1. Parametric synthesis of the controller by metaheuristic algorithm in the SimInTech environment

Abstract: Efficiency and quality of operation of local automatic control systems as part of an autonomous object is mainly determined by the regulator in their composition and the used control law, for the synthesis of which is necessary to use modern computer-aided design systems. The article presents the results of the application of genetic algorithm as a method of parametric synthesis of the PID-controller implemented in the SimInTech visual dynamic modeling environment, which currently is used by enterprises in the rocket and space sectors of Russian Federation. Object of research is the dynamic negative feedback system on the example of the automatic angular position of artificial satellite control system. The functional scheme and the simplified mathematical model of the system in the form of the transfer functions of its links are presented. The computer model of the system as well as a process of synthesis of the controller are implemented as a package of projects, based on standard blocks and submodels SimInTech. Projects interact using a common signal base, which provides information exchange between projects, making the system model flexible and versatile. An interactive computing environment for programming language Python, Jupyter Notebook, is used as a third-party software. The organization of interaction between SimInTech and Jupyter Notebook is described, scripts of programs for its implementation are presented.

2. Prospects of applying diamond-like coatings for parts of friction units operating in a corrosion-abrasive medium

Abstract: The results of studying the microstructure and microhardness of ring lugs on discs made of a steel–copper pseudoalloy after laser thermal treatment using a fiber laser with a power of 1 kW are given. It is revealed that the partial melting region in which melting occurs in the volumes of the low-melting component (copper) in the initial structure and contacting segments of steel matrix is formed in the material in addition to the total melting zone. Then the quenching zone from the solid state follows, in which the maximal hardness up to 1000 HV is attained for best samples in the volume of martensite, which is formed in perlite colonies of the initial steel–copper material.

3. Short-Wave Orotrons and Oro-Multipliers

Abstract: The capabilities of two types of terahertz sources based on stimulated Smith – Purcell radiation of electrons in open cavities are discussed. A series of developed pulsed orotrons provides coherent radiation at frequencies up to 410 GHz with an output power of 1–0.1 W with high frequency stability. A promising alternative source with smaller currents than in orotrons is a frequency multiplier based on the excitation of a surface wave of a periodic structure and Smith – Purcell radiation of arising electron bunches inside the orotron cavity. A possibility of frequency doubling inside the orotron cavity is experimentally demonstrated at a frequency of 190 GHz

Abstract: We have developed a series of low-voltage orotrons operated in the short-wave part of the millimeter and long-wave section of the submillimeter wavelength range. The use of an open resonator as the electrodynamic system of the orotron ensures high stability of the radiation frequency and a wide band of frequency tuning. The output orotron power achieved experimentally amounts to hundreds of milliwatts, which is sufficient for many promising spectroscopy methods.

Abstract: The scale along the coordinate axes in the equations of motion hitherto used for an H-maser (in relative units) is shown to depend on Q as the quality factor for an H-maser. This hinders both analysis of the operation of the H-maser and optimization of its parameters. The modified equations of motion proposed here do not have that shortcoming. We determine the atomic flux into the bulb that gives the H-maser the best metrological characteristics.

Abstract: We demonstrate that in the currently used equations of motion for the hydrogen maser (in relative units) the scale along the coordinate axes dependeds on the q factor of the hydrogen maser. This makes it difficult to analyze the H-maser operation and to optimize its parameters. We propose modified equations of motion equations without this shortcoming.

Abstract: Due to high selectivity of open cavity, wider electron beams with higher current can be used in orotron as compared with other Cherenkov devices. It provides important advantages at short millimeter and submillimeter waves. Theory and preliminary experiments in the short part of millimeter and long part of submillimeter wavelength range show that orotron with operating voltage of few kV can generate significantly higher output power than existing BWOs. Combined electronic and mechanical frequency tuning in the band of about of octave has been obtained.

