Abstract Introduction Abdominal surgical emergencies have a high mortality rate. Effective management primarily relies on the early identification of patients at high risk of postoperative complications. The objective of our study was to determine the prognostic factors associated with poor outcomes from abdominal surgical emergencies in Senegal and to establish a predictive score for mortality for preoperative risk evaluation (NDAR (New Death Assessment Risk) score). Methodology This was a retrospective national cross-sectional study conducted over one year in 14 regions of Senegal. Adult patients (aged > 15 years) who presented with a traumatic or non-traumatic abdominal surgical emergency were included. The studied variables included clinical and paraclinical data. The variable of interest was death within 30 days of the surgery. Logistic regression was used to identify the factors independently associated with mortality. Risk factors identified after logistic regression analysis were weighted using odds ratio (OR) values rounded to the nearest whole number. The predictive capacity of the score was evaluated by analyzing the ROC (Receiver Operating Characteristic) curve based on the area under the curve (AUC). Results A total of 1114 patient records were included, with a mortality rate of 4.4%. Diagnoses were observed in patients included appendicitis in 39.8% of cases (n = 444), followed by peritonitis in 22.3% (n = 249), intestinal obstruction in 18.5% (n = 205), strangulated hernias in 10.5% (n = 117), and abdominal trauma in 6.1%. Logistic regression, established the following scores: age > 40 years (score 2), ASA status grade 2 or higher (score 1), presence of a positive QSIRS score (score 2), diagnosis of peritonitis (score 2), diagnosis of intestinal obstruction (score 1), and the presence of intestinal necrosis (score 3). The score is positive if the total is strictly greater than 5, indicating a 17.7% risk of mortality. This score had a high predictive capacity with an AUC of 0.7397. Conclusion This study enabled the establishment of a score that allows for the early identification of at-risk patients, even in constrained resource settings, facilitating appropriate perioperative management and timely surgical intervention to reduce the risk of complications. This approach, focused on early recognition of high-risk patients, is crucial for improving clinical outcomes in abdominal surgical emergencies.

This review thoroughly assesses the viability and effectiveness of biological and advanced coagulation methods for treating leachate. It offers a comprehensive analysis of landfill leachate, with a specific focus on its composition and the presence of diverse pollutants and contaminants. The study investigates leachate toxicity and evaluates advanced coagulation processes and strategies designed to remove or reduce organic contaminants by employing coagulants. The novelty of this review lies in its emphasis on demonstrating the efficacy of coagulation and flocculation methods utilizing chemicals such as alum, ferric chloride, and aluminum sulfate in leachate treatment. These methods facilitate the removal of recalcitrant pollutants and demonstrate considerable removal efficiency. Noteworthy removal effectiveness is evident in coagulation/flocculation procedures targeting various contaminants, including suspended particles, heavy metals, and xenobiotic organic compounds. Additionally, the study examines the leachate’s toxicity both before and after treatment with the coagulation process.

Family farms play an important role in food security and nutrition in West Africa. Family farms are rapidly changing and face many constraints. Thus, characterizing them is necessary for policy purposes. This study aimed to characterise family farms in Niger, focusing on the current constraints to family farms and existing agroecological intensification (AEI) options. A survey was conducted using a questionnaire administered to 108 family farms across the selected six study locations. We used descriptive statistics to characterise the family farms and the factorial analysis of mixed data (FAMD) and the Hierarchical Clustering of Principal Components (HCPC) to identify the types of family farms. Results showed that family farms, on average, encompass three households, and the majority (87.2%) of the family farms surveyed were managed by married persons where 25.7% of whom are female. Agriculture remains the primary source of income for 98.2% of people in the study locations, and the main second source of family income (47.7%) is animal husbandry. Family farms are mainly characterised by the presence of the main field (MF) (98%) led by the heads of households and the presence of the women fields (WF) (78%) led by the women. Regarding farm size, the average MF and the WF farm sizes were 3.0 ha and 1.5 ha, respectively. The main identified constraints to family farms were soil fertility decline (84.33%), a recurrent early end of the rainy season (84.33%), farmers' low income (41.15%), and limited access to the market (24.70%). Twelve potential AEIs options across the study area were explored. In the MF, the major AEI options practised by farmers were: the application of organic manure as fertilizer (73.27%), cereallegume intercropping (68.83%), and Farmer Managed Natural Regeneration (35.85%). Whereas in the WF, the AEI practices were the application of organic manure (55.28%), and cereal-legume intercropping (28.98%). Co-building an integrative approach that combines multiple AEI options in the same field is necessary to tackle the main drivers of the farming systems. Abstract Providing sustainable, affordable, and reliable electricity through low-carbon energy development in the Nigerian energy sector is fundamental to ensuring energy security. Currently, efforts to harness the potential of renewable energy, to provide universal electricity access for all have not translated into significant economic development in Nigeria. Investment in green hydrogen could strengthen Nigeria’s net-zero transition plan (NETP) and achieve sustainable energy access. The study explored the role of green hydrogen among five Electricity Distribution Companies (DisCos), from three geopolitical zones in Nigeria—North West, North Central, and North East. A bottom-up optimization linear programming methodology based on an open energy modelling framework (OEMOF) was used as the modelling paradigm. Secondary data mined from the Nigeria Energy Commission, Nigeria Electricity Regulatory Commission, NECAL 2050 report and international reports, and 2020 was used as a reference year to benchmark the model. The basic characteristics of the generation of electricity from green hydrogen, fuel cells, electrolyzers, and hydrogen storage, among other existing generation plants, were modelled till 2060 using modelled daily data obtained from Toktarova et al. (Electrical Power and Energy Systems 111:160–181, 2019). Outcomes from benchmarking led to two planning scenarios; these investigated possible insights that explored green hydrogen in Nigeria. Results showed that an integrated distributed approach would enhance harnessing green hydrogen in Nigeria, that is, electricity distribution among the DisCos. The study also revealed the following (1) the levelized cost of electricity could drop by about 8%, so also the cost of the investment; (2) access to electricity showed an improvement compared to the base year; and (3) emissions were cut in the power sector. To attain sustainable NETP with green hydrogen, the study recommends that a distributed generation approach among DisCos would support the national net-zero transition plan.

Background: Anemia remains a significant public health concern in Gabon, particularly among children, adolescents, and females. Gabon is also home to two major species of filarial worms, Loa and Mansonella spp., which cause microfilaremia. The epidemiological nexus between hemoglobin (Hb) concentrations and microfilaremia in Gabonese first-time blood donors remains unknown. Aims: To understand better the epidemiological relationship between anemia and microfilaremia to improve donor selection and management protocols. Study Design: A retrospective cohort study. Methods: This study was conducted among first-time blood donors in Lambaréné between March 2018 and October 2019. Participants aged 16-65 years old and weighing a minimum of 50 kg were enrolled using standard donor selection criteria. An automatic hematological analyzer was used to quantify Hb concentrations, and microscopy techniques were used to detect the presence of microfilariae. Results: Microfilariae were found in 4.8% (35/723) of the 723 first-time blood donors from Lambaréné. Anemia was classified as mild in 35.5% (257/723) and moderate in 1% (7/723). No significant associations were found between the distribution of microfilariae and variables such as age, sex, socioprofessional classification, marital status, or residence. Blood group O donors had a higher prevalence of microfilariae (6%) than non-O donors (2.7%). However, the observed difference was not statistically significant (AOR =2.3, p = 0.052). Furthermore, microfilariae were associated with increased moderate anemia (3.7% vs. 29%, AOR =15.6, p = 0.003). Conclusion: Our findings highlight microfilaremia as a possible etiological cause of anemia among Gabonese blood donors, emphasizing the need for further research and a potential review of donor management strategies.

Objectives: Despite evidence of praziquantel's (PZQ) safety for treating schistosomiasis in pregnancy, many countries withhold treatment. Only two randomized controlled trials have investigated PZQ in pregnancy, none involving Schistosoma haematobium. Methods: Pregnant women during the second trimester in Lambaréné (Gabon) were screened for S. haematobium infection using urine microscopy and circulating anodic antigen detection. Participants positive for either test were randomized (3:1) to single-dose PZQ 40 mg/kg during pregnancy versus no treatment during pregnancy. Investigators were blinded for allocation. Primary outcomes were reduction of egg (egg reduction rate [ERR]) and antigen production (infection reduction rate [IRR]) while explorative outcomes included assessment of cure rate, adverse events, maternal hemoglobin levels, maternal anemia prevalence at delivery, pregnancy outcomes, and newborn anthropometric parameters. Results: Of 761 women screened 165 were eligible and randomized (intervention n = 124, control n = 41). Of them, 124 completed the study (n = 90 and n = 34, respectively). Treatment led to a significantly higher ERR (95.0% [91-97%] vs 27.0% [−42-63%]) and IRR (95% [91-97%] vs 56% [14-78%]). Common adverse events were dizziness, nausea, and vomiting. Maternal anemia at delivery was significantly lower in the intervention group (odds ratio: 0.40 [0.16;0.96], P = 0.04). No increased risk for adverse pregnancy outcomes was observed. Conclusions: This first randomized controlled trial investigating PZQ in pregnant women with S. haematobium found PZQ to be safe, effective, and reducing maternal anemia. We recommend treating confirmed infections to prevent morbidity in pregnant women.

Despite a significant reduction in the burden of malaria in children under five years-old, the efficient implementation of seasonal malaria chemoprevention (SMC) at large scale remains a major concern in areas with long malaria transmission. Low coverage rate in the unattainable areas during the rainy season, a shift in the risk of malaria to older children and the rebound in malaria incidence after stopping drug administration are mainly reported in these areas. These gaps represent a major challenge in the efficient implementation of SMC measures. An open randomized study was conducted to assess the effect of a fifth additional round to current regime of SMC in older children living in Dangassa, a rural malaria endemic area. Poisson regression Model was used to estimate the reduction in malaria incidence in the intervention group compared to the control group including age groups (5-9 and 10-14 years) and the use of long-lasting insecticidal nets (LLINs; Yes or No) with a threshold at 5%. Overall, a downward trend in participation rate was observed from August (94.3%) to November (87.2%). In November (round 4), the risk of malaria incidence was similar in both groups (IRR = 0.66, 95%CI [0.35-1.22]). In December (round 5), a decrease of 51% in malaria incidence was observed in intervention group compared to control group adjusted for age groups and the use of LLINs (IRR = 0.49, 95%CI [0.26-0.94]), of which 17% of reduction is attributable to the 5th round in the intervention group. An additional fifth round of SMC resulted in a significant reduction of malaria incidence in the intervention group. The number of SMC rounds could be adapted to the local condition of malaria transmission.

The Sahel region is projected to be highly impacted by the more frequent hazards associated with climate change, including increased temperature, drought and flooding. This systematic review examined the evidence for climate change-related health consequences in the Sahel. The databases used were Medline (PubMed), Embase (Ovid), Web of Science (Clarivate) and CABI Global Health. Hand searches were also conducted, which included directly engaging Sahelian researchers and hand-searching in the African Journals Online database. Of the 4153 studies found, 893 were identified as duplicates and the remaining 3260 studies were screened (title and abstract only) and then assessed for eligibility. A total of 81 studies were included in the systematic review. Most studies focused on vector-borne diseases, food security, nutrition and heat-related stress. Findings suggest that mosquito distribution will shift under different climate scenarios, but this relationship will not be linear with temperature, as there are other variables to consider. Food insecurity, stunting (chronic malnutrition) and heat-related mortality are likely to increase if no action is taken owing to the projected impact of climate change on environmental factors and agriculture. Seventy-one per cent of manuscripts (n = 58) had first authors from institutions in North America or Europe, of which 39.7% (n = 23) included co-authors from African institutions.

