Our feasibility objectives were: 1) recruitment rate/week; 2) recruitment of ≥ 90% of eligible patients; and 3) > 90% follow-up. Secondary objectives were incidence and intensity of persistent post-surgical pain (PPSP) and other clinical and safety outcomes.Methods: Using computerized randomization, patients were allocated to one of four groups: NMDA active with steroid placebo; NMDA placebo with steroid active; both NMDA and steroid active; both NMDA and steroid placebo. Patients, health providers, and data analysts were blinded to allocation. Patients were followed for 3 months after randomization.Results: The trial was initiated in May 2017 at Hamilton and, after subsequent regulatory and ethics approval, in April 2018 at Cleveland. The trial had to be stopped after only 1 month of recruitment in Cleveland because the packaged study medications (memantine) expired and we were unable to procure the dosage required. Among 41 eligible patients, 27 (66%) were randomized. The recruitment rate/week was 0.63, 95% confidence interval (CI): 0.47– 0.79 in Hamilton; and 1, 95% CI: 0.83– 1.17 in Cleveland. Follow-up was complete for all 24 patients (100%) in Hamilton, and 3 of 4 patients in Cleveland. In total, only 4 patients (15%), and 2 patients (7%) had persistent pain at rest and with movement, respectively. There were no significant differences between groups for other outcomes.Conclusion: The trial had to be stopped prematurely due to non-availability of study medications. Trial feasibility objectives of recruiting 90% of eligible patients and recruiting at least one patient/week per site were not met. 5. A Deterministic Analysis of Decimation for Sigma-Delta Quantization of Bandlimited Functions

Abstract

We study Sigma-Delta (Σ Δ) quantization of oversampled bandlimited functions. We prove that digitally integrating blocks of bits and then down-sampling, a process known as decimation, can efficiently encode the associated Σ Δ bit-stream. It allows a large reduction in the bit-rate while still permitting good approximation of the underlying bandlimited function via an appropriate reconstruction kernel. Specifically, in the case of stable rth order Σ Δ schemes we show that the reconstruction error decays exponentially in the bit-rate. For example, this result applies to the 1-bit, greedy, first-order Σ Δ scheme.

Abstract

Quantization of compressed sensing measurements is typically justified by the robust recovery results of Cand\`es, Romberg and Tao, and of Donoho. These results guarantee that if a uniform quantizer of step size $\delta$ is used to quantize $m$ measurements $y = \Phi x$ of a $k$-sparse signal $x \in \R^N$, where $\Phi$ satisfies the restricted isometry property, then the approximate recovery $x^#$ via $\ell_1$-minimization is within $O(\delta)$ of $x$. The simplest and commonly assumed approach is to quantize each measurement independently. In this paper, we show that if instead an $r$th order $\Sigma\Delta$ quantization scheme with the same output alphabet is used to quantize $y$, then there is an alternative recovery method via Sobolev dual frames which guarantees a reduction of the approximation error by a factor of $(m/k)^{(r-1/2)\alpha}$ for any $0 < \alpha < 1$, if $m \gtrsim_r k (\log N)^{1/(1-\alpha)}$. The result holds with high probability on the initial draw of the measurement matrix $\Phi$ from the Gaussian distribution, and uniformly for all $k$-sparse signals $x$ that satisfy a mild size condition on their supports.

16. Global Neuroblastoma Network: An international multidisciplinary neuroblastoma tumor board for resource-limited countries

Abstract

BACKGROUND: Tumor boards are part of standard care of patients with complex cancers, but appropriate multidisciplinary expertise and infrastructure are often not available in low- and middle-income countries (LMIC) for pediatric cancers, such as neuroblastoma. Our goal was to review results of a Global Neuroblastoma Network (GNN) tumor board accessible to LMIC. METHODS: De-identified clinical cases presented via internet conference during a weekly GNN virtual tumor board from 2010 through 2020 were evaluated in a standardized format, including diagnostic imaging, pathology, therapy information, resource limitations, and questions for discussion. Information summarized included the presentations, a survey of the impact on care, and a resource questionnaire. RESULTS: Registered GNN participants included 575 individuals from 77 countries, with a median of 39 participants per session. Total 412 cases were presented from 32 countries, including 351 unique neuroblastoma patients, 52 follow-up cases, and nine non-neuroblastoma diagnoses. Twenty-eight educational sessions were presented. Limited critical resources for diagnostics and staging of cases included MYCN analysis (54.7%), metaiodobenzylguanidine (MIBG) scans (38.7%), and International Neuroblastoma Pathology Classification (49%). Therapies were also limited, with markedly decreased use of radiation and autologous stem cell transplant for high-risk cases, and no availability of anti-GD2 antibody in LMIC. Limited sampling with a post-presentation survey showed that 100% found the GNN helpful, and 70% altered the care plan based on the discussion. CONCLUSION: This report shows the utility of an international tumor board for LMIC focused on a challenging solid tumor where local expertise may be limited, with international multidisciplinary expert participation and educational sessions.