Abstract: A highly sensitive intracavity millimeter-wave spectrometer was developed for the investigation of the absorption spectra of van der Waals complexes in a supersonic jet. The key element of the spectrometer is a tunable oscillator, called OROTRON, which generates the millimeter-wave radiation through the interaction of an electron beam with the electromagnetic field of a high quality (Q≈104) Fabry–Perot resonant cavity. This cavity consists of a movable spherical mirror and a fixed planar mirror with the periodic structure imprinted on its surface. The electron beam moves along the periodic structure of the plane mirror. This part separated from the rest of the resonator by a mica foil is kept under ultrahigh vacuum conditions. The molecular jet is injected by a pulsed valve into the other part of the resonator. The absorption in the jet is sensitively detected by measuring the electric current in a special collector circuit of the OROTRON. Abstract: The global topologies of inclusive three- and four-jet events produced in $\overline{p}p$ interactions are described. The three- and four-jet events are selected from data recorded by the D0 detector at the Fermilab Tevatron Collider operating at a center-of-mass energy of $\sqrt{s}=1800$ GeV. The measured, normalized distributions of various topological variables are compared with parton-level predictions of tree-level QCD calculations. The parton-level QCD calculations are found to be in good agreement with the data. The studies also show that the topological distributions of the different subprocesses involving different numbers of quarks are very similar and reproduce the measured distributions well. The parton-shower Monte Carlo generators provide a less satisfactory description of the topologies of the three- and four-jet events.

Abstract: We have searched for associated production of the lightest chargino W 1 and next-to-lightest neutralino Z 2 of the minimal supersymmetric standard model in pp collisions at √s=1.8 TeV using the D0 detector at the Fermilab Tevatron collider. Data corresponding to an integrated luminosity of 12.5±0.7pb -1 were examined for events containing three isolated leptons. No evidence for W 1 Z 2 pair production was found. Limits on s(W 1 Z 2 )B(W 1 →lνZ 1 )B(Z 2 →llZ 1 ) are presented.

Abstract: We present a search for pair produced top squarks, the supersymmetric partners of the top quark, using the D0 detector at the Fermilab Tevatron pp collider. We consider a scenario in which the lighter of the two top squarks t 1 decays with 100% branching fraction to a charm quark and the lightest neutralino χ 1 0 yielding a signal of two acollinear jets with missing transverse energy. We observe 3 events while we expect 3.5±1.2 events from the known standard model processes. We exclude at the 95% confidence level a significant region of the m t 1 - m χ 1 0 parameter space. The highest m t 1 value we exclude is 93 GeV/c 2 with a corresponding m χ 1 0 value of 8 GeV/c 2 .

Abstract: A study of the particle multiplicity between jets with large rapidity separation has been performed using the D\O\ detector at the Fermilab Tevatron $p\bar{p}$ Collider operating at $\sqrt{s}=1.8$\,TeV. A significant excess of low-multiplicity events is observed above the expectation for color-exchange processes. The measured fractional excess is $1.07 \pm 0.10({\rm stat})~{ + 0.25}_{- 0.13}({\rm syst})\%$, which is consistent with a strongly-interacting color-singlet (colorless) exchange process and cannot be explained by electroweak exchange alone. A lower limit of $0.80\%$ (95\% C.L.) is obtained on the fraction of dijet events with color-singlet exchange, independent of the rapidity gap survival probability.

Abstract: We report on a search for second generation scalar leptoquarks with the D0 detector at the Fermilab Tevatron pp collider at √s = 1.8 TeV. This search is based on 12.7 pb-1 of data. Second generation leptoquarks are assumed to be produced in pairs and to decay into a neutrino and quark with branching ratio β or into a neutrino and quark with branching ratio 1 - β. We obtain cross section times branching ratio limits as a function of leptoquark mass and set lower limits on the leptoquark mass at the 95% confidence level.

Abstract: The ratio of the number of {ital W}+1 jet to {ital W}+0 jet events is measured with the D0 detector using data from the 1992--93 Tevatron Collider run. For the {ital W}{r_arrow}{ital e}{nu} channel with a minimum jet {ital E}{sub {ital T}} cutoff of 25 GeV, the experimental ratio is 0.065{plus_minus}0.003(stat){plus_minus}0.007(syst). Next-to-leading order QCD predictions for various parton distributions agree well with each other and are all over 1 standard deviation below the measurement. Varying the strong coupling constant {alpha}{sub {ital s}} in both the parton distributions and the partonic cross sections simultaneously does not remove this discrepancy. {copyright} {ital 1995} {ital The} {ital American} {ital Physical} {ital Society}.

