Purpose: To analyze the clinical features, visual acuity, and full-field electroretinogram (ERG) findings of 15 patients with the neuronal ceroid lipofuscinosis (NCL) phenotype and to establish the role of ERG testing in NCL diagnosis. Methods: The medical records of five patients with infantile NCL, five with Jansky-Bielschowsky disease, and five with juvenile NCL who underwent full-field ERG testing were retrospectively analyzed. Results: Progressive vision loss was the initial symptom in 66.7% of patients and was isolated or associated with ataxia, epilepsy, and neurodevelopmental involution. Epilepsy was present in 93.3% of patients, of whom 86.6% presented with neurodevelopmental involution. Fundus findings ranged from normal to pigmentary/atrophic abnormalities. Cone-rod, rod-cone, and both types of dysfunction were observed in six, one, and eight patients, respectively. Conclusion: In our study, all patients with the NCL phenotype had abnormal ERG findings, and the majority exhibited both cone-rod and rod-cone dysfunction. We conclude that ERG is a valuable tool for the characterization of visual dysfunction in patients with the NCL phenotype and is useful for diagnosis. RESUMO Objetivo: Analisar o quadro clínico, a acuidade visual e o eletrorretinograma de campo total (ERG) de 15 pacientes com o fenótipo da lipofuscinose ceróide neuronal (LCN), estabelecendo o papel do eletrorretinograma no seu diagnóstico. Métodos: Eletrorretinograma foi realizado em 5 pacientes com lipofuscinose ceróide neuronal infantil, 5 com doença de Jansky-Bielschowsky e 5 com lipofuscinose ceróide neuronal juvenil sendo feita uma análise retrospectiva dos registros médicos. Resultados: A perda progressiva da acuidade visual foi o sintoma inicial em 66,7%; isolada ou associada à ataxia, epilepsia e involução do desenvolvimento neuropsico motor. Epilepsia foi o sintoma inicial em 93,3% e 86,6% apresentaram involução do desenvolvimento neuropsicomotor. Achados fundoscópicos variaram de normal a alterações pigmentares/atróficas. Disfunção de cone-bastonete foi constatada em 6 pacientes, bastonete-cone em 1 e em 8 pacientes observou-se disfunção proporcional de ambos os sistemas. Conclusão: O eletrorretinograma foi alterado em todos os pacientes, e o achado mais frequente foi o comprometimento de cones e bastonetes. O eletrorretinograma constitui, portanto, uma ferramenta valiosa para caracterizar a disfunção visual em pacientes com o fenótipo da lipofuscinose ceróide neuronal, contribuindo para seu diagnóstico.

Purpose: To investigate the contributions of transient pattern-reversal visual evoked potentials in the diagnosis of ocular malingering at a Brazilian university hospital. Methods: Adult patients with suspected malingering in one or both eyes were referred for visual evoked potential testing. Data from patients' medical records were reviewed and analyzed retrospectively. Data analysis included the distance optotype visual acuity based on a ETDRS retro-illuminated chart and the transient pattern-reversal visual evoked potential parameters of latency (milliseconds) and amplitude (microvolts) for the P100 component, using checkerboards with visual subtenses of 15' and 60'. Motivations for malingering were noted. Results: The 20 subjects included 11 (55%) women. Patient ages ranged from 21 to 61 years (mean= 45.05 ± 11.76 years; median= 49 years). In 8 patients (6 women), both eyes exhibited reduced visual acuity with normal pattern-reversal visually evoked potential parameters (pure malingerers). The remaining 12 patients (7 men) exhibited reduced vision in only 1 eye, with simulated reduced vision in the contralateral eye (exaggerators). Financial motivation was noted in 18 patients (9 men). Conclusion: Normal pattern-reversal visually evoked potential parameters with suspected ocular malingering were observed in a 20 patient cohort. This electrophysiological technique appeared to be useful as a measure of visual pathway integrity in this specific population. RESUMO Objetivo: Investigar a contribuição dos potenciais visuais evocados por padrões reversos no diagnóstico de simulação de baixa de visão em um hospital universitário do Brasil. Métodos: Um grupo de pacientes adultos com suspeita de simulação de baixa de visão em um ou ambos os olhos foi avaliado e os dados analisados