Amaç: Bal antiödem etkisi, granülasyonu hızlandırıcı etkisi, enzimatik debridman ve immün sistemi güçlendirici etkisi bilinen bir doğal gıdadır. Bu deneysel çalışmanın amacı bir alkali ajan olan sodyum hidroksit ile oluşturulan koroziv özofajitte darlık gelişimi üzerine balın ve prednisolonun etkilerini araştırıldı. Yöntemler: Bu çalışma, ağırlıkları 200-250 gram arasında değişen Wistar tipi albino sıçanlar üzerinde gerçekleştirildi. Denekler, eşit sayıda dört gruba bölündü. Birinci, ikinci ve üçüncü gruplar özofajit modeli grupları olup, bu gruplardaki tüm deneklerde %37,5’luk sodyum hidroksit ile koroziv özofajit oluşturuldu. Birinci gruba herhangi bir tedavi uygulanmazken, ikici gruba oral bal tedavisi ve üçüncü gruba intraperitoneal prednisolon tedavisi uygulandı. Kontrol grubunun özofagusuna herhangi bir işlem yapılmadan şam laparotomi uygulandı. Tüm denekler 28. gün sakrifiye edildi ve deneklerin distal özofaguslarından alınan 20 mm uzunluğundaki örnekler histopatolojik incelemeye tabi tutuldu. Grupların doku hasarı skorları ve stenoz indeksi skorları ölçülerek kıyaslandı.Bulgular: Çalışmaya toplam 32 denek dahil edildi ve her bir grupta sekiz denek mevcuttu. Stenoz indeksi skoru ortalama değeri ve doku hasarı skoru ortalama değeri balla tedavi edilen grupta, prednisolon verilen ve tedavi verilmeyen gruplara kıyasla anlamlı olarak düşüktü (P=0,001).Sonuç: Bal, prednisolona kıyasla, alkalen ajanla oluşturulan deneysel koroziv özofajit modelinde gelişen özofagus darlıkları üzerinde daha anlamlı düzeyde olumlu etkiye sahiptir., Aim: Honey is well known for its beneficial wound healing-related effects, including anti-edematous effects, stimulation of granulation tissue formation, chemical debridement, and immune system enhancement. In this experimental study, the effects of honey and prednisolone on stricture formation in sodium hydroxide-induced corrosive esophagitis were investigated.Methods: This study was carried out on Wistar albino rats weighing between 200-250 grams. Rats were divided into four equal groups. Corrosive esophagitis induced by 37.5% sodium hydroxide was formed in three groups. The first group received no specific treatment. The second group was treated with honey via the oral route, and the third group was treated by prednisolone intraperitoneally. The control group underwent a sham laparotomy. All subjects were sacrificed by the end of the 28th day. A 20-mm long segment of the distal esophagus was harvested for histopathological examination. The tissue damage scores and stenosis index scores of the groups were measured and compared.Results: A total of 32 rats were included in the study, with eight subjects in each group. The mean values of stenosis index score and tissue damage score were significantly lower in the honey-treated group (P=0.001).Conclusion: Oral honey treatment seems to reduce the severity of esophageal strictures associated with corrosive esophagitis, when compared to untreated and prednisolone-treated groups.

Introduction: Sialadenoma papilliferum (SP) is a rare benign tumor, which originates from the minor salivary gland. It occurs at sites that have minor salivary glands, such as the palate, retromolar pads, buccal mucosa, and lips. The most common location for tumor development is on the hard palate. A differential diagnosis consists of ruling out other salivary gland tumors. Transoral robotic surgery (TORS) is a new technology used in head and neck surgery within certain centers around the world. Case Report: Herein, we present the first successful case of SP tumor removal by TORS. Conclusion: This particular case highlights the identification of this rare tumor in an unusual location. Furthermore, it demonstrates the utilization of TORS, leveraging the superior visualization to obtain excellent local control with minimal acute and long-term morbidity, in comparison to conventional transoral surgical approaches.

Bukkal mukoza tutulumu nadir görülmektedir. Ayırıc tanısına diğer tükürük bezi tümörleri girmek- tedir. Tedavisi temiz cerrahi sınırlarla total eksizyondur. The hard and soft palate are the most common sites of PA of the minor salivary glands. PA of the buccal minor salivary gland is seen rarely. A differential diagnosis consists of ruling out other salivary gland tumors. The typical treatment for PA is complete excision with clear surgical margins. Herein, we report the diagnosis, treatment and follow-up in process of a case PA of minor salivary glands in buccal mucosa and reviewed the relevant literature.Key words: Buccal mucosa; Minor salivary gland; Pleomorphic adenoma

Kaposi's sarcoma is a slowly progressive systemic multicentric tumor which usually involves the skin. The extracutaneous Kaposi's sarcoma involves lymph nodes, gastrointestinal tract and lungs at most. The most commonly involved parts of the gastrointestinal tract are the stomach and the duodenum. In this article, we present a patient who was diagnosed as Kaposi's sarcoma by the upper gastrointestinal tract endoscopy that was performed because of dyspeptic complaints., Kaposi sarkomu genellikle cildi tutan, sistemik multisentrik ve yavaş ilerleyen bir tümördür. Ekstrakutanöz Kaposi sarkomu en sık lenf düğümleri, gastrointestinal sistem ve akciğerleri tutar. Gastrointestinal traktın en sık tutulan yerleri arasında ise mide ve duodenum gelmektedir. Bu yazıda dispeptik şikayetleri nedeniyle yapılan üst gastrointestinal sistem endoskopisinde Kaposi sarkomu saptanan bir hasta sunulmuştur.

