Oxidative stress, characterized by an overproduction of reactive oxygen species that overwhelm the body's physiological defense mechanisms, is a key factor in the progression of parasitic diseases in both humans and animals. Scabies, a highly contagious dermatological condition caused by the mite Sarcoptes scabiei var. hominis, affects millions globally, particularly in developing regions. The infestation leads to severe itching and skin rashes, triggered by allergic reactions to the mites, their eggs, and feces. Conventional scabies treatments typically involve the use of scabicidal agents, which, although effective, are often associated with adverse side effects and the increasing threat of resistance. In light of these limitations, there is growing interest in the use of medicinal plants as alternative therapeutic options. Medicinal plants, rich in bioactive compounds with antioxidant properties, offer a promising, safer, and potentially more effective approach to treatment. This review explores the role of oxidative stress in scabies pathogenesis and highlights how medicinal plants can mitigate this by reducing inflammation and oxidative damage, thereby alleviating symptoms and improving patient outcomes. Through their natural antioxidant potential, these plants may serve as viable alternatives or complementary therapies in the management of scabies, especially in cases where resistance to conventional treatments is emerging. Introduction: Scabies, caused by the mite Sarcoptes scabiei, is a highly contagious skin infestation. Poor personal hygiene often contributes to its prevalence. This study aims to identify specific aspects of personal hygiene that influence the incidence of scabies among residents of the Institute Rehabilitation Center of The Vagrants, Scroungers, and Individuals with Mental Disabilities in the South Sumatra Region. Methods: This observational-analytic study employed a case-control design. Data collection involved clinical records, observations, and questionnaire-based interviews. A total of 160 occupants, comprising 80 cases and 80 controls, were selected through simple random sampling. Data analysis utilized chi-square test and multiple logistic regression tests. Results and Discussion: Chi-square analysis revealed significant associations (p-value = 0.000) between scabies incidence and bathing habits, cleanliness of mattresses and bed linen, cleanliness of clothing, and open defecation behavior. Multiple logistic regression indicated that open defecation behavior (OR = 56.336; 95% CI = 17.281 – 158.500) was the primary personal hygiene factor influencing scabies incidence. Thus, maintaining proper open defecation practices is crucial for residents' hygiene and health. Conclusion: Poor personal hygiene practices, including inadequate bathing habits, unclean bedding, dirty clothing, and open defecation behavior, contribute to the occurrence of scabies. Background: Scabies is a highly contagious skin disease caused by scabies mite infestation, resulting in intense itching. In 2023, the Korean Dermatological Association is actively implementing the "Eradication of Scabies, National Health Project" as a key initiative. Under this project, educational programs for sanatoriums and other group residential facilities, as well as nationwide public awareness campaigns, are being conducted. An expert committee of dermatologists has developed standardized clinical guidelines for the diagnosis and treatment of scabies. Current Concepts: The clinical presentation of scabies varies depending on the age and health status of the patient, the number of scabies mites, and the mode of transmission. Characteristic skin symptoms include intense itching that worsens at night and the presence of burrows and red, inflamed papules on typical areas such as finger webs, inner wrists, scrotum, and around the umbilicus. In older adults or immunocompromised infants, scabies may involve the scalp, face, palms, and soles. Atypical skin findings such as excessive hyperkeratosis, nodules, and other unusual manifestations may also occur. Discussion and Conclusion: Using the scabies diagnostic and treatment algorithm, healthcare professionals in Korean clinical settings can make rapid medical decisions when examining patients with scabies. Dillenia ochreata were used traditionally to treat wounds and scabies. D. ochreata leaves have also been reported to have antibacterial, anti-inflammatory, and analgesic activities. This study aimed to determine the subchronic toxicity of an ethanol extract of D. ochreata leaves in Wistar rats. Tests were performed 28 days after administration of the extract. This study used 50 rats (25 males and 25 Females), each consisting of five groups: one control group (I) and four treatment groups (II-V) with doses of 100, 200, 400 and 800 mg/kg body weight (BW). Haematological parameters, biochemistry and histopathology of the liver, kidneys and heart were evaluated. The results showed that the treatment groups' haematological levels and blood biochemical levels were still within normal, except for group V, which showed a significant difference (p <0.05) from the group I. The organ histopathology results in group V showed liver and kidney damage with fat degeneration and moderate necrosis in both male and female rats. Administration of the ethanol extract of D. ochreata leaves up to a dose of group IV showed no toxic symptoms, but toxic effects were shown at a dose of group V. Introduction: Bullous scabies (BS) is an infrequent and atypical presentation of scabies, with a predilection for elderly males. The mechanism of BS is not fully understood; superinfection, friction due to pruritus, autoeczemation, direct injury from mite's lytic enzymes, cross-reactivity of scabies protein with basal membrane zone antigens are considered to be possible reasons. Aim: To define clinical features of paediatric BS cases, which is an extremely rare subtype of scabies. Material and methods: This is a retrospective study of paediatric BS cases seen at two tertiary care