Abstract

The $^{18}$Ne(d,t)$^{17}$Ne and $^{18}$Ne(d,$^3$He)$^{17}$F single-nucleon pickup reactions were measured at 16.5 MeV/nucleon in inverse kinematics together with elastic and inelastic scattering channels. The full set of measured exclusive differential cross sections was compared with the mirror reaction channels on stable $^{18}$O after consistent reanalysis using coupled reaction channels calculations. Within this interpretation scheme, most of the spectroscopic factors extracted for the population of unbound states in $^{17}$F match within uncertainties with their mirror partners in $^{17}$O. However, for the deeply-bound neutron removal channel to $^{17}$Ne, a significant symmetry breaking with the mirror proton-removal channel leading to $^{17}$N is evidenced by an overall single-particle strength reduction.

Abstract

The AGATA tracking detector array represents a significant improvement over previous Compton suppressed arrays. The construction of AGATA led to numerous technological breakthroughs in order to meet the requirements and the challenges of building a mobile detector across Europe. This paper focuses on the design and implementation of the data acquisition system responsible of the readout and control of the germanium detectors of AGATA. Our system is highly versatile, capable of instrumenting AGATA and seamlessly adapting it to various configurations with a wide range of ancillary detectors and/or spectrometers. It consists of three main components: an autonomous and independent infrastructure, a dedicated application core ensuring overall consistency, and a high--performance software package providing a fully integrated data flow management including the setting-up, the supervision and the slow control of the instrument. In this paper, we present a comprehensive analysis of the system's design and performance, particularly under high-counting rate conditions.

Abstract

Classical novae are thermonuclear explosions in stellar binary systems, and important sources of $^{26}$Al and $^{22}$Na. While gamma rays from the decay of the former radioisotope have been observed throughout the Galaxy, $^{22}$Na remains untraceable. The half-life of $^{22}$Na (2.6 yr) would allow the observation of its 1.275 MeV gamma-ray line from a cosmic source. However, the prediction of such an observation requires good knowledge of the nuclear reactions involved in the production and destruction of this nucleus. The $^{22}$Na($p,\gamma$)$^{23}$Mg reaction remains the only source of large uncertainty about the amount of $^{22}$Na ejected. Its rate is dominated by a single resonance on the short-lived state at 7785.0(7) keV in $^{23}$Mg. In the present work, a combined analysis of particle-particle correlations and velocity-difference profiles is proposed to measure femtosecond nuclear lifetimes. The application of this novel method to the study of the $^{23}$Mg states, combining magnetic and highly-segmented tracking gamma-ray spectrometers, places strong limits on the amount of $^{22}$Na produced in novae, explains its non-observation to date in gamma rays (flux < 2.5x$10^{-4}$ ph/(cm$^2$s)), and constrains its detectability with future space-borne observatories., Comment: 18 pages, 3 figures, 1 table

13. Effects of home-based exercise alone or combined with cognitive training on cognition in community-dwelling older adults: A randomized clinical trial

Abstract

Background: Structured and supervised physical exercise and cognitive training are two efficient ways to enhance cognition in older adults. Performing both within a combined intervention could maximize their effect on cognition due to their potential synergy on brain functions. During the COVID-19 pandemic, these interventions were particularly relevant due to the collateral impact of social restrictions regarding physical activity and the level of cognitive stimulation. However, the benefits of remotely monitored intervention combining physical exercise and cognitive training for older adult cognition remain to be demonstrated. Methods: 127 older adults (age: 65.20 ± 7.95) were randomized in two arms, encouraging self-engagement in six months of home-based physical exercise alone or combined with cognitive training, monitored by phone once a week. Neuropsychological assessment was performed under videoconference supervision at baseline and after three and six months. Composite Z-scores were calculated for processing speed, executive functioning, working, and episodic memory to assess changes after three and six months of training. The weekly metabolic expenditure of self-reported activities was estimated using the compendium of physical activity to distinguish participants performing higher and lower doses of exercise (median split). Results: 106 participants (83.46 %) completed the 6-month training. Results showed a greater Z-score change in executive functioning for participants in the combined arm than those who only exercised (F = 4.127, p = 0.046, ηp2 = 0.050). Group x Exercise dose interaction was observed for episodic memory Z-score change (F = 6.736, p = 0.011, ηp2 = 0.070), with a greater improvement for participants performing higher doses of exercise compared to those who performed a lower dose, only in exercise alone arm. Performing a higher dose of exercise increased the working memory Z-score change in both intervention arms compared to a lower dose (F = 7.391, p = 0.008, η p2 = 0.076). Conclusion: Remote combined training may lead to larger improvement in executive functioning than exercise alone. Physical exercise showed a dose-related improvement in working and episodic memory performances. The combination of cognitive interventions mitigated the effects of exercise on episodic memory. These results suggest that home-based exercise and cognitive training may help improve older adults' cognition. Trial registration: COVEPIC was retrospectively registered on December 03, 2020. Abstract

