ObjectiveTo describe the burden of colorectal cancer (CRC) in Mexico and understand mortality patterns based on sex, geography, and insurance status.Materials and methodsMortality data (1998-2018) from the Instituto Nacional de Estadística y Geografía was obtained. We included colon (C18.0, C18.2-18.9) and rectal cancer ICD-10 codes (C19, C20), and estimated age-standardized national, state-level and health insurance mortality rates. We estimated the average annual percent change using joinpoint regression.ResultsBetween 1998 and 2018, the observed women and men mortality rate increased annually by 1.3 and 2.7%, respectively. Higher CRC mortality was observed in northern and more urbanized states and in groups with greater access to health insurance, which currently facilitates but does not routinely cover screening.ConclusionCRC mortality in Mexico is increasing rapidly, with marked differences based on sex, geography, and insurance status. Our findings underscore potential benefits of increased investment in comprehensive screening, diagnosis, and treatment strategies for the general population.

Objective. To determine the magnitude of mortality due to acute lymphoblastic leukemia (ALL) nationally and by age group, sex, state of residence and insurance status, as well as to evaluate time trends during the period 1998-2018 Materials and methods. We obtained ALL mortality data and estimated age-standardized national, state-level and health insurance mortality rates. We conducted a joinpoint regression analysis to describe mortality trends across the study period and estimate the average annual percent change (AAPC). Results. In a 20-year period, age-standardized ALL mortality rates increased from 1.6 per 100 000 in 1998 to 1.7 in 2018. Nationally, a constant annual increase in mortality was observed for both sexes (1998-2002 AAPC 0.6 in boys, and 1998-2002 AAPC 0.3 in girls). We observed heterogeneity in childhood ALL at a state level. Conclusion. Our results reflect the social, economic, geographic diversity of the country. Monitoring and surveillance of this disease is crucial to assess quality of care.

- Abstract
In Costa Rica, all the large volcanoes have been converted into national parks, which has limited human occupation and productive activities in its closest areas. The country has been promoted worldwide as an important tourist destination due to its biodiversity and commitment to conservation, increasing tourism and people's exposure to different volcanic hazards. Public universities and the National Commission for Risk Prevention and Emergency Attention (CNE) carry out informative and educational activities for the population. In 2020, the CNE promoted a ‘geoliteracy’ campaign, through the design and installation of signs with preventive and geo-educational information on volcanoes in Spanish and English. As of December 2022, more than 250 signs had been installed in collaboration with National Parks, the Costa Rican Electricity Institute, communities, private companies, the tourism sector, and the Community Aqueduct and Sewer Management Associations. These actions have generated a sense of empowerment and belonging in the actors involved.

Abstract: This project started with the analysis of how coaches from a running club store and then access and analyse data from training. The authors of this paper realized how much of a delay there is in the development of technologies that allow this process, compared with the tools that are already in use in other sports. In order to get as much information as possible on this topic, the authors of this studio visited a training centre, where it was possible for them to see how the athletes were doing their training routines. As a matter of fact, it was crucial to have a close relationship with athletes in order to be able to give a solution to the problem mentioned before. To solve this, in the studio is presented what the authors believe is an optimal architecture. To arrive at this point and to develop this project, it was very important to hear coaches’ opinions on the matter, as it has been done. In order to design this app, there have been used three clearly differenced components. First, a very intuitive interface, where the users could add and then access their data. Angular, a scalable database, was used to create this interface. After considering the different options that Angular was offering, it was decided to use MYSQL. This decision was taken because of how powerful it is and because of the great usability when storing related data. Finally, an intermediate layer was used to communicate the two components previously mentioned. PHP was selected for this task because of its useability. The implementation of this project has been developed in an incremental way. The main focus has been to ensure the high quality of each process, along with ensuring high client satisfaction. The result of this project has been successful and the goal of creating a functional and useful app for the Club Asociación de Atletismo Moratalaz was achieved.