centres. Previously described bacterial culture, antibiogram and follow-up records were investigated retrospectively. Confirmed scabies cases, according to the "International Alliance for the Control of Scabies (IACS)" with bullae were included. All cases were treated with 10% sulfur ointment for 3 consecutive days, two cycles. Households of cases were also treated simultaneously. Systemic antibiotics were added to patients with elevated acute phase reactants according to the antibiogram results. Informed consent was obtained from patients' parents. Results: Five BS cases were included. Three cases were male, two cases were female. Four cases had staphylococcus aureus, one had group-A beta haemolytic streptococcus positive bullae culture. All cases achieved a rapid complete resolution of symptoms after topical 10% sulfur ointment. Conclusions: Paediatric BS is an extremely rare entity of scabies. Due to the growing aging population and the increased number of long-term patients staying in nursing facilities, the prevalence of scabies has recently been increasing, even in developed countries. This study aimed to identify the actual status of hospitalized patients with scabies in South Korea using the national health insurance claims data. From 2010 to 2019, 2586 patients were hospitalized with scabies (B86) as the primary diagnosis. There were more females than males (χ2 = 31.960, p < 0.001) and patients aged 80 years or older in long-term care hospitals (χ2 = 431.410, p < 0.001). Scabies patients were mainly hospitalized in internal medicine, family medicine, and dermatology for all provider types (χ2 = 170.033, p < 0.001). In long-term care hospitals, the rate of accompanying dementia was 31.9% (χ2 = 193.418, p < 0.001), cerebral infarction was 10.4% (χ2 = 106.271, p < 0.001), and cancer was 2.1% (χ2 = 17.963, p < 0.001), which was higher than other provider types. Additionally, 20.6% in general hospitals (χ2 = 198.952, p < 0.001) had an indwelling catheter, while 49.1% in hospitals and 41.1% in general hospitals were administered steroids (χ2 = 214.440, p < 0.001). The KOH smear test was performed in 11.3% of all inpatients with scabies. We suggest recognizing these characteristics of scabies patients and thoroughly checking the skin lesions during physical examination for early diagnosis and prevention of scabies infection. Community nurses may encounter scabies in homeless people, those living in prisons, people living at home or in people living in care homes. Scabies is more prevalent in older adults and there is evidence that this infection is becoming more prevalent and difficult to treat. The community nurse can help reduce such risks. Purpose: Scabies is a World Health Organization-defined neglected tropical disease and a growing public health issue worldwide. It is difficult to obtain reliable data on prevalence due to the lack of standardized tests. The aim of this study was to assess scabies online search behavior in Germany to identify local differences using Google search volume. Methods: Google Ads Keyword Planner was used to investigate the scabies-related search volume for Germany as a whole, its 16 federal states, and 15 large cities for the period from January 2016 to December 2019. The identified search terms were qualitatively categorized and critically analyzed. Results: A total of 572 keywords with an overall search volume of 11,414,180 searches regarding scabies were identified in Germany. The number of searches was higher in winter than in summer, with a national peak in March 2018. Around 30.6% of the searches regarding scabies therapy (n = 978,420) were related to home remedies. Regarding body localization, most searches focused on the whole body (n = 109,050), followed by head (n = 89,360) and the genital area (n = 28,640). Conclusions: The analysis of Google search data provides an overview of the populations' interest regarding scabies. The analysis can detect local peaks and assess the relevance of scabies at individual localizations of the body. The study highlighted current possible shortcomings in the therapy of scabies. Scabies infestation is a pruritic condition that is commonly seen in the pediatric population. Children are affected more than adults in certain geographic regions and can have variable clinical presentations. To avoid delays in diagnosis, healthcare providers should be aware of these differences. This article provides an overview of epidemiology and pathophysiology, clinical presentation and diagnosis, and management of scabies infestation, with a particular focus on infants and children. Scabies is a parasitic infestation of the skin caused by the mite Sarcoptes scabiei var. hominis. It is common in tropical areas, including the sub-tropical areas of Southern Africa. Predisposing factors are overcrowding, poor personal hygiene, low socio-economic standards and impaired host immunity. Although it can occur at any age, scabies is commonly seen in children and young adults. It is not a fatal disease; however, it can cause severe morbidity and poor quality of life. Scabies can complicate with pyoderma which may result in post-streptococcal glomerulonephritis. As a novel method of ivermectin (IVM) administration for the treatment of scabies, we devised a whole‐body bathing (WBB), in which patients are immersed in a fluid that contains IVM. A multi‐institutional trial for elderly patients with scabies was conducted to investigate the efficacy and safety of IVM‐WBB. Seven elderly patients with scabies were enrolled and received IVM‐WBB up to four times at 1‐week interval. The cure for scabies was defined as the absence of mites in two consecutive microscopic or dermoscopic examinations at weekly intervals and the absence of new skin lesions indicative of scabies. Consequently, the cure rate on day 22, the primary end‐point, was 71.4%, and all patients had been cured until day 29. Additionally, neither significant adverse events nor clinically problematic abnormal blood test values were obtained. Furthermore, no IVM was detected in the optional plasma (five cases) collected for IVM measurement after bathing. These results suggest that IVM‐WBB was effective to treat scabies, causing no serious adverse events and with a very low internal exposure of IVM. [ABSTRACT FROM AUTHOR]

