Abstract Clostridioides difficile, a cause of healthcare-associated infections, poses a significant global health threat. This multi-institutional retrospective study focuses on epidemic dynamics, emphasizing minor and toxin-negative clinical isolates through high-resolution genotyping. The genotype of the C. difficile clinical isolates during 2005 to 2022 was gathered from 14 hospitals across Japan (N = 982). The total number of unique genotypes was 294. Some genotypes were identified in every hospital (cross-regional genotypes), while others were unique to a specific hospital or those in close geographic proximity (region-specific genotypes). Notably, a hospital located in a sparsely populated prefecture exhibited the highest prevalence of region-specific genotypes. The isolation rate of cross-regional genotypes positively correlated with the human mobility flow. A 6-month interval analysis at a university hospital from 2019 to 2021 revealed a temporal transition of the genotype dominance. The frequent isolation of identical genotypes over a brief timeframe did not always align with the current criteria for defining nosocomial outbreaks. This study highlights the presence of diverse indigenous C. difficile strains in regional environments. The cross-regional strains may have a higher competency to spread in the human community. The longitudinal analysis underscores the need for further investigation into potential nosocomial spread.

The generation of a plasma with an ultrahigh energy density of 1.2 GJ/cm3 (which corresponds to about 12 Gbar pressure) is investigated by irradiating thin stainless-steel foils with high-contrast femtosecond laser pulses with relativistic intensities of up to 1022 W/cm2. The plasma parameters are determined by X-ray spectroscopy. The results show that most of the laser energy is absorbed by the plasma at solid density, indicating that no pre-plasma is generated in the current experimental setup.

This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic Games. Data on 567 injuries and other illnesses of athletes treated at the on-site clinics were collected from the Tokyo 2020 Organizing Committee. Of these, 84 athletes who required outpatient care during the Games were registered for this survey. During the Olympic and Paralympic Games, 66 (8.3/1 000) and 18 (7.2/1 000) athletes, respectively, consulted external medical facilities. In the Olympic Games, the reasons for these visits included 48 cases (72.7%) of injuries, 13 (19.7%) cases of illnesses, and 5 (7.6%) cases of heat stroke illness (HSI). Of these patients, 56 (84.9%) were treated as outpatients and 10 (15.1%) were hospitalized, while three of these patients required hospitalization for > 7 days. On the other hand, in the Paralympics Games, there were 7 (38.8%) cases of injuries, 9 (50.0%) other illnesses, 1 (5.6%) case of HSI, and 1 (5.6%) other cases, of which 11 (61.1%) were treated as outpatients and 7 (38.9%) were hospitalized, but none was hospitalized for > 7 days. Injuries accounted for 70% of the total cases at the 2021 Olympic Games, but only three (0.05%) were severe cases that required hospitalization for more than 1 week. In contrast, in the Paralympic Games, other illnesses accounted for approximately half of the total cases. This study provides details on the extent of injuries and other illnesses that were transferred to outside facilities, which has not been documented in previous games.

We investigated imaging conditions for the distribution of lutetium oxodotreotide (Lu-177 DOTATATE) in the body during peptide receptor radionuclide therapy for neuroendocrine tumor (NET). We investigated imaging conditions using gamma rays emitted from the radionuclide. The gamma rays had energy peaks at 113 and 208 keV and characteristic X-rays at 56 keV. Image quality was compared by utilizing a combination of low–medium-energy general-purpose (LMEGP) and medium-energy general-purpose (MEGP) collimators. This study included the measurement of total spatial resolution (Full Width at Half Maximum) using a line source phantom. We compared the image quality of static images using a plane phantom and SPECT images using a cylindrical phantom. This comparison involved assessing recovery coefficient curves, count ratio, and %CV. Imaging evaluation was also performed on one NET patient. In phantom studies and the clinical study, comparing the combination of the three energy peaks (56 + 113 + 208 keV) using the LMEGP collimator and the conventional combination (113 + 208 keV) using the MEGP collimator revealed a count ratio of 1.9 times the maximum, stable %CV, and the best image quality.

Laser shock peening (LSP) is a powerful technique for improving the fatigue performance of metallic components by customizing compressive residual stresses in the desired near-surface regions. In this study, the residual stress distribution characteristics of 6061-T6 aluminum alloy induced by LSP were identified by the X-ray diffraction method, and their dependent factors (i.e., LSP coverage, LSP energy, and scanning path) were evaluated quantitatively by numerical simulations, exploring the formation mechanism of LSP residual stresses and the key role factor of the distribution characteristics. The results show that LSP is capable of creating anisotropic compressive residual stresses on the specimen surface without visible deformation. Compressive residual stresses are positively correlated with LSP coverage. The greater the coverage, the higher the residual stress, but the longer the scanning time required. Raising LSP energy contributes to compressive residual stresses, but excessive energy may lead to a reduction in the surface compressive residual stress. More importantly, the anisotropy of residual stresses was thoroughly explored, identifying the scanning path as the key to causing the anisotropy. The present work provides scientific guidance for efficiently tailoring LSP-induced compressive residual stresses to improve component fatigue life.

