Abstract

Acute perforated appendicitis is rare in neonates and is associated with high morbidity and mortality. This is mainly because the rarity of the pathology and the abnormal clinical features cause delays in diagnosis and definitive management. We report a case of a premature neonate who presented with sudden onset of abdominal distension associated with an inability to pass stools. The initial abdominal X-ray showed free air under the right hemi-diaphragm. An emergency laparotomy was performed revealing a perforation at the appendicular tip. An appendectomy was done, and the neonate recovered well with a mild surgical site infection during the course of recovery. Neonatal perforated appendicitis is rare, and clinicians need to consider it as a differential diagnosis due to the atypical presentations. This preterm neonate presented with clinical features of intestinal obstruction and was found at laparotomy to have a perforated appendix at the tip due to neonatal appendicitis.

The application aims to provide an efficient and user-friendly way for people to monitor their health status and detect chronic diseases at an early stage. The system utilizes various data analysis and Machine Learning algorithms, such as Random Forest and Decision Trees, to predict and analyze health data. The application is designed to enable users to perform health tests online and get immediate results with recommendations for further actions. Moreover, Chronic Ai stores the user's health records and comprehensively analyzes their health history. The application's front end is designed to be user-friendly and easy to navigate, ensuring an excellent user experience. Additionally, the application has the functionality of sending test results to the user's email address, enabling them to keep track of their health records conveniently. Overall, Chronic Ai is an innovative solution to chronic disease prediction and prevention, providing a cost-effective and efficient way to monitor health status and prevent chronic diseases. Abstract

Multiple myeloma (MM) commonly presents with anemia and bone pain. Besides bone marrow involvement, MM has a tendency to involve other organs with gastrointestinal (GI) tract, especially lower GI tract involvement being uncommon. Our case shows an elderly male with anemia due to suspected GI malignancy. Investigations revealed anemia, thrombocytopenia, positive stool occult blood and inconclusive diagnostic endoscopic findings. An elevated serum total protein, serum calcium and serum creatinine gave rise to the suspicion of MM which was later supported by the fulfilment of the CRAB criteria, expressed as hypercalcemia, renal failure, anemia and lytic lesions on skull X-ray. The relationship between multiple myeloma and GI bleeding is rare and indicates extramedullary disease which is a sign of poor prognosis. Early detection may alter the prognostic course of MM.

41. The Reno-Protective Effect of Ethanolic Extract of Cassia Fistula (Amaltas) Leaves on Streptozotocin Induced Diabetic Nephropathy in Rats

Abstract

Objective: To determine the Reno-protective effect of ethanolic extract of Cassia fistula (Amaltas) leaves on streptozotocin induced diabetic nephropathy in rats. Study Design: An animal experimental study Place of Study: The study was done in Biochemistry department, Islamic International Medical College,Rawalpindi and NIH.Islamabad. Materials and Methods: Single injection of STZ was given intraperitoneally to rats and those rats that showed fasting blood glucose level over 280mg/dl were included in the study. After induction of diabetes all rats were divided into, normal control group (A), diabetes positive control group (B), and the two groups (C and D) served as experimental groups while group E served as standard as it received glibenclamide. Group C and D diabetic experimental rats received ethanolic extract of Cassia fistula leaves at 400 mg/kg and 500mg/kg of body weight orally for eight weeks on daily basis. On the other hand group E rats received glibenclamide at 0.5 mg/kg bodyweight orally for eight weeks. Blood samples were collected after eight weeks to find reno-protection against STZ induced diabetic nephropathy. Results: The diabetic positive group rats showed variable increase in levels of serum glucose, serum urea, serum creatinine, total urinary protein and microalbuminuric levels. Body weight decreased and urine volume increased in diabetic groups. Cassia fistula ethanolic extract of 400mg/kg and 500mg/kg body weight dose and glibenclamide significantly decreased the levels of these parameters in rats. On comparison Cassia fistula ethanolic extract of 500mg/kg dose reduced levels of biochemical parameters more effectively than the 400mg/kg dose of Cassia fistula and glibenclamide. Cassia fistula constituents, especially polyphenols and flavonoids have strong anti-oxidant activity which might be involved in reno-protection. Conclusion: Cassia fistula ethanolic leaf extract showed reno-protection against STZ induced diabetic nephropathy in rats.
