Abstract

High-resolution ultrasound has become the primary tool in the investigation of patients with atherosclerotic carotid disease. Apart from defining the degree of stenosis non-invasively, it provides valuable information about the features of the carotid plaque. Overcoming the subjectivity of ultrasonographic carotid plaque characterisation is essential if this is to be applied reliably in a multi-centre study. Computer-assisted plaque characterisation with B-mode image normalisation and its application under different settings has become an essential tool for a meaningful comparison of carotid plaque ultrasonographic features with risk factors for atherosclerosis and ultimately incidence of ischaemic stroke. The aim of this thesis was to identify the association of clinical and biochemical risk factors, ultrasonographic carotid plaque characterisation and neuroimaging of the brain by means of CT-brain scan with the incidence of ischaemic stroke and cardiac mortality in patients with asymptomatic carotid stenosis. These risk factors may help identify a vulnerable carotid plaque and patients at increased risk for stroke. The ultrasonographic features of the vulnerable carotid plaque were identified in three cross-sectional studies and subsequentiy applied in a prospective study of asymptomatic carotid stenosis with ischaemic stroke and cardiac death as the primary end-points. Hypoechoic plaques were associated with a large necrotic core on histology, increased prevalence of neurological ischaemic symptoms and increased prevalence of embolic silent CT-brain infarcts.

Abstract

A survey conducted by the UK Aortic Society aimed to assess the current state of aortic disease treatment in the UK and its compliance with best practices. The survey revealed significant variations in how aortic services are provided across the UK, with some centers offering advanced treatments while others lack crucial resources. The findings highlight the need for improvement in several areas, including concentrating complex aortic surgeries in high-volume centers, establishing regional rotas, implementing standardized protocols, and investing in regional educational programs for aortic diseases. Abstract

Background: The purpose of this study was to determine the cerebrovascular risk stratification potential of baseline degree of stenosis, clinical features, and ultrasonic plaque characteristics in patients with asymptomatic internal carotid artery (ICA) stenosis. Methods: This was a prospective, multicenter, cohort study of patients undergoing medical intervention for vascular disease. Hazard ratios for ICA stenosis, clinical features, and plaque texture features associated with ipsilateral cerebrovascular or retinal ischemic (CORI) events were calculated using proportional hazards models. Results: A total of 1121 patients with 50% to 99% asymptomatic ICA stenosis in relation to the bulb (European Carotid Surgery Trial [ECST] method) were followed-up for 6 to 96 months (mean, 48). A total of 130 ipsilateral CORI events occurred. Severity of stenosis, age, systolic blood pressure, increased serum creatinine, smoking history of more than 10 pack-years, history of contralateral transient ischemic attacks (TIAs) or stroke, low grayscale median (GSM), increased plaque area, plaque types 1, 2, and 3, and the presence of discrete white areas (DWAs) without acoustic shadowing were associated with increased risk. Receiver operating characteristic (ROC) curves were constructed for predicted risk versus observed CORI events as a measure of model validity. The areas under the ROC curves for a model of stenosis alone, a model of stenosis combined with clinical features and a model of stenosis combined with clinical, and plaque features were 0.59 (95% confidence interval [CI] 0.54-0.64), 0.66 (0.62-0.72), and 0.82 (0.78-0.86), respectively. In the last model, stenosis, history of contralateral TIAs or stroke, GSM, plaque area, and DWAs were independent predictors of ipsilateral CORI events. Abstract

BACKGROUND AND PURPOSE: Our hypothesis was that symptomatic and asymptomatic carotid plaques are different. The aim of this study was to identify the sonographic tissue and surface characteristics of plaques and their degree of stenosis that corresponded to these two clinical presentations. METHODS: We studied 81 symptomatic and 111 asymptomatic plaques (150 patients) having 50% to 99% stenosis on duplex scanning. These plaques were imaged on duplex and captured in a computer. We evaluated the gray-scale median (GSM) to distinguish hypoechoic (low GSM) from hyperechoic (high GSM) plaques, and the bending energy (BE), to distinguish plaques with irregular (high BE) versus smooth (low BE) surfaces. RESULTS: The symptomatic group corresponded to hypoechoic (median GSM, 4) and severely stenosed (median stenosis, 85%) plaques, whereas the asymptomatic group corresponded to hyperechoic (median GSM, 35) and moderately stenosed (median stenosis, 70%) plaques (P < .05 for both variables). The BE failed to separate the two groups; the mean BEs were 1.63 and 1.68 for the symptomatic and asymptomatic groups, respectively (P = .38). CONCLUSION: Our results suggest that echogenic characteristics and the degree of stenosis are the strongest predictors of carotid plaque behavior. The sonographic surface characteristics failed to qualify as an index of plaque instability.

