Objectives: Idiopathic granulomatous mastitis (IGM) is a rare, benign, chronic inflammatory breast disease of unknown etiology. There is no standardized treatment because its etiology is still unclear. In this multicenter study, we presented the demographic, treatment, and follow-up characteristics of IGM cases monitored by rheumatology clinics. Material and Methods: This retrospective study was conducted in 13 different rheumatology centers. A total of 108 patients with IGM were included in the study. Demographic and clinical data were retrospectively obtained from patient files. Results: The most commonly administered drugs were, in order of frequency, corticosteroid (CS) (91.7%), methotrexate (MTX) (74.1%), antibiotics (63%), non-steroidal anti-inflammatory drugs (41.7%), azathioprine (AZA) (13.9%), colchicine (5.6%), and tumor necrosis factor (TNF) inhibitors (0.9%). The most commonly used form of immunosuppressive (IS) treatment was the MTX and CS combination 78 (72.2%). The ratio of patients receiving CS alone was 19 (17.6%). The ratio of patients who were operated on only and did not use IS drugs was 6.5%. The ratio of patients who received no treatment was 2.8%. Among the drugs used, MTX and CS alone use were independent risk factors for relapse; (p=0.027, p=0.011, respectively). The relapse rate was higher in patients receiving CS alone. Conclusion: IS drugs including CS, MTX, AZA, and TNF inhibitors seem to be efficient for treating IGM. CS alone use is associated with relapse, and the use of other IS drugs such as MTX is particularly effective in reducing relapse in IGM. [ABSTRACT FROM AUTHOR]

Goltz syndrome is a rare X-linked dominant multisystem disorder that presents with ectoderm and mesoderm-derived symptoms. Skin manifestations including congenital patchy skin aplasia, congenital nodular fat herniation, congenital hypo- or hyperpigmentation along Blaschko's lines, telangiectasia, and congenital ridged dysplastic nails are typical in this disorder. Almost all cases of Goltz syndrome correspond to female newborns and that hemizygosis makes the syndrome fetal in males. Triple X syndrome is a relatively common congenital disorder that presents with mild to no symptoms in the developmental and psychiatric realm. The patient reported here was born with multisystem anomaly affecting the eyes, craniofacial region, cardiovascular system, skin, and limbs. A G-banding chromosomal study revealed 47, XXX. She was diagnosed with Goltz syndrome owing to her distinctive skin manifestations. The congenital cervical skin defect healed with conservative treatment. The facial cleft, cleft lip-palate, and syndactyly were successfully treated with multiple surgical treatments. The combination of triple X syndrome and Goltz syndrome is very rare. Introduction: Pathological scars occur from the hyperproliferation of fibroblasts and can be classified into hypertrophic scars and keloids. Basically, hypertrophic scars do not grow beyond the limits of the original wound, while keloids grow horizontally in a nodular form. Despite the diversity of instruments used to guide the prevention, treatment and follow-up of pathological scars, there is a need for instruments that address local realities. The objective is to carry out a narrative review of the literature on scar management algorithms and create an updated algorithm. Methods: Descriptive study of narrative literature review, with a search in PubMed, SciELO, LILACS, MEDLINE and Cochrane databases, from November 2010 to November 2020, published in English, Portuguese and Spanish. The descriptors used were: “cicatrix,” “keloid,” “algorithms,” and “wound healing.” The sample selection consisted of identifying the articles, reading the titles and abstracts, and selecting studies related to the topic. Subsequently, the full reading of the selected studies and classification according to the eligibility criteria were carried out. Results: 209 articles were found, and 116 were eliminated due to duplicity, resulting in 45 articles. A total of 8 articles that met the inclusion criteria were identified. Four articles were excluded after analysis and consensus meeting due to the absence of algorithms with scientific rigor; this study is composed of four articles. Conclusion: Four algorithms were found in the literature review that resulted in the development of an updated algorithm for scars.