Abstract: The distribution of the transverse energy in jets has been measured in p p collisions at s =1.8 TeV TeV using the DO detector at Fermilab. This measurement of the jet shape is made as a function of jet transverse energy in both the central and forward rapidity regions. Jets are shown to narrow both with increasing transverse energy and with increasing rapidity. Next-to-leading order partonic QCD calculations are compared to the data. Although the calculations qualitatively describe the data, they are shown to be very dependent on renormalization scale, parton clustering algorithm, and jet direction definition and they fail to describe the data in all regions consistently.

Abstract: A search for a heavy charged gauge boson, W′, using the decay channels W′ → eν and W′ → τν → eνν ν is reported. The data used in the analysis were collected by the DO experiment at the Fermilab Tevatron during the 1992-93 p p collider run from an integrated luminosity of 13.9 ± 0.8 pb−1 at s =1.8 TeV . Assuming that the neutrino from W′ decay is stable and has a mass significantly less than mW′, an upper limit at the 95% confidence level is set on the cross section times branching ratio for p p → W′ → eν . A W′ with the same couplings to quarks and leptons as the standard model W boson is excluded for mW′

Abstract: The inclusive cross sections times leptonic branching ratios for $W$ and $Z$ boson production in $p\overline{p}$ collisions at $\sqrt{s}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}1.8$ TeV were measured using the D0 detector at the Fermilab Tevatron collider: ${\ensuremath{\sigma}}_{W}B(W\ensuremath{\rightarrow}e\ensuremath{\nu})\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}2.36\ifmmode\pm\else\textpm\fi{}0.07\ifmmode\pm\else\textpm\fi{}0.13\mathrm{nb}$, ${\ensuremath{\sigma}}_{W}B\left(W\ensuremath{\rightarrow}\ensuremath{\mu}\ensuremath{\nu}\right)\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}2.09\ifmmode\pm\else\textpm\fi{}0.23\ifmmode\pm\else\textpm\fi{}0.11\mathrm{nb}$, ${\ensuremath{\sigma}}_{Z}B\left(Z\ensuremath{\rightarrow}{e}^{+}{e}^{\ensuremath{-}}\right)\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}0.218\ifmmode\pm\else\textpm\fi{}0.011\ifmmode\pm\else\textpm\fi{}0.012\mathrm{nb}$, and ${\ensuremath{\sigma}}_{Z}B\left(Z\ensuremath{\rightarrow}{\ensuremath{\mu}}^{+}{\ensuremath{\mu}}^{\ensuremath{-}}\right)\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}0.178\ifmmode\pm\else\textpm\fi{}0.030\ifmmode\pm\else\textpm\fi{}0.009\mathrm{nb}$. The first error is the combined statistical and systematic uncertainty, and the second reflects the uncertainty in the luminosity. For the combined electron and muon analyses we find ${\ensuremath{\sigma}}_{W}B(W\ensuremath{\rightarrow}l\ensuremath{\nu})/{\ensuremath{\sigma}}_{Z}B(Z\ensuremath{\rightarrow}{l}^{+}{l}^{\ensuremath{-}})\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}10.90\ifmmode\pm\else\textpm\fi{}0.49$. Assuming standard model couplings, this result is used to determine the width of the $W$ boson, $\ensuremath{\gamma}(W)\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}2.044\ifmmode\pm\else\textpm\fi{}0.093\mathrm{GeV}$.

Abstract: We performed a direct search for the anomalous {ital ZZ}{gamma} and {ital Z}{gamma}{gamma} couplings by studying {ital p{bar p}}{r_arrow}ll{gamma}+{ital X} (l={ital e},{mu}) events at {radical}{ital s}=1.8 TeV with the D0 detector at the Fermilab Tevatron Collider. A fit to the transverse energy spectrum of the photon in the signal events, based on the data set corresponding to an integrated luminosity of 14.3pb{sup {minus}1} (13.7pb{sup {minus}1}) for the electron (muon) channel, yields the following 95% confidence level limits on the anomalous {ital CP}-conserving {ital ZZ}{gamma} couplings: {vert_bar}{ital h}{sub 30}{sup {ital Z}}{vert_bar}{lt}1.8 ({ital h}{sub 40}{sup {ital Z}}=0) and {vert_bar}{ital h}{sub 40}{sup {ital Z}}{vert_bar}{lt}0.5 ({ital h}{sub 30}{sup {ital Z}}=0), for a form-factor scale {Lambda}=500 GeV. Limits for the {ital Z}{gamma}{gamma} couplings and {ital CP}-violating couplings are also discussed.

