Abstract: Rationale & objectiveUnderlying kidney disease is an emerging risk factor for more severe coronavirus disease 2019 (COVID-19) illness. We examined the clinical courses of critically ill COVID-19 patients with and without pre-existing chronic kidney disease (CKD) and investigated the association between the degree of underlying kidney disease and in-hospital outcomes.Study designRetrospective cohort study.Settings & participants4,264 critically ill patients with COVID-19 (143 patients with pre-existing kidney failure receiving maintenance dialysis; 521 patients with pre-existing non-dialysis-dependent CKD; and 3,600 patients without pre-existing CKD) admitted to intensive care units (ICUs) at 68 hospitals across the United States.Predictor(s)Presence (vs absence) of pre-existing kidney disease.Outcome(s)In-hospital mortality (primary); respiratory failure, shock, ventricular arrhythmia/cardiac arrest, thromboembolic events, major bleeds, and acute liver injury (secondary).Analytical approachWe used standardized differences to compare patient characteristics (values>0.10 indicate a meaningful difference between groups) and multivariable-adjusted Fine and Gray survival models to examine outcome associations.ResultsDialysis patients had a shorter time from symptom onset to ICU admission compared to other groups (median of 4 [IQR, 2-9] days for maintenance dialysis patients; 7 [IQR, 3-10] days for non-dialysis-dependent CKD patients; and 7 [IQR, 4-10] days for patients without pre-existing CKD). More dialysis patients (25%) reported altered mental status than those with non-dialysis-dependent CKD (20%; standardized difference=0.12) and those without pre-existing CKD (12%; standardized difference=0.36). Half of dialysis and non-dialysis-dependent CKD patients died within 28 days of ICU admission versus 35% of patients without pre-existing CKD. Compared to patients without pre-existing CKD, dialysis patients had higher risk for 28-day in-hospital death (adjusted HR, 1.41 [95% CI, 1.09-1.81]), while patients with non-dialysis-dependent CKD had an intermediate risk (adjusted HR, 1.25 [95% CI, 1.08-1.44]).LimitationsPotential residual confounding.ConclusionsFindings highlight the high mortality of individuals with underlying kidney disease and severe COVID-19, underscoring the importance of identifying safe and effective COVID-19 therapies in this vulnerable population.

2. Shannon entropies and Fisher information of K-shell electrons of neutral atoms

Abstract: We represent the two K-shell electrons of neutral atoms by Hylleraas-type wave function which fulfils the exact behavior at the electron-electron and electron-nucleus coalescence points and, derive a simple method to construct expressions for single-particle position- and momentum-space charge densities, $\rho(\vec{r})$ and $\gamma(\vec{p})$ respectively. We make use of the results for $\rho(\vec{r})$ and $\gamma(\vec{p})$ to critically examine the effect of correlation on bare (uncorrelated) values of Shannon information entropies ($S$) and of Fisher information ($F$) for the K-shell electrons of atoms from helium to neon. Due to inter-electronic repulsion the values of the uncorrelated Shannon position-space entropies are augmented while those of the momentum-space entropies are reduced. The corresponding Fisher information are found to exhibit opposite behavior in respect of this. 5. Fisher information for quasi-one-dimensional hydrogen atom

Abstract: The coordinate-space wave function $\psi(x)$ of quasi-one-dimensional atoms is defined in the $x\geq 0$ region only. This poses a typical problem to write a physically acceptable momentum-space wave function $\phi(p)$ from the Fourier transform of $\psi(x)$. We resolve the problem with special attention to the behavior of real and imaginary parts of the complex-valued function $\phi(p)$ as a function of $p$ and confirm that $\phi_i(p)$ (the imaginary part of $\phi(p)$) represents the correct momentum-space wave function. We make use of the results for $\psi(x)$ and $\phi_i(p)$ to express the position- and momentum-space Fisher information in terms of the principal quantum number and energy eigen value of the system and provide some useful checks on the result presented with particular attention on the information theoretic uncertainty relation.

