Abstract

Novel information retrieval methods to identify citations relevant to a clinical topic can overcome the knowledge gap existing between the primary literature (MEDLINE) and online clinical knowledge resources such as UpToDate. Searching the MEDLINE database directly or with query expansion methods returns a large number of citations that are not relevant to the query. The current study presents a citation retrieval system that retrieves citations for evidence-based clinical knowledge summarization. This approach combines query expansion, concept-based screening algorithm, and concept-based vector similarity. We also propose an information extraction framework for automated concept (Population, Intervention, Comparison, and Disease) extraction. We evaluated our proposed system on all topics (as queries) available from UpToDate for two diseases, heart failure (HF) and atrial fibrillation (AFib). The system achieved an overall F-score of 41.2% on HF topics and 42.4% on AFib topics on a gold standard of citations available in UpToDate. This is significantly high when compared to a query-expansion based baseline (F-score of 1.3% on HF and 2.2% on AFib) and a system that uses query expansion with disease hyponyms and journal names, concept-based screening, and term-based vector similarity system (F-score of 37.5% on HF and 39.5% on AFib). Evaluating the system with top K relevant citations, where K is the number of citations in the gold standard achieved a much higher overall F-score of 69.9% on HF topics and 75.1% on AFib topics. In addition, the system retrieved up to 18 new relevant citations per topic when tested on ten HF and six AFib clinical topics.

Abstract

Aims: Compared with those without obesity, patients with obesity‐related heart failure with preserved ejection fraction (HFpEF) have worse symptoms, haemodynamics, and outcomes. Current weight loss strategies (diet, drug, and surgical) work through decreased energy intake rather than increased expenditure and cause significant loss of skeletal muscle mass in addition to adipose tissue. This may have adverse implications for patients with HFpEF, who already have reduced skeletal muscle mass and function and high rates of physical frailty. Mitochondrial uncoupling agents may have unique beneficial effects by producing weight loss via increased catabolism rather than reduced caloric intake, thereby causing loss of adipose tissue while sparing skeletal muscle. HU6 is a controlled metabolic accelerator that is metabolized to the mitochondrial uncoupling agent 2,4‐dinotrophenol. HU6 selectively increases carbon oxidation from fat and glucose while also decreasing toxic reactive oxygen species (ROS) production. In addition to sparing skeletal muscle loss, HU6 may have other benefits relevant to obesity‐related HFpEF, including reduced specific tissue depots contributing to HFpEF; improved glucose utilization; and reduction in systemic inflammation via both decreased ROS production from mitochondria and decreased cytokine elaboration from excess, dysfunctional adipose. Methods: We describe the rationale and design of HuMAIN‐HFpEF, a Phase 2a randomized, double‐blind, placebo‐controlled, dose‐titration, parallel‐group trial in patients with obesity‐related HFpEF to evaluate the effects of HU6 on weight loss, body composition, exercise capacity, cardiac structure and function, metabolism, and inflammation, and identify optimal dosage for future Phase 3 trials. 50. Stabilization of Rapidly Progressive Cardiac Allograft Vasculopathy Using mTOR Inhibition After Heart Transplantation.

Abstract

• This study included 216 patients taking tacrolimus after heart transplantation, 81 of whom were switched from mycophenolate to sirolimus after 1 year. • Treatment with sirolimus was associated with a reduction in intimal thickness on serial IVUS studies, driven by patients diagnosed with rapidly progressive CAV. • However, the transition to sirolimus was not associated with cardiovascular events after a median follow-up of 8.6 years. Inhibition of the mammalian target of rapamycin (mTor) pathway after heart transplantation has been associated with reduced progression of coronary allograft vasculopathy (CAV). The application of low-dose mTOR inhibition in the setting of modern immunosuppression, including tacrolimus, remains an area of limited exploration. This retrospective study included patients who received heart transplantation between January 2009 and January 2019 and had baseline, 1-year and 2–3-year coronary angiography with intravascular ultrasound (IVUS). Intimal thickness in 5 segments along the left anterior descending artery was compared across imaging time points in patients who were transitioned to low-dose mTOR inhibitor (sirolimus) vs standard treatment with mycophenolate on a background of tacrolimus. Long-term adverse cardiovascular outcomes (revascularization, severe CAV, retransplant, and cardiovascular death) were also assessed. Among 216 patients (mean age 51.5 ± 11.9 years, 77.8% men, 80.1% white), 81 individuals (37.5%) were switched to mTOR inhibition. mTOR inhibition was associated with a reduction in intimal thickness by 0.05 mm (95% CI 0.02–0.07; P < 0.001). This reduction was driven by patients who met the criteria for rapidly progressive CAV 1-year post-transplant (0.12 mm; P = 0.016 for interaction). After a median follow-up of 8.6 (IQR 6.6–11) years, 40 patients had major adverse cardiovascular outcomes. The use of mTOR inhibitors was not significantly associated with cardiovascular outcomes (P = 0.669). Transitioning patients after heart transplantation to an immunosuppression regimen composed of low-dose mTOR inhibition and tacrolimus was associated with a lack of progression of CAV, particularly in those with rapidly progressive CAV at 1 year, but not with long-term cardiovascular outcomes.