Aim: Scabies is a contagious parasitic skin disorder, which is characterized by severe pruritus. Scabies usually presents with papules and excoriations on hands, feet, axillae, umbilicus and genitalia. Recently, both increased prevalence of the disease and resistance to anti-scabies treatment have been reported. Therefore, early diagnosis and appropriate treatment of scabies are crucial. The aim of this study was to evaluate the characteristics of patients with scabies and treatments that the patients received in order to provide successful management of scabies. Material and Methods: Medical records of the patients who were diagnosed with scabies between June 2019 and February 2020 were reviewed retrospectively. Age, gender, disease duration, similar symptoms in household members, dermatological examination, prior treatments and treatments that we recommended were recorded. Results: The study included 74 patients (39 female, 35 male) with scabies. The mean age of the patients was 40.6±16.7 years. The mean disease duration was 53.5±5.5 days. Pruritus was observed in the household members of 37 (50%) patients. 18 (24.3%) patients received only symptomatic treatment since they were diagnosed as other pruritic dermatoses including xerosis or dermatitis, previously. However, we treated 58 (78.4%) patients using topical permethrin 5%. In addition, 4 (5.4%) patients were treated with topical sulfur 12.5% and 3 (4.1%) patients were treated with balsam of Peru. 9 (12.2%) patients who were resistant to topical treatment received oral ivermectin for complete cure of scabies. Conclusion: The results indicated that most of the patients with scabies (78.4%) were treated with topical permethrin successfully, whereas 12.2% patients required oral ivermectin in addition to topical anti-scabies treatment. Nevertheless, delay in diagnosis of 24.3% patients might increase the risk of spread of scabies. Therefore, we recommend topical permethrin, oral ivermectin and immediate further clinical and laboratory evaluation for indeterminate cases. [ABSTRACT FROM AUTHOR]

The purpose and prospective of the work exertion is to formulate and carryout evaluation study of medicated cream composed of gelling agent and emulsifier for the treatment of Scabies. An o/w cream for scabies loaded with Permethrine is prepared by dispersion method. Performed comparative diffusion studies by discriminating diffusion media. Carbomer homopolymer type B was used as a gelling agent. Glyceryl monostearate, polyoxyethylene cetyl ethers and lanoline alcohol were used in combination as emulsifiers. In this formulation mineral oil and fractionated coconut oil was used as oil phase. An FTIR and DSC study suggests that no chemical interactions present between drug and polymer. Solubility studies were carried at different compendial medium. Optimised cream was further evaluated for pH, Viscosity, particle size, spreadability, drug content, water content, formaldehyde content and comparative diffusion with reference to the Reference Listed Drug (RLD), using Ethanol: PBS (50:50) and Ethanol: PBS (80:20) as a diffusion medium. Stability studies were carried out on prepared formulation for homogeneity and drug content. Thereof the prepared, developed and formulated o/w cream to be effective for the treatment of Scabies. Scabies is a highly contagious, globally prevalent, parasitic skin infestation caused by Sarcoptes scabiei var. hominis, also known as the itch mite. There have been outbreaks not only in the developing world, but also in the developed world among refugees and asylum seekers. Once infested with scabies mites, symptomatic patients, as well as asymptomatic carriers, quickly spread the disease through direct skin-to-skin contact. Typically, symptoms of scabies are characterized by an erythematous, papular, pruritic rash associated with burrows. Treatment of scabies involves using topical or systemic scabicides and treating secondary bacterial infections, if present. Given the prevalence and contagiousness of scabies, measures to prevent its spread are essential. Through application of the novel Identify-Isolate-Inform (3I) Tool, emergency medical providers can readily identify risk factors for exposure and important symptoms of the disease, thus limiting its spread through prompt scabicide therapy; isolate the patient until after treatment; and inform local public health authorities and hospital infection prevention, when appropriate. Ultimately, these three actions can aid public health in controlling the transmission of scabies cases, thus ensuring the protection of the general public from this highly contagious skin infestation. Currently, there are three known subtypes of scabies: ordinary, crusted, and bullous. The worldwide prevalence of scabies remains high in the 21st century. To decrease the social, economic, and psychological impact on the enormous population infected, a lot of important work has been completed over the last 20 years concerning the management of scabies. For example, a standardization of guidelines for the treatment of scabies has been completed and programs have been designed for the prevention and treatment in endemic populations, called mass drug administrations. Unfortunately, these only apply to the ordinary form of scabies. Moreover, resistance to the drugs currently used in treatment is growing, which imposes the need to search for new treatments. For this purpose, new acaricides are being developed to enhance the therapeutic options for the patients’ benefit and effectively treat this disease. There is also the necessity for prevention before the development of scabies. An effective vaccine has the potential to protect people before this disease, especially in endemic areas. Unfortunately, there are no such vaccines against Sarcoptes yet.

