Abstract Background The availability and use of telehealth to support health care access from a distance has expanded in response to the COVID-19 pandemic. Telehealth services have supported regional and remote health care access for many years and could be augmented to improve health care accessibility, acceptability and overall experiences for both consumers and clinicians. This study aimed to explore health workforce representatives’ needs and expectations to move beyond existing telehealth models and plan for the future of virtual care. Methods To inform recommendations for augmentation, semi-structured focus group discussions were held (November–December 2021). Health workforce representatives with experience in health care delivery via telehealth across country Western Australia were approached and invited to join a discussion. Results Focus group participants included 53 health workforce representatives, with between two and eight participants per discussion. In total, 12 focus groups were conducted: seven were specific to regions, three with staff in centralised roles, and two with a mixture of participants from regional and central roles. Findings identified four key areas for telehealth augmentation: improvements required to existing service practice and processes; equity and access considerations; health workforce-focussed opportunities; and consumer-focussed opportunities. Conclusions Following the onset of the COVID-19 pandemic and the rapid increase in health services delivered via telehealth modalities, it is timely to explore opportunities to augment pre-existing models of care. Workforce representatives consulted in this study suggested modifications to existing process and practice that would improve the current models of care, and recommendations on ways to improve clinician and consumer experiences with telehealth. Improving experiences with virtual delivery of health care is likely to support continued use and acceptance of this modality in health care delivery.

The neodymium (Nd) isotopic composition of seawater is a widely used ocean circulation tracer. However, uncertainty in quantifying the global ocean Nd budget, particularly constraining elusive non-conservative processes, remains a major challenge. A substantial increase in modern seawater Nd measurements from the GEOTRACES programme, coupled with recent hypotheses that a seafloor-wide benthic Nd flux to the ocean may govern global Nd isotope distributions (εNd), presents an opportunity to develop a new scheme specifically designed to test these paradigms. Here, we present the implementation of Nd isotopes (143Nd and 144Nd) into the ocean component of the FAMOUS coupled atmosphere–ocean general circulation model (Nd v1.0), a tool which can be widely used for simulating complex feedbacks between different Earth system processes on decadal to multi-millennial timescales. Using an equilibrium pre-industrial simulation tuned to represent the large-scale Atlantic Ocean circulation, we perform a series of sensitivity tests evaluating the new Nd isotope scheme. We investigate how Nd source and sink and cycling parameters govern global marine εNd distributions and provide an updated compilation of 6048 Nd concentrations and 3278 εNd measurements to assess model performance. Our findings support the notions that reversible scavenging is a key process for enhancing the Atlantic–Pacific basinal εNd gradient and is capable of driving the observed increase in Nd concentration along the global circulation pathway. A benthic flux represents a major source of Nd to the deep ocean. However, model–data disparities in the North Pacific highlight that under a uniform benthic flux, the source of εNd from seafloor sediments is too non-radiogenic in our model to be able to accurately represent seawater measurements. Additