydrogenation of β-campholenol (= 2,3,3-trimethylcyclopent-1-ene-1-ethanol) gave access via the cis-alcohol rac-23a, to the cis-aldehyde rac-24.

Nous présentons les résultats de ces analyses, suivis de la liste complète des tables des matières, l'index des auteurs et celui des titres des articles et des rapports.
