Abstract

There are several uses for silver nanoparticles in the field of biomedicine because of their exceptional features. The crucial use of silver nanoparticles is the development of antibiotics that are clinically effective against resistant bacteria. Due to their non-toxicity, environmental friendliness, and faster action, green syntheses of silver nanoparticles have gained some attraction. Plant extracts could be used as efficient sources of reductants and stabilizers in this process and also investigated for antibacterial action. This study mainly aimed to synthesize silver nanoparticles using Lablab purpureus flowers extracts and evaluate the antibacterial properties of the green synthesized sliver nanoparticles. The silver nanoparticles were synthesized and characterized by uv-vis, FT-IR spectroscopy and SEM analysis. Additionally, the disk diffusion method was used to evaluate the antibacterial activity of the silver nanoparticles against one gram positive and five gram negative bacteria. The particle size of synthesized Silver nanoparticles (AgNPs) was less than 1 µm. In bacterial susceptibility study, flower extract of L. purpureus showed moderate antibacterial activity but nanoparticles showed promising antibacterial potential against gram positive S. aureus and gram negative E. coli, moderate antibacterial activity against S. typhi bacteria, and mild activity against P. vulgaris, K. pneumonia and P. aeruginosa. This study indicates that AgNPs made from L. purpureus flower extract exhibit potent antibacterial activity, suggesting these nanoparticles may be used to develop effective anti-bacterial drugs for the treatment of bacterial infections. 8. Impact of Multi-Walled CNT Incorporation on Dielectric Properties of PVDF-BaTiO3 Nanocomposites and Their Energy Harvesting Possibilities

Abstract

The current study investigated the fabrication of multi-walled carbon nanotubes (MWCNTs) adhering to Barium titanate (BaTiO3) nanoparticles and poly(vinylidene fluoride) (PVDF) nanocomposites, as well as the impact of MWCNT on the PVDF-BaTiO3 matrix in terms of dielectric constant and dielectric loss with a view to develop a high performance piezoelectric energy harvester in future. The capacity and potential of as-prepared nanocomposite films for the fabrication of high-performance flexible piezoelectric nanogenerator (PNG) were also investigated in this work. In particular, five distinct types of nanocomposites and films were synthesized: PB (bare PVDF–BaTiO3), PBC-1 (PVDF–BaTiO3-0.1 wt% CNT), PBC-2 (PVDF–BaTiO3-0.3 wt% CNT), PBC-3 (PVDF–BaTiO3-0.5 wt% CNT), and PBC-4 (PVDF–BaTiO3-1 wt% CNT). The dielectric constant and dielectric loss increased as MWCNT concentration increased. Sample PBC-3 had the optimum dielectric characteristics of all the as-prepared samples, with the maximum output voltage and current of 4.4 V and 0.66 μA, respectively, with an applied force of ~2N. Fine-tuning the BaTiO3 content and thickness of the PNGs is likely to increase the harvester’s performance even more. It is anticipated that the work would make it easier to fabricate high-performance piezoelectric films and would be a suitable choice for creating high-performance PNG.

Abstract

The prediction of a difficult cholecystectomy has traditionally been based on certain pre-operative clinical and imaging factors. Most of the previous literature reported small patient cohorts and have not used an objective measure of operative difficulty. The aim of this study was to develop a pre-operative score to predict difficult cholecystectomy, as defined by a validated intra-operative difficulty grading scale. Two cohorts from prospectively maintained databases of patients who underwent laparoscopic cholecystectomy were analysed: the CholeS Study (8755 patients) and a single surgeon series (4089 patients). Factors potentially predictive of difficulty were correlated to the Nassar intra-operative difficulty scale. A multivariable binary logistic regression analysis was then used to identify factors that were independently associated with difficult laparoscopic cholecystectomy, defined as operative difficulty grades 3 to 5. The resulting model was then converted to a risk score, and validated on both internal and external datasets. Increasing age and ASA classification, male gender, diagnosis of CBD stone or cholecystitis, thick-walled gallbladders, CBD dilation, use of pre-operative ERCP and non-elective operations were found to be significant independent predictors of difficult cases. Abstract

