Abstract

Microbes have inhabited the earth for hundreds of millions of years longer than humans. The microbiota–gut–brain axis (MGBA) represents a bidirectional communication pathway. These communications occur between the central nervous system (CNS), the enteric nervous system (ENS), and the emotional and cognitive centres of the brain. The field of research on the gut–brain axis has grown significantly during the past two decades. Signalling occurs between the gut microbiota and the brain through the neural, endocrine, immune, and humoral pathways. A substantial body of evidence indicates that the MGBA plays a pivotal role in various neurological diseases. These include Alzheimer's disease (AD), autism spectrum disorder (ASD), Rett syndrome, attention deficit hyperactivity disorder (ADHD), non-Alzheimer's neurodegeneration and dementias, fronto-temporal lobe dementia (FTLD), Wilson–Konovalov disease (WD), multisystem atrophy (MSA), Huntington's chorea (HC), Parkinson's disease (PD), multiple sclerosis (MS), amyotrophic lateral sclerosis (ALS), temporal lobe epilepsy (TLE), depression, and schizophrenia (SCZ). Furthermore, the bidirectional correlation between therapeutics and the gut–brain axis will be discussed. Conversely, the mood of delivery, exercise, psychotropic agents, stress, and neurologic drugs can influence the MGBA. By understanding the MGBA, it may be possible to facilitate research into microbial-based interventions and therapeutic strategies for neurological diseases. [ABSTRACT FROM AUTHOR]

11. Serum copeptin, lactate, and shock index as predictors of morbidity and mortality in shocked acutely poisoned patients.

Abstract

Background: Poisoning-induced shock is a serious medical emergency with a high mortality rate. Hospitalized poisoned individuals experience multiple adverse cardiovascular events that could progress to cardiac arrest. This study was designed to compare the prognostic role of the admission shock index and plasma copeptin level in shocked poisoned patients and to evaluate their associations with initial patients' characteristics and outcomes. Methods: We conducted a prospective study on acutely poisoned adult patients. Results: A total of 41 patients were enrolled in the study. The mean age of all patients was 27.05 ± 10.99 years and most of the patients were females (n = 27, 66%). Pesticides were the most common type of poisoning (n = 18, 44%), followed by cardiovascular drugs (n = 12, 29.3%). Eleven (26.8%) patients died during the hospital stay length. The initial serum copeptin level and shock index could predict organ dysfunction indexed by sequential organ assessment score (SOFA) with area under the curve (AUCs) of 0.862 and 0.755, respectively. Initial serum copeptin and lactate levels, SOFA score, and their combination can strongly differentiate between survivors and non-survivors with an AUC of 0.944, 0.885, and 0.959, and 0.994, respectively. Conclusion: We concluded that the shock index, serum lactate level, and SOFA score may help in risk stratifying patients and predicting outcomes in critically ill patients with poisoning-induced shock. Copeptin is superior to the shock index in predicting mortality among the studied patients. However, a combination of SOFA score, serum copeptin level, and serum lactate level can develop a more predominant prediction for overall clinical outcomes in these patients. [ABSTRACT FROM AUTHOR]

Abstract

The essential objective of this work was to co‐pyrolyze a binary mixture of date and cherry seeds in a fixed‐bed reactor. The experimental variables influencing the output of products, such as the pyrolysis temperature, pyrolysis duration, particle size, and heating rate, were optimized. The maximum liquid fraction yield (55.55 % with a bio‐oil yield of 29.70 %) was attained at 500 °C for 60 min at a heating rate of 40 °C min−1 and a particle size of 0.25 mm. The Fourier transform infrared and gas chromatography‐mass spectrometry results revealed that the bio‐oil was predominantly composed of oxygenated hydrocarbons, primarily higher acids (52.05 %), esters (23.67 %), and phenols (13 %). Biochar was converted into activated carbon, which possessed a Brunaer‐Emmett‐Teller surface area of 1055.55 m2g−1. 16. Evaluation of the new poisoning mortality score in comparison with PSS and SOFA scoring systems to predict mortality in poisoned patients admitted to the intensive care unit.

