A recanalização espontânea tardia da carótida interna é um evento incomum e pouco estudado. Os autores relatam o caso de paciente de 73 anos, masculino, hipertenso, com antecedente de acidente vascular cerebral há 3 anos, com seqüela motora e sensitiva em membro superior direito, apresentando ao mapeamento dúplex e arteriografia oclusão total da carótida interna. Evoluiu após 2 anos com novos episódios de ataques isquêmicos transitórios de repetição. Submetido a novos exames de imagem para avaliação da circulação cerebral extra e intracraniana, evidenciou-se recanalização do vaso, com estenose severa. Foi realizada endarterectomia de carótida esquerda, sem intercorrências. Paciente evoluiu sem novos episódios após 1 mês de seguimento. Considerando a raridade do caso e a parca literatura sobre recanalização tardia de carótida, passível de reparo cirúrgico, optamos pela apresentação do caso enfocando a importância do acompanhamento de oclusões carotídeas.

Late spontaneous recanalization of internal carotid artery is an unusual event that has received little attention. The authors report a case of a 73-year-old male patient, hypertensive, with previous history of cerebral vascular accident 3 years ago, with sensorimotor sequela in the right upper limb. Duplex scanning and arteriography showed total occlusion of the internal carotid artery. The patient progressed after 2 years with new episodes of recurrent transient ischemic attacks. When submitted to new imaging examinations to assess extra- and intracranial circulation, vessel recanalization was demonstrated, with severe stenosis. Left carotid endarterectomy was uneventfully performed. The patient progressed without new episodes after a 1-month follow-up. Considering the case rarity and the lack of literature on late carotid artery recanalization that can be surgically repaired, we decided to present this case focusing on the importance of following carotid artery occlusions.

However, the information provided by this test can go beyond the meters covered. The aim of this thesis was to expand the use of the 6MWT in patients with PAD, describing their relative values compared to peers without the disease and using the product of distance by body weight (DW) as a new way to interpretation 6MWT data. Furthermore, the second objective was to analyze the relationship of new interpretations with functional capacity and cardiovascular variables. Therefore, 227 patients with symptomatic PAD participated in the study. The 6MWT was performed and the onset claudication distance and the total walking distance in six minutes were obtained. The results of the 6MWT were then relativized and expressed as a percentage of a healthy individual without PAD. Additionally, from the obtained absolute distance, the DW was calculated. Functional capacity was objectively assessed using the Short Physical Performance Battery and the handgrip strength test, and subjectively using questionnaires (Walking Impairment Questionnaire and Walking Estimated-Limitation Calculated by History). The indicators of the cardiovascular system were obtained by measuring brachial blood pressure (BP), central blood pressure and arterial stiffness indicators (pulse pressure, augmentation index itself and corrected by 75 bpm, and pulse wave velocity). The results of the 6MWT were compared between genders using the independent t test. To analyze the influence of different age categories (≤59 years, 60-69 years and ≥70 years), one-way ANOVA was applied. Although peripheral arterial disease has a similar prevalence between men and women, studies have shown that the consequences of the disease are extremely different between the sexes. Studies show that women have lower functional capacity and lower levels of physical activity than men. However, the sexes are different from the barriers to the practice of physical activity and cardiovascular health indicators have not yet been studied. Objective: To compare the barriers to the practice of physical activity, cardiovascular function and arterial rigidity among men and women with peripheral arterial disease. Material and Method: this is a descriptive epidemiological study. The study included 267 patients with peripheral arterial disease and symptoms of intermittent claudication divided into two studies, 102 patients in the first study and 167 patients in the second study. The patients were submitted to the evaluation of physical activity level and barriers to the practice of physical activity, as well as cardiovascular health indicators (auscultatory arterial pressure, autonomic cardiovascular modulation, and arterial stiffness). Results: Women had a higher rate of increase by 34% compared to men who presented 29%, women had a rate of increase corrected by 75 beats per minute 31%, men had 25%. Women with peripheral arterial disease had lower levels of physical activity with moderate and vigorous intensity and reported more frequently than the following barriers to physical activity: lack of companionship to practice physical activity, fear of physical activity aggravate the disease, no place to sit when pain is felt and no place to practice physical activity. Conclusion: Regarding the cardiovascular variables, women present higher reflected wave indicators than men, as well as presented more barriers to physical activity compared to men.