Abstract

Resumen Introducción: El bosque seco tropical (Bs-T) es uno de los ecosistemas más amenazados, en Colombia, presenta la mayor extensión en la región Caribe, está sometido a continua transformación, que ha ocasionado la disminución de su cobertura. Objetivo: Caracterizar la diversidad florística en términos de composición y estructura en seis fragmentos de Bs-T en las subregiones bajo y medio Sinú del Departamento de Córdoba. Métodología: Se siguió la metodología de Parcelas de Muestreo Rápido de Vegetación - RAP modificada. Se determinó la riqueza y abundancia, el Índice de valor de Importancia por especie y por familia,se construyeron clases diamétricas y altimétricas y se calculó el Índice de Diversidad α de Fisher e Índice de Similaridad de Sørensen. Resultados: Se registraron 1 336 individuos, agrupados en 68 familias, 161 géneros y 273 especies en el medio Sinú y 2 178 individuos (226 especies/151 géneros/59 familias) en el bajo Sinú. Las familias más importantes por riqueza de especies fueron: Fabaceae, Rubiaceae y Bignoniaceae, los mayores valores de abundancia se presentaron en Fabaceae, Capparaceae y en las especies: Astronium graveolens Jacq.y Capparidastrum frondosum (Jacq.) Cornejo & Iltis. La distribución por clases diamétricas y altimétricas presentó un comportamiento de J invertida. Los arbustos representaron el hábito de crecimiento dominante. Attalea butyracea (Mutis ex L.f.) Wess.Boer, Cavanillesia platanifolia (Humb. & Bonpl.) y Sterculia apetala Sterculia apetala (Jacq.) H.Karst. registraron los mayores valores de importancia. Conclusiones: Los bosques estudiados presentaron la composición florística que caracteriza el Bs-T en Colombia. La información obtenida contribuye a la gestión integral del bosque seco en Córdoba. Objective: To characterize the floristic diversity in terms of composition and structure in six TDF fragments in the lower and middle Sinú subregions of the Department of Córdoba. Methods: The methodology of Rapid Vegetation Sampling Plots - modified RAP was followed. Wealth and abundance were determined, The Importance Value Index by species and family, diametric and altimetric classes were constructed and the Fisher’s α Diversity Index and Sørensen´s Similarity Index were calculated. Results: 1 336 individuals were registered, grouped into 68 families, 161 genera and 273 species in the middle Sinú and 2 178 individuals (226 species / 151 genera / 59families) in the lower Sinú. The most important families by species richness were Fabaceae, Rubiaceae and Bignoniaceae, the highest abundance values were found in Fabaceae, Capparaceae and in the species Astronium graveolens Jacq. and Capparidastrum frondosum (Jacq.) Cornejo & Iltis. The distribution by diametric and altimetric classes presented an inverted J behavior. The bushes represented the dominant growth habit. Attalea butyracea (Mutis ex L.f.) Wess.Boer, Cavanillesia platanifolia (Humb. & Bonpl.) Kunth and Sterculia apetala (Jacq.) H.Karst. recorded the highest values of importance. Conclusions: The forests studied presented the floristic composition that characterizes the TDF in Colombia. The information obtained contributes to the integral management of the Tropical Dry Forest in Córdoba.

21. PIVOT-10: A phase II study of bempegaldesleukin (NKTR-214) in combination with nivolumab (NIVO) in cisplatin (cis) ineligible patients with previously untreated locally advanced or metastatic urothelial cancer (mUC).

Abstract

Background: Checkpoint inhibitors can achieve durable responses in cis-ineligible 1L mUC. However, use is restricted to patients whose tumors are PD-L1 high. Approximately 70% of cis-ineligible patients have tumors with low PD-L1 expression, leaving a significant proportion of 1L mUC patients in need of new treatment options. Bempegaldesleukin (BEMPEG; NKTR-214) is a CD122-preferential IL-2 pathway agonist designed to provide sustained signaling through the IL-2 sy receptor. NIVO is an anti-PD-1 antibody that is approved for treatment in several types of cancers, including 2L mUC after treatment with a platinum agent. Early BEMPEG plus NIVO data in 1L mUC (cis-eligible and -ineligible) patients found an objective response rate (ORR) of 48% (13/27) in the efficacy evaluable population (defined as having undergone at least one post-baseline scan) and a CR rate of 19%, prompting this further exploration of BEMPEG plus NIVO in a phase 2 study (Siefker-Radke, 2019). Method(s): This Phase 2 multi-national trial evaluates BEMPEG plus NIVO in previously untreated patients with cis-ineligible mUC. Eligibility also requires tumor tissue be analyzed by central laboratory to document PD-L1 status. Approximately 205 patients will be enrolled. BEMPEG (0.006 mg/kg) and NIVO (360 mg) are given intravenously (IV) on Day 1 of each 3-week cycle. The primary endpoint is ORR assessed per RECIST 1.1 by blinded independent central review (BICR) in patients with low PD-L1 expression (defined as Combined Positive Score [CPS] < 10). Secondary endpoints include ORR and duration of response in all treated patients, safety, and tolerability. Tumor and blood samples will be collected for biomarker analyses. Enrollment is ongoing.