Background/aim: In the pathogenesis of intestinal type gastric cancer, the importance of the process of atrophic gastritis-intestinal metaplasiadysplasia- cancer is well known. In this study, assuming that it will provide a good database in gastric cancer follow-up programs, we intended to establish the rates of gastric precancerous lesion (atrophy, intestinal metaplasia, and dysplasia) in patients with dyspepsia. Materials and methods: From June 2003 - February 2004, 310 consecutive patients (172 females, 138 males, mean age 55.8, range 16-80) who applied for gastrointestinal endoscopy due to dyspepsia were included in the study. Five biopsies were taken from the antrum and corpus of these patients using the updated Sidney system and were examined histopathologically. Results: In these patients, we determined 8% (24 patients) with atrophic gastritis, 20% (61 patients) with intestinal metaplasia (complete or incomplete), and 4% (13 patients) with dysplasia. While H. pylori was (+) in 90% (89 patients) of 98 cases with precancerous lesions, H. pylori was (+) in 61% (115 patients) of 187 patients without precancerous lesions, and the difference was statistically significant. Conclusion: Several precancerous lesions may be detected in the stomach of patients in whom endoscopic examinations are performed due to dyspepsia. Verifying the strong relationship between intestinal type gastric cancers and H. pylori, this bacteria was found at a high rate in patients with precancerous lesions., Giriş ve amaç: İntestinal tip mide kanserinin patogenezinde atrofik gas trit-intestinal metaplazi-displazi- kanser sürecinin önemi bilinmektedir. Bu çalışmada mide kanser izlem programları içinde iyi bir veri tabanı sağlayabileceği öngörüsüyle dispepsili olgularda midedeki prekanseröz lezyon (atrofi, intestinal metaplazi, displazi) oranlarını belirlemeyi amaçladık. Gereç ve yöntem: Çalışmaya 2003 Haziran-2004 Şubat ayları arasında dispepsi nedeni ile üst gastrointestinal endoskopi yapılan ardı şık 285 hasta (161 kadın, 124 erkek, yaş ortalaması 55.8, yaş aralığı 16-80) alındı. Bu hastalardan güncelleştirilmiş Sidney sistemine göre antrum ve korpustan toplam 5 biyopsi alındı ve histopatolojik değerlendirme yapıldı. Bulgular: Bu hastaların %8'inde (24 hasta) atrofik gastrit, %21'inde (61 hasta) intestinal metaplazi (komplet ve inkomplet), %4'ünde (13 hasta) displazi saptandı. Mide kanseri için prekanseröz lezyonlar saptanan 98 olgunun %90'ında (89 hasta) H. pylori (+) iken, prekanseröz lezyon saptanmayan 187 olgunun ise %61'inde (115 hasta) H. pylori (+) bulundu ve istatiksel olarak anlamlı fark bulundu. Sonuç: Dispepsi nedeniyle üst gastrointestinal endoskopi yapılan hastalarda midenin prekanseröz lezyonları (atrofi, intestinal metaplazi, displazi) saptanabilir. İntestinal tip gastrik kanserle H. pylori arasındaki güçlü ilişkiyi doğrulayacak şekilde prekanseröz lezyon bulunan hastalarda bu bakteri yüksek oranda saptanmıştır.

Malign melanom (MM) deri kanserleri içerisinde 3. sıklıkta görülen tüm dünyada ve ülkemizde insidansı artmakta olan, mortalite oranı en yüksek cilt kanseridir. Malign melanomlu hastaların tedavisinde amaç primer melanomun ve bölgesel lenf nodu metastazlarının ve nadiren uzak metastazların cerrahi olarak tedavisidir. Erken tanı ve yeterli primer cerrahi rezeksiyon malign melanom tedavisinin temelidir. Lokal nüks, patolojik olarak ilk cerrahi rezeksiyonu geniş lokal eksize edilmiş cerrahi skarın 2 cm dışına kadar gelişen melanomdur ve in-transit metastaz ise orijinal lezyon ve bölgesel lenf nodları alanı arasında kalan alanda orijinal lezyondan 2 cm’den daha uzak gelişen herhangi bir lezyondur. Malign melanomun lokal nüks ve in-transit metastaz görülme sıklığı ve mortalite oranı yaşla artar. Sunduğumuz bu olguda 85 yaşında kadın hastanın ayak tabanında tümör kalınlığı 4,75 mm olan malign melanom, geniş lokal eksize edilmiş ve inguinal diseksiyon uygulanmıştır. Takiplerine gelmeyen hasta 2 yıl sonra tüm bacağa yayılmış olan çok sayıda in-transit metastaz ve lokal rekürrens ile başvurmuştur. Son zamanlarda yapılan bilimsel çalışmalar lokal nüks ve in-transit metastazların özellikle 65 yaşın üzerinde, başlangıç tümör kalınlığı 4 mm’den fazla, mitotik hızı mm2 de 11’in üzerinde olan ve anjiyolenfatik invazyon bulunan hastalarda görüldüğünü göstermiş ve yaşla ilgili olarak lenfatik fonksiyonların değişerek nod tutulumunun azaldığını ve yaşlı hastalarda in-transit metastazların arttığını saptamışlardır. Böyle yüksek riskli hastalar özellikle teşhisten sonraki ilk 18 ayda erken nükslerin saptanabilmesi için sıkı takip edilmelidirler., Malignant melanoma (MM) is the third most common skin cancers seen around the world and our country with an increasing incidence and the highest mortality rate of skin cancer. Objective in the treatment of patients with malignant melanoma is adequate surgical management of primary melanoma and regional lymph node metastasis, and rarely distant metastasis. Early diagnosis and adequate surgical resection is the primary basis for treatment of malignant melanoma. Local recurrence (LR) is melanoma occurring within 2cm of the surgical scar of wide local excision of the primary tumor with pathologic evidence of complete initial resection and in-transit metastasis (ITM) is any lesion greater than 2 cm from the original lesion but between the original lesion and regional lymph node basin. The incidence of LR/ITR of malignant melanoma and the mortality rate of MM increase with age. We present 85-year-old female patient in this case report which was in the bottom of her feet, tm thickness 4.75 mm and wide local excision of malignant melanoma and inguinal dissection was performed. She didn’t come her follow-up and 2 years later she was presented with a lot of in-transit metastasis and local recurrence in all right lower extremity. Recent studies showed that older patients (>65) with thicker tumors (>4mm) have higher risk for local and in-transit recurrence and presented age related altered lymphatic function reduce nodal positivity and increased in-transit disease in older patients. Such higher-risk patients should receive aggressive clinical follow-up evaluation, particularly within the first 18 months of diagnosis for early recurrence.

Drug-induced pseudolymphoma cases are frequently seen. However, these cases usually develop as a result of systemic use of the drug. Pseudolymphoma cases due to the use of local agents have also been reported. We are also presenting localized cutaneous pseudolymphoma, with erythematous patch and plaque lesions on injection sites; due to subcutaneous injection of insulin and interferon. According to our literature knowledge our cases are the first pseudolymphoma cases that seemed on insulin and interferon injection sites., İlacın indüklediği psödolenfoma olgularına oldukça fazla rastlanmaktadır. Ancak bu olgular daha çok ilacın sistemik kullanımı sonucu gelişmektedir. Ajanların lokal kullanımına bağlı psödolenfoma olguları da bildirilmiştir. Bu çalışmada subkutan insülin ve interferon enjeksiyonuna bağlı; enjeksiyon bölgelerine lokalize, eritemli patch şeklinde lezyonlarla kendini gösteren iki psödolenfoma olgusu sunuyoruz. Literatür bilgilerine göre olgularımız insülin ve interferon enjeksiyon bölgelerinde lokalize gözlenen ilk psödolenfoma olgularıdır.

PubMedID: 22798108 Background/aims: The aim of this multicenter study was to determine the histopathological features and immunohistochemical profiles of gastrointestinal stromal tumors diagnosed in Turkish patients. Material and Methods: Twenty-eight participating centers registered their gastrointestinal stromal tumor cases on a nationwide database. The diagnosis of gastrointestinal stromal tumor relied upon hematoxylin & eosin features and the results of antibody panel including CD117, CD34, desmin, smooth muscle actin, S-100 protein, and Ki67. The database consisted of parameters including age, gender, location, and all other histopathological and immunohistochemical findings. Statistical analysis was performed using Pearson, Kruskal-Wallis, Mann-Whitney U, and Spearman tests. Results: From all of the gastrointestinal stromal tumors in the database, 1160 cases with a male to female ratio of 1.22 and a mean age of 56.75 years were included in the study. The most common location was the stomach (45.0%), followed by the small intestine, omentum-peritoneum, large intestine, and esophagus (32.0%, 12.6%, 9.3%, 1.1%, respectively). The risk groups were distributed as: 6.1% very low, 21.7% low, 19.3% intermediate, and 53% high-risk cases. Many histopathologic findings were correlated with risk groups. CD117 was positive in 95.3% of gastrointestinal stromal tumors, whereas CD34 was positive in 74.9%, smooth muscle actin in 45.9%, desmin in 9.2%, and S-100 in 19.1.%. Though no significant relation was found between CD117 expression and tumor location, CD34, smooth muscle actin and Ki67 expressions significantly varied in different locations (p=0.001) and risk groups. Conclusions: The results of this multicenter study demonstrated that features other than tumor size and mitosis and immune markers other than CD117 and Ki67 included in the antibody panel seem to be useful as predictive risk factors.