Most developing countries' electric power system is stressed by an unprecedented demand growth as well as obstacles that call for urgent actions. Therefore, tackling the present-day power-related challenges and ensure dependable and safe electricity may result in improving living conditions. This research aims to comprehend the primary factors that impede power companies in emerging economies and propose ways of addressing them with a focus on Togolese electricity system as a case study., The methodology utilized to study a complex and dynamic system like electricity sector is an integrated model composed of a survey and review of available literature, an interview with energy experts and the SWOT/PESTLE analysis to perform an in-depth and all-encompassing analysis. The study revealed that the electrification poverty was 39.47 % at countrywide level that requires an additional power of 220.95 MW to that of 2021 to achieve 100 % of electricity access by 2030. Moreover, the system's performance is hindered by a number of internal and external bottlenecks. They include but not limited to limitations in policies and regulations; technical difficulties in the transmission, distribution and off-grid subsystems; insufficient investments; and a lack of incentives and taxes rebates. In light of these findings, a model prioritizing a resilient power system was proposed for transforming the outdated power infrastructure in developing countries laying stress upon energy mix planning, transmission and distribution subsectors innovation and effective regional collaboration.

Soils have recently received attention in the policy area due to their various connections to climate change, human health and their key role in sustaining human societies in general. In this context, agricultural production and healthy nutritious food are linked to soil health and the diversity of their (micro-)biome, which depend on organic carbon materials as an energy and nutrient source. In this paper, we review the evidence showing that carbon-rich soils improve the resilience of human societies to pandemics and other crises. We indicate pathways for how the loss of soil carbon due to farming could be reversed by transformations within our food systems. Moreover, we argue that soil carbon has a strong role to play in enhancing environmental and human health in addition to mitigating and adapting to climate change. This multifaceted role requires a transdisciplinary dialogue and multi-stakeholder collaboration.

The Rigler's Triad consists by three radiological signs, including intestinal obstruction, pneumobilia, and an aberrant gallstone in the bowel. It is an inconstant triad considered being pathognomonic of gallstone ileus. Gallstone ileus is an exceptional complication of cholelithiasis due to the passage of one or more gallstones from the bile ducts into the lumen of the bowel through a biliodigestive fistula. We report the case of an 83-year-old female patient with a history of ischemic heart disease and an asymptomatic large gallstone. The patient was admitted to the emergency department for bowel obstruction, abdominal pain, and bilious vomiting. A clinical examination found a patient with an alteration in general condition and a distended abdomen with tenderness. An abdominal CT scan revealed Rigler's triad, allowing the diagnosis of gallstone ileus. A midline exploratory laparotomy was performed to find a giant gallstone blocked in the last ileum loop. A simple enterolithotomy was performed, allowing the extraction of giant lithiasis from an 8-cm major axis. The postoperative evolution was uneventful, and the patient was discharged 4 days after surgical treatment.

Abstract Objective To evaluate the surgical management of thyroid pathologies at the Reference General Hospital. Methods This was a retro-prospective study over 4 years 6 months carried out in the departments of General and Digestive Surgery (GDS) and Otorhinolaryngology and Cervico Facial Surgery (ORL/FCS). It involved 182 patients who underwent thyroid surgery. Results A frequency of thyroidectomy of 9.46% was found. Females predominated with a sex ratio of 0.1. The average age of patients was 42.85 years, a standard deviation 12.80. 84.06% of patients had consulted for anterior cervical mass. EU-TIRADS score 3 represented 7,14% of cases. Heteromultinodular goiter was the main indication for thyroid surgery (59.34%). Total thyroidectomy was the most commonly performed gesture in general surgery in 88,23% (n = 105), in Otorhinolaryngology, it was in the same proportion as lobo-isthmectomy at 47.61% (n = 30). The first route was video-assisted thyroidectomy 2.2% (n = 4). The recurrent laryngeal nerve was dissected and seen in 159 cases (87.36%) and parathyroid glands were also seen and preserved in 58.24% of cases (n = 106). In immediate postoperative surgery, the main complications were unilateral recurrent paralysis with dysphonia in 3.3% (n = 6) and compressive hematoma in 2.2% (n = 4). No deaths had been recorded. Conclusion Total thyroidectomy was the most performed procedure in department of General and Digestive Surgery. Routine oral calcium and vitamin D supplementation in the general surgery ward, reduces the occurrence of hypocalcemia after total thyroidectomy and allows a safe and early exit. Standardizing protocols will further reduce complications.

