The Frailty screening should be widely performed; however, simple and inexpensive biomarkers are missing. Biomarkers that can be routinely assessed in many patients are desirable. Recently, the hemoglobin-to-red cell distribution width ratio (Hb/RDW, HRR) has been suggested as a new prognostic marker and has been reported to be associated with inflammation, one of the factors contributing to frailty. Therefore, we aimed to address the role of HRR in frailty among 557 older outpatients (aged 65-96 years). Frailty was assessed using the Japanese version of the Cardiovascular Health Study criteria, and HRR was calculated from clinical records. Participants were classified into five groups based on a sex-stratified quintile of HRR (Q1-Q5). Of the participants, 20.3% were frail. Using multiple logistic regression models with the Q5 group as a reference, after adjusting for sex, age, body mass index, polypharmacy, pre-orthopedic surgery, and the use of iron medications, the multivariable-adjusted odds ratios (95% confidence intervals) of the Q4 to Q1 groups were 0.92 (0.58-1.47), 1.04 (0.67-1.61), 1.29 (0.84-1.96), and 1.85 (1.22-2.82), respectively, indicating that a lower HRR was significantly associated with frailty. The robustness of these results was also shown in the multiple imputation analysis. The results suggest that HRR measurement may be one of the indicators to identify frail older adults in routine practice.

TJ-II stellarator results on modelling and validation of plasma flow asymmetries due to on-surface potential variations, plasma fuelling physics, Alfvén eigenmodes (AEs) control and stability, the interplay between turbulence and neoclassical (NC) mechanisms and liquid metals are reported. Regarding the validation of the neoclassically predicted potential asymmetries, its impact on the radial electric field along the flux surface has been successfully validated against Doppler reflectometry measurements. Research on the physics and modelling of plasma core fuelling with pellets and tracer encapsulated solid pellet injection has shown that, although post-injection particle radial redistributions can be understood qualitatively from NC mechanisms, turbulence and fluctuations are strongly affected during the ablation process. Advanced analysis tools based on transfer entropy have shown that radial electric fields do not only affect the radial turbulence correlation length but are also capable of reducing the propagation of turbulence from the edge into the scrape-off layer. Direct experimental observation of long range correlated structures show that zonal flow structures are ubiquitous in the whole plasma cross-section in the TJ-II stellarator. Alfvénic activity control strategies using ECRH and ECCD as well as the relation between zonal structures and AEs are reported. Finally, the behaviour of liquid metals exposed to hot and cold plasmas in a capillary porous system container was investigated.

We present recent highlights from the most recent operation phases of Wendelstein 7-X, the most advanced stellarator in the world. Stable detachment with good particle exhaust, low impurity content, and energy confinement times exceeding 100 ms, have been maintained for tens of seconds. Pellet fueling allows for plasma phases with reduced ion-temperature-gradient turbulence, and during such phases, the overall confinement is so good (energy confinement times often exceeding 200 ms) that the attained density and temperature profiles would not have been possible in less optimized devices, since they would have had neoclassical transport losses exceeding the heating applied in W7-X. This provides proof that the reduction of neoclassical transport through magnetic field optimization is successful. W7-X plasmas generally show good impurity screening and high plasma purity, but there is evidence of longer impurity confinement times during turbulence-suppressed phases. Objectives: To evaluate the effects of β-hydroxy-β-methylbutyrate (HMB) on muscle strength, physical performance, and muscle mass without additional exercise training in older adults with low physical function. Design: Randomized, controlled trial (Open-label study). Setting: Outpatients. Participants: 34 senior outpatients with low physical function who do not exercise regularly. Intervention: 2.4 g of HMB (3.0 g of calcium β-hydroxy-β-methylbutyrate [CaHMB]) per day was given for 60 days, and subjects in the control group were asked to engage in daily activities as normal. Measurements: Weakness or low function was defined by the Asian Working Group for Sarcopenia criteria, then the participants were assigned to the HMB group or the control group. All participants underwent several evaluations such as grip strength, the timed up and go test, the 5-times chair stand test (5CS), and skeletal muscle mass index by the bioimpedance method at baseline and at the end of intervention or control period. Results: An intragroup comparison of pre- to post-treatment values showed significant improvement in grip strength and the 5CS in the HMB group (grip strength: HMB, 16.6±6.1 kg to 18.2±6.4 kg, P=.001; control, 16.5±4.3 kg to 16.7±4.7 kg, P=.729; 5CS: HMB, 11.0 [8.8-13.0] s to 10.1 [8.5-12.6] s, P=.011; control, 11.1 [8.6-13.8] s to 10.0 [8.8-11.3] s, P=.246). Two-way repeated measures analysis of variance (ANOVA) used to compare the HMB and control groups showed a significant improvement in grip strength in the HMB group compared with the control group (P=.029). Conclusion: A supplementation of HMB without additional exercise may improve muscle strength in older patients with low muscle strength.

We assess the magnetic field configuration in modern fusion devices by comparing experiments with the same heating power, between a stellarator and a heliotron. The key role of turbulence is evident in the optimized stellarator, while neoclassical processes largely determine the transport in the heliotron device. Gyrokinetic simulations elucidate the underlying mechanisms promoting stronger ion scale turbulence in the stellarator. Similar plasma performances in these experiments suggests that neoclassical and turbulent transport should both be optimized in next step reactor designs.

A heating source with off-axis electron cyclotron heating (ECH) alone produced a plasma with a quasi-steady-state hollow electron-temperature profile in the Large Helical Device. The clear formation of this quasi-steady-state hollow electron-temperature profile can be explained by adding the outward heat convection term to the diffusion term, as a simple model to describe the electron heat flux, using the energy conservation equation. In addition, we directly observed the non-locality of the non-diffusive (convective) contribution in transient electron thermal transport in the condition that power-modulated on-axis ECH was applied to the plasma sustained by off-axis ECH. The experimentally evaluated flux-gradient relation shows two different positive values of the electron heat flux at zero temperature gradient by going back and forth between positive and negative temperature gradient regions in the transport hysteresis phenomenon.

Recently, the validity range of the approximations commonly used in neoclassical calculation has been reconsidered. One of the primary motivations behind this trend is observation of an impurity hole in LHD (Large Helical Device), i.e. the formation of an extremely hollow density profile of an impurity ion species, such as carbon $\text{C}^{6+}$ , in the plasma core region where a negative radial electric field ( $E_{r}$ ) is expected to exist. Recent studies have shown that the variation of electrostatic potential on the flux surface, $\unicode[STIX]{x1D6F7}_{1}$ , has significant impact on neoclassical impurity transport. Nevertheless, the effect of $\unicode[STIX]{x1D6F7}_{1}$ has been studied with radially local codes and the necessity of global calculation has been suggested. Thus, we have extended a global neoclassical code, FORTEC-3D, to simulate impurity transport in an impurity hole plasma including $\unicode[STIX]{x1D6F7}_{1}$ globally. Independently of the $\unicode[STIX]{x1D6F7}_{1}$ effect, an electron root of the ambipolar condition for the impurity hole plasma has been found by global simulation. Hence, we have considered two different cases, each with a positive (global) and a negative (local) solution of the ambipolar condition, respectively. Our result provides another support that $\unicode[STIX]{x1D6F7}_{1}$ has non-negligible impact on impurity transport. However, for the ion-root case, the radial $\text{C}^{6+}$ flux is driven further inwardly by $\unicode[STIX]{x1D6F7}_{1}$ . For the electron-root case, on the other hand, the radial particle $\text{C}^{6+}$ flux is outwardly enhanced by $\unicode[STIX]{x1D6F7}_{1}$ . These results indicate that how $\unicode[STIX]{x1D6F7}_{1}$ affects the radial particle transport crucially depends on the profile of the ambipolar- $E_{r}$ , which is found to be susceptible to $\unicode[STIX]{x1D6F7}_{1}$ itself and the global effects.