Abstract: The {ital WW}{gamma} gauge boson couplings were measured using {ital p{bar p}}{r_arrow}l{nu}{gamma}+{ital X} (l={ital e},{mu}) events at {radical}{ital s}=1.8 TeV observed with the D0 detector at the Fermilab Tevatron Collider. The signal, obtained from the data corresponding to an integrated luminosity of 13.8pb{sup {minus}1}, agrees well with the standard model prediction. A fit to the photon transverse energy spectrum yields limits at the 95% confidence level on the {ital CP}-conserving anomalous coupling parameters of {minus}1.6{lt}{Delta}{kappa}{lt}1.8 ({lambda}=0) and {minus}0.6{lt}{lambda}{lt}0.6 ({Delta}{kappa}=0). Similar limits are obtained for the {ital CP}-violating coupling parameters.

Abstract: The results of a search for W boson pair production in p{bar p} collisions at {radical}s = 1.8 TeV with subsequent decay to dilepton (e{mu}, ee, and {mu}{mu}) channels are presented. One event is observed with an expected background of 0.56 {plus_minus} 0.13 events with an integrated luminosity of approximately 14 pb{sup {minus}1}. Assuming equal strengths for the WWZ and WW{gamma} gauge boson coupling parameters {kappa} and {lambda}, limits on the CP-conserving anomalous coupling constants are {minus}2.6 < {delta}{kappa} < 2.8 and {minus}2.1 {lambda} < 2.1 at the 95% confidence level.

Abstract: We present a search for events consistent with the production and decay of the squarks and gluinos of the minimal supersymmetric standard model (MSSM) in the D0 detector at the Fermilab Tevatron {ital p{bar p}} collider. We examined data for events containing large missing transverse energy and three or more jets. We observed no excess of events above the expected yield from standard model processes. For a choice of MSSM parameter values, we set a lower limit at the 95% confidence level on the mass of the gluino of 144 GeV/{ital c}{sup 2} for all squark masses and a lower limit of 212 GeV/{ital c}{sup 2} for equal squark and gluino masses.

Abstract: We report a measurement of the inclusive muon and {ital b}-quark production cross sections in {ital p{bar p}} collisions at {radical}{ital s}=1.8 TeV using the D0 detector at the Fermilab Tevatron collider. The inclusive muon spectrum extends over the kinematic range {vert_bar}{ital y}{sup {mu}}{vert_bar}{lt}0.8 and 3.5{lt}{ital p}{sub {ital T}}{sup {mu}}{lt}60GeV/{ital c}, and is well described by the expected contributions from various known sources. The {ital b}-quark production cross section for {vert_bar}{ital y}{sup {ital b}}{vert_bar}{lt}1.0 and {ital p}{sup {ital b}}{sub {ital T}}{gt}6GeV/{ital c} is extracted, and agrees with next-to-leading order QCD predictions within the experimental and theoretical uncertainties.

Abstract: We present new results on the search for the top quark in p anti-p collisions at S**(1/2) = 1.8 TeV with an integrated luminosity of 13.5 +- 1.6 pb**(-1). We have considered t anti-t production in the Standard Model using electron and muon dilepton decay channels (t anti-t -> e mu + jets, e e + jets, and mu mu + jets) and single-lepton decay channels (t anti-t -> e + jets and mu + jets) with and without tagging of b quark jets. From all channels, we have 9 events with an expected background of 3.8 +- 0.9. If we assume that the excess is due to t anti-t production, and assuming a top mass of 180 GeV/c**2, we obtain a cross section of 8.2 +- 5.1 pb.