This study uses a multi-stakeholder analysis approach to consolidate low-carbon energy planning and facilitate energy transition (ET). To do this, we assess factors that influence ET strategies in the Nigerian power sector. Here we employ both quantitative and qualitative data obtained via well-structured, and concise questionnaires and semi-structured interviews. While our study brings to light a sustainable approach to achieving a low-carbon future, it identified technology, motivation and finance as variables that are significant in accelerating the country’s transition to clean energy. It revealed that prioritising low-interest rate and tax holiday for investors’ of solar energy and consistence expansion in energy efficiency (EE) is pertinent to achieving a sustainable low-carbon future in Nigeria. We conclude that robust policy frameworks that prioritise investment in solar energy and EE through incentivising energy management solutions with appropriate financial tools and fostering active verbal support through advocacy are essential to ET in Nigeria.

The aim of this study was to describe the frequency, clinical signs, management, and outcomes of snakebite patients admitted to the envenomation treatment center of the Institut de Recherche en Biologie Appliquée de Guinée (IRBAG). This was a retrospective review combining aggregated annual statistics (2011–2015) and routine data (from January to October 2021) from the IRBAG treatment center. There were 1345 (57.2%) snakebite victims out of a total of 2352 consultations at the center during the study period. Males (67.7%), persons aged ≥45 years (29%) and ≤14 years (27.7%), farmers/housewives (44.5%), workers (23.9%), and those residing in the Kindia Prefecture (53.5%) were the most commonly affected. The majority of victims (84.5%) were admitted three hours after snakebite, with bites mainly occurring in rural areas (86.5%) and during the rainy season (83.2%). Pain (100%), edema (76.8%), and bleeding (65.2%) were the most common clinical presentations. Almost all victims received antivenom serum (98%), antibiotics (87.7%), and analgesics or anti-inflammatory drugs (88.4%). Six out of the one hundred and fifty-five patients died. Snakebites are a frequent public health problem in rural Guinea. The majority of victims seek medical attention too late. There is an urgent need to include snakebite in the country’s list of priority NTDs in order to promote access to antivenom serum.

Le mil est l’une des plantes alimentaires les plus cultivées au Niger. Cette étude, menée dans le village de Digargo au Niger, a pour objectif d’évaluer les effets combinés du zaï et du nombre de plants par poquet sur les paramètres de croissance et de rendement du mil. Un dispositif expérimental en randomisation totale avec trois (3) répétitions a été mis en place et deux (2) facteurs ont été étudiés. Le facteur (A) a deux (2) niveaux A1 et A2 où le mil est semé respectivement sur un sol traité en zaï et sur un sol témoin et le facteur (B) nombre de plants par poquet avec cinq (5) niveaux (B1, B2, B3, B4 et B5). Les meilleurs rendements en grains ont été obtenus sur les sols traités en zaï avec trois (3) et quatre (4) plants par poquet, notamment A1B3 et A1B4, avec respectivement 2523 et 2187 kg/ha. D’autre part, l’ACP montre que les traitements A1B3 et A1B4 se caractérisent par un bon rendement en épis, en grains et un poids élevé des mille grains. Le traitement en zaï contribue à améliorer les rendements des cultures et peut être proposé pour une gestion agroécologique de la fertilité des sols. Abstract Background Aedes albopictus and Aedes aegypti are known for their potential as vectors of dengue (DENV) and chikungunya (CHIKV) viruses. However, entomological surveys are mostly carried out during epidemics. In Gabon where outbreaks of both viruses have occurred, there is no vector control program targeting these arboviruses. Therefore, we assessed the presence of Aedes species along a rural–urban gradient in Lambaréné (Gabon) and its surroundings and determined ecological factors associated to their presence. Methods An entomological survey was conducted in Lambaréné and its surrounding rural areas. Mosquitoes were collected with aspirators around human dwellings, and ecological and environmental data were collected from each study area. Morphological identification keys were used to identify Aedes species. RNA was extracted from pools of female mosquitoes and amplified by RT-qPCR to detect the presence of DENV and CHIKV. Results Overall, the most common vector collected was Aedes albopictus (97%, 4236/4367 specimens), followed by Aedes aegypti (3%, 131/4367). Albopictus vectors was more abundant in the rural area (Wilcoxon signed-rank test, Z = 627, P = 0.043) than in the urban area. In the urban area, a higher number of mosquitoes (45%) were recorded in the economic zone (zone 3) than in the historical and administrative zones (zone 1 and 2). In the rural area, the proportions of species numbers were significantly higher along the south rural transect (92%) compared to the north rural transect (Wilcoxon signed-rank test, Z = 43, P ˂ 0.016). Abstract Background Intussusception is a rare condition in adults, accounting for 5% of intestinal intussusception and being responsible for approximately 1% of all adult bowel obstructions. Neoplastic origin is the most common etiology of intestinal intussusception in adults, unlike pediatric intussusception, which is usually idiopathic. Intussusception due to the appendiceal mucocele is exceptional, and only a few cases have been reported in the medical literature. Case presentation We report the case of a 25-year-old black African male patient with no medical history. He presented to the emergency department for abdominal pain, nausea, and bilious vomiting. The abdominal examination revealed typical signs of acute bowel obstruction. Enhanced abdominopelvic computed tomography showed an invagination of the last ileal loop, cecum, and ascending colon into the lumen of the transverse colon, with a rounded image with hypodense content and some calcifications compatible with an appendiceal mucocele. An emergency exploratory laparoscopy was performed and confirmed the ileocecocolic intussusception. Right hemicolectomy and ileocolic anastomosis were performed. The patient recovery postoperatively was uneventful, and he was discharged 4 days later. Histological examination of the surgical specimen confirmed the diagnosis of mucinous cystadenoma. Conclusion The symptoms of bowel intussusception with the appendiceal mucocele as the lead point in adults are similar to any other bowel intussusception. Differential diagnosis is often carried out thanks to the injected abdominal computed tomography scan.