Abstract Costa Rica is known worldwide as a tourist mecca. The death of an American tourist and its Costa Rican guide in the year 2000, as well as the sudden eruption of the Poás in 2017, coupled with the persistent eruptive activity of Turrialba for more than 10 years, were the mechanisms of four major changes in the Management and safety of the Costa Rican Volcanic Parks: a) The generation of the decree to restrict the use of land around the Arenal in 2001 and its updating in the 2006, b) the improvement of volcanic monitoring systems in real time, c) the construction of volcanic ballistic protection screens in the viewpoint of Active crater of the Poás volcano in 2018 and controlled management measures; and the construction of volcanic bomb protection screens in Turrialba in 2018, for the personnel that attends the communications towers, the volcanologists and technicians specialized in surveillance Volcanic, and d) the preventive and information labels in several volcanic parks. Previously, in the Irazú volcano, the ICE had already built in 1964 a bunker so that the volcanologists and guards could make geophysical and volcanological observations. This bunker is located within the first five temporary observatories in the world and within the first -if not the first- in the American continent and in the world to build a work of protection of this type. PURPOSE Hispanics and Indigenous women are underrepresented in cancer research. We aimed to estimate the incidence of breast cancer (BC) among indigenous and non-indigenous women and describe reproductive and lifestyle risk factors. METHODS The baseline questionnaire was completed by 115,307 women (2006-2008). Indigenous ancestry was defined by self-adscription and/or speaking an indigenous language. Incident BC-cases were confirmed using self-reports, administrative and clinical databases, cancer registries, and death certificates. We calculated person-years from the baseline questionnaire to the date of diagnosis, death, or the end of follow-up (December 31, 2019). We age-standardized reproductive and lifestyle information. RESULTS After a median follow-up of 10.8 years, we confirmed 1,212 BC-cases. The crude incidence rate per 100,000 person-years was 55 for indigenous and 95 for non-indigenous women; the mean age at diagnosis was 48.2 and 50.8, respectively. In indigenous BC-cases, early menarche (11.9 v 26.7%), first pregnancy > 25 years (36.8% v 51.7%), and nulliparity (11.7% v 14.1%) were less frequent compared to non-indigenous. The number of children (3.0 v 2.6) and breastfeeding > 12 months (71.6% v 45.2%) were higher among indigenous. Indigenous had earlier menopause (44.8 v 47.5 years) and more premenopausal-BC (27.8% v 25.1%). Oral contraceptives use (42.6% v 48.0%), hormone-replacement therapy (12.4% v 24.6%), family history of BC (11.9% v 15.1%) and benign breast-disease (17.8% v 23.0%) were less frequent in indigenous cases. Physical activity (> 150 min/week) was higher in indigenous women (34.3% v 27.8%). Smoking (6.6% v 11.2%) and alcohol consumption (47.9% v 61.3%) were lower in indigenous women; however, they had more diabetes (14.6% v 6.0%) and were at the highest tertile of the dietary glycemic index (40.2% v 35.0%). CONCLUSION In the MTC, BC-incidence in indigenous women is lower than in non-indigenous; this might be explained by a lower prevalence of hormonal and reproductive risk factors and higher physical activity among indigenous women.

Abstract From April 12 until April 22 2017 Poás volcano experienced a new explosive phase with ballistic fragments reachingup to ~2,2 km. At its final phase non juvenile bocks predominated, product both of a 1953 lava body destruction and a lacustrinefloor, followed by an increment of juvenile fragments (glassy bread crust blocks, scoriaceous bombs and, rarely, fusiformbombs). The largest ones measured up to 20 m in diameter and 1,5 m in thickness, reaching up to 200 m from the crater. Thejuvenile fragments showed a varied vesiculation (15-50% vesicles) with a basaltic andesitic composition (phenocrysts of plag 20-22%, cpx 6-8%, opx 3-5%, lo 1-3%, mt 0-1%) within an intersertal matrix. The juvenile phreatomagmatic ash content increasedfrom approximated 9% to 85% at the end, consisting of a strombolian activity. When hitting the floor, the bombs generated perpendicularcooling joints and shear stress conjugate pairs. While cooling, the joints were parallel to impact surface generating asymmetric conjugated rectangular pattern. When fallon surfaces with a slope the pattern was more complex due to viscous flow. The ballistic initial velocity was estimated in the 100-300 ms-1 range, the final velocity was estimated in the 36-120 ms-1 range, anda terminal velocity (defined in the text) in the 124-250 ms-1 range. Flight time from bottom of Main crater to the visitor area (Viewpoint) was estimated in the 8-15 secs range. At the end, a set of recommendations for future volcanic ballistic studies is includedbased on a totally new physics model using actual range and impact angle. It is here recommended that more shelters be built onthe path between the active and Botos craters.

Abstract The Turrialba volcano has a complex graben on its summit with an approximate strike of N50°E, a length of 3.5 km and a maximum width of 2.8 km. The boundaries of the graben are a complex system of left-lateral, strike-slip faults with normal component, several craters oriented N45°E and a collapse caldera oriented to the NE of 6 km long and 2.5 km wide. Two secondary cones are on its SW flank with N30°E orientation; all these elements show a NE volcanotectonic trend along 9 km. The analysis of secondary faults presents on its flanks, the morphoneotectonics and seismicity, together with morphovolcanic features, suggests a volcanotectonic model more complex than those proposed in the literature for this volcano. We propose the existence of a regional left-lateral strike-slip fault, oriented SW-NE, which forms a flower structure at the surface, generating a complex graben bounded by the Elia and Ariete faults. The transtensive stress favored the formation of the craters and escarpments, whereas the transpresive stress produced a dome fold structure and highly fractured areas. The zones of weakness and possible folding favored the formation of a sectoral collapse toward the NE, with the associated development of one or several volcanic debris avalanche deposits. A similar mechanism could be the trigger of the Angostura volcanic debris avalanche 17 ka ago, from the Coliblanco avalanche caldera on the SW flank of Turrialba. The Ariete and Elia faults have a seismic potential of Mw 6.2 and 6.0, respectively. The generation of new eruptive volcanic vents and sector collapses along these faults can also occur in the future.