Monosulfiram is a drug used topically in the treatment of scabies. Upon its application it shows similar effects as seen in case of disulfiram, a drug used in alcohol aversion therapy. Previous reports have concluded a light induced conversion of monosulfiram (MS) into disulfiram (DS). In the present study a computational approach has been involved to investigate the mechanism of this conversion. Structures have been optimized using MP2 and DFT approach. Insights on their reactivities have been assessed through conceptual DFT. Time-dependent DFT investigation has been undertaken to obtain excitation energies for singlet and triplet states. Bond dissociation energies of both molecules have also been obtained and analyzed. It has been found that conversion of MS into DS occurs in both thermal and photochemical situations. Abstract: The scabies mite, Sarcoptes scabiei var hominis, is an obligate ectoparasite of humans. It has been a source of distress for humanity since antiquity. The troublesome mite is emerging triumphant over current acaricidal agents with reports of emerging resistance and treatment failures. Scabies in endemic areas and crusted scabies offer additional management challenges. Exploration of indigenous plants and better understanding of mite biology and pathogenesis provide opportunities for the development of novel agents for this common pest. We review the recent diverse approaches to scabies, including the use of novel plant products with a better safety profile, translating the use of moxidectin from veterinary practice to human scabies, vaccination, immunotherapy, and development of drugs that directly target mite molecules. Background: Scabies and head lice are ubiquitous ectoparasitic infestations that are common across the Pacific Islands. Ivermectin is an effective treatment for both conditions, although the doses used vary. At a community level, mass drug administration (MDA) with ivermectin is an effective strategy to decrease prevalence of scabies. To what extent MDA with ivermectin will also reduce prevalence of head lice is unknown. Methodology: Head lice prevalence was assessed before and after MDA with oral ivermectin (at a dose of 200 micrograms per kilogram of body weight) administered on day 1 and day 8. The primary outcome was the change in prevalence of head louse infestation at two weeks compared to baseline. Longer term efficacy was assessed three months after MDA. Results: 118 participants were enrolled. Baseline prevalence of active head louse infestation was 25.4% (95% CI 18.4–34.0). At three-month follow-up, prevalence was 7.5% (95% CI 2.7–12.3), a relative reduction of 70.6% (95% CI 72.7%-91.4%, p <0.001). Head louse infestation was associated with younger age (age ≤10 years: prevalence 46.7%; adjusted odds ratio compared to adults of 7.2, 95%CI 2.0–25.9) and with having at least one other member of the household with active head louse infestation (adjusted odds ratio 4.3, 95%CI 1.7–11.1). Conclusions: Head louse infestation is common in the Solomon Islands. This proof of principle study shows that oral ivermectin at a dose of 200 micrograms per kilogram can reduce the burden of active head louse infestation, offering an additional collateral benefit of MDA with ivermectin for scabies control. Abstract: Background: Protothecosis is an uncommon infection caused by the achlorophyllic algae found more commonly in tropical areas. Only a limited number of cases have been reported. Objective: We aimed to evaluate the clinicopathological features and treatment outcomes of cutaneous protothecosis. Methods: We retrospectively identified 20 pathology‐confirmed cases of cutaneous protothecosis based on skin biopsies in two tertiary medical centres in Taiwan from 1997 to 2015. Results: The age of the patients at the time of diagnosis ranged from 48 to 85 years (mean age of 74 years). All lesions developed on the limbs. Twelve (60%) patients had adrenal insufficiency, but no patients had active malignancy at diagnosis. Interestingly, four (20%) patients had concurrent scabies infestation. Clinically, most lesions were erythematous plaques studded with punctate ulcers. Microscopically, the most common finding was granulomatous inflammation. Nineteen (95%) cases were successfully treated with itraconazole for 14–148 days with only one case of recurrence. Concomitant scabies should be suspected if pruritus is recalcitrant despite itraconazole treatment. Conclusion: Despite its rarity, cutaneous protothecosis has become more significant due to an increased prevalence of immunocompromised individuals. Steroid overuse or iatrogenic adrenal insufficiency predisposes individuals to high‐risk infections. Neglecting the disease leads to a chronic and incurable state. Protothecosis should be suspected in chronic eczematous and ulcerative plaques on the limbs refractory to conventional antibacterial and antiviral treatments, especially in patients with adrenal insufficiency. Background: Scabies outbreaks in residential and nursing care homes for elderly people are common, subject to diagnostic delay, and hard to control. We studied clinical features, epidemiology, and outcomes of outbreaks in the UK between 2014 and 2015.Methods: We did a prospective observational study in residential care homes for elderly people in southeast England that reported scabies outbreaks to Public Health England health protection teams. An outbreak was defined as two or more cases of scabies (in either residents or staff) at a single care home. All patients who provided informed consent were included; patients with dementia were included if a personal or nominated consultee (ie, a family member or nominated staff member) endorsed participation. Dermatology-trained physicians examined residents at initial clinical visits, which were followed by two mass treatments with topical scabicide as per local health protection team guidance. Follow-up clinical visits were held 6 weeks after initial visits. Scabies was diagnosed through pre-defined case definitions as definite, probable, or possible with dermatoscopy and microscopy as appropriate.Findings: 230 residents were examined in ten outbreaks between Jan 23, 2014, and April 13, 2015. Median age was 86·9 years (IQR 81·5-92·3), 174 (76%) were female, and 157 (68%) had dementia. 