Understanding vegetation seasonality and its driving mechanisms improves decision-making in the management of ecological systems in a warming global climate. Using multiple statistical methods (i.e., trend analysis, abrupt changes, and partial correlation analysis), this study analyzed the spatiotemporal variations in the Normalized Difference Vegetation Index (NDVI) in the Equatorial Africa (EQA) region and their responses to climate factors from 1982 to 2021. The NDVI values declined at a rate of 0.00023 year−1, while the precipitation (P) and mean temperature (TMEAN) values increased at rates of 0.22 mm year−1 and 0.22 °C year−1, respectively. The mean minimum temperature (TMIN) had a higher rate of 0.2 °C year−1 than the mean maximum temperature (TMAX) at 0.02 °C year−1. An abrupt change analysis showed that the TMAX, P, and NDVI breakpoints occurred in 2000, 2002, and 2009, respectively; TMEAN and TMIN breakpoints occurred in 2001. The NDVI trends declined in forest and cropland areas but increased in shrubland and grassland areas. The summer NDVI trends declined for all vegetation types and were reversed in the winter season. The NDVI positively correlated with the P (r = 0.50) and TMEAN (r = 0.60). All seasonal analyses varied across four seasons. A temporal analysis was conducted using partial correlation analysis (PCR), and the results revealed that TMIN had a greater impact on the NDVI (PCR = −0.45), followed by the TMAX (PCR = 0.31) and then the P (PCR = −0.19). The annual trend showed that areas with significant greening were consistent with stronger wetter and weaker warming trends. Both precipitation and temperature showed a positive relationship with vegetation in semi-arid and arid regions but a negative relationship with humid regions. Our findings improve our insight into scientific knowledge on ecological conservation.

The study entitled "A Study on Reading Habits of the University Students in Nigeria: A Case of Selected Students of Economics Department at the Nile University of Nigeria" analyzed the major factors that hinder regular or systematic reading habits among university students. To determine reading habits, the study examined the concept of reading, its types, reading culture, the importance of reading, and many others through the literature review. Using a carefully-designed questionnaire and nonparticipatory observations as its research instruments, data was obtained from sixty randomly selected students of the Department of Economics who constituted the population of the study. From the analysis of the data using the quantitative paradigm as the theoretical framework, the findings indicated that there were some linguistic factors responsible for the decline in reading habits among the students included in our target group. The adverse impact of the Internet, the lack of interest, television addiction, and the lack of books made reading difficult. The study concluded by reiterating the importance of reading not just for the individual but the nation at large. Thereafter, the study offered some recommendations to the students, the management of the University, and the government at large.