Isotope effects of ECRH plasma in LHD were investigated in detail. A clear difference of transport and turbulence characteristics in H and D plasmas was found in the core region, with normalized radius ρ < 0.8 in high collisionality regime. On the other hand, differences of transport and turbulence were relatively small in low collisionality regime. Power balance analysis and neoclassical calculation showed a reduction of the anomalous contribution to electron and ion transport in D plasma compared with H plasma in the high collisionality regime. In core region, density modulation experiments also showed more reduced particle diffusion in D plasma than in H plasma, in the high collisionality regime. Ion scale turbulence was clearly reduced at ρ < 0.8 in high collisionality regime in D plasma compared with H plasma. The gyrokinetic linear analyses showed that the dominant instability ρ = 0.5 and 0.8 were ion temperature gradient mode (ITG). The linear growth rate of ITG was reduced in D plasma than in H plasma in high collisionality regime. This is due to the lower normalized ITG and density gradient. More hollowed density profile in D plasma is likely to be the key control parameter. Present analyses suggest that anomalous process play a role to make hollower density profiles in D plasma rather than neoclassical process. Electron scale turbulence were also investigated from the measurements and linear gyrokinetic simulations.

A 64-year-old male on regular hemodialysis who was a human T lymphotrophic virus Type I (HTLV-I) carrier developed cryptococcal pleuritis. The initial manifestations of the present case were a persistent cough and the accumulation of unilateral pleural effusion. A culture of the pleural fluid of the patient grew cryptococcus neoformans and a test for antigens against cryptococcus neoformans in the pleural fluid was also positive, therefore, cryptococcal pleuritis was diagnosed. Pleural cryptococcosis per se is rare and it is extremely rare for a dialysis patient to develop pleural cryptococcosis. To our knowledge, only a few cases of cryptococcal pleuritis have so far been reported in patients on dialysis. Furthermore, an isolated occurrence of cryptococcal pleuritis with no cryptococcal pulmonary parenchymal lesions, as was seen in the present case, is rare because cryptococcal pleuritis is usually associated with underlying cryptococcal pulmonary parenchymal lesions. Patients on chronic dialysis are susceptible to developing pleural effusion from many etiologies such as congestive heart failure, infection (tuberculosis, bacterial, viral, parasitic, fungal), collagen vascular disease, drug reaction, metastasis, or uremia itself. Cryptococcal pleuritis developing in a dialysis patient is extremely rare, but physicians should consider cryptococcal infection as a possible cause when pleural effusion develops in a dialysis patient and no other cause is identified, as occurred in the present case.

ProtozoaDB (http://www.biowebdb.org/protozoadb) is being developed to initially host both genomics and post-genomics data from Plasmodium falciparum, Entamoeba histolytica, Trypanosoma brucei, T. cruzi and Leishmania major, but will hopefully host other protozoan species as more genomes are sequenced. It is based on the Genomics Unified Schema and offers a modern Web-based interface for user-friendly data visualization and exploration. This database is not intended to duplicate other similar efforts such as GeneDB, PlasmoDB, TcruziDB or even TDRtargets, but to be complementary by providing further analyses with emphasis on distant similarities (HMM-based) and phylogeny-based annotations including orthology analysis. ProtozoaDB will be progressively linked to the above-mentioned databases, focusing in performing a multi-source dynamic combination of information through advanced interoperable Web tools such as Web services. Also, to provide Web services will allow third-party software to retrieve and use data from ProtozoaDB in automated pipelines (workflows) or other interoperable Web technologies, promoting better information reuse and integration. We also expect ProtozoaDB to catalyze the development of local and regional bioinformatics capabilities (research and training), and therefore promote/enhance scientific advancement in developing countries.

It is required to understand the tritium behavior in concrete for establishment of tritium safety technology of a fusion reactor or a tritium handling facility because the concrete wall is used as the final containment to prevent tritium release to the environment. This paper discusses about the effect of adsorption and diffusion of water and isotope exchange reaction between physically adsorbed water and chemically adsorbed water or structural water. It is known in this study that a large amount of tritium can be trapped to the concrete wall because cement paste has the nature of porous hydrophilic material.