Abstract: The design and operation of a gas millimetre-wave spectrometer based on a tunable generator orotron are presented. The absorption cell occupies a part of the high quality (Q approximately=104) orotron Fabry-Perot cavity. The limit of sensitivity gamma min approximately=(3-5)*10-10 cm-1 has been achieved with source frequency modulation at the output band of a receiver of 1 Hz. The narrow spectral width of the orotron radiation (10-15 kHz without locking) provides the Doppler resolution of spectral lines. An absorption signal of gas under investigation is detected by variations of the collector current of the orotron. New results on rotational transition of SiH4 in a vibrational dyad v2/v4 with the absorption coefficient gamma approximately=10-8 cm-1 are briefly given as an illustration of the sensitivity of this spectrometer.

Abstract: The aim of this study was to examine the development and aging of chosen grapevine leaves in situ during the growing season (130 d) using chlorophyll (Chl) a fluorescence measurements and determining the changes in pigment contents. During the course of photosystems development, the increase of Chl and decrease of anthocyanin contents in leaves was observed simultaneously. On 28th day, the maximum content of Chl and minimum content of anthocyanins was measured. However, the maximal photosynthetic performance was found one week later, when the content of Chl started to diminish. Our study proved that the achievement of maximal photosynthetic performance of each leaf took about quarter of organ life and this state lasted very shortly. In this work, we described and discussed for the first time the dynamics of Chl, anthocyanins, and flavonols combined with photosynthetic efficiency changes during the leaf life in situ.

Abstract: Quality is an essential element of conducting successful information systems (IS) initiatives. While quality is a critical issue, statistics indicate that between one and two thirds of IS projects fail and of the most expensive software projects, about half eventually will be canceled for being out of control. This article examines the three phases of quality management and how these relate to IS development. This article illustrates the challenges faced in incorporating quality into an IS development effort and concludes with observations about the general states of IS development quality.

Abstract: We present results on the search for the top quark in {ital p{bar p}} collisions at {radical}{ital s} =1.8 TeV with an integrated luminosity of 13.5{plus_minus}1.6 pb{sup {minus}1}. We have considered {ital t{bar t}} production in the standard model using electron and muon dilepton decay channels ({ital t{bar t}}{r_arrow}{ital e}{mu}+jets, {ital ee}+jets, and {mu}{mu}+jets) and single-lepton decay channels ({ital t{bar t}}{r_arrow}{ital e}+jets and {mu}+jets) with and without tagging of {ital b} quark jets. An analysis of these data optimized for top quark masses below 140 GeV/{ital c}{sup 2} gives a lower top quark mass limit of 128 GeV/{ital c}{sup 2}. An analysis optimized for higher top quark masses yields 9 events with an expected background of 3.8{plus_minus}0.9. If we assume that the excess is due to {ital t{bar t}} production, and assuming a top quark mass of 180 GeV/{ital c}{sup 2}, we obtain a cross section of 8.2{plus_minus}5.1 pb.

Abstract: The DO collaboration reports on a search for the Standard Model top quark in pbar-p collisions at Sqrt(s)=1.8TeV at the Fermilab Tevatron, with an integrated luminosity of approximately 50pb-1. We have searched for t-tbar production in the dilepton and single-lepton decay channels, with and without tagging of b-quark jets. We observed 17 events with an expected background of 3.8+/-0.6 events. The probability for an upward fluctuation of the background to produce the observed signal is 2.0E-6 (equivalent to 4.6 standard deviations). The kinematic properties of the excess events are consistent with top quark decay. Abstract: The design and operation of a gas millimeter-wave spectrometer based on a tunable generator orotron are presented. The absorption cell occupies a part of the high quality (Q approximately equals 10 4 ) orotron Fabry-Perot cavity. The limit of sensitivity (gamma) min approximately equals (3- 5)X 10 -10 cm -1 has been achieved with source frequency modulation at the output band of a receiver of 1 Hz. The narrow spectral width of the orotron radiation (10 - 15 kHz without locking) provides the Doppler resolution of spectral lines.