Mucopolysaccharidosis (MPS)‐VII, called Sly disease, is a lysosomal storage disorder that can cause fetal hydrops, including fetal hydrothorax (FHT). We describe two fetal cases that received thoracoamniotic shunting for FHT, which was later found to be associated with MPS‐VII by exome sequencing. Bilateral FHT accompanied by skin edema and ascites was found before 20 weeks of gestation in both cases. One fetus died in utero at 35 weeks of gestation, and the other survived with preterm delivery at 30 weeks of gestation. Both cases inherited compound pathogenic variants of GUSB from parents. Comparison with previously reported primary FHT cases revealed distinct clinical features in MPS‐VII‐associated FHT: early gestational age at diagnosis (<26 weeks), bilateral effusion, skin edema with ascites, and poor survival. A genetic analysis would be considered for FHT cases, with consideration of shunting when they show early‐onset bilateral effusions with skin edema and ascites. [ABSTRACT FROM AUTHOR]

Melanoma is the most lethal of all skin cancers. This necessitates the need for a machine learning-driven skin cancer detection system to help medical professionals with early detection. We propose an integrated multi-modal ensemble framework that combines deep convolution neural representations with extracted lesion characteristics and patient meta-data. This study intends to integrate transfer-learned image features, global and local textural information, and patient data using a custom generator to diagnose skin cancer accurately. The architecture combines multiple models in a weighted ensemble strategy, which was trained and validated on specific and distinct datasets, namely, HAM10000, BCN20000 + MSK, and the ISIC2020 challenge datasets. They were evaluated on the mean values of precision, recall or sensitivity, specificity, and balanced accuracy metrics. Sensitivity and specificity play a major role in diagnostics. The model achieved sensitivities of 94.15%, 86.69%, and 86.48% and specificity of 99.24%, 97.73%, and 98.51% for each dataset, respectively. Additionally, the accuracy on the malignant classes of the three datasets was 94%, 87.33%, and 89%, which is significantly higher than the physician recognition rate. The results demonstrate that our weighted voting integrated ensemble strategy outperforms existing models and could serve as an initial diagnostic tool for skin cancer. [ABSTRACT FROM AUTHOR]

Introduction: The number of animals used in research has increased following the progress and development of medical technology. Every year, millions of animals are used around the world. The pain, distress and death experienced by animals during scientific experiments have been a matter of debate for many years. In addition to the strong ethical issue, there are further drawbacks of these experiments, such as the need for skilled human resources, time consuming protocols and high costs. Now, several alternatives to animal testing have been developed to overcome the disadvantages associated with them but also to avoid unethical procedures. Aim: The aim of this review was to designate the specific field of tissue engineering that can be an essential tool in the future and lead to the reduction of animal use and the application of the 3RS. Material-Methods: We used the online data base PUBMED to choose the studies that describe the products of tissue engineering that are being used instead of living animals in experiments. The research took place the months June-August 2018 and again in March of 2021. Results: Five studies are analyzed: 3 refer to full thickness skins and experimentally demonstrate that it is possible to manufacture materials that can be used for in vitro investigations and the other 2 refer to rebuilt human epidermis where experimentally using different chemicals controls their reaction to that's all. Conclusion: The new technologies developed in the field of tissue engineering allow us to build functional tissues that can be used as models for risk assessment for humans that may conceal some chemical substances used in the production. Aplasia cutis congenita (ACC) is a rare entity characterized by a congenital localized absence of the skin, dermal appendages, and subcutaneous tissue. The cause of ACC is not clear, but inheritance is the most widely accepted cause. We report a rare case of a full-term (37 + 5 weeks) female newborn who showed complete absence of skin in localized areas of the upper and lower extremities. The patient was diagnosed with ACC associated with epidermolysis bullosa (EB; a disease that causes the skin to blister easily) and was initially treated using conservative measures. We applied mupirocin topical ointment and a nonocclusive polyester mesh impregnated with hydrocolloid and petroleum jelly daily. Complete healing of the affected areas occurred after 3 weeks. Managing patients with ACC is often challenging and based on the severity of the lesions, the approach may include both surgical and conservative treatment. Our case report suggests that a conservative approach can be effective for managing certain types of ACC and EB lesions. Abstract Since its first introduction into medical practice, reflectance confocal microscopy (RCM) has been a valuable non-invasive diagnostic tool for the assessment of benign and malignant neoplasms of the skin. It has also been used as an adjunct for diagnosing equivocal cutaneous neoplasms that lack characteristic clinical or dermoscopic features. The use of RCM has led to a decreased number of biopsies of benign lesions. Multiple published studies show a strong correlation between RCM and histopathology thereby creating a bridge between clinical aspects, dermoscopy, and histopathology. Dermatopathologists may potentially play an important role in the interpretation of confocal images, by their ability to correlate histopathologic findings. RCM has also been shown to be an important adjunct to delineating tumoral margins during surgery, as well as for monitoring the non-surgical treatment of skin cancers. Advanced technology with smaller probes, such as the VivaScope 3000, has allowed access to lesions in previously inaccessible anatomic locations. This review explains the technical principles of RCM and describes the most common RCM features of normal skin with their corresponding histological correlation.