Prognoses in cases of uterine cervical squamous cell carcinoma treated with radiotherapy were investigated in association with immunohistochemical expression of an angiogenic factor, thymidine phosphorylase (TP). Forty-six cases of uterine cervical squamous cell carcinoma mainly treated with radiotherapy during 1992–2001 at our clinic were studied. All were diagnosed as stages IIB to IVA. Paraffin-embedded biopsy specimens excised before radiotherapy were stained immunohistochemically using anti-TP monoclonal antibody. The extent of staining in both tumor and interstitial cells was graded as (−), (±), (+), and (2+). Specimens with TP expression levels of (2+) and (+) were regarded as positively stained and those with TP expression levels of (±) and (−) as negatively stained. The efficacy of radiotherapy in both groups was analyzed by the Kaplan–Meier method. With tumor cells, 5-year survival rates for the positive (n = 38) and negative (n = 8) staining groups were 73.9% and 42.9%, respectively; the rate being significantly higher for the TP-positive group (log rank, P = 0.0096). Contrarily, with staining for interstitial cells, the 5-year survival rates for the positive (n = 20) and negative (n = 26) staining groups were 74.1% and 64.6%, respectively, with no significant difference (log rank, P = 0.406). The efficacy of radiotherapy in the group with positive staining of tumor cells was significantly better than in the negative staining group. Immunohistochemical expression of TP in tumor cells is suggested as a useful prognostic factor for uterine cervical squamous cell carcinomas treated with radiotherapy. Choosing therapy for individual cases by referring to factors including TP expression should contribute to an improved prognosis.

Aims: To compare the antimicrobial resistance, serotypes and flagellin gene types of Campylobacter isolated from humans and food-producing animals and thereby facilitate elucidation of the origin of Campylobacter causing human infection in Japan. Methods and Results: The MIC values of ampicillin, dihydrostreptomycin, gentamicin, erythromycin, oxytetracycline, nalidixic acid and enrofloxacin for Campylobacter isolated from humans (134 isolates), cattle (38 isolates), pigs (69 isolates), layers (84 isolates) and broilers (51 isolates) were compared. The MIC90 values of ampicillin for Campylobacter jejuni isolates from poultry were higher than those from humans and cattle. Campylobacter coli that was resistant to dihydrostreptomycin and erythromycin was observed at a higher frequency in humans and pigs than in poultry. The restriction fragment profiles of flaA of human, bovine and broiler isolates were analysed by clustering, and the isolates were classified into five clusters. Cluster I contained only human and bovine isolates. Clusters III, IV and V contained human, bovine and broiler isolates. Conclusions: Campylobacter isolates from humans included isolates that exhibited characteristics identical to those of the bovine, porcine and poultry isolates. Significance and Impact of the Study: In addition to poultry, cattle and pigs are believed to be sources of campylobacteriosis in Japan.

The Japanese regional population of the Oriental white stork (Ciconia boyciana) became extinct in 1986. Mitochondrial DNA (mtDNA) D-loop region from 20 mounted specimens preserved at public facilities in Toyooka City, Hyogo Prefecture, Japan and its vicinity (n = 17), the area inhabited by the last of the Japanese population, and samples originating from China (n = 3) which were kept at a zoo was analyzed. After extracting DNA from small pieces of skin from mounted specimens, a 1210-bp region of the mtDNA D-loop region was analyzed. The haplotypes among 11 specimens of storks captured or found dead at Toyooka City just before the population became extinct were completely identical. Four haplotypes observed among the mounted specimens preserved in the vicinity of Toyooka City were differentiated from those of captive storks originating from China and Russia in a previous study. Therefore, the last Japanese population might have been a genetically unique group. However, phylogenetic analysis using the maximum likelihood method showed that haplotypes found in the Japanese regional population were closely related to the Chinese and Russian lineages (sequence difference = 2.1%). One mounted specimen collected in 1935 at Izushi village, in the vicinity of Toyooka City, showed the same haplotype as the captive storks from China, suggesting that genetic flow may have historically occurred between the populations of Japan and the continent. Recently, reintroduction for the Oriental white stork has been planned in Toyooka City. The planning for the recovery of extinct populations should not only involve translocation of species to the range from which it disappeared, but also reconstruction of regional populations while considering the genetic lineage between the extinct and introduced populations.