Abstract: Efficiency and quality of operation of local automatic control systems as part of an autonomous object is mainly determined by the regulator in their composition and the used control law, for the synthesis of which is necessary to use modern computer-aided design systems. The article presents the results of the application of genetic algorithm as a method of parametric synthesis of the PID-controller implemented in the SimInTech visual dynamic modeling environment, which currently is used by enterprises in the rocket and space sectors of Russian Federation. Object of research is the dynamic negative feedback system on the example of the automatic angular position of artificial satellite control system. The functional scheme and the simplified mathematical model of the system in the form of the transfer functions of its links are presented. The computer model of the system as well as a process of synthesis of the controller are implemented as a package of projects, based on standard blocks and submodels SimInTech. Projects interact using a common signal base, which provides information exchange between projects, making the system model flexible and versatile. An interactive computing environment for programming language Python, Jupyter Notebook, is used as a third-party software. The organization of interaction between SimInTech and Jupyter Notebook is described, scripts of programs for its implementation are presented.

Abstract: Background & Aims: Few factors have been identified that can be used to predict response of patients with eosinophilic esophagitis (EoE) to topical steroid treatment. We aimed to determine whether baseline clinical, endoscopic, histologic, and molecular features of EoE can be used to predict histologic response., Methods: We collected data from 97 patients with EoE, from 2009 through 2015, treated with a topical steroid for 8 weeks; 59 patients had a histologic response to treatment. Baseline clinicopathologic features and gene expression patterns were compared between patients with a histologic response to treatment (<15 eos/hpf) and non-responders (≥15 eos/hpf). We performed sensitivity analyses for alternative histologic response definitions. Multivariate logistic regression was performed to identify predictive factors associated with response to therapy, which were assessed with area under the receiver operator characteristic (AUROC) curves., Results: Baseline dilation was the only independent predictor of non-response (odds ratio [OR], 0.30; 95% CI, 0.10-0.89). When an alternate response (<1 eos/hpf) and non-response (<50% decrease in baseline eos/hpf) definition was used, independent predictors of response status were age (OR, 1.08; 95% CI, 1.02-1.14), food allergies (OR, 12.95; 95% CI, 2.20-76.15), baseline dilation (OR, 0.17; 95% CI, 0.03-0.88), edema or decreased vascularity (OR, 0.20; 95% CI, 0.04-1.03), and hiatal hernia (OR, 0.07; 95% CI, 0.01-0.66). Using these 5 factors, we developed a predictive model that discriminated complete responders from non-responders with an AUROC of 0.88. Baseline gene expression patterns were not associated with treatment response and did not change with different histologic response thresholds., Conclusions: In an analysis of 97 patients with EoE, we found dilation to be the only baseline factor associated with non-response to steroid treatment (<15 eos/hpf). However, a model comprising 5 clinical, endoscopic, and histologic factors identified patients with a complete response (<1 eos/hpf). Abstract: Pharmacogenetics influences oral tacrolimus exposure; however, little data exist regarding i.v. tacrolimus. We investigated the impact of genetic polymorphisms in CYP3A4, CYP3A5, and ABCB1 on i.v. tacrolimus exposure and toxicity in adult patients receiving an allogeneic hematopoietic stem cell transplant for hematologic malignancies. Germline DNA was extracted from buccal swabs and genotyped for CYP3A4, CYP3A5, and ABCB1 polymorphisms. Continuous i.v. infusion of tacrolimus .03 mg/kg/day was initiated on day +5 post-transplant, and steady-state blood concentrations were measured 4days later. We evaluated the association between phenotypes and prevalence of nontherapeutic target concentrations (below or above 5 to 15 ng/mL) as well as tacrolimus-related toxicities. Of 63 patients, 28.6% achieved the target concentration; 71.4% were >15ng/mL, which was more common in CYP3A4 intermediate/normal metabolizers (compared with rapid) and those with at least 1 ABCB1 C2677T loss-of-function allele (P < .05). ABCB1 C2677T was significantly associated with concentrations >15ng/mL (odds ratio, 6.2; 95% confidence