Abstract

The relationships between subjective status and perceived legitimacy are important for understanding the extent to which people with low status are complicit in their oppression. We use novel data from 66 samples and 30 countries (N = 12,788) and find that people with higher status see the social system as more legitimate than those with lower status, but there is variation across people and countries. The association between subjective status and perceived legitimacy was never negative at any levels of eight moderator variables, although the positive association was sometimes reduced. Although not always consistent with hypotheses, group identification, self-esteem, and beliefs in social mobility were all associated with perceived legitimacy among people who have low subjective status. These findings enrich our understanding of the relationship between social status and legitimacy.

Abstract

Abstract

Introduction: The Tropical Dry Forest (TDF) is one of the most threatened ecosystems, in Colombia it has the greater extension in the Caribbean region, undergoing continuous transformation, causing the reduction of its coverage. Objective: To characterize the floristic diversity in terms of composition and structure in six TDF fragments in the lower and middle Sinú subregions of the Department of Córdoba. Methods: The methodology of Rapid Vegetation Sampling Plots - modified RAP was followed. Wealth and abundance were determined, The Importance Value Index by species and family, diametric and altimetric classes were constructed and the Fisher's a Diversity Index and S0rensen's Similarity Index were calculated. Results: 1 336 individuals were registered, grouped into 68 families, 161 genera and 273 species in the middle Sinú and 2 178 individuals (226 species / 151 genera / 59families) in the lower Sinú. The most important families by species richness were Fabaceae, Rubiaceae and Bignoniaceae, the highest abundance values were found in Fabaceae, Capparaceae and in the species Astronium graveolens Jacq. and Capparidastrum frondosum (Jacq.) Cornejo & Iltis. The distribution by diametric and altimetric classes presented an inverted J behavior. The bushes represented the dominant growth habit. Attalea butyracea (Mutis ex L.f.) Wess.Boer, Cavanillesiaplatanifolia (Humb. & Bonpl.) Kunth and Sterculia apetala (Jacq.) H.Karst. recorded the highest values of importance. Conclusions: The forests studied presented the floristic composition that characterizes the TDF in Colombia. The information obtained contributes to the integral management of the Tropical Dry Forest in Córdoba. [ABSTRACT FROM AUTHOR]

Abstract

Inspired by the popular Arab protests against oppressive regimes that began in 2010, people around the world protested in sympathy with the Arab peoples. The present research draws on two major theories of intergroup relations to develop an initial integrative model of sympathetic collective action. We incorporate social dominance theory’s (SDT) concept of (rejectionist) legitimizing myths with the solidarity and emotional mediation concept of the social identity model of collective action (SIMCA) to understand motivations for sympathetic collective action among bystanders. Using data from 12 nations (N = 1,480), we tested three models: (a) SIMCA (i.e., solidarity, anger, and efficacy), (b) a social dominance theory model of collective action (i.e., social dominance orientation and ideologies concerning Arab competence), and (c) an integrated model of sympathetic collective action combining both theories. Results find the greatest support for an integrated model of collective action. Discussion focuses on theoretical pluralism and suggestions for future research.

Abstract

We tested the internal reliability and predictive validity of a new 4-item Short Social Dominance Orientation (SSDO) scale among adults in 20 countries, using 15 languages (N = 2,130). Low scores indicate preferring group inclusion and equality to dominance. As expected, cross-nationally, the lower people were on SSDO, the more they endorsed more women in leadership positions, protecting minorities, and aid to the poor. Multilevel moderation models showed that each effect was stronger in nations where a relevant kind of group power differentiation was more salient. Distributions of SSDO were positively skewed, despite use of an extended response scale; results show rejecting group hierarchy is normative. The short scale is effective. Challenges regarding translations, use of short scales, and intersections between individual and collective levels in social dominance theory are discussed. © The Author(s) 2013.