Objectives: This study aimed to explore the association between high–risk fertility behaviors and neonatal mortality in Ethiopia. Study design: A community-based cross-sectional study was conducted using data from the 2019 Ethiopian Mini-Demographic and Health Survey. Methods: Mixed-effects logit regression models were fitted to 5527 children nested within 305 clusters. The definition of high-risk fertility behavior was adopted from the 2019 EMDHS. The fixed effects (the association between the outcome variable and the explanatory variables) were expressed as adjusted odds ratios (ORs) with 95 % confidence intervals and measures of variation explained by intra-class correlation coefficients, median odds ratio, and proportional change invariance. Abstract

BackgroundPremature deliveries are a major public health issue, with high health, economic, and productivity costs associated with lengthy hospitalizations in neonatal critical care units. The goal of this study was to determine the number of premature births in Ethiopia's Debre Berhan Comprehensive Specialized Hospital and the factors that influence them.MethodsThe Debre Berhan Comprehensive Specialized Hospital conducted an institution-based cross-sectional study between February and April 2020. A total of 325 study participants were selected using systematic random sampling. Face-to-face interviews using a pre-tested semi-structured questionnaire were used to collect data. For data entry and analysis, Epi data version 3.1 and SPSS version 20 were used. At a P-value of 0.2, bivariate logistic regression analysis was used to categorize candidate variables to the next level, and variables in multivariate logistic regression models with a p-value of 0.05 were considered statistically significant.ResultPreterm births accounted for 16.1% of all births at Debre Berhan Comprehensive Specialized Hospital. Cesarean section [AOR = 2.412; 95% CI (1.154, 5.0370)], twin pregnancy [AOR = 3.524; 95% CI (1.114, 11.150)], and maternal anemia during pregnancy [AOR = 3.124; 95% CI (1.417, 6.887)] were statistically significant associations with the outcome variable in the final logistic regression model.Conclusion and recommendationPreterm birth was found to be greater in the study area than in the Global Action Report for Sub-Saharan Africa and a few other countries. Efforts should be made to prevent maternal health issues that lead to caesarean section, and all pregnant mothers should be supplemented with iron and folic acid as soon as feasible. This study suggests that there is still a gap in the field in terms of health service intervention.

Abstract

The present study was undertaken to evaluate in-vitro antifungal activity of Semecarpus anacardium Linn. oil against four fungal pathogens, viz. Curvularia penniseti, Curvularia lunata, Fusarium oxysporum f. sp. ciceris and Helminthosporium maydis using poisoned food technique. The DMSO extract of S.anacardium oil was found to be more or less active against almost all tested pathogenic fungi with a varied spectrum of reduced growth. C.lunata has shown 93.3% inhibition and F.oxysporum and H.maydis have shown 94.4% inhibition and 100% mycelial inhibitions at 15% and 18% concentrations of the extract respectively. Whereas, C.penniseti was found to be quite sensitive that showed 88.9 inhibitions at 10% concentration but it showed 100% inhibition at 18% concentration.

Abstract

This study used a numerical simulation approach to examine the effects of external modifications in reducing aerodynamic drag on passenger vehicles. During the simulation, modifications included reducing mirror size by replacing the side mirrors with cameras and covering the wheel area. The resulting changes in drag force for different combinations of modifications were compared with a conventional baseline model to determine the most aerodynamic configuration. The study found that side view cameras reduced drag forces by almost 2.6% due to their smaller frontal areas and improvement in the overall aerodynamics of the vehicle. Besides, an increase in wheel coverage decreased the drag causing up to 2.7% of drag force reduction for a wheel with an 87% coverage area. This is because of the reduction in wake formation caused by the wheel rims. Finally, using a combination of smaller cameras and wheels with larger coverage areas resulted in a maximum drag reduction of about 4.3%. [ABSTRACT FROM AUTHOR]