The increasing incidence of sexually transmitted infection (STI) in young people is one of the most important social and health problems in Japan,1 and recent changes in the sexual behaviour may be an important factor.2 Cervical human papillomavirus (HPV) infection is one STI and is the major cause of cervical cancer, which is the cancer with the second highest incidence rate and fifth highest cause of cancer death in women worldwide.3 Japan is an area with the second lowest incidence of cervical cancer worldwide.3 To investigate the prevalence of STIs, we performed a cross sectional study of women attending gynaecology departments in the Hokuriku area of Japan from July 2000 to July 2003. In all, 797 women between 15 and 62 years old were selected from about 15 000 women who had visited and had a cervical cytology test in one of five clinics during this period. After obtaining written informed consent, all the subjects were given to pelvic examinations, a cervical cytology test, and an STI test using the cervical scraped cell samples. DNA for high risk HPV (HPV types 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, and 68), Chlamydia trachomatis , and Neisseria gonorrhoeae was detected using hybrid capture assays (Digene, USA) performed at the Mitsubishi Kagaku Clinical Laboratory in Tokyo (Japan). Rat liver is known to contain a regulatory protein (RP) that inhibits glucokinase competitively with respect to glucose. By an immunohistochemical technique and by cell fractionation in glycerol, we investigated the distribution of RP in rat liver and its changes in response to high glucose. RP was detected almost exclusively in the nuclei of the parenchymal cells of both fed and fasted rats. In situ perfusion of the liver with 20 mM glucose for 10 min caused a marked decrease in nuclear RP and an increase in cytoplasmic RP. These results indicate that RP is present predominantly in the nuclei of hepatocytes and is translocated from the nucleus to the cytoplasm in response to high glucose.

The synergic extraction of trivalent lanthanoids (Ln: La, Nd, Sm, Eu, Yb and Lu) with 2-thenoyltrifluoroacetone (Htta) and 2,2′∶6′, 2″-terpyridine (tpy) into benzene has been studied. The partition coefficient (Ps) of tpy was obtained experimentally in order to calculate the equilibrium concentration of tpy in the organic phase. From the slope analysis, it was shown that these lanthanoids were extracted as Ln(tta)3(tpy). The adduct formation constant (βs,1) and the synergic extraction constant were obtained for each lanthanoid. The (βs,1) decreases with increasing atomic number of lanthanoids and the trend of (βs,1) is compared with that for bidentate and unidentate heterocyclic amines.

Ozonolysis of acenaphthylene in the presence of a nitrone 2 yields the corresponding polycyclic peroxide 5 containing the novel dihydro-1,2,5,7,4-tetroxazocine ring system; the structure of the crystalline adduct 5a has been unambiguously determined by X-ray crystallographic analysis.

Conventional IEEE802.11 wireless LANs employ the binary exponential back-off scheme to determine a back-off window size. Performance evaluations of the binary exponential back-off scheme and several enhanced ones have been done under an assumption that each station can store away only one packet with its transmission buffer. Under such assumption, enhanced schemes provide better performance, however they do not work well under a realistic condition that each station has more larger transmission buffer. In this paper, we propose a back-off scheme in which each station changes the back-off window size according to the load conditions. In order to evaluate performance of the proposed scheme, we build a queueing model and execute computer simulation. Simulation results show that the proposed scheme provides a higher throughput than the conventional ones. Moreover, under the proposed scheme, the number of stations has a slight influence on the performance.

VCCI* Technical Sub-committee has investigated the dispersion of radiated emission measurement results . It was confirmed that one of the main cause of the dispersion of radiated emission measurement results was the difference of the power line-to-ground impedance of the power supply for the equipment under test (EUT). For the improvement of the dispersion, VCCI manufactured the experimental VHF-LISN (very high frequency-line impedance stabilization network). And we confirmed the validity of this VHF-LISN.