61 (27%) residents were diagnosed with definite, probable, or possible scabies, of whom three had crusted scabies. Physical signs differed substantially from classic presentations. 31 (51%) of the 61 people diagnosed with scabies were asymptomatic, and only 25 (41%) had burrows. Mites were visualised with dermatoscopy in seven (11%) patients, and further confirmed by microscopy in three (5%). 35 (57%) cases had signs of scabies only on areas of the body that would normally be covered. Dementia was the only risk factor for a scabies diagnosis that we identified (odds ratio 2·37 [95% CI 1·38-4·07]). At clinical follow-up, 50 people who were initially diagnosed with scabies were examined. No new cases of scabies were detected, but infestation persisted in ten people.Interpretation: Clinical presentation of scabies in elderly residents of care homes differs from classic descriptions familiar to clinicians. This difference probably contributes to delayed recognition and suboptimal management in this vulnerable group. Dermatoscopy and microscopy were of little value. Scabies is a skin infestation with the mite Sarcoptes scabiei causing itch and rash and is a major risk factor for bacterial skin infections and severe complications. Here, we evaluated the treatment outcome of 2866 asylum seekers who received (preventive) scabies treatment before and during a scabies intervention programme (SIP) in the main reception centre in the Netherlands between January 2014 and March 2016. A SIP was introduced in the main national reception centre based on frequent observations of scabies and its complications amongst Eritrean and Ethiopian asylum seekers in the Netherlands. On arrival, all asylum seekers from Eritrea or Ethiopia were checked for clinical scabies signs and received ivermectin/permethrin either as prevention or treatment. A retrospective cohort study was conducted to compare the reinfestations and complications of scabies in asylum seekers who entered the Netherlands before and during the intervention and who received ivermectin/permethrin. In total, 2866 asylum seekers received treatment during the study period (January 2014 –March 2016) of which 1359 (47.4%) had clinical signs of scabies. During the programme, most of the asylum seekers with scabies were already diagnosed on arrival as part of the SIP screening (580 (64.7%) of the 897). Asylum seekers with more than one scabies episode reduced from 42.0% (194/462) before the programme to 27.2% (243/897) during the programme (RR = 0.64, 95% CI = 0.55–0.75). Development of scabies complications later in the asylum procedure reduced from 12.3% (57/462) to 4.6% (41/897). A scabies prevention and treatment programme at start of the asylum procedure was feasible and effective in the Netherlands; patients were diagnosed early and risk of reinfestations and complications reduced. Scabies is an infestation of the skin by the mite Sarcoptes scabiei. It manifests with pruritic erythematous papules and excoriations, in addition to the pathognomonic burrows. Multiple drugs can be used for treatment, but resistance to conventional therapy is increasing throughout the years. Scabies is caused by Sarcoptes scabiei var. hominis. The disease can be sexually transmitted. Patients' main complaint is nocturnal itch. Disseminated, excoriated, erythematous papules are usually seen on the anterior trunk and limbs. Crusted scabies occurs in immunocompromised hosts and may be associated with reduced or absent pruritus. Recommended treatments are permethrin 5% cream, oral ivermectin and benzyl benzoate 25% lotion. Alternative treatments are malathion 0.5% aqueous lotion, ivermectin 1% lotion and sulphur 6-33% cream, ointment or lotion. Crusted scabies therapy requires a topical scabicide and oral ivermectin. Mass treatment of large populations with endemic disease can be performed with a single dose of ivermectin (200 micrograms/kg of bodyweight). Partner management needs a look-back period of 2 months. Screening for other STI is recommended. Patients and close contacts should avoid sexual contact until completion of treatment and should strictly observe personal hygiene rules when living in crowded spaces. The disease scabies is one of the earliest diseases of humans for which the cause was known. It is caused by the mite, Sarcoptes scabiei, that burrows in the epidermis of the skin of humans and many other mammals. This mite was previously known as Acarus scabiei DeGeer, 1778 before the genus Sarcoptes was established (Latreille 1802) and it became S. scabiei. Research during the last 40 years has tremendously increased insight into the mite's biology, parasite-host interactions, and the mechanisms it uses to evade the host's defenses. This review highlights some of the major advancements of our knowledge of the mite's biology, genome, proteome, and immunomodulating abilities all of which provide a basis for control of the disease. Advances toward the development of a diagnostic blood test to detect a scabies infection and a vaccine to protect susceptible populations from becoming infected, or at least limiting the transmission of the disease, are also presented. The article presents a case study of a 10-year-old girl who was diagnosed with Down syndrome and presented hyperkeratotic papules and plaques on her whole body. Topics discussed include hand and foot deformity, treatment of patient with daily therapy of benzyl benzoate emulsion and diagnosis of Norwegian scabies.

