3. Progressive perceptual-motor impairment without generalized dementia: a type of cortical degenerative syndrome

- Abstract
A patient developed slowly progressive clumsiness of both upper extremities with associated impainnent of daily activities without loss of muscle strength or abnormal muscle tone. Her sensory function was normal in the primary modalities but was impaired in the discriminative tasks. A single photon emission computed tomography using 123I-iodoamphetamine showed the areas with reduced uptake in the postcentral cortices. Based on the analysis of her motor impairment, we conclude that clumsiness of hands results from higher sensory disturbance.

The frequency profiles and time courses of oscillatory changes when reading words are not fully understood, although there have been many reports that oscillatory dynamics reflect local brain function. In order to clarify oscillatory dynamics, we investigated the frequency and spatiotemporal distributions of neuromagnetic activities during silent reading of words in 23 healthy subjects. Individual data were divided into the following frequency bands: theta (5–8 Hz), alpha (8–13 Hz), beta (13–25 Hz), low gamma (25–50 Hz), and high gamma (50–100 Hz), and were analyzed by synthetic aperture magnetometry (SAM). The time window was consecutively moved in steps of 50 ms. Group analysis was performed to delineate common areas of brain activation. A transient power increase in the theta band occurred first in the bilateral occipital cortices, and then rapidly propagated to the left temporo-occipital areas, left inferior and middle frontal gyri, bilateral medial prefrontal cortices, and finally to the left anterior temporal cortices, which possibly reflects a serial cognitive process. This serial propagation of the transient power increase in the theta band was followed by sustained power decreases in the alpha, beta and low gamma bands. These results suggest that the transient power increase in the theta bands may be associated with priming and propagation of local activities, while sustained power decreases in the alpha, beta and low gamma bands reflect parallel neural processes related to silent reading words. Our results showed a relationship between frequency bands of oscillatory changes and locations. This may have implications in the relationship between frequency bands and functions.

The education system for medical technologists has recently been revolutionized, their educational periods vary from 2 to 9 years, and some already have doctoral degrees. In such a new situation, our faculty thinks that the most important point for new medical technologists is the ability to have a broad view of the clinical fields, especially the view of patients. Special training in bed-side education and a stint in several divisions, such as the surgical operation room, rehabilitation. radiological examination room, pharmacy, central storage room of medical records, and medical informatics, and so on, of the hospital is a powerful tool to obtain a broad view of the various clinical fields and can be essential for developing high performance medical technologists. As nine years have passed since starting this education, we evaluated this practice through systematic personal communication. As a result, it was found to be extremely effective for many reasons such as having a continuous image of the patient when they examine the blood sample in the hospital laboratory, showing advanced laboratory performance, and having no mental barrier to visiting the wards and so on. The abilities of our alumni are praised highly by many large scale hospitals around the country and 50% of them are working in the clinical laboratory division of these hospitals. About 40% are working in the division of research and development in various companies. We express sincere thanks to the director and all cooperative individuals for this course in the Osaka University Hospital.

7. Long latency response in soleus muscle evoked by magnetic stimulation at the foramen magnum

We performed magnetic stimulation at the level of foramen magnum in healthy subjects to evaluate the long latency response in lower limb muscle. Subjects assumed an upright stance and we recorded electromyographic activities in soleus muscle. A late response at the onset latency of approximately 40 ms was elicited. The late response wasn't induced in other lower limb muscles; anterior tibial muscle, quadriceps femoris muscle, and biceps femoris muscle. Additionally, magnetic stimulation to foot motor cortex, basal occiput and cervical nerve roots did not evoke the response with latency of 40 ms. These results reveal the late response in soleus muscle that has not been previously reported. We speculate that it is involved with the long-loop reflex.

A repetitive speech disorder resulting from infarcts in the paramedian thalami and the midbrain is reported. Although the speech disorder seemed like stuttering, the compulsive repetitions, constant rate and monotonous tone were not associated with ordinary stuttering. Since repetition was restricted to the first syllable, the speech disorder in our patient could be distinguished from palilalia. The extrapyramidal system is considered responsible for repetitive speech disorders resulting from infarcts in the paramedian thalami and the midbrain but without good reason. Repetitive speech disorder in patients with infarcts in the supplementary motor area (SMA) have similar clinical features to our patient. It is suggested that interruption in the projective system to the SMA is a possible cause of "stuttering like repetition".

We examined eight patients with Kearns-Sayre syndrome (KSS) to investigate a dysfunction in the central nervous system (CNS) using PTN-SEP, MN-SEP and BAEP. We found a significant increase in the P37 latency of PTN-SEPs and the central conduction time of MN-SEPs, and interpeak latencies of BAEPs. Delayed SEPs or BAEPs were caused by a dysfunction of the somatosensory or lateral lemniscus pathways which could be related to mitochondrial abnormalities in the CNS. Long-term therapy with CoQ showed an improvement of the latencies of SEPs after about half a year from the start of CoQ therapy in our patients. The improvement of the latencies of SEPs were preserved during CoQ therapy. It could be demonstrated that CoQ therapy had the beneficial effects on abnormal functions of the CNS in patients with KSS.

Single large-scale deletions of mitochondrial DNA (mtDNA) are found in 70 to 80% of Kearns-Sayre syndrome (KSS) patients. Most deletions are flanked by direct repeats up to 13 nucleotides in length. The incidence of ragged-red fibers and cytochrome c oxidase-negative fibers in muscle is correlate with the amount of deleted mtDNA. Recently, study with 'cybrid' cell lines, which have different proportions of deleted mtDNA, showed that accumulation of deleted mtDNA to over 60% of the total mtDNA resulted in progressive inhibition of overall mitochondrial translation, as well as, reduction of cytochrome c oxidase activity. These results suggest that deletion of mtDNA alone is sufficient for the mitochondrial dysfunction in KSS.

We reported here a 64-year-old man with a central apnea resulted from unilateral medullary infarction. He was admitted because of cerebellar ataxia, dysarthria and dysphasia of abrupt onset. After the injection of diazepam for alcohol forbidden syndrome, he induced complete apnea and required the endotracheal intubation. At the spontaneous respiration under room air, his arterial blood gas showed hypercapnea without hypoxemia, and he fell into severe hypoventilation when hypnotic drug was injected. Respisomnogram revealed the frequent presence of central apnea both while he was awake and asleep. MRI demonstrated an abnormal high intensity area on T2 weighted image at the right lateral medulla just below the ponto-medullary junction. At autopsy, areas of the infarction were limited within the right lateral medulla, including lateral portion of the medullary reticular formation, the ambigual nucleus, one part of the solitary nuclear complex, the inferior cerebellar peduncle and the spinal trigeminal nucleus. However, the dorsomotor nucleus of vagus was completely free from the infarct lesion. There was no other lesion within central nervous system. Such a distribution seemed the minimal extent of the lesion responsible for central, apnea compared to the previous reports. We suggest that central apnea occurs not infrequently in the cases of Wallenberg's syndrome.

Four different genes encoding variant transthyretins (TTR) have been known in Japanese cases with familial amyloidotic polyneuropathy (FAP); TTR Met30, Gly42, Arg50 and Cys114. First three mutant genes can be detected by restriction fragment length polymorphism using NsiI, Cfr13I and MvaI, respectively. Since a single base change responsible for TTR Cys114 produces no new restriction site, RFLP is not directly applicable for the detection of this gene. In this study, TTR Cys114 gene was amplified by polymerase chain reaction using mismatch primer to produce a new restriction site for HgiAI. The enzyme digestion of the product resulted in the appearance of extra fragments in the presence of the normal fragment. The accurate detections of all the four mutant genes are hereafter possible by these procedures.

33. [A case of SSPE that showed watershed infarction like image on MRI initially, and progressed to whole brain atrophy]

We reported a 15-year-old male patient of subacute sclerosing panencephalitis (SSPE), progressed from stage I to stage IV. On his admission, his clinical state was stage I with disturbance of generalized attention, myoclonus of upper limbs and no focal cortical signs. After rapid progression to stage III in a month, his clinical state became fairly stable and gradually advanced to stage IV. We followed him with serial CT and MR images. On imagings we found initial lesion in left parietoocipital lobe and spread in right parietoocipital lobe, anterior lobe and then brain stem, showing low density on plain CT scans and high intensity of T2 weighed MR images. This initial lesion situated in watershed area was hardly distinguishable from watershed infarction. In addition, we found blood flow decrease in vertebral artery system with SPECT and Doppler flowmetry. These findings suggested vascular pathogenesis of SSPE. We showed the imaging processes of SSPE progression from formation of initial lesion to demyelination and gliosis of total brain.

34. [A case of paraneoplastic neuropathy with necrotizing arteritis localized in the peripheral nervous system]

A 59-year-old demented Japanese man who was proven to have high titer of serum alpha-fetoprotein (AFP) and carcino-embryonic antigen (CEA) was admitted to our hospital. Neurological examinations revealed moderate dementia with deterioration and loss of memory, and decreased deep tendon reflexes in all extremities. Sensory disturbances were not obvious. There were no significant changes in the usual laboratory findings including CSF, except for elevated serum AFP and CEA. Three months after admission, he died of gastric cancer and its metastases in liver and lymph nodes. Post-mortem examination in the central nervous system (CNS) revealed many senile plaques and neurofibrillary tangles throughout the cerebral cortex and hippocampus. There was marked loss of neurons in the hippocampus. All the neuropathological findings in the CNS were consistent with those in Alzheimer disease. In the peripheral nervous system, necrotizing arteritis was found throughout the length of sciatic nerve. Large myelinated fibers seemed to be preferentially degenerated with proximo-distal gradient. Teased fiber preparation revealed de/remyelination and axonal degeneration more frequently at the distal portion. Immunohistologically, the serum IgG of this patient specifically reacted to the endothelial cells of all vessels in control organs, which strongly suggested the autoimmune mechanism for the necrotizing arteritis in this patient. The pathogenetic role of this antibody for necrotizing arteritis, found selectively in the peripheral nervous system, still remained unclear. However, paraneoplastic neuropathy due to necrotizing arteritis is a distinct entity in addition to common form of paraneoplastic subacute sensory neuropathy.

This peculiar astrocytoma showed no hemorrhage, hemosiderin deposits, melanin or iron. Shortening of T1 relaxation time may be caused by protein hydration layer due to protein rich tumor cells.

Echocardiographic findings in five patients with pulmonary embolism were studied. Tricuspid echocardiograms showed abnormalities in valve motion, that is a monophasic triangular wave during diastole in all the patients as well as an increased dimension of the right ventricle. An "a' dip of the pulmonary valve echocardiogram was also recognised in all five. Later tricuspid echocardiograms regained the normal M-shaped configuration. The monophasic triangular pattern of the tricuspid valve during diastole may be related to the shorter duration of tricuspid valve opening compared with that of the mitral valve. Tachycardia cannot explain this difference between tricuspid and mitral valve motion, which seems to be caused by a prolonged isovolumic relaxation time of the right ventricle resulting in a delayed opening of the tricuspid valve. These results were obtained by comparing these data with those of control subjects and patients with chronic right ventricular overloading resulting from atrial septal defect.

Interruption of the aorta is a very rare congenital anomaly. The third case in a living adult has been reported here with a brief review of the clinicopathological data concerning living patients reported in the literature. According to the site of the interruption of the aortic arch, three basic types of absence of the aortic arch are recognized. In the majority of patients, the interruption of the aorta is associated with the presence of VSD, patent ductus arteriosus, and atrial septal defect. Other cardiovascular abnormalities are transposition of aorta, aortic stenosis, and so on. We have briefly speculated on why the majority of patients with “Steidele's anomaly” die early in life and very few survive for long periods. The case reported here is one of the very rare cases of “Steidele's complex” in a living adult.

Rendu-Osler-Weber病は欧米においては数多く報告されているが,本邦では比較的稀な疾患である.本症に肝内動静脈瘻を伴うものは非常に珍しい.また肝内動静脈瘻も稀な疾患である.本症例は吐血および鼻出血を主訴とした52才の主婦で,末梢血管拡張,その部よりの出血および遺伝性を認め,同時に血管造影にて肝内動静脈瘻を認めた.肝内動静脈瘻を伴うRendu-Osler-Weber病は3例報告されており, 1例にportosystemic encephalopathyを認める.本症例においても説明のつかない脳症状をおこし死亡したが,単にportosystemic encephalopathyとするには疑問の余地があり,血圧低下に起因する脳のanoxiaも一因と考える.

Echocardiographic study was performed on the mitral valve echogram in cases with clinical as well as experimental acute myocardial infarction (AMI). The echocardiograms were recorded in 88 patients with AMI within 7 days from the onset. In 8 patients (9%), and abnormal monophasic triangular diastolic pattern of the mitral valve was observed. Among them, 6 were examined within 12 hours after the onset and the remainder 2 within 24 hours. In all 8, markedly prolonged isovolumic relaxation time (IRT) was present. Mitral valve motion, IRT and peak negative dp/dt of the left ventricle were examined in 5 mongrel dogs following obstruction of the main left coronary artery, one of which was followed up for next 24 hours. Following coronary occlusion, the IRT progressively prolonged from the control value of 20 msec to 130 msec (15 min), 130 msec (30 min), 140 msec (1 hr), 150 msec (6 hr) and 150 msec (24 hr), respectively. The respective values of peak negative dp/dt were 2,000, 800, 840, 840, and 620 mmHg/sec. The possible explanation for these changes was the impaired active relaxation of the left ventricle, resulting in a delayed opening (or at least the lack of usual diastolic opening) of the mitral value, which gave an abortive E-point or a markedly delayed D point. We conclude that the diastolic monophasic triangular pattern of the mitral valve echogram in a reflection of the prolonged left ventricular isovolumic relaxation, and an index of impaired left ventricular diastolic relaxation.

In a patient with Kearns-Sayre syndrome, concentration of coenzyme Q10, a component of the mitochondrial electron transport system, was decreased in serum and in the mitochondrial fraction of skeletal muscle. Serum concentrations of lactate and pyruvate were abnormally high, especially after exercise or oral glucose loading. Levels of folic acid in plasma and CSF were decreased. ECG showed a first-degree atrioventricular block. After administration of coenzyme Q10 60 to 120 mg daily for 3 months, serum levels of lactate and pyruvate became normal, with improvement of atrioventricular block and ocular movements.