interval, 1.8 to 23.6; P = .004) and tacrolimus-related toxicities (odds ratio, 7.5; 95% confidence interval, 1.6 to 55.2; P = .02). ABCB1 C2677T and CYP3A4 are important determinants of i.v. tacrolimus exposure, whereas ABCB1 C2677T also impacts tacrolimus-related toxicities in stem cell transplants., (Copyright © 2019 American Society for Blood and Marrow Transplantation. Abstract: Background: Aromatase inhibitors are the mainstay of therapy for patients with hormone receptor-positive breast cancer in both adjuvant and metastatic settings. Their use in clinical practice has been challenged by significant inter-individual variability in response and tolerability. Hence, the purpose of this paper is to provide a succinct review of the literature on the genetic factors contributing to this variability., Design: A systematic search in PUBMED was conducted to identify studies that investigated the association between germline polymorphisms and disposition, clinical response and toxicities of aromatase inhibitors, as well as those evaluating the implications of mutations in ESR1 on clinical response., Results: Polymorphisms in genes coding for phase I and phase II enzymes (pharmacokinetic genes) significantly modulated exposure to aromatase inhibitors; however, there is a paucity of data linking interindividual variability in drug exposure to clinical response. Furthermore, pharmacogenetic studies interrogating relationship between polymorphisms in CYP19A1 (the target site of aromatase inhibitors, i.e. a pharmacodynamic gene) and response yielded conflicting results. Acquired mutations in ESR1 receptors have been identified as the underlying mechanism of resistance to aromatase inhibitors, and likely predict drug response. Although some pharmacogenetic studies have implicated polymorphisms in CYP19A1 and ESR1 with drug-related side effects, the putative role of these genes in predicting toxicity warrants further validation., Conclusion: Genetic polymorphisms in pharmacokinetic and pharmacodynamic genes appear to influence aromatase inhibitor disposition, response and/or toxicity; however, prospective interventional studies are needed to understand the application of genomics to personalize aromatase inhibitor therapy in breast cancer patients., (Copyright © 2018 Elsevier Ltd. Abstract: Background and Aims: No prospective studies substantiate 15 eos/hpf as an appropriate endpoint for treatment of eosinophilic esophagitis (EoE). We aimed to determine a histologic cutpoint that identifies successful treatment of EoE by assessing symptomatic and endoscopic improvement., Methods: We performed a prospective cohort study of 62 consecutive adult patients undergoing outpatient esophagogastroduodenoscopy at the University of North Carolina from 2009 through 2014. At diagnosis of EoE and after 8 weeks of standard treatment, symptom and endoscopic responses were measured using a visual analogue scale and an endoscopic severity score (ESS), and eosinophil counts were assessed. Receiver operator curves and logistic regression models evaluated the histologic threshold that best predicted symptomatic and endoscopic response. For symptoms, analysis was limited to patients without baseline esophageal dilation., Results: The mean eosinophil count at diagnosis was 124 eos/hpf, falling to 35 eos/hpf after treatment. The mean visual analogue scale decreased from 3.4 at baseline to 1.7 after treatment, and the mean ESS decreased from 3 to 1.6. Twenty-nine patients had symptom responses (47%) and 34 had endoscopic responses (55%). Post-treatment eosinophil count thresholds of 8, 15, and 5 eos/hpf best predicted symptom, endoscopic and combined responses, respectively. On logistic regression, decreasing eosinophil count was significantly associated with the probability of symptomatic (P = .01) and endoscopic response (P < .001)., Conclusions: In a prospective study of patients with EoE, we found that a cutpoint of <15 eos/hpf identifies most patients with symptom and endoscopic improvements, providing support for the current diagnostic threshold. A lower threshold (<5 eos/hpf) identifies most patients with a combination of symptom and endoscopic responses; this cutpoint might be used in situations that require a stringent histologic threshold., (Copyright © 2018 AGA Institute. Abstract: There are few data exploring modifiable risk factors for eosinophilic esophagitis (EoE). We aimed to determine if smoking, alcohol consumption, and nonsteroidal anti-inflammatory drug (NSAID) use were risk factors for EoE, and to assess their impact on EoE phenotypes and treatment outcomes. We performed a case-control study analyzing data collected from a prospective cohort of adults undergoing upper endoscopy for symptoms of esophageal dysfunction. Incident EoE cases were diagnosed via consensus guidelines. Exposure data were collected via standardized patient questionnaire. Follow-up assessments for cases were made after treatment, with histologic response defined as <15 eosinophils per high-power field (eos/hpf). Exposures were compared between EoE cases and controls, among EoE cases with and without fibrostenosis, and among EoE responders and nonresponders. A total of 115 cases and 225 controls were analyzed. Cases were less likely to have ever smoked cigarettes (23% vs. 47%, P < 0.001) or currently use NSAIDs (17% vs. 40%, P < 0.001) compared to controls. These relations persisted after multivariate analysis. Although alcohol use was more common among cases (75% vs. 51%, P < 0.001), the effect was abrogated after multivariate analysis. Smoking, alcohol, and NSAID use were not associated with the fibrostenotic phenotype. There was a trend toward improved histologic response among EoE patients concomitantly using NSAIDs (87% vs. 63%, P = 0.08; aOR 6.97 (95% CI: 0.81-60.3). In conclusion, NSAID and smoking were inversely associated with EoE compared to endoscopy-based controls. Alcohol use was more prevalent in the EoE cases, although not an independent risk factor. Abstract: Many studies of eosinophilic esophagitis (EoE) use expert pathology review, but it is unknown whether less experienced pathologists can reliably assess EoE histology. We aimed to determine whether trainee pathologists can accurately quantify esophageal eosinophil counts and identify associated histologic features of EoE, as compared with expert pathologists. We used a set of 40 digitized slides from patients with varying degrees of esophageal eosinophilia. Each of 6 trainee pathologists underwent a teaching session and used our validated protocol to determine eosinophil counts and associated EoE findings. The same slides had previously been evaluated by expert pathologists, and these results comprised the criterion standard. Eosinophil counts were correlated, and agreement was calculated for the diagnostic threshold of 15 eosinophils per high-power field as well as for associated EoE findings. Peak eosinophil counts were highly correlated between the trainees and the criterion standard (ρ ranged from 0.87 to 0.92; P<.001 for all). Peak counts were also highly correlated between trainees (0.75-0.91; P<.001), and results were similar for mean counts. Agreement was excellent for determining if a count exceeded the diagnostic threshold (κ ranged from 0.83 to 0.89; P<.001). Agreement was very good for eosinophil degranulation (κ = 0.54-0.83; P<.01) and spongiosis (κ = 0.44-0.87; P<.01) but was lower for eosinophil microabscesses (κ = 0.37-0.64; P<.01). In conclusion, using a teaching session, digitized slide set, and validated protocol, the agreement between pathology trainees and expert pathologists for determining eosinophil counts was excellent. Abstract: Background: Because eosinophilic esophagitis (EoE) causes dysphagia, esophageal narrowing, and strictures, it could result in low body mass index (BMI), but there are few data assessing this., Aim: To determine whether EoE is associated with decreased BMI., Methods: We conducted a prospective study at the University of North Carolina from 2009 to 2013 enrolling consecutive adults undergoing outpatient EGD. BMI and endoscopic findings were recorded. Incident cases of EoE were diagnosed per consensus guidelines. Controls had either reflux or dysphagia, but not EoE. BMI was compared between cases and controls and by endoscopic features., Results: Of 120 EoE cases and 297 controls analyzed, the median BMI was lower in EoE cases (25 vs. 28 kg/m 2 , p = 0.002). BMI did not differ by stricture presence (26 vs. 26 kg/m 2 , p = 0.05) or by performance of dilation (26 vs. 27 kg/m 2 for undilated; p = 0.16). However, BMI was lower in patients with narrow caliber esophagus (24 vs. 27 kg/m 2 , p < 0.001). EoE patients with narrow caliber esophagus also had decreased BMI compared to controls with narrow caliber esophagi (24 vs. 27 kg/m 2 , p = 0.001). On linear regression after adjustment for age, race, and gender, narrowing decreased BMI by 2.3 kg/m 2 [95% CI -4.1, -0.6]., Conclusions: BMI is lower in EoE cases compared to controls, and esophageal narrowing, but not focal stricture, is associated with a lower BMI in patients with EoE. Weight loss or low BMI in a patient suspected of having EoE should raise concern for esophageal remodeling causing narrow caliber esophagus.