In the academic area, the Internet is used as a scientific resource. However, finding appropriate information on the Web remains difficult. To simplify this process, we designed the document retrieval framework to retrieve scientific publications from the Web. In this paper, we present the architecture of the proposed tool and results of preliminary test evaluating its quality. This tool can be used by researchers and students when they search for scientific information on the Internet.

The new concept of tribological material for internal combustion engine components, aiming at reduction of friction loss and control of gas leakage has been proposed. To develop these technologies, the inorganic-organic hybrids with new properties, such as wettability with oil, mechanical strength and thermal conductivity, have been synthesized by sol-gel process. In this paper, it was found that the friction property of inorganic-organic hybrid was significantly improved by adding only 10% organic component with a good wettability with oil.

Determination of the rates of diffusion of beta-lactam antibiotics through purified Pseudomonas aeruginosa porins C, D2, and E in liposomes yielded the following results. (i) The rates of carbapenem (imipenem and meropenem) diffusion through the protein D2 pore were roughly 2 to 70 times higher than those through other porin pores. It is not clear why the protein D2 pore allowed rapid diffusion of carbapenems. The rates of diffusion of glucosamine and triglycine through the protein D2 pore were about 14 and 4 times higher, respectively, than that of an uncharged test solute with a similar Mr, glucose. (ii) The rates of diffusion of antipseudomonal anionic beta-lactams such as piperacillin, ceftazidime, cefsulodin, and aztreonam through the protein C pore were higher than those through other porin pores. This was probably due to the slightly larger pore size and the slight anion selectivity of protein C, since the apparent exclusion limit of the protein C pore for uncharged saccharides is higher than that of other porins and the rate of diffusion of gluconic acid through the protein C pore is about double that for glucose. (iii) The rates of diffusion of cefoperazone through all three species of porin were relatively high. These results indicate that the antipseudomonal beta-lactams permeate the P. aeruginosa outer membrane via newly identified porins.

The limits and the measurement methods for the radiated emissions above 1 GHz have been published as CISPR22 Ed. 5 Amendment 1 and CISPR16-2-3 Ed. 1 Amendment 1, respectively, VCCI Technical Sub-committee has examined the measurement methods for the radiated emissions above 1 GHz. This paper describes the matter of the signal to noise ratio (S/N) for the radiated emission measurement system above 1 GHz, the setting for the video band width (VBW) of a spectrum analyzer and the influence that the rotational speed of a turntable and the sweep time of a spectrum analyzer exert on radiated emission measurement above 1 GHz, based on the measurement results.

The Japan Society for Quality in Health Care (JSQua) created "Standards for the Evaluation of Hospital Infection Control Policies and Procedures, the First Version" in 1998 and carried out third-party surveys. Through the experience of those surveys, we revised the standards and created a second version in 1999. The surveyors felt that in using the second version of the standards it would be easier to evaluate the quality of hospital care and that these standards would be more widely applied.

AML with trilineage myelodysplasia (AML/TMDS) is recognized as having of poor prognosis due to its resistance to chemotherapy in comparison with de novo AML. An AML/TMDS patient who failed to respond to ordinary induction therapy achieved complete remission with combination therapy consisting of all-trans retinoic acid (ATRA) and low-dose Ara-C. No serious toxicity was observed. ATRA combined with low-dose Ara-C could be an alternative treatment for this kind of patient.

A case of adenoma of the ampulla of Vater is presented in a 48-year-old man who was admitted with epigastric pain. Gastroendoscopy revealed a sessile tumor about 3 cm in diameter at the ampulla of Vater, and the pathological diagnosis of a biopsy specimen was adenoma with moderate atypia. Because of the possibility of a malignant component, pylorus-preserving pancreatoduodenectomy was performed to avoid local recurrence. The final pathological diagnosis was adenoma with severe atypia.