Background: Scabies is an ectoparasitic infection, which occurs because of direct skin-to skin contact. The ideal treatment modality is still unclear and further research on this topic is warranted. The aim of the study was to com- pare the efficacy and safety of the topical scabicides: permethrin, crotamiton and sulfur ointment. Methods: Fifty four patients with diagnosed scabies were randomly divided into three treatment groups. The first group received 5% permethrin cream twice with one week interval, the patients from the second group were given crotamiton lotion for two days twice with one week interval, while the third group received 10% sulfur ointment for two or three weeks. All patients were followed up at 1, 2 and 4 weeks intervals. Results: At one-week follow up the cure rate was significantly higher at permethrin-treated group when compared to crotamiton group (P< 0.001) and sulfur group (P< 0.001). At the end of two-week interval, the cure rate at permethrin group was 100%, while at crotamiton group, 66.7% and in sulfur group 38.9% (P< 0.001). At 4-week follow up the applied treatment was effective in all studied individuals. Conclusion: The topical application of permethrin, crotamiton and sulfur was equally efficacious at 4-week follow up, however permethrin cream showed faster improvement at first and second follow up. Acquiring permethrin is considered as expensive option and crotamiton lotion seems to be cost-less alternative to this cream. [ABSTRACT FROM AUTHOR]

Scabies is an infection caused by Sarcoptes scabiei. In developed countries, scabies remains an important public health problem in hospitals and care facilities among elderly or immunocompromised patients. There are a number of medical providers who have not experienced scabies and there has been confusion surrounding its management and prevention. Therefore, the aim of the present study was to identify the optimal approach for the management and prevention of scabies. A scabies outbreak occurred between June 2014 and October 2014 in the Toho University School of Medicine, Omori Hospital (Tokyo, Japan), and the current study investigates factors concerning the outbreak, such as disease recognition, diagnostic strategy, medical staff experience and correspondence after the outbreak occurred. Six patients were newly diagnosed with scabies including patients, medical staff and family of the medical staff. An infection control committee was implemented and required a follow-up survey of 181 people (144 patients and 37 medical staff). It took ~4 months to resolve the outbreak. Scabies is highly infectious, and sufficient knowledge is required to care for the patients and prevent the infection of healthy people. Background: Scabies is one of the commonest dermatological conditions globally; however it is a largely underexplored and truly neglected infectious disease. Foremost, improvement in the management of this public health burden is imperative. Current treatments with topical agents and/or oral ivermectin (IVM) are insufficient and drug resistance is emerging. Moxidectin (MOX), with more advantageous pharmacological profiles may be a promising alternative. Methodology/Principal Findings: Using a porcine scabies model, 12 pigs were randomly assigned to receive orally either MOX (0.3 mg/kg once), IVM (0.2 mg/kg twice) or no treatment. We evaluated treatment efficacies by assessing mite count, clinical lesions, pruritus and ELISA-determined anti-S. scabiei IgG antibodies reductions. Plasma and skin pharmacokinetic profiles were determined. At day 14 post-treatment, all four MOX-treated but only two IVM-treated pigs were mite-free. MOX efficacy was 100% and remained unchanged until study-end (D47), compared to 62% (range 26–100%) for IVM, with one IVM-treated pig remaining infected until D47. Clinical scabies lesions, pruritus and anti-S. scabiei IgG antibodies had completely disappeared in all MOX-treated but only 75% of IVM-treated pigs. MOX persisted ~9 times longer than IVM in plasma and skin, thereby covering the mite’s entire life cycle and enabling long-lasting efficacy. Conclusions/Significance: Our data demonstrate that oral single-dose MOX was more effective than two consecutive IVM-doses, supporting MOX as potential therapeutic approach for scabies. The aim of this study is to evaluate four drug regimens for treatment of scabies as regard their efficacy, acceptability and cost effectiveness. Two hundred cases with ordinary scabies were randomized into four groups. First group received ivermectin 200 μg/kg body weight single oral dose, repeated after one week. The second received benzyl benzoate 20% cream. The third received permethrin 2.5%–5% lotion, whereas the fourth group received 5–10% sulfur ointment. Topical treatments were applied for five consecutive nights. Patients were followed up for two weeks for cure rate and adverse effects. At the end of the study, permethrin provided a significant efficacy of 88% and acceptability in 100% of cases, but had higher cost to treat one case (20.25 LE). Ivermectin provided efficacy and acceptability rates of 84% and 96%, respectively, and had a cheaper cost (9.5 LE). Benzyl benzoate provided 80% for both rates and was the cheapest drug. Sulfur ointment provided the least rates, and it was the most expensive. Treatment choice will depend on the age, the general condition of cases, patient compliance to topical treatment and his ability to stick to its roles, and the economic condition of the patient. With the increasing number of pets in home the human-animal relationship is increasingly close and care about control disease growing. Ivermectin (IVM) is frequently used because its proven safety. IVM is recommended for the treatment of demodectic scabies and prevention of heartworm in dogs, but informally is extremely used to control of Ctenocephalides felis felis and Rhipicephalus sanguineus . The aim of this study is evaluate the use of IVM in dogs, by the oral route at 0.6 μg/kg dose, against experimental infection of these parasites using the construction of the plasma concentration curve and efficacy study. A IVM quantification method in canine plasma using HPLC-FL was developed and validated based on RE n°899/03 ANVISA. The samples collected during the efficacy test was analyzed by this validated method and prove C max of 350 ng/mL at 4 h (t max ) and AUC of 8411 ng/h/mL. Spite of formulation have shown good absorption, the highest efficiency values found for Rhipcephalus sanguineus and Ctenocephalides felis felis were very low, 35% and 67% respectively, demonstrating this not