Abstract

Background: Accurate measurement is indispensable for effectively managing hypertension (HTN); any error in technique or instrumentation can lead to misdiagnosis and improper management. Thus, the present study aimed to assess the knowledge and skills of blood pressure (BP) measurement among nurses at a tertiary care cardiac center in Karachi. Materials and methods: Nursing staff responsible for BP assessment at various stations were identified, observed, and interviewed to evaluate their skill and knowledge levels regarding BP measurement techniques. Nurses' skill levels were assessed using a checklist based on the American Heart Association (AHA) guidelines for BP assessment. Results: Seventy-five nurses participated in the study, with 49 (65.3%) being male and a mean age of 32.1 ± 6.2 years. Only 25 (33.3%) nurses reported reading the AHA guidelines for BP measurement. None of the nurses demonstrated excellent skills; 19 (25.3%) showed good skills, while 56 (74.7%) showed poor skills in BP measurement. A poor compliance was observed on a total of 14/31 steps with compliance rate of less than 50%. Similarly, none of the nurses exhibited excellent knowledge; only 3 (4%) had good knowledge, while 72 (96%) had poor knowledge about BP measurement. A poor knowledge was observed on a total of 18/36 items with correct response rate of less than 50%. Conclusion: Nurses working at various stations of a tertiary cardiac center exhibited inadequate skills and knowledge regarding BP measurement. Abstract

To evaluate the shared genetic etiology of type 2 diabetes (T2D) and coronary heart disease (CHD), we conducted a genome-wide, multi-ancestry study of genetic variation for both diseases in up to 265,678 subjects for T2D and 260,365 subjects for CHD. We identify 16 previously unreported loci for T2D and 1 locus for CHD, including a new T2D association at a missense variant in HLA-DRB5 (odds ratio (OR) = 1.29). We show that genetically mediated increase in T2D risk also confers higher CHD risk. Joint T2D-CHD analysis identified eight variants-two of which are coding-where T2D and CHD associations appear to colocalize, including a new joint T2D-CHD association at the CCDC92 locus that also replicated for T2D. The variants associated with both outcomes implicate new pathways as well as targets of existing drugs, including icosapent ethyl and adipocyte fatty-acid-binding protein.

Abstract

A major goal of biomedicine is to understand the function of every gene in the human genome. Loss-of-function mutations can disrupt both copies of a given gene in humans and phenotypic analysis of such 'human knockouts' can provide insight into gene function. Consanguineous unions are more likely to result in offspring carrying homozygous loss-of-function mutations. In Pakistan, consanguinity rates are notably high. Here we sequence the protein-coding regions of 10,503 adult participants in the Pakistan Risk of Myocardial Infarction Study (PROMIS), designed to understand the determinants of cardiometabolic diseases in individuals from South Asia. Abstract

Objectives: This study assessed the efficacy of using steri-strips in conjunction with conventional pneumatic trans-radial (TR) bands for achieving radial artery hemostasis after diagnostic coronary angiography (CA). With varying methods aimed at reducing compression duration post-CA via trans-radial access (TRA), our aim was to evaluate time dynamics. Methodology: We randomly assigned 209 patients in a 1:1 ratio to receive either the TR band with steri-strips (treatment) or conventional TR band alone (control) post -diagnostic CA, and followed them for one month. The primary endpoint was time to achieve adequate hemostasis, defined by the total TR band application duration. Secondary endpoints included radial artery occlusion (RAO) and major bleeding. The treatment group comprised 106 patients with steri - strips alongside the TR band, while the control group had 103 patients with the TR band alone. Median TR band application duration was 60 [IQR: 60-60] min in the treatment group and 250 [IQR: 240-360] min in the control group; p=0.001. Results: The incidence of site bleed requiring TR band re-application was 9.4% vs. 4.9%, p=0.364, and hematoma (type I only) was 7.5% vs. 9.7%, p=0.578 in the treatment and control groups, respectively. Radial artery palpability after 24 hours was 95.3% vs. 85.4%; p=0.016, and at one month it was 96.2% vs. 88.3%; p=0.032 in the treatment and control arms, respectively. Conclusion: Steri-strips significantly reduce TR band compression duration and improve radial artery patency post-diagnostic angiography without causing hematoma or major bleeding complications. Abstract

Following percutaneous coronary intervention with stent placement to treat acute coronary syndromes, international clinical guidelines generally recommend dual antiplatelet therapy with aspirin plus a P2Y12receptor inhibitor for 12 months to prevent myocardial infarction and stent thrombosis. However, data on single antiplatelet therapy with a potent P2Y12inhibitor earlier than 12 months after percutaneous coronary intervention for patients with an acute coronary syndrome are scarce. The aim of this trial was to assess whether the use of ticagrelor alone, compared with ticagrelor plus aspirin, could reduce the incidence of clinically relevant bleeding events without an accompanying increase in major adverse cardiovascular or cerebrovascular events (MACCE).

Objective: This study was conducted to compare the predictive power of Shock Index (SI), TIMI Risk Index (TRI), LASH Score, and ACEF Score for the prediction of in-hospital mortality in a contemporary cohort of ST-segment elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary intervention (PCI) at a tertiary care cardiac center of a developing country. Methods: Consecutive patients diagnosed with STEMI and undergoing primary PCI were included in this study. SI, TRI, LASH, and ACEF were computed and their predictive power was assessed as the area under the curve (AUC) on the receiver operating characteristics (ROC) curve analysis for in-hospital mortality. Results: We included 977 patients, 780 (79.8%) of which were male, and the mean age was 55.6 ± 11.5 years. The in-hospital mortality rate was 4.3% (42). AUC for TRI was 0.669 (optimal cutoff: ≥17.5, sensitivity: 76.2%, specificity: 45.6%). AUC for SI was 0.595 (optimal cutoff: ≥0.9, sensitivity: 21.4%, specificity: 89.8%). AUC for LASH score was 0.745 (optimal cutoff: ≥0, sensitivity: 76.2%, specificity: 66.9%). AUC for the ACEF score was 0.786 (optimal cutoff: ≥1.66, sensitivity: 71.4%, specificity: 73.5%). Conclusion: In conclusion, ACEF showed sufficiently high predictive power with good sensitivity and specificity compared to other three scores. These simplified indices based on readily available hemodynamic parameters can be reliable alternatives to the computational complex scoring systems for the risk stratification of STEMI patients.