Ultrafast optical probing is a widely used method of underdense plasma diagnostic. In relativistic plasma, the motion blur limits spatial resolution in the direction of motion. For many high-power lasers the initial pulse duration of 30–50 fs results in a 10–15 μm motion blur, which can be reduced by probe pulse post-compression. Here we used the compression after compressor approach [Phys.-Usp. 62, 1096 (2019); JINST 17 P07035 (2022)], where spectral broadening is performed in thin optical plates and is followed by reflections from negative-dispersion mirrors. Our initially low-intensity probe beam was down-collimated for a more efficient spectral broadening and higher probe-to-self-emission intensity ratio. The setup is compact, fits in a vacuum chamber and can be implemented within a short experimental time slot. We proved that the compressed pulse retained the high quality necessary for plasma probing.

Tight focusing with very small f-numbers is necessary to achieve the highest at-focus irradiances. However, tight focusing imposes strong demands on precise target positioning in-focus to achieve the highest on-target irradiance. We describe several near-infrared, visible, ultraviolet and soft and hard X-ray diagnostics employed in a ∼1022 W/cm2 laser–plasma experiment. We used nearly 10 J total energy femtosecond laser pulses focused into an approximately 1.3-μm focal spot on 5–20 μm thick stainless-steel targets. We discuss the applicability of these diagnostics to determine the best in-focus target position with approximately 5 μm accuracy (i.e., around half of the short Rayleigh length) and show that several diagnostics (in particular, 3 $\omega$ reflection and on-axis hard X-rays) can ensure this accuracy. We demonstrated target positioning within several micrometers from the focus, ensuring over 80% of the ideal peak laser intensity on-target. Our approach is relatively fast (it requires 10–20 laser shots) and does not rely on the coincidence of low-power and high-power focal planes.

Absolute Kα line spectroscopy is proposed to study laser-plasma interactions taking place in cone-guided fast ignition targets. A transmission-type spectrometer was developed, and its absolute sensitivity was measured with laser-generated x-rays and radioisotopes, showing good agreement with theoretical predictions. The absolute yield of Au Kα line was mesured in the fast ignition experimental campaign perfomed at ILE, Osaka University. Assuming a single Maxwellian electron spectrum and one-way travel of hot electrons through the cone, energy transfer efficiency of incident LFEX laser to hot electrons was derived.

Kα line emission from Mo was experimentally and theoretically studied using clean, ultrahigh-intensity femtosecond laser pulses. The absolute yields of Kα x-rays at 17 keV from Mo were measured as a function of the laser pulse contrast ratio and irradiation intensity. Significantly enhanced Kα yields were obtained by employing high contrast ratio at optimum irradiance. Conversion efficiencies of 4.28 × 10−5/sr, the highest values obtained to date, was demonstrated with contrast ratios in the range of 10−10 to 10−11.

Introduction Among the 43 venues of Tokyo 2020 Olympic Games (OG) and 33 venues of Paralympic Games (PG) were held, the heat island effect was highly expected to cause heat-related illnesses in the outdoor venues with maximum temperatures exceeding 35°C. However, the actual number of heat-related illness cases during the competition was lower than that was initially expected, and it was unclear under what conditions or environment-related heat illnesses occurred among athletes.Object To clarify the cause and factors contributing to the occurrence of heat-related illness among athletes participating in the Tokyo 2020 Olympic and Paralympic Games.Method This retrospective descriptive study included 15 820 athletes from 206 countries. From 21 July 2021 to 8 August 2021 for the Olympics, and from 24 August 2021 to 5 September 2021 for the Paralympics. The number of heat-related illness cases at each venue, the incidence rate for each event, gender, home continent, as well as the type of competition, environmental factors (such as venue, time, location and wet-bulb globe temperature (WBGT)), treatment factor and the type of competition were analysed.Results More number of heat-related illnesses among athletes occurred at the OG (n=110, 76.3%) than at the PG (n=36, 23.7%). A total of 100 cases (100%) at the OG and 31 cases (86.1%) at the PG occurred at the outdoors venues. In the OG, a total of 50 cases (57.9%) occurred during the competition of marathon running and race walking at Sapporo Odori Park. Six of those, were diagnosed with exertional heat illness and treated with cold water immersion (CWI) at OG and one case at PG. Another 20 cases occurred in athletics (track and field) competitions at Tokyo National Olympic Stadium. In total, 10 cases (10.0%) were diagnosed with severe heat illness in the OG and 3 cases (8.3%) in the PG. A 27‐year‐old nulliparous woman presented with a feeling of fullness in the lower abdomen and abdominal pain. A left ovarian tumor, uterus didelphys, left renal agenesis, and left vaginal atresia were observed on imaging. The ovarian tumor was presumed to have caused the abdominal pain, and an abdominal left adnexectomy was performed. After 3 months, she reported severe lower abdominal pain during menstruation. Transvaginal ultrasonography revealed uterine enlargement. After 17 days, the patient presented with abdominal pain and fever. She was diagnosed with peritonitis due to infection and left uterine hematometra. Because she did not improve with antibiotic treatment, left laparoscopic hysterectomy was performed. Subsequently, she did not experience the lower abdominal pain. Appropriate diagnosis and treatment based on the morphology of the reproductive tract and symptoms must be considered in patients with Herlyn–Werner–Wunderlich syndrome.