The effect of starvation on carbohydrate metabolism in the last instar larvae of the silkworm Bombyx mori was examined. Trehalose concentration in the hemolymph increased slightly during the first 6 h of starvation and decreased thereafter, whereas glucose concentration decreased rapidly immediately after diet deprivation. Starvation-induced hypertrehalosemia was completely inhibited by neck ligation, suggesting that starvation stimulates the release of a hypertrehalosemic factor(s) from the head. The percentage of active glycogen phosphorylase in the fat body increased within 3 h of starvation and its glycogen content decreased gradually. These observations suggest that production of trehalose from glycogen is enhanced in starved larvae. However, hypertrehalosemia during starvation cannot be explained by the increased supply of trehalose into hemolymph alone, as similar changes in phosphorylase activity and glycogen content in the fat body were observed in neck-ligated larvae, in which hemolymph trehalose concentration did not increase but decreased gradually. When injected into larvae, trehalose disappeared from hemolymph at a rate about 40% lower in starved larvae than neck-ligated larvae. The hemolymph lipid concentration increased during starvation, suggesting that an increased supply of lipids to tissues suppresses the consumption of hemolymph trehalose and this is an important factor in hypertrehalosemia.

Clinical electrophysiological studies(EPS) have become useful tools for specific cardiac arrhythmias. However, these studies are expensive and occasionally cause major as well as minor complications such as serious arrhythmias, cardiac tamponade or thromboembolism. It is important to consider guidelines for clinical application of invasive studies. The indications for clinical electrophysiological studies have been categorized into three classes. Class 1: All experts agree that EPS is useful and important for patients treatment and patients with these condition benefit from EPS. Class 2: There is less certainty about usefulness of the information from EPS. Class 3: EPS does not provide useful information. EPS should be performed by trained cardiologist in adequately equipped laboratory. Such studies are performed either by intracardiac pacing and recording or esophageal pacing and recording, and evaluate electrophysiological properties such as automaticity, conduction and refractoriness; initiate and terminate tachycardias; mechanism of tachycardia by activation mapping and pace mapping; evaluate for antiarrhythmic drug, catheter ablation, and intracardiac defibrillator. EPS is useful tool for analysis of bradyarrhythmias and tachyarrhythmias and bring us useful information for the choice of treatment but has its limitation and pitfalls. In general, EPS is not so important to determine the indication of pacemaker for bradyarrhythmias such as sick sinus syndrome or atrioventricular(AV) block. But, it is very useful to analysis tachyarrhythmias and indicate antiarrhythmic drugs or catheter ablation for patients with Wolf-Parkinson-White(WPW) syndrome, AV nodal reentrant tachycardia, atrial tachycardia, atrial flutter, or ventricular tachycardia to cure the tachyarrhythmias.

Thirty-one patients with clear cell ovarian carcinoma who underwent primary surgery and postoperative therapy were retrospectively evaluated. Eighteen patients (58%) had International Federation of Gynecology and Obstetrics (FIGO) stage I disease, 3 patients (9.7%) stage II disease, and 10 patients (32.3%) stage III and IV disease. Patients with stage III and IV disease demonstrated a significantly poor prognosis compared with patients who had stage I or II disease (p0.01). No patients with stage III and IV disease survived 5 years. p53 protein expression and proliferative activity (PA) were studied by immunohistochemical methods using p53 molecule and antibodies to PCNA (proliferative cell nuclear antigen). Intranuclear accumulations of p53 product were observed in 15 of 31 (48.4%). On the other hand, 15 of 31 (48.4%) patients stained positively for PCNA (or = 60% of cancer cells stained positively). Positive p53 staining and highly PA were associated with poor survival. Two patients with stage I a relapsed were positive p53 and highly PA. Accordingly, consolidation chemotherapy is necessary for patients with stage I a who are positive p53 and highly PA. Platinum-based chemotherapy for patients who had minimal residual tumor was effective, but 5 patients who hador = 2 cm tumor burden were not effective at all. The response rate for platinum-based chemotherapy was 20% (1/5) among p53 positive, in contrast to 66.7% (4/6) among p53 negative patients. So it seems that p53 positive patients are chemoresistant.

Telomerase is a ribonucleoprotein that synthesizes telomeric DNA onto chromosomal ends. The expression of telomerase is thought to be required for cellular immortality and oncogenesis. Telomerase activity has been detected not only in most cancers but also in some types of premalignant lesions, such as squamous intraepithelial lesions (SILs). In the present study, we used the telomerase assay to detect uterine cervical lesions in cervical scraping samples. A total of 82 cervical scraping samples were obtained from women with or without cervical lesions and examined by nonradioisotope telomeric repeat amplification protocol assay. Fifteen of 17 (88%) cervical cancer specimens exhibited telomerase activity, whereas 5 of 8 (63%) and 14 of 24 (58%) specimens from low-grade and high-grade SILs, respectively, also exhibited telomerase activity. In contrast, 3 of 33 (9%) specimens from normal cervices exhibited telomerase activity. Dilution telomeric repeat amplification protocol assay was performed to estimate telomerase activity; it revealed that high levels of activity were often expressed in cervical cancer. Cytological examination was also performed by Pap smear test, and 4 of 8 (50%) low-grade SILs, 21 of 24 (88%) high-grade SILs, and 16 of 17 (94%) cervical cancers were found to have cytological abnormalities. There were discordances in some cases between findings of smear abnormality and telomerase positivity. In particular, we found five cases of SILs without smear abnormality but with telomerase activity, suggesting that some lesions with false negative cytology can be detected by telomerase assay. These findings suggest that telomerase assay using cervical scrapings might be a useful screening method for cervical lesions especially when combined with a Pap smear test.

Summary: Growth of genome data and analysis possibilities have brought new levels of difficulty for scientists to understand, integrate and deal with all this ever-increasing information. In this scenario, GARSA has been conceived aiming to facilitate the tasks of integrating, analyzing and presenting genomic information from several bioinformatics tools and genomic databases, in a flexible way. GARSA is a user-friendly web-based system designed to analyze genomic data in the context of a pipeline. EST and GGS data can be analyzed using the system since it accepts (1) chromatograms, (2) download of sequences from GenBank, (3) Fasta files stored locally or (4) a combination of all three. Quality evaluation of chromatograms, vector removing and clusterization are easily performed as part of the pipeline. A number of local and customizable Blast and CDD analyses can be performed as well as Interpro, complemented with phylogeny analyses. GARSA is being used for the analyses of Trypanosoma vivax (GSS and EST), Trypanosoma rangeli (GSS, EST and ORESTES), Bothrops jararaca (EST), Piaractus mesopotamicus (EST) and Lutzomyia longipalpis (EST). A comparative study of energy transport for medium- to high-density discharges in the stellarator-heliotrons TJ-II, W7-AS and LHD is carried out. The specific discharge parameters are chosen to apply a recently concluded benchmarking study of neoclassical (NC) transport coefficients (Beidler et al 2011 Nucl. Fusion 51 076001) to perform this validation study. In contrast to previous experiments at low densities for which electron transport was predominant (Yokoyama et al 2007 Nucl. Fusion 47 1213), the current discharges also exhibit significant ion energy transport. As it affects the energy transport in 3D devices, the ambipolar radial electric field is addressed as well. For the discharges described, ion-root conditions, i.e. a small negative radial electric field were found. The energy transport in the peripheral region cannot be explained by NC theory. Within a 'core region'(r/a < 1/2 ~ 2/3), the predicted NC energy fluxes comply with experimental findings for W7-AS. For TJ-II, compliance in the core region is found for the particle transport and the electron energy transport. For the specific LHD discharges, the core energy transport complied with NC theory except for the electron energy transport in the inward-shifted magnetic configuration. The NC radial electric field tends to agree with experimental results for all devices but is measured to be more negative in the core of both LHD and TJ-II. As a general observation, the energy confinement time approaches the gyro-Bohm-type confinement scaling ISS04 (Yamada et al 2005 Nucl. Fusion 45 1684). Three-dimensional effects on plasma flows have been experimentally studied in the large helical device with 3D configurations. Spontaneous toroidal flow without net driving force using the combination of perpendicular neutral beam injection (NBI) heating and balanced tangential NBI heating has been investigated with two magnetic configurations. Co- and counter-directed spontaneous flows have been observed depending on the collisionality. Toroidal flow shear changes the sign at 0.4
