Uterine adenosarcoma is an uncommon biphasic tumor with benign epithelial and malignant mesenchymal components, often presenting difficulties for morphological diagnosis. We describe 5 cases of adenosarcoma of the uterine corpus and cervix, and vaginal stump in patients aged 46-76 years. Clinical data, ultrasound results, morphological data, including immunohistochemical studies with antibodies to CD10, estrogen and progesterone receptors, desmin, smooth muscle actin, and Ki-67 are presented. Large polypoid mass of the epithelial-mesenchymal structure within the uterine cavity in women of peri - and postmenopausal age require the exclusion of malignancy of the mesenchymal component with searchig for diagnostic criteria - periglandular cuffing of the stromal cells and mitoses.Аденосаркома матки — редкая бифазная опухоль с доброкачественным эпителиальным и злокачественным мезенхимальным компонентами, нередко представляющая трудности для морфологической диагностики. Описано 5 наблюдений аденосаркомы тела, шейки матки и культи влагалища у пациенток 46—76 лет. Представлены клинические данные, результаты ультразвукового исследования, морфологические данные, включая иммуногистохимическое исследование с антителами к CD10, рецепторам эстрогена и прогестерона, десмину, гладкомышечному актину, Ki-67. Крупные полиповидные образования тела и шейки матки эпителиально-мезенхимального строения у женщин пери- и постменопаузального возраста требуют исключения злокачественности мезенхимального компонента с прицельным поиском диагностических критериев — наличие камбиального слоя и митозов.

A case of persistent gestational trophoblastic disease that developed after ectopic (cervical) pregnancy with complete hydatidiform mole (CHM) in a 56-year-old patient is presented. The diagnosis of CHM was made retrospectively based on immunohistochemical analysis of archival material using p57 and Ki67 antibodies. Observation shows the difficulty of objective diagnosis of hydatidiform mole in early pregnancy due to the lack of typical cystic transformation of the stroma of villi, focal proliferation of villous trophoblast. Application of the p57 marker for the differential diagnosis of CHM and other variants of cystic villi transformation may be especially important in cases of ectopic pregnancy, in which morphological changes in the chorion do not always correspond to the classical picture. The presence of ectopic pregnancy with CHM in a 56-year-old patient requires special attention of clinicians.Представлено наблюдение персистирующей трофобластической болезни, развившейся после эктопической (шеечной) беременности с полным пузырным заносом (ППЗ) у 56-летней пациентки. Диагноз ППЗ был поставлен ретроспективно при анализе архивного материала с помощью иммуногистохимического исследования с антителами p57 и Ki-67. Наблюдение демонстрирует объективную сложность диагностики пузырного заноса на ранних сроках беременности — отсутствие типичной кистозной трансформации стромы ворсин, очаговый характер пролиферации ворсинчатого трофобласта. Подтверждена целесообразность использования маркера p57 для дифференциального диагноза ППЗ и других вариантов кистозной трансформации ворсин, что особенно актуально для эктопической беременности, при которой морфологические изменения хориона не всегда соответствуют классической картине. Наличие эктопической беременности с ППЗ у 56-летней пациентки требует особого внимания клиницистов.

Cesarean section (CS) is the most widespread operation despite of high frequency of intra and post operational complications as opposed to spontaneous delivery. The aim of investigation (objective) is the analysis of outcomes of CS in every stages during and after CS. Material and methods. There is a retrospective analysis of 675 patients after CS. 60 women after pregnancy; 75 patients with insolvent scar after previous CS during planning of the next pregnancy; 540 scarred pregnant women in term and 24 pregnant women with implantation in the scar. Results. The application of the secondary sutures is indicated in the case of insufficient sutures on the uterine wall. The metroplastic operation is indicated during the next pregnancy. The insufficiency of the previous uterine scar is the indication for CS in more than 70 %. The earliest diagnosis of the localization of the placenta in the scar is desirable. The most cautious method of pregnancy interruption should be used with immediate or postponed metroplastic.