be the most appropriate treatment for the control of these parasites. The effect of lemon oil ( Citrus limon) on Sarcoptes scabiei var. cuniculi was evaluated in vitro and in vivo. The mite samples were collected from naturally infected rabbits. The lemon oil was prepared in six concentrations by dilution with distilled water (2.5, 5, 10, 20, 50, and 100 %). In vitro application was done in five replicates for each concentration in petri dishes in the laboratory. The treated mites were observed at 1, 12, and 24 h post application (PA) for lemon oil effect. In addition, oxidative stress profile was evaluated for the treated mite. Dependent on in vitro results, 20 % lemon oil was used in vivo trial. Twenty-four naturally infected rabbits were divided into three groups of eight: 20 % lemon oil, deltamethrin, and untreated control. The infected parts of rabbits were treated topically once a week for four successive weeks. In vitro application results showed that lemon oil 10 and 20 % diluted in water caused mortality to 100 % of mites after 24 h PA. The oxidative stress profile revealed that mites treated with 20 % lemon oil had significantly ( P < 0.05) higher hydrogen peroxide and malondialdehyde when compared with mites treated with deltamethrin or distilled water. In vivo application of 20 % lemon oil on naturally infected rabbits showed complete recovery from clinical signs, absence of mite in microscopic examination from the second week of treatment. In addition, productive performance was significantly better than infected untreated group. Also, the treated tissue showed stoppage of scale formation and hair growth faster than deltamethrin-treated rabbits. Objective Indigenous Australians suffer higher mortality and have statistically worse outcomes for many chronic disease states than the non-Indigenous population. Although many of these people are prescribed pharmaceutical drugs for their illnesses, some still use medicines that were traditional to their culture. This report reviews some of the traditional medicines used for ailments seen in Indigenous Australian communities. Design A literature search was conducted, with the period between the publication of an 'Aboriginal Pharmacopoeia' in 1988 and 'current' our target interval for searching. The ethics of publishing knowledge belonging to Aboriginal people is briefly discussed in this context. Results Ailments were grouped into communicable diseases, pain and inflammation, skin disorders and gastrointestinal disorders. Although cancer is regarded as a disease of the 'white man', it is briefly discussed in the context that several traditional medicines and foods may have provided some protective effects. Where known, the ethnopharmacology of these medicines is discussed, as well as a brief description of their preparation and application. Conclusion Some Indigenous Australians continue to use traditional medicines. We have tabulated these according to ailment, and have listed pharmacological actions where known. What is not known, however, is their potential to interact with pharmaceutical drugs. Background: Scabies, or mange as it is called in animals, is an ectoparasitic contagious infestation caused by the mite Sarcoptes scabiei. Sarcoptic mange is an important veterinary disease leading to significant morbidity and mortality in wild and domestic animals. A widely accepted hypothesis, though never substantiated by factual data, suggests that humans were the initial source of the animal contamination. In this study we performed phylogenetic analyses of populations of S. scabiei from humans and from canids to validate or not the hypothesis of a human origin of the mites infecting domestic dogs. Methods: Mites from dogs and foxes were obtained from three French sites and from other countries. A part of cytochrome c oxidase subunit 1 (cox1) gene was amplified and directly sequenced. Other sequences corresponding to mites from humans, raccoon dogs, foxes, jackal and dogs from various geographical areas were retrieved from GenBank. Phylogenetic analyses were performed using the Otodectes cynotis cox1 sequence as outgroup. Maximum Likelihood and Bayesian Inference analysis approaches were used. To visualize the relationship between the haplotypes, a median joining haplotype network was constructed using Network v4.6 according to host. Results: Twenty-one haplotypes were observed among mites collected from five different host species, including humans and canids from nine geographical areas. The phylogenetic trees based on Maximum Likelihood and Bayesian Inference analyses showed similar topologies with few differences in node support values. The results were not consistent with a human origin of S. scabiei mites in dogs and, on the contrary, did not exclude the opposite hypothesis of a host switch from dogs to humans. Conclusions: Phylogenetic relatedness may have an impact in terms of epidemiological control strategy. Our results and other recent studies suggest to re-evaluate the level of transmission between domestic dogs and humans. [ABSTRACT FROM AUTHOR]

Lindane is a neurotoxicant used for the treatment of lice and scabies present on human skin. Due to its pharmaceutical application, an extensive pharmacokinetic database exists in humans. Mathematical diffusion models allow for calculation of lindane skin permeability coefficients using human kinetic data obtained from in vitro and in vivo experimentation as well as a default compound-specific calculation based on physicochemical characteristics used in the absence of kinetic data. A dermal model was developed to describe lindane diffusion into the skin, where the skin compartment consisted of homogeneous dermal tissue. This study utilized Fick's law of diffusion along with chemical binding to protein and lipids to determine appropriate dermal absorption parameters which were then incorporated into a physiologically based pharmacokinetic (PBPK) model to describe in vivo kinetics. The estimation of permeability coefficients using chemical binding in combination with in vivo data demonstrates the advantages of combining physiochemical properties with a PBPK model to predict dermal absorption. Sheep scab, caused by infestation with the mite Psoroptes ovis, is highly contagious, causing intense pruritus and represents a major welfare and economic concern. Disease control strategies rely upon chemotherapy, however, sustainability is questionable due to issues of chemical residues, eco-toxicity and acaricide resistance. Control by vaccination is supported by demonstration of protective immunity in sheep