retrospectivamente. Foram medidos: acuidade visual de optotipos informada para longe utilizando a tabela ETDRS, parâmetros dos potenciais visuais evocados por padrões reversos de latência (milissegundos) e amplitude (microvolts) para o componente P100 com estímulos de ângulos visuais de 15' e 60'. A motivação do paciente para a simulação foi anotada. Resultados: Os participantes foram 20 indivíduos com 11 (55%) do sexo feminino. A idade variou de 21 a 61 anos (média= 45,05 ± 11,76 anos; mediana= 49 anos). Em 8 pacientes (6 mulheres) ambos os olhos tinham acuidade visual reduzida com parâmetros dos potenciais visuais evocados por padrão reverso normais para ambos os olhos (simuladores puros). Uma subsérie separada de 12 pacientes (7 homens) tinha visão reduzida em apenas um olho e estavam simulando redução da visão no outro olho (exacerbadores). A motivação financeira foi observada em 18 pacientes (9 homens). Conclusões: Parâmetros dos potenciais visuais evocados por padrões reversos normais foram encontrados neste grupo de 20 pacientes com suspeita de simulação. Esta técnica eletrofisiológica pode ser útil como uma medida da integridade do sistema visual nesta população de doentes.

OBJETIVOS: Avaliar a função macular por eletrorretinografia focal e angiofluoresceinografia em pacientes com degeneração macular relacionada à idade neovascular submetidos à terapia fotodinâmica com verteporfina. MÉTODOS: Estudo prospectivo incluindo 22 pacientes (12 homens) com degeneração macular relacionada à idade e lesão neovascular coroídea subfoveal predominantemente clássica, não consecutivos, tratados com terapia fotodinâmica com verteporfina padrão e acompanhados por um período de 12 meses. A acuidade visual melhor corrigida foi medida utilizando tabela ETDRS, as alterações das lesões foram avaliadas pela angiofluoresceinografia e a função dos cones foram obtidas pela eletrorretinografia focal na visita inicial e nas subsequentes a cada três meses. RESULTADOS: Todos os pacientes completaram as visitas programadas. Após uma média de 3,5 sessões por paciente, a variação da acuidade visual melhor corrigida não foi significante no decorrer do estudo. Em 50% dos pacientes a variação da acuidade visual melhor corrigida foi >1 linha. Ao final do estudo observou-se ausência de vazamento em 86% dos pacientes. A média da amplitude e latência do ERG focal na visita inicial foram de, respectivamente, 194,88 nV e 29,19 ms e as variações no decorrer do estudo não foram significantes. CONCLUSÕES: Não foram encontradas diferenças na amplitude e latência do eletrorretinografia focal após um período de 9 meses. A acuidade visual melhor corrigida não sofreu variações significativas ao longo dos 12 meses. A lesão foi significativamente reduzida ao longo do tempo e houve uma correlação negativa entre a amplitude e a acuidade visual melhor corrigida. PURPOSE: To evaluate macular function by focal electroretinography and fluorescein angiography (FA) in patients with neovascular age-related macular degeneration submitted to verteporfin photodynamic therapy (VPT). METHODS: Prospective study involving 22 patients with age-related macular degeneration and predominantly classic subfoveal neovascular membrane, in non consecutive series, treated with VPT and followed for 12 months. They had their best corrected visual acuity measured by ETDRS chart, changes of lesion measured by fluorescein angiography and cone function assessed by focal electroretinography at baseline and each 3month follow-up. RESULTS: All 22 patients completed the scheduled follow-up. After a mean of 3.5 sessions of treatment per patient, the mean visual acuity variation was not significant at the end of study. Eleven patients showed variation >1 line. 86% of patients achieved stabilization of lesion leakage at the end of the study. Focal electroretinography showed a mean of 194.88 nV in amplitude and 29.19 ms in latency and did not present a significant variation during treatment. CONCLUSIONS: There were no significant differences in focal electroretinography amplitudes and latencies after a 9-month period. Visual acuity did not show important variations during the 12 months. The decrease of lesion size showed a significant difference at 12 months with negative correlation between the amplitude of focal electroretinography and best corrected visual acuity.