To reveal the morphological characteristics of simultaneously diagnosed leiomyoma of the corpus uteri and vulva.The paper describes a case of multiple uterine leiomyomas concurrent with vulvar leiomyoma in a 39-year-old patient with progressive tumor nodule growth over 2 years. Vulvar tumor was biopsied simultaneously with extirpation of the uterus; vulvar leiomyoma was removed six months later. Histological and immunohistochemical studies: such as hematoxylin and eosin staining, the expression of smooth muscle actin, desmin, and progesterone and estrogen receptors, S100, CD10, and determination of Ki-67 proliferation index, were conducted.The largest (14-cm) multiple tumor nodule in the corpus uteri had the structure of leiomyoma of uncertain malignant potential; the large (8-cm) vulvar tumor was a leiomyoma with hyalinosis. The immunohistochemical profile of uterine and vulvar leiomyoma (smooth muscle actin+, desmin+, progesterone+, estrogen+ receptors, CD117-, and Ki-67) was the same (1-3%). The vulvar leiomyoma was assumed to be a tumor of metastatic origin.Vulvar leiomyoma is rare; it can arise from smooth muscle tissue of various anatomical structures of the skin and soft tissues. The pathogenesis of the so-called metastatic leiomyoma is unclear; there are concepts of a metaplastic transformation of subcelomic mesenchyme and multifocal smooth muscle proliferation. The presented case demonstrates the synchronous development of uterine and vulvar leiomyoma.Цель исследования - выявить морфологические особенности лейомиомы тела матки и лейомиомы вульвы, диагностированные одновременно. Материал и методы. Представлено наблюдение множественной лейомиомы тела матки в сочетании с лейомиомой вульвы у пациентки 39 лет с прогрессивным ростом опухолевых узлов в течение 2 лет. Биопсия опухолевого образования вульвы выполнена одновременно с экстирпацией матки, лейомиома вульвы удалена через полгода. Проведены гистологическое и иммуногистохимическое исследования: окраска гематоксилином и эозином, экспрессия гладкомышечного актина, десмина, рецепторов прогестерона и эстрогена, S100, CD10, а также определение индекса пролиферации Ki-67. Результаты. Наибольший из множественных опухолевых узлов тела матки (14 см) имел строение лейомиомы с неясным злокачественным потенциалом, опухоль вульвы больших размеров (8 см) - лейомиома с гиалинозом. Иммуногистохимический профиль лейомиомы тела матки и вульвы был одинаков: гладкомышечный актин+, десмин+, рецепторы к прогестерону+, эстрогену+, CD117-, Ki-67 1-3%. Предположено метастатическое происхождение лейомиомы вульвы. Заключение. Лейомиома вульвы встречается редко, может развиваться из гладкомышечной ткани различных анатомических структур кожи и мягких тканей. Патогенез так называемой метастатической лейомиомы неясен, существуют концепции метапластической трансформации субцеломической мезенхимы и мультифокальной гладкомышечной пролиферации. Представленное наблюдение демонстрирует синхронное развитие лейомиомы тела матки и вульвы.

Introduction:Cicatricial deformity of the uterine cervix is a relevant and understudied problem in medicine. It is important to conduct preoperative preparation aimed at creating a favorable background for operative treatment as well as to conduct post-operative treatment for improving and accelerating the reparative processes.Objective:Improving the results of surgical treatment of cicatricial deformity of the uterine cervix with the help of preoperative preparation and evaluating the efficiency of application of the Flamena gel complex in the post-operative period.Method:There were 30 patients having cicatricial deformity of the uterine cervix aged 25 to 45 under observation. With all patients, the clinical, laboratory and instrumental examination was conducted which included: microscopic examination of the intensity of leukocytosis in the vaginal and cervical secretion, cytological examination, bacteriological inoculation from the cervical canal, the PCR method test for sexually transmitted infections (the STIs), detection of the human papillomavirus, the content of immunoglobulines (IgA, IgG, IgM) in the venous blood serum and in the mucous secretion of the cervical canal, colposcopy, ultrasonography, and histological examination of the uterine cervix.Results:After normalizing the biocenosis of the vagina, the Superlimf 25 units immunomodulating agent was prescribed to all patients, as another preoperative preparation stage. Then surgical treatment was performed. In the post-operative period, the Flamena gel complex was prescribed. The combined therapy contributed to improving the results of the surgical treatment.Conclusion:The algorithm suggested by the authors which includes preoperative preparation, surgical treatment and post-operative care can be used as an integrated approach to the management of patients having cicatricial deformity of the uterine cervix.