In this study, regarding the attachment styles, sociobehavioral problems in first-grade children with experience of preschool and in those taught by their mothers at-home are compared. A casual-comparative method was used to compare children in the two groups. A total of 320 first-grade children participated in the study. The study measures included a separation anxiety test, an adaptive behavior scale, and a children’s symptom inventory. Data were analyzed using multivariate statistics. Secure attachment in the group with experience of preschool was significantly higher than that in the at-home group. None of the variables, including parents’ education and father’s income, significantly affected attachment style. Neither father’s education, father’s income, or attachment significantly influenced adjustment. Father’s education significantly influenced children’s symptoms. Attachment style and hours of preschool attendance had no effect on Child Symptom Inventory scores. Associations among age at joining preschool, attachment style, and behavioral and adaptive problems in first-grade children were nonlinear and multivariate. By taking into account parents’ awareness, sensitivity, and responsiveness, relative welfare, appropriate quality of child-care centers, and having fewer hours of preschool attendance, the risk factors for early parent–child separation and institutional care can be reduced. Abstract Background The cornerstone of diabetes management is the self-care behavior of diabetics. However, many people with diabetes do not fully engage in these activities. Effective stress management behaviors have a positive impact on diabetes self-care. The purpose of this study was to investigate the relationship between self-care behaviors in people with diabetes and stress coping behaviors in people with type II diabetes. Method A facility-based cross-sectional study was undertaken in the North Shoa zone from March 2 to 29, 2022. The study involved 432 types II diabetic patients who were chosen at random from eight public hospitals. Eight item stress coping techniques tools was used to measure stress management behavior. Data were entered into Epi Data V.3.1 and analyzed using SPSS version 22. Data for continuous variables were reported as means and standard deviations and percentages for categorical variables. Descriptive statistic was used to summarize study variables. Binary logistic regression models were used to assess associations between sociodemographic variables, stress-coping behaviors, and self-care behaviors. Binary logistic regression model was used investigate the association between diabetic self-care behaviors and stress-coping behaviors. A p-value ≤ 0.05 and an OR with a 95% CI are considered statistically significant associations. Result the study showed that stress management behavior was observed in more than half of the patients (51.2; 95% CI; (46.5, 55.6). 34. Efficient integrated production of bioethanol and antiviral glycerolysis lignin from sugarcane trash

Abstract

Abstract Background Sugarcane trash (SCT) represents up to 18% of the aboveground biomass of sugarcane, surpassing 28 million tons globally per year. The majority of SCT is burning in the fields. Hence, efficient use of SCT is necessary to reduce carbon dioxide emissions and global warming and establish agro-industrial biorefineries. Apart from its low costs, conversion of whole biomass with high production efficiency and titer yield is mandatory for effective biorefinery systems. Therefore, in this study, we developed a simple, integrated method involving a single step of glycerolysis pretreatment to produce antiviral glycerolysis lignin (AGL). Subsequently, we co-fermented glycerol with hydrolyzed glucose and xylose to yield high titers of bioethanol. Results SCT was subjected to pretreatment with microwave acidic glycerolysis with 50% aqueous (aq.) glycerol (MAG50); this pretreatment was optimized across different temperature ranges, acid concentrations, and reaction times. The optimized MAG50 (opMAG50) of SCT at 1:15 (w/v) in 1% H2SO4, 360 µM AlK(SO4)2 at 140 °C for 30 min (opMAG50) recovered the highest amount of total sugars and the lowest amount of furfural byproducts. Following opMAG50, the soluble fraction, i.e., glycerol xylose-rich solution (GXRS), was separated by filtration. A residual pulp was then washed with acetone, recovering 7.9% of the dry weight (27% of lignin) as an AGL. AGL strongly inhibited the replication of encephalomyocarditis virus (EMCV) in L929 cells without cytotoxicity. The pulp was then saccharified in yeast peptone medium by cellulase to produce a glucose concentration similar to the theoretical yield. The total xylose and arabinose recoveries were 69% and 93%, respectively. GXRS and saccharified sugars were combined and co-fermented through mixed cultures of two metabolically engineered Saccharomyces cerevisiae strains: glycerol-fermenting yeast (SK-FGG4) and xylose-fermenting yeast (SK-N2). By co-fermenting glycerol and xylose with glucose, the ethanol titer yield increased to 78.7 g/L (10% v/v ethanol), with a 96% conversion efficiency. Conclusion The integration of AGL production with the co-fermentation of glycerol, hydrolyzed glucose, and xylose to produce a high titer of bioethanol paves an avenue for the use of surplus glycerol from the biodiesel industry for the efficient utilization of SCT and other lignocellulosic biomasses.

Abstract

A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets.Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall's tau for dichotomous variables, or Jonckheere-Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis.A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p 0.001).We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.

Abstract

Abstract Background Diabetes is a significant global public health issue that necessitates self-management. However, this is difficult to put into practice and requires a new approach. The purpose of this study was to assess the effects of a physical activity promotion program on adherence to recommended physical activity and lessons to improve self-management. Methods A quasi-experimental study was conducted from January 2020 to February 2021 at North Shoa Zone Public Hospital. The study enrolled 216 type II diabetic patients from four public hospitals. Data were entered into Epi Data V.3.1 and analyzed using SPSS version 22. Data were presented as means of standard deviations for continuous variables and percentages for categorical variables. Intervention and control groups were compared before and after intervention using independent t-tests. A p-value less than 0.05 was considered significant for all statistical tests. As of October 2015, 1595 cases of MERS have been reported to the WHO; 80% from Saudi Arabia with a high overall mortality (35%). Few studies have examined the clinical course of critically ill patients; most from a single-center and without a comparison group. Therefore, it remains unclear whether clinical presentations, co-morbidities and outcomes differ from severe acute respiratory infection (SARI) cases of other etiologies.

Systematic corticosteroids are commonly used among critically ill patients with the Middle East Respiratory Syndrome (MERS), but their impact on outcome is uncertain. Studies that adjust only for baseline characteristics do not account for the clinical condition at the time of corticosteroid initiation. The objective of this study is to investigate the association of corticosteroids on mortality accounting for time-dependent variables during critical illness up-to the time of corticosteroid initiation.

46. The impact of staff-poaching among radio stations in the Kumasi metropolis

Abstract

Poaching key employees from close competitors has become a common phenomenon in Ghana. The study investigates the extent to which staff-poaching has hit the radio industry in Kumasi Metropolis and discusses its impact on the radio stations and their listenership. Relevant data was obtained from three key players in the radio industry: management, presenters/producers and listeners who are the regular and ardent radio listeners. The paper employs exploratory method to assess the dimensions of staff-poaching by giving an in-depth analysis of the factors influencing the phenomenon of staff-poaching among radio stations. The study found out that there is a bond of loyalty that unconsciously tends to be formed between listeners and particular radio stations and presenters. Consequently, loyalties or interests can change when there is poaching of staff. This has both positive and negative impacts. The study reveals that staff-poaching becomes rife when there is emergence of new private radio stations, and lack of proper contract leads to staff-poaching. The study therefore recommends that there should be proper contract and good relationship between employers and employees which will help manage the high attrition rate in the radio stations.Key words: Staff-poaching, Attrition, Non-compete clause (CNC), predatory hiring, competitive poaching

Abstract

Background The aims of this prospective population-based cohort study were to identify the patient and hospital characteristics associated with emergency cholecystectomy, and the influences of these in determining variations between hospitals. Methods Data were collected for consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing the performance of emergency cholecystectomy were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 4744 cholecystectomies from 165 hospitals. Increasing age, lower ASA fitness grade, biliary colic, the need for further imaging (magnetic retrograde cholangiopancreatography), endoscopic interventions (endoscopic retrograde cholangiopancreatography) and admission to a non-biliary centre significantly reduced the likelihood of an emergency cholecystectomy being performed. The multilevel model was used to calculate the probability of receiving an emergency cholecystectomy for a woman aged 40 years or over with an ASA grade of I or II and a BMI of at least 25·0 kg/m2, who presented with acute cholecystitis with an ultrasound scan showing a thick-walled gallbladder and a normal common bile duct. The mean predicted probability of receiving an emergency cholecystectomy was 0·52 (95 per cent c.i. 0·45 to 0·57). The predicted probabilities ranged from 0·02 to 0·95 across the 165 hospitals, demonstrating significant variation between hospitals. Conclusion Patients with similar characteristics presenting to different hospitals with acute gallbladder pathology do not receive comparable care.