Abstract

BACKGROUND AND PURPOSE: Our hypothesis was that the carotid plaques associated with retinal and cerebrovascular symptomatology and asymptomatic presentation may be differ from each other. The aim of this study was to identify the sonographic and histopathologic characteristics of plaques that corresponded to these three clinical manifestations. METHODS: The echo process involved duplex preoperative imaging of 71 plaques (67 patients, 21 plaques were associated with retinal, 25 with cerebrovascular symptoms, and 25 were asymptomatic), which was performed in a longitudinal fashion. Appropriate frames were captured and digitized via S-video signal in a computer and digitized sonograms were normalized by two echo-anatomic reference points: the gray scale median (GSM) of the blood and that of the adventitia. The GSM of the plaques was evaluated to distinguish dark (low-GSM) from bright (high-GSM) plaques. Subsequent to endarterectomy, the plaques were sectioned transversely, and a slice at the level of the largest plaque area was examined for the relative size of necrotic core and presence of calcification and hemorrhage. RESULTS: Retinal symptomatology was associated with a hypoechoic plaque appearance (median GSM: 0), asymptomatic status with a hyperechoic plaque appearance (median GSM: 34), and cerebrovascular symptomatology with an intermediate plaque appearance (median GSM: 16) (P = .001). The histopathologic characteristics did not disclose differences between the three clinical groups. The hypoechoic plaque appearance was associated only with the presence of hemorrhage (median GSM for the hemorrhagic plaques, 6, and for the non-hemorrhagic ones, 20 [P = .04]). The relative necrotic core size and the presence of calcification did not show any echomorphologic predilection. CONCLUSION: Our results showed that distinct echomorphologic characteristics of plaques were associated with retinal and cerebrovascular symptomatology and asymptomatic status. Histopathologically, only the presence of hemorrhage proved to have an echomorphologic predilection.

Abstract

The objective of this work was to develop a computer aided system which will facilitate the automated characterization of carotid plaques recorded from high resolution ultrasound images for the identification of individuals with asymptomatic carotid stenosis at risk of stroke. The plaques were classified into (i) symptomatic or (ii) asymptomatic. Ten different texture feature sets were extracted from the segmented plaque image using the following algorithms: fast order statistics, spatial gray level dependence matrices, gray level difference statistics, neighborhood gray tone difference matrix, statistical feature matrix, Laws texture energy measures, fractal dimension texture analysis, Fourier power spectrum and shape parameters. Although the statistics of all features extracted for the two classes indicated a high degree of overlap, a classification of the plaques was possible using the unsupervised self-organizing feature map (SOFM) classifier and combining techniques. The classification results of the different feature sets were combined using (i) majority voting and (ii) weighted averaging based on a confidence measure derived from the SOFM. Combining the classification results of the ten different feature sets improved significantly the classification results obtained by the individual feature sets, reaching an average diagnostic yield of 75%. 5 3591 3596 Cited By :15

Abstract

Various forms of renal replacement therapies are available to treat acute renal failure (ARF) after cardiac surgery. The objective of this study was to assess the incidence of ARF developing postoperatively necessitating continuous veno-venous hemofiltration (CVVH) in adult patients requiring cardiopulmonary bypass (CPB), to determine the factors which influence the outcome in these patients and to assess the outcome following the use of early and intensive CVVH. During the study period, i.e. August 2000 to July 2002, 2355 adult patients underwent surgery under CPB, of whom 159 (6.7%) developed renal failure (creatinine >200 micromol/l) and 116 (5%) needed CVVH. Patients excluded were those who died within 24 h and those who underwent coronary artery bypass grafting without utilising CPB, thoracoabdominal aneurysm operations and pericardial surgery. Average age, Parsonnet score and Euroscore in the study population were 69.9 years, 21 and 7.70, respectively. Of the 116, 45 died in the intensive care unit (38.8% mortality). Relatively more non-survivors suffered from diabetes and preoperative renal dysfunction (P<0.05). Adverse outcome was also more likely if patient suffered from postoperative cardiac failure or had gastrointestinal complications or had more than two organ systems failing (P<0.05). Mortality was 100% if hepatic failure ensued.