Abstract

Objective: Mortality prediction in acute poisoning patients aids in prompt and effective treatment. This study aimed to evaluate the effectiveness of the new Poisoning Mortality Score (PMS) in comparison with the Poison Severity Score (PSS) and Sequential Organ Failure Assessment (SOFA) scoring systems in poisoned patients admitted to the intensive care unit (ICU). Material and Methods: The medical records of 523 poisoned patients admitted to the ICU of the Poison Control Centre from September 2021 to June 2022 were examined retrospectively. The PMS, PSS, and SOFA scores were calculated based on the worst values of the first 24 h of admission. Results: A total of 100 patients were enrolled in the study, and the in-hospital mortality rate was 28%. The best cut-off points for predicting mortality for PMS, PSS, and SOFA scores were > 53, > 2, and > 6, with sensitivities of 67.9%, 85.7%, and 82.4% and specificities of 73.6%, 84.7%, and 83.3% respectively. In a pairwise comparison of the AUCs for PMS, PSS, and SOFA scores, SOFA displayed significantly greater accuracy than PSS and PMS. Conclusion: The PMS, PSS, and SOFA scoring systems were significant predictors of mortality in ICU-admitted poisoned patients, however, the SOFA score showed the best performance (OR = 1.77, and 95% CI = 1.42-2.54) with significant P-value (0.002) as a predictor of mortality and highest AUC(0.904). 17. Decision-Making of Patients Towards Treatment Modalities of Failed Root Canal Therapy

Abstract

OBJECTIVES To determine the awareness and perception of patients towards treatment modalities of failed root canal therapy. METHODOLOGY A cross-sectional study was conducted at the department of Operative Dentistry and Endodontics, Altamash Institute of Dental Medicine, Karachi, from 15 August’2021 to 14 October’2021. A total of 292 participants, 155(63%) female and 104(37%) males, were selected for the study. All the participants included had at least one root canal-treated tooth. A well-constructed and validated performa was used to evaluate the patient’s decision for different treatment modalities against a tooth with failed root canal treatment. RESULTS Among the subjects, most patients opted for endodontic re-treatment (60%) in case of root canal treatment failure, whereas the remaining opted for extraction (40%). Out of this, 40% (8%) choose removal of partial denture, (12%) bridge, and implant (20%). CONCLUSION Re-treatment is the first-line treatment option for the previously failed root canal. Our study concluded that most patients would opt for an endodontic re-treatment. The patient's education and awareness level affected the treatment options.

Abstract

Introduction: Since the start of pandemic new variants have been evolving and moving from one country to another either by air travel or ground crossings.Objective: To find out factors associated with noncompliance of recommended guidelines by international passengers so that we can improve the arrangements at airports where required, improve future preparedness, and give recommendations to concerned authorities for improvement in enforcing guidelines.Methodology: A cross-sectional study was carried out at Islamabad International Airport during the months of June and July 2021. The study population included international arriving passengers aged 12 years and above. Questions were asked about following guidelines before boarding, on board, and after disembarkation. Twenty questions were asked to assess compliance level. A median cut off value was set for assessment of noncompliance.Results: The male to female ratio was 1:1. The age range was 12 – 75 years. The odds of noncompliance to guidelines was higher in females compared to males. The results revealed a significant association between region of arrival of respondents and noncompliance. Passengers arriving from Afghanistan, France , UK and USA were more likely to be noncompliant to guidelines (p-value = 0.00). There was a significant association between occupation of participants and noncompliance. Housewives and retired were more likely to be noncompliant (p-value = 0.00). A significant association was observed between vaccinated people and noncompliance.Conclusion: Travelers arriving from some specific countries were noncompliant to guidelines. Vaccinated were most noncompliant which indicates still we need to work on awareness and need legislations, fines or travel restrictions to noncompliant travellers.