MAB, newborn male, e.t , gestational age of 37 weeks and 5 days. During his initial care, no changes were noted, except for skin lesions: cutaneous aplasia on limbs, back, inguinal region and mucous membranes, as well as bullous lesions and nail alterations. The e.t o nt the anatomopathological exam was epidermolysis bullosa. The patient had a bad e.t o nt and developed neonatal sepsis, and several antibiotic therapy and dressings were performed. After presenting n e.t o ntent clinical worsening and refractoriness to the proposed treatment, he died at 57 days of life. Barts syndrome is a genetic condition, and its diagnosis is clinical, made through the triad: cutaneous aplasia, epidermolysis bullosa and nail dystrophies. This syndrome is linked to autosomal dominant inheritance and occur e.t oto a mutation in the type VII collagen gene. Despite having a good prognosis, its diagnosis must be early, so the treatment, which is based on topical dressings and antibiotics, can be started as soon as possible to prevent possible complications such as: sepsis, hemorrhages, hydroelectrolytic disorder and death.

Infantile hemangioma (IH) is the most common vascular tumor in infancy. Although IHs can regress spontaneously, some problematic IHs still need treatment. However, either treated or untreated IHs may leave skin sequelae which can cause permanent disfigurement. Many studies evaluated the short-term efficacy of different kinds of treatment, but now, few studies are focusing on long-term skin sequelae. The objectives of our systemic review were to identify skin sequelae of IH thoroughly, determine specific factors associated with long-term IH sequelae, and learn how to improve these sequelae. We searched the following electronic databases: PubMed, EMBASE, Cochrane Library, and Clinicaltrials.gov. Three independent authors assessed the articles, and we reported this systemic review following PRISMA guidelines. Of 4448 articles initially identified, 62 underwent full-text review, and 17 met inclusion criteria. The overall rate of sequelae ranged from 5.3 to 93.5%. Factors associated with skin sequelae included patients' demographics, hemangioma characteristics, and treatment factors. What is Known: • Infantile hemangioma is the most common vascular tumor during infancy. • Infantile hemangiomas can regress spontaneously but either treated or untreated patients may leave permanent skin sequelae. Ectodermal dysplasias (EDs) represent a heterogeneous group of genetic disorders characterized by the abnormal development of ectodermal-derived tissues. They include the involvement of the hair, nails, skin, sweat glands, and teeth. Pathogenic variants in EDA1 (Xq12–13.1; OMIM*300451), EDAR (2q11-q13; OMIM*604095), EDARADD (1q42-q43, OMIM*606603), and WNT10A (2q35; OMIM*606268) genes are responsible for most EDs. Bi-allelic pathogenic variants of WNT10A have been associated with autosomal recessive forms of ED, as well as non-syndromic tooth agenesis (NSTA). The potential phenotypic impact of associated modifier mutations in other ectodysplasin pathway genes has also been pointed out. We present on an 11-year-old Chinese boy with oligodontia, with conical-shaped teeth as the main phenotype, and other very mild ED signs. The genetic study identified the pathogenic variants WNT10A (NM_025216.3): c.310C > T; p. (Arg104Cys) and c.742C > T; p. (Arg248Ter) in compound heterozygosis, confirmed by parental segregation. In addition, the patient had the polymorphism EDAR (NM_022336.4): c.1109T > C, p. (Val370Ala) in homozygosis, named EDAR370. A prominent dental phenotype with minor ectodermal symptoms is very suggestive of WNT10A mutations. In this case, the EDAR370A allele might also attenuate the severity of other ED signs. Background: In reconstructive and plastic surgery, split-thickness skin graft (STSG) treatments are frequently used to treat a variety of skin abnormalities. However, managing donor sites is difficult because of issues including infection and delayed recovery. Although collagen-based dressings have showed potential in the field of wound healing, STSG donor sites have not been the subject of indepth research. Methods: 100 patients getting STSG