48. A qualitative study exploring effects of the 2003 coalition-led invasion of Iraq on the quality of medical care provided to patients at a local Iraqi Kurdish hospital

Abstract

Media reports have suggested that the Kurdistan region is the only state to have economically and politically benefited from the Iraq War. However, the literature regarding the impact of the war on medical care in the region is minimal. A descriptive, interview-based study was conducted with the aim of assessing whether the quality of medical care provided to patients in a local Kurdish hospital has improved since the fall of Saddam Hussein’s regime. With more studies such as these will improve the breadth of knowledge surrounding the changes to Kurdistan following the Iraq War. Twenty patients from a local district general hospital were interviewed: 11 men between the ages of 35 to 70 and nine women between the ages of 33 to 60. The data were interpreted and coded into specific and fundamental themes. The derived themes included lack of punctuality (90% of patients described this), long waiting times (70%), lack of one-on-one patient-doctor consultations (95%), lack of female doctors in the hospital (50%), financial incentives by inappropriate private clinic referrals (100%), lack of vital investigations (100%), lack of anesthesiologists and cancellation of surgical operating lists (100%), lack of documentation (40%) and poor hygiene (100%). There was general dissatisfaction with the quality of services provided at the local hospital, and all of the patients stated the quality of healthcare had remained the same or declined since the Iraq War. Abstract

"nBackground: There are multiple diagnostic methods for evaluation of patients with low back pain with a radicular pattern, each one has it's own accuracy and diagnostic ability and so comparing them with each other would help the clinicians to find the best diagnostic method. The main objective of our study was to compare the diagnostic accommodation of electromyography (EMG) and Magnetic Resonance Imaging (MRI) findings in patients with low back pain and radiculopathy. "n"nMethods: In this descriptive cross-sectional study, 101 patients with low back pain and radiculopathy attending to Sina Hospital in Tehran, Iran from 2007 to 2009 that had indication for both EMG and MRI were evaluated for their demographic characteristics and disease-related factors. "n"nResults: Totally, 90 out of 101 patients (89%) had abnormal EMG findings and 94 out of 101 subjects (93%) had abnormal MRI results. The concordance rate was 88% (89 patients) with no significant difference (p> 0.05). The sensitivity and specificity of MRI were 95.6% and 27.3%, respectively, and the sensitivity and specificity of EMG were 91.5% and 60%, respectively. "n"nConclusion: According to the results of this study and in comparison with other studies it may be concluded that MRI and EMG are both accurate and use of them in a concomitant manner would result in increased diagnostic ability in patients with low back pain and radiculopathy.

Abstract

In this investigation, the photothermal heating of superparamagnetic Fe3O4 nanoparticles was carried out by irradiating with either 785 nm or 808 nm near infrared (NIR) lasers. The effects of nanoparticle configuration, arrangement, and surface coating on the photothermal heating behavior were investigated for different Fe3O4 nanoparticle systems. Depending on the preparation method, Fe3O4 nanoparticles with mean hydrodynamic diameter ranging from 30 nm to 250 nm were synthesized. Photothermal transduction efficiency is a measure of light to thermal energy conversion; the highest efficiency obtained was 56% by 785 nm and 42% by 808 nm light irradiation for poly(acrylic) acid (PAA) coated Fe3O4 samples. With this conversion efficiency, the PAA-coated Fe3O4 nanoparticles raised the solution temperature ∼   1 2 ∘ C above physiological temperature, which is sufficient for cancer therapeutics. Photothermal transduction efficiency was found to decrease as the particle hydrodynamic diameter increased. Nanoparticle absorption and scattering properties were found different due to surface modifications. UV-VIS-NIR absorption spectroscopy was carried out and results were analyzed using the Mie scattering theory. Experimental photothermal transduction efficiency was found to scale with the theoretical results for a particular wavelength. These results have significance in the design and development of the Fe3O4 nanoparticle systems for effective cancer therapy with NIR light. [ABSTRACT FROM AUTHOR]