OBJETIVO: Traçar perfil clínico, anatômico e funcional da comoção retiniana. MÉTODOS: Pacientes com trauma ocular contuso e quadro fundoscópico de comoção retiniana unilateral há menos de 72 horas foram submetidos ao exame de acuidade visual, biomicroscopia, oftalmoscopia binocular indireta, retinografia, angiografia fluoresceínica, tomografia de coerência óptica e eletrorretinografia de campo total. A eletrorretinografia foi repetida após 15 e 30 dias. A angiografia após 30 dias. RESULTADOS: Foram incluídos 16 pacientes no estudo. No primeiro exame, havia diferença estatisticamente significante entre o olho atingido e o olho controle na amplitude de todas as respostas, sem alteração da relação b/a, e na latência da resposta de cones isolados e do flicker a 30 Hz. No segundo exame, manteve-se a diferença para os potenciais oscilatórios, que desapareceu no último exame. Na angiofluoresceinografia, todos os olhos acometidos mostravam áreas de hiperfluoresceência por transmissão alternadas com áreas de hipofluorescência por bloqueio. Este defeito mantém-se após 30 dias. A tomografia de coerência óptica mostrou diminuição da refletividade na camada dos fotorreceptores. CONCLUSÕES: As alterações encontradas refletiram acometimento de fotorreceptores e de células ganglionares, mas não da camada de células bipolares, além de mobilização precoce de pigmento do epitélio pigmentado da retina. As alterações eletrorretinográficas desapareceram após 30 dias do trauma. PURPOSE: To investigate clinical, anatomic and electroretinographic changes in eyes that suffered blunt ocular trauma with commotio retinae. METHODS: Patients who presented commotio retinae after unilateral blunt ocular trauma less then 72 hours before were submitted to visual acuity testing, biomicroscopy, binocular indirect ophthalmoscopy, fluorescein angiography, optical coherence tomography and full-field electroretinography. Full-field ERG was repeated after 15 and 30 days. RESULTS: Sixteen patients were included in the study. On the first examination there was a statistically significant difference between affected and fellow eye in all response amplitudes, without b/a ratio alteration, and a delay in single-flash cone response and 30-Hz flicker implicit time. On the second examination, the difference between the eyes remained for oscillatory potentials, but disappeared on the last examination. In fluorescein angiography, all patients presented mottled hyperfluorescence and hypofluorescent areas, due to alterations in the pigment barrier. On optical coherence tomography, we found optically empty spaces at the site of the lesion. CONCLUSION: Found changes suggested photoreceptor and ganglion cells, but not Müller cell functional alterations, as well as pigment mobilization. These changes disappeared 30 days after the trauma.

OBJETIVO: Determinar a freqüência de diferentes categorias diagnósticas em um laboratório de eletrofisiologia visual clínica de um hospital universitário. MÉTODOS: Os pacientes participaram de um estudo transversal sendo submetidos a testes eletrodiagnósticos visuais entre outubro de 1998 e outubro de 2000 (N=570). Os exames de eletrorretinografia (ERG) foram realizados em 385 pacientes (68%) e de potenciais visuais evocados (PVE) por reversão de padrão e/ou flashes e varredura em 185 pacientes (32%). RESULTADOS: Após avaliação da eletrorretinografia, as categorias diagnósticas mais freqüentes foram identificadas na seguinte ordem: retinose pigmentária, doença de Stargardt, distrofia de cones, degeneração do tipo cone-bastonete, retinotoxicidade devido a vários agentes (cloroquina, tamoxifeno, etc) e trauma ocular, além de perdas visuais de causas idiopáticas. Em 23% dos casos, não foi possível obter o diagnóstico final ao primeiro exame. O grupo mais freqüente referido para o eletrorretinografia foi para documentar ou excluir as degenerações tapeto-retinianas (42%). Neste grupo, 110 casos de retinose pigmentária foram identificados: 91 casos isolados e 19 sindrômicos. Para o exame de potenciais visuais evocados, as categorias diagnósticas mais freqüentes foram: doenças que afetam somente o nervo óptico (atrofia de nervo óptico, neurites ópticas, edema de disco óptico, etc.); condições neurológicas afetando a via visual (esclerose múltipla, paralisia cerebral, trauma crânio-encefálico, tumores, etc.), condições neurológicas e/ou doenças oculares pediátricas (catarata congênita, deficiência