Purpose of the study. Was to assess the prospects for the use of vacuum aspiration biopsy in terms of breast cancer diagnosis in patients with focal breast changes categorised by 3 and 4a according to the BIRADS scale.Materials and methods. In the period from February 2020 to July 2021, 41 (n = 41) the patient with nodules of 3 and 4a categories according to BIRADS has been passed 41 vacuum aspiration biopsies (VAB) (n = 41) under local infiltration anesthesia with ultrasound navigation. The 17 % (n = 7) of the nodular neoplasms were represented by multiple lesions. The ultrasound examination was mandatory for all the patients and the 17 % (n = 7) was combined with mammography. Invasive diagnostics of tissue material from tumors subject to VAB was previously performed in 5 patients. Trephine biopsy was also performed in 4 cases and the fine needle aspiration biopsy (TAB) was performed in 1 case. Tissue atypia was not detected in all cases.Results. The removed lesions were benign in 85.3 % (n = 35) of the cases. Breast cancer was diagnosed in 4.8 % of cases (n = 2), cancer in situ was diagnosed in 9.7 % (n = 4). The cases of invasive cancer were observed in both BIRADS categories, while in situ cancer was diagnosed in only 4a BIRADS categories. The residual tissue of the formation was detected in 1 case (n = 1). During the procedure, no complications were revealed. Moreover, in the early postoperative period, seroma formation has occurred in 2 patients 4.8 % (n = 2). All the patients noted the satisfactory tolerance to the manipulation.Conclusion. The data obtained suggest that vacuum aspiration biopsy has a high diagnostic accuracy and certain advantages in this category of patients, in particular cases of urgent need to conduct more precise invasive diagnostics without performing the excisional biopsy examination.

Objective: to analyze the 3-year experience of vacuum-assisted aspiration biopsy under ultrasound guidance in patients with focal mammary neoplasms in categories BIRADS 2–5.Materials and methods. The totals of 121 interventions were performed in 121 patients. In 9 (7.4 %) cases there were more than 1 node in the mammary gland.Results. In 97.7 % cases the removed formations were benign. The breast cancer was detected in 5 (2.3 %) cases; there were cases of malignant tumors in each category according to the BIRADS scale. The complications were registered in 15 (12.3 %) cases and did not require surgical treatment. In 77 (63.3 %) cases the results of ultrasound monitoring were obtained in 6 months after the removal of formations, residual tissue was detected in 6 (4.9 %) patients.Conclusions. The vacuum-assisted aspiration biopsy, thus, has a high diagnostic accuracy; the number of completely removed benign formations allows us to consider the method as an alternative to sectoral resection. It is worth saying that a high percentage of complications is associated with the stage of mastering the method; all complications were resolved conservatively.

The article presents an analysis of 34.903 esophagoscopy results performed for population of Leningrad region at the period from 2007 to 2013. A prevalence of erosive esophagitis was 4,9%, peptic strictures - 0,18%, columnar-celled metaplasias - 1,43% and esophageal adenocarcinoma - 0,645%. The data obtained showed a high prevalence of GERD-associated syndromes with injury of mucous coat of esophagus in the local population of Russians.

The incidence of esophageal adenocarcinoma among the population in economically developed countries has been increasing significantly for last three decades. Columnar-lining esophagus (Barrett's esophagus) is one of the proven factors of developing esophagus cancer. The prevalence of Barrett's esophagus and esophageal adenocarcinoma in Russia is not investigated enough. The analysis of the results of 34903 endoscopic examinations of the upper gastrointestinal tract in population of Leningrad region with symptoms of gastric dyspepsia during the period from 2007 to 2013 is presented in this article.