Introduction Reduced thrombin generation is an important component of post cardiopulmonary bypass (CPB) coagulopathy. To replenish coagulation factors and enhance thrombin generation in bleeding surgical patients, frozen plasma (FP) and four-factor prothrombin complex concentrate (4F-PCC) are used. However, the efficacy–safety balance of 4F-PCC relative to FP in cardiac surgery is unconfirmed.Methods and analysis LEX-211 (FARES-II) is an active-control, randomised, phase 3 study comparing two coagulation factor replacement therapies in bleeding adult cardiac surgical patients at 12 hospitals in Canada and the USA. The primary objective is to determine whether 4F-PCC (Octaplex/Balfaxar, Octapharma) is clinically non-inferior to FP for haemostatic effectiveness. Inclusion criteria are any index (elective or non-elective) cardiac surgery employing CPB and coagulation factor replacement with 4F-PCC or FP ordered in the operating room for bleeding management. Patients will be randomised to receive 1500 or 2000 international units of 4F-PCC or 3 or 4 units of FP, depending on body weight. The primary endpoint of haemostatic treatment response is ‘effective’ if no additional haemostatic intervention is required from 60 min to 24 hours after the first initiation of 4F-PCC or FP; or ‘ineffective’ if any other haemostatic intervention (including a second dose of study drug) is required. An estimated 410 evaluable patients will be required to demonstrate non-inferiority (one-sided α of 0.025, power ≥90%, non-inferiority margin 0.10). Secondary outcomes include transfusions, bleeding-related clinical endpoints, coagulation parameters and safety.Ethics and dissemination The trial has been approved by the institutional review boards of all participating centres. Trial completion is anticipated at the end of 2024, and results will be disseminated via publications in peer-reviewed journals and conference presentations in 2025. The results will advance our understanding of coagulation management in bleeding surgical patients, potentially reducing the need for allogeneic blood products and improving outcomes in surgical patients.Trial registration number NCT05523297.

Background Few studies describe rationales for intraspecialty (‘lateral’) general surgery (GS) resident program transfers.Objective We aimed to describe the key features of GS residency programs reporting lateral transfers, to characterize reasons behind transfer events, and to compare transferring resident skill sets against their new institutional peers.Methods A survey was administered in October 2020 to capture program features and demographic information of residents who transferred into or out of a GS residency program during a 5-year period. This survey was approved and distributed by the Association of Program Directors in Surgery to all participating GS program directors and coordinators. Survey responses were collected, and descriptive analyses were performed.Results Of 69 program responses (21.5% survey response rate), 42 (61%) indicated the presence of any type of transfer event (in or out); 19 of 69 (27.5%) programs reported having at least one categorical GS resident transfer out, and 31 of 69 (44.9%) programs reported having at least one transfer in. Most transfer-out events (94.7%) were resident initiated, and the most commonly cited rationale was family obligation (78.9%). Most programs reported that residents who transferred in were on par with the existing resident cohort with respect to their medical knowledge, administrative abilities, and communication skills.Conclusion GS transfers were not uncommon and most were resident initiated secondary to family obligations. The majority of transfer resident skill sets met institutional expectations by the time of graduation. Programs surveyed were content with their decision to accept transfer residents.Level of evidence III.

Abstract

The low-lying excited states in the neutron-deficient $N=Z+1$ nucleus $^{87}_{43}$Tc$^{\ }_{44}$ have been studied via the fusion-evaporation reaction $^{54}$Fe($^{36}$Ar, $2n1p$)$^{87}$Tc at the Grand Acc\'el\'erateur National d'Ions Lourds (GANIL), France. The AGATA spectrometer was used in conjunction with the auxiliary NEDA, Neutron Wall, and DIAMANT detector arrays to measure coincident prompt $\gamma$-rays, neutrons, and charged particles emitted in the reaction. A level scheme of $^{87}$Tc from the (9/2$^{+}_{g.s.}$) state to the (33/2$^{+}_{1}$) state was established based on 6 mutually coincident $\gamma$-ray transitions. The constructed level structure exhibits a rotational behavior with a sharp backbending at $\hbar\omega\approx 0.50$ MeV. A decrease in alignment frequency and increase in alignment sharpness in the odd-mass isotonic chains around $N=44$ is proposed as an effect of the enhanced isoscalar neutron-proton interactions in odd-mass nuclei when approaching the $N=Z$ line.