visual cortical, glaucoma congênito, etc.), e opacidade de meios (leucoma, trauma ocular, etc.). CONCLUSÕES: A análise deste amplo grupo de pacientes, mostra as indicações mais freqüentes e significativas para registros eletrofisiológicos visuais e decisões diagnósticas. Vários diagnósticos podem ser de difícil conclusão sem os testes eletrofisiológicos visuais. Dentre essas doenças incluem-se estágios iniciais da retinose pigmentária, distrofia progressiva de cones, retinopatia tóxica sem alterações fundoscópicas, função de nervo óptico ou retiniana em opacidades de meios, envolvimento de nervo óptico em esclerose múltipla e avaliação da acuidade visual em doenças oculares infantis com ou sem alterações neurológicas. PURPOSE: To determine the frequency of diagnostic categories obtained in a clinical vision electrophysiology laboratory at a university hospital. METHODS: Patients who underwent visual electrodiagnostic testing from October 1998 to October 2000 (N=570) were cross-sectionally studied. Electroretinogram (ERG) was performed in 385 (68%) patients and pattern and/or flash visually evoked potentials were performed in 185 (32%) patients. RESULTS: After electroretinogram assessment, the most frequently identified diagnostic categories were (in this order): retinitis pigmentosa, Stargardt's disease, cone dystrophy, cone-rod degeneration, retinotoxic effects due to several agents (chloroquine, tamoxifen, methanol, etc) and ocular trauma, besides unclear visual loss. In 23% of the cases, it was impossible to obtain a final diagnosis on the first examination. The largest group referred to electroretinogram testing was for documentation or exclusion of tapetoretinal degenerations (42%). In this group, 110 cases of RP were identified: 91 isolated and 19 syndromic cases. Visually evoked potential testing helped to identify the following diagnostic categories: diseases affecting only the optic nerve (optic nerve atrophy, optic neuritis, optic disk edema, etc); neurological conditions affecting the visual pathway (multiple sclerosis, cerebral palsy, blunt head trauma, tumors, etc): pediatric eye and/or neurological conditions (congenital cataract, cortical visual impairment, congenital glaucoma, etc) and opaque media (leucoma, ocular trauma, etc). CONCLUSIONS: Frequent and meaningful indications for visual electrophysiologic recording and diagnostic decisions arise from this relatively large group of studied patients. A number of diagnoses can hardly, if not all, be established without visual electrophysiologic testing. These include early stages of retinitis pigmentosa, progressive cone dystrophy, toxic retinopathy without fundus changes, optic nerve or retinal dysfunction in opaque media, optic nerve involvement in multiple sclerosis and pediatric eye disease with or without neurological damage.

Purpose: The photopic negative response (PhNR) is a slow negative component of a flash photopic full-field ERG that has been shown to be specific for retinal ganglion cell (RGC) activity. Direct evaluation of RGC function is desirable in patients with Leber's hereditary optic neuropathy (LHON) in which the loss of central acuity can make it difficult to monitor patients with standard metrics. The purpose of this study was to evaluate the use of PhNR as an objective noninvasive clinical metric in LHON.Methods: Full-field photopic ERG recordings were collected in subjects with the mt.11778G>A/ND4 LHON mutation using a red on blue stimulus. The PhNR was identified using a computer-based automated detection system, and data were manually examined to remove movement artifacts.Results: The PhNR amplitude was compared between controls (n = 13), carriers (n = 17), and affected (n = 6). Mean PhNR amplitude decreased significantly across groups (P < 0.0001). Post hoc Tukey's test revealed a significant decrease in PhNR amplitude between carriers and controls (P < 0.05) and between carriers and affected (P < 0.01).Conclusions: We are able to demonstrate that the PhNR amplitude is significantly decreased in patients affected by LHON compared to carriers in a well-described pedigree. Surprisingly, there was also a decrease in PhNR in carriers, suggesting potential subclinical RGC dysfunction in some carriers. This is important in patients affected with LHON who typically have a dense central scotoma. The PhNR may be a useful objective outcome measure for future clinical trials.