6. On the correlation measure of two-electron systems

Abstract: We make use of a Hylleraas-type wave function to derive an exact analytical model to quantify correlation in two-electron atomic/ionic systems and subsequently employ it to examine the role of inter-electronic repulsion in affecting (i) the bare (uncorrelated) single-particle position- and momentum-space charge distributions and (ii) corresponding Shannon's information entropies. The results presented for the first five members in the helium iso-electronic sequence, on the one hand, correctly demonstrate the effect of correlation on bare charge distributions and, on the other hand, lead us to some important results for the correlated and uncorrelated values of the entropies. These include the limiting behavior of the correlated entropy sum (sum of position- and momentum-space entropies) and geometrical realization for the variation of information entropies as a function of Z. We suggest that, rather than the entropy sum, individual entropies should be regarded as better candidates for the measure of correlation., Comment: 10 pages, 8 figures, 2 tables

7. National Trends in Emergency Room Visits of Dialysis Patients for Adverse Drug Reactions

Abstract: BACKGROUND:Various medications are cleared by the kidneys, therefore patients with impaired renal function, especially dialysis patients are at risk for adverse drug events (ADEs). There are limited studies on ADEs in maintenance dialysis patients. METHODS:We utilized a nationally representative database, the Nationwide Emergency Department Sample, from 2008 to 2013, to compare emergency department (ED) visits for dialysis and propensity matched non-dialysis patients. Log binomial regression was used to calculate relative risk of hospital admission and logistic regression to calculate ORs for in-hospital mortality while adjusting for patient and hospital characteristics. RESULTS:While ED visits for ADEs decreased in both groups, they were over 10-fold higher in dialysis patients than non-dialysis patients (65.8-88.5 per 1,000 patients vs. 4.6-5.4 per 1,000 patients respectively, p < 0.001). The top medication category associated with ED visits for ADEs in dialysis patients is agents primarily affecting blood constituents, which has increased. After propensity matching, patient admission was higher in dialysis patients than non-dialysis patients, (88 vs. 76%, p < 0.001). Dialysis was associated with a 3% increase in risk of admission and 3 times the odds of in-hospital mortality (adjusted OR 3, 95% CI 2.7-2.3.3). CONCLUSIONS:ED visits for ADEs are substantially higher in dialysis patients than non-dialysis patients. In dialysis patients, ADEs associated with agents primarily affecting blood constituents are on the rise. ED visits for ADEs in dialysis patients have higher inpatient admissions and in-hospital mortality. Further studies are needed to identify and implement measures aimed at reducing ADEs in dialysis patients.

Abstract: BackgroundThe epidemiology and outcomes of acute kidney injury (AKI) in prevalent non-renal solid organ transplant recipients is unknown.MethodsWe assessed the epidemiology of trends in acute kidney injury (AKI) in orthotopic cardiac and liver transplant recipients in the United States. We used the Nationwide Inpatient Sample to evaluate the yearly incidence trends (2002 to 2013) of the primary outcome, defined as AKI requiring dialysis (AKI-D) in hospitalizations after cardiac and liver transplantation. Abstract: We examine the effect of dissipation on traveling waves in nonlinear dispersive systems modeled by Benjamin- Bona- Mahony (BBM)-like equations. In the absence of dissipation the BBM-like equations are found to support soliton and compacton/anticompacton solutions depending on whether the dispersive term is linear or nonlinear. We study the influence of increasing nonlinearity of the medium on the soliton- and compacton dynamics. The dissipative effect is found to convert the solitons either to undular bores or to shock-like waves depending on the degree of nonlinearity of the equations. The anticompacton solutions are also transformed to undular bores by the effect of dissipation. But the compactons tend to vanish due to viscous effects. The local oscillatory structures behind the bores and/or shock-like waves in the case of solitons and anticompactons are found to depend sensitively both on the coefficient of viscosity and solution of the unperturbed problem., Comment: 25 pages, 39 figures

Abstract: The traditional method of factorization can be used to obtain only the particular solutions of the Li\'enard type ordinary differential equations. We suggest a modification of the approach that can be used to construct general solutions . We first demonstrate the effectiveness of our method by dealing with a solvable form of the modified Emden-type equation and subsequently employ it to obtain the solitary wave solutions of the KdV, mKdV, Rosenau-Hyman (RH) and NLS equations. The solution of the mKdV equation, via the so-called Muira transform, leads to a singular solution of the KdV equation in addition to the well-known soliton solution supported by it. We obtain the solution of the non-integrable RH equation in terms of the Jacobi function and show that, although robust, it is structurally different from the KdV soliton. We also find the soliton solution of the NLS equation that accounts for the evolution of complex envelopes in an optical medium., Comment: two figures

Abstract: In an attempt to look for the root of nonstandard Lagrangians in the theories of the inverse variational problem we introduce a logarithmic Lagrangian (LL) in addition to the so-called reciprocal Lagrangian (RL) that exists in the literature. The equation of motion that follows from the RL could easily be solved by using its first or Jacobi integral. This is not, however, true for the similar equation resulting from the LL. We make use of a method of factorization to find the particular solutions for equations following from both RL an LL and subsequently derive a novel approach to obtain their general solutions from the particular ones. The case studies presented by us include the modified Emden-type equation for which we also construct a new expression for the Lagrangian function (NL). We point out that NL is not related to RL by a total derivative or gauge term. In spite of that NL leads to the same equation of motion as that given by the RL., Comment: 8 pages, 0 figures

Abstract: The dynamical behavior of matter wave solitons of two-component Bose-Einstein condensates (BEC) in combined linear and nonlinear optical lattices (OLs) is investigated. In particular, the dependence of the frequency of the oscillating dynamics resulting from initially slightly displaced components is investigated both analytically, by means of a variational effective potential approach for the reduced collective coordinate dynamics of the soliton, and numerically, by direct integrations of the mean field equations of the BEC mixture. We show that for small initial displacements binary solitons can be viewed as point masses connected by elastic springs of strengths related to the amplitude of the OL and to the intra and inter-species interactions. Analytical expressions of symmetric and anti-symmetric mode frequencies, are derived and occurrence of beatings phenomena in the displaced dynamics is predicted. These expressions are shown to give a very good estimation of the oscillation frequencies for different values of the intra-species interatomic scattering length, as confirmed by direct numerical integrations of the mean field Gross-Pitaevskii equations (GPE) of the mixture. The possibility to use displaced dynamics for indirect measurements of BEC mixture characteristics such as number of atoms and interatomic interactions is also suggested., Comment: 8 pages, 21 figures

Abstract: National data on patient characteristics, treatment, and outcomes of critically ill coronavirus disease 2019 (COVID-19) solid organ transplant (SOT) patients are limited. We analyzed data from a multicenter cohort study of adults with laboratory-confirmed COVID-19 admitted to intensive care units (ICUs) at 68 hospitals across the United States from March 4 to May 8, 2020. From 4153 patients, we created a propensity score matched cohort of 386 patients, including 98 SOT patients and 288 non-SOT patients. We used a binomial generalized linear model (log-binomial model) to examine the association of SOT status with death and other clinical outcomes. Among the 386 patients, the median age was 60 years, 72% were male, and 41% were black. Death within 28 days of ICU admission was similar in SOT and non-SOT patients (40% and 43%, respectively; relative risk [RR] 0.92; 95% confidence interval [CI]: 0.70-1.22). Other outcomes and requirement for organ support including receipt of mechanical ventilation, development of acute respiratory distress syndrome, and receipt of vasopressors were also similar between groups. There was a trend toward higher risk of acute kidney injury requiring renal replacement therapy in SOT vs. non-SOT patients (37% vs. 27%; RR [95% CI]: 1.34 [0.97-1.85]). Death and organ support requirement were similar between SOT and non-SOT critically ill patients with COVID-19.

Abstract: IMPORTANCE: Therapies that improve survival in critically ill patients with coronavirus disease 2019 (COVID-19) are needed. Tocilizumab, a monoclonal antibody against the interleukin 6 receptor, may counteract the inflammatory cytokine release syndrome in patients with severe COVID-19 illness. OBJECTIVE: To test whether tocilizumab decreases mortality in this population. DESIGN, SETTING, AND PARTICIPANTS: The data for this study were derived from a multicenter cohort study of 4485 adults with COVID-19 admitted to participating intensive care units (ICUs) at 68 hospitals across the US from March 4 to May 10, 2020. Critically ill adults with COVID-19 were categorized according to whether they received or did not receive tocilizumab in the first 2 days of admission to the ICU. Data were collected retrospectively until June 12, 2020. A Cox regression model with inverse probability weighting was used to adjust for confounding. EXPOSURES: Treatment with tocilizumab in the first 2 days of ICU admission. MAIN OUTCOMES AND MEASURES: Time to death, compared via hazard ratios (HRs), and 30-day mortality, compared via risk differences. RESULTS: Among the 3924 patients included in the analysis (2464 male [62.8%]; median age, 62 [interquartile range {IQR}, 52-71] years), 433 (11.0%) received tocilizumab in the first 2 days of ICU admission.