participated in this prospective randomized controlled trial, which was run between December 2020 and June 2022. The collagen dressing group (n = 50) and the standard saline dressing group (n = 50) were randomly assigned to the patients. Secondary outcomes were patient-reported outcomes and cosmetic evaluation, whereas primary results included healing rate and complication incidence. Results: Complete wound closure was achieved by 85% of patients in the collagen dressing group by postoperative day 30, compared to 62% in the regular saline dressing group. This represents a significant improvement in healing rates. When compared to the standard saline dressing group (18%), complications were lower in the collagen dressing group (6%). Patients who received collagen dressings reported less discomfort and greater satisfaction with their wound treatment. The collagen dressing group had better aesthetic results, according to cosmetic evaluation (80% rated "excellent"). Conclusion: At STSG donor sites, collagen dressings dramatically improve healing, lower complications, and enhance patient experiences. The use of collagen dressings in reconstructive and cosmetic surgery is supported by these data, which also point to prospective advantages. This study examined the associations of polysubstance use, mood disorders, and chronic conditions with the history of anxiety disorder among patients with opioid use disorder (OUD). We performed a secondary analysis of the baseline data from a clinical trial including 1,645 individuals with OUD, of which 513 had anxiety disorder. Substance use disorders (SUDs) included alcohol, amphetamines, cannabis, cocaine, and sedative use disorders. Mood disorders included major depressive disorder (MDD) and bipolar disorder (BD). Chronic conditions were allergies, gastrointestinal problem(s), skin problem(s), and hypertension. Sedative use disorder, MDD, BD, skin problems, and hypertension were significantly associated with anxiety disorder (p < 0.05). Additionally, more than two SUDs, two mood disorders, and more than two chronic conditions were significantly associated with anxiety disorder (p < 0.05). These findings highlight the comorbid mental health and physical health problems in individuals with OUD, as well as the need for integrated multidisciplinary treatment plans. [ABSTRACT FROM AUTHOR]

Inherited epidermolysis bullosa (EB) simplex is a heterogeneous group of skin fragility disorders caused by mutations in genes encoding cell‐cell or cell‐matrix adhesion proteins. A recently identified, rare subtype of EB simplex is due to bi‐allelic mutations in the EXPH5 gene, which encodes exophilin5, an effector protein of the Rab27B GTPase involved in intracellular vesicle trafficking and exosome secretion. The EXPH5 EB subtype is characterized by early‐onset skin blisters and scars, mainly on extremities, and varying degrees of pigmentary alterations. Here, we present a 31‐year‐old female with diffuse guttate hypopigmentation on the trunk and extremities since early childhood, with no apparent blisters or scars. We employed whole exome sequencing of germline DNA extracted from the patient's leukocytes to determine the genetic aetiology of the phenotype. A novel homozygous variant in EXPH5, c.1153C>T causing a premature stop codon at amino acid Glutamine 385, was identified. Histologic examination after skin pricking disclosed focal keratinocyte detachment typical to EB. Additionally, we identified a deleterious‐predicted variant in ENPP1, a gene associated with disturbed transfer of melanosomes to keratinocytes in Cole disease. Our report expands the clinical spectrum of inherited EB simplex with a possible di‐genic synergism contributing to co‐presentation with guttate leukoderma. [ABSTRACT FROM AUTHOR]

Annular erythema of infancy (AEI) is characterized by self‐limited eruptions of erythematous, annular to polycyclic patches and plaques, the etiology of which is thought to involve a hypersensitivity reaction to an unknown antigen. We present a case of AEI mistaken for systemic mastocytosis due to elevated serum tryptase. We were unable to find prior reports of an association between AEI and elevated tryptase in the literature.