previously infested with P. ovis. We identified vaccine candidates for P. ovis based on: (1) antigens selected by their interaction with host signalling pathways and the host immune-response; and (2) those shown to be either immunogenic or involved in mite feeding. This resulted in the development and validation, in repeated immunisation and challenge trials, of a seven recombinant protein sub-unit cocktail vaccine. Sheep were inoculated on three occasions, 2 weeks apart, along with QuilA adjuvant. Vaccination resulted in highly significant reductions in both lesion size (up to 63%) and mite numbers (up to 56%) following challenge. Mean lesion size in vaccinates was significantly smaller than controls from 1 week post infestation (wpi) until the end of the experiment at 6 wpi. All antigens elicited serum IgG responses following immunisation and prior to infestation, whereas controls did not produce antigen-specific IgG during the pre-infestation period. Vaccinated animals showed an amnestic response, with levels of antigen-specific IgG against muGST, Pso o 1 and Pso o 2 increasing following infestation. This vaccine represents the greatest reduction in lesion size to date with a sheep scab vaccine, providing encouragement for future production of a commercially-viable means of immunoprophylaxis. Patients with chronic itch suffer from higher levels of depression and anxiety than their healthy counterparts. Furthermore, psychological factors, such as stress, are known to aggravate itch. The mere act of thinking about itching can induce the sensation. Interventions like habit reversal training and arousal reduction have been shown to have positive effects on itch relief. Yet, there is still limited data on the psychological management to control the itch scratch cycle and a description of methods suitable to address itch. In this review, we describe different psychological interventions shown to be effective in the treatment of chronic itch. Abstract: Background: Skin bacterial colonizations and superinfections are well‐known complications of scabies, in particular, in tropical and subtropical countries. However, only very rare studies on skin bacterial colonizations and superinfections have been carried out in immunocompetent patients with scabies living in Western countries. Methods: Eighty‐nine Caucasian adult immunocompetent patients with scabies living in Milan, Italy, were subjected to bacteriologic examinations of the skin. Clinical diagnosis of scabies was confirmed in all patients by means of microscopic examinations: they were considered positive when adults or eggs or feces of Sarcoptes scabiei var. hominis were visible. In all patients, six skin swabs (three for aerobic and three for anaerobic bacteria) were taken in three different areas before the beginning of the treatment. Results: No clinical manifestations of pyoderma were observed. Bacteriologic examinations were positive in 5/89 patients (5.6%). Bacteriologic cultures were positive for Staphylococcus aureus in all five patients; in one patient, they were also positive for Escherichia coli. No growth of anaerobic bacteria was recorded. Conclusions: According to the results of this study, skin bacterial colonizations and superinfections in immunocompetent adult patients with scabies living in Milan are uncommon. ABSTRACT Many medications are available for scabies treatment including oral and topical ivermectin. However, studies comparing these two forms as a scabies treatment are few. This study compares efficacy and safety of topical versus oral ivermectin as scabies treatment. The study included 62 confirmed uncomplicated scabies patients, divided into: Group I (32 patients, received topical ivermectin) and Group II (30 patients, received oral ivermectin). Patients were assessed, clinically and by KOH smear at 1, 2 and 4 weeks. Treatment was repeated after one week in patients with persistent infection. Adverse events were recorded. Most patients (87.5% and 73.5% in group I and group II respectively) were symptom free after a single treatment. A second treatment was required in 4 patients of group I and 8 patients of group II. However, 2 weeks after treatment symptoms and signs completely resolved in all cases with no recurrence at 4 weeks. This study suggests that both topical and oral ivermectin are safe and equally effective in treatment of uncomplicated scabies. Single treatment, whether topical or oral, is associated with high cure rate in a week post treatment. However, repeating treatment after one week may be required to achieve 100% cure. Introduction Scabies is a major public health problem in the Pacific and is associated with an increased risk of bacterial skin infections, glomerulonephritis and rheumatic fever. Mass drug administration with ivermectin is a promising strategy for the control of scabies. Mass treatment with ivermectin followed by active case finding was conducted in five communities in the Solomon Islands between 1997 and 2000 and resulted in a significant reduction in the prevalence of both scabies and bacterial skin infections. Methods We conducted a prospective follow-up study of the communities where the original scabies control programme had been undertaken. All residents underwent a standardised examination for the detection of scabies and impetigo. Results Three hundred and thirty eight residents were examined, representing 69% of the total population of the five communities. Only 1 case of scabies was found, in an adult who had recently returned from the mainland. The prevalence of active impetigo was 8.8% overall and 12.4% in children aged 12 years or less. Discussion We found an extremely low prevalence of scabies 15 years after the cessation of a scabies control programme. The prevalence of impetigo had also declined further since the end of the control programme. Our results suggest that a combination of mass treatment with ivermectin and intensive active case finding may result in long term control of scabies. Larger scale studies and integration with other neglected tropical disease control programmes should be priorities for scabies control efforts. [ABSTRACT FROM AUTHOR]

The article presents a study that aimed to define the house dust mite (HDM) allergens that bind immunoglobulin E (IgE) from patients with scabies. The study used nine purified or recombinant HDM allergens to measure antibody-binding activities of IgE, IgG1 and IgG4. The authors suggest that epitopes with useful immuno-modulatory properties can be defined by understanding the environment or genetic interactions in allergy and asthma.