- Abstract

In this report, the cytological features and differential diagnosis of a case of giant-cell tumor (GCT) in soft tissues in a patient with osseous Paget's disease (PD) are described, with histological confirmation. Characteristic cytological findings include a diffuse cellular population of neoplastic cells composed of two types: one group of polygonal or round mononuclear small cells, exhibiting a thin-rim, dense cytoplasm and single hyperchromatic nuclei; and the second group of multinucleated, osteoclast-type giant cells with dozens of round nuclei, showing occasional micronucleoli. A prominent feature is the presence of numerous capillary structures surrounded by tumor cells. The differential diagnosis includes a number of other neoplastic and reactive processes in which giant cells may be abundant. The cytological features of GCT appear to be characteristic enough to allow a suggestive diagnosis. However, the final diagnosis should be made only after the lesion has been studied histologically.

- Abstract
Purpose: We estimated the 5-year overall, age at diagnosis- and stage-specific colorectal cancer survival in patients treated through their coverage with Seguro Popular., Methods: We conducted a retrospective study using a dataset that included 1418 colorectal cancer patients covered by Seguro Popular (Mexico's public health insurance system covering 60 % of the population) between 2013 and 2016. Deaths were identified using the Epidemiologic Death Statistics Subsystem registry, with a specialized algorithm for record linkage. The Kaplan-Meier method was used to estimate overall survival curves and the proportion of patients alive at various follow-up time points. We compared survival curves across subgroups using the log-rank test., Results: In this study the average age at diagnosis was 56 years with 31.9 % of patients diagnosed before the age of 50. Most cases (78.1 %) were diagnosed in advanced stages (i.e., III and IV), with nearly half of the cases originating in the rectum. The overall 5-year survival was 50 %, with higher survival (74 %) for patients with stage I-II and lower survival for those with stage III (58 %) and IV (33 %). While age at diagnosis was not associated with survival for early-stage colorectal cancer, younger patients with metastatic disease had a worse prognosis compared to older patients., Conclusion: The 5-year overall colorectal cancer survival was 50 %, with variation by clinical stage. Almost 80 % of the population was diagnosed with advanced stages, underscoring the need for screening programs. Younger patients with metastatic disease exhibited a worse prognosis, highlighting the need for targeted interventions., Competing Interests: Declaration of Competing Interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (Copyright © 2024 Elsevier Ltd. - Abstract
Introduction: Resistance to antimicrobial drugs has increased in recent years. The aim of this multicenter study was to determine the current situation in Spain regarding the resistance patterns of the most common infectious agents to antimicrobial drugs., Material and Methods: A total 1,475 strains isolated in 38 hospitals in October 2001 were studied. These included 294 methicillin-resistant Staphylococcus aureus (MRSA), 149 clinically significant coagulase-negative staphylococci (CNS), 96 Enterococcus faecium, 1 ampicillin-resistant Enterococcus faecalis, 204 penicillin-resistant Streptococcus pneumoniae, 100 ampicillin-resistant Haemophilus influenzae, 361 ciprofloxacin-resistant Escherichia coli, 160 Pseudomonas aeruginosa, and 110 Acinetobacter baumannii. Sensitivity to various antibiotics was studied by a broth microdilution method. Additionally, an epidemiologic survey was conducted in the participating hospitals to determine the incidence of these multi-drug resistant microorganisms., Results: Overall incidence of ciprofloxacin-resistant E. coli and MRSA strains was 24% in both cases. The pneumococci were resistant to several antibiotics. Telithromycin showed excellent activity against all the pneumococci, including those resistant to macrolides. Most MRSA isolates were resistant to quinolones and macrolides. All the staphylococci were susceptible to linezolid and quinupristin-dalfopristin. Only 50% of A. baumannii strains were susceptible to imipenem., Conclusion: The results of this study evidence increasing multi-drug resistance among various microorganisms, such as MRSA, CNS, ampicillin-resistant enterococci, and penicillin-resistant pneumococci, and underline the need for surveillance programs to gain information on this significant therapeutic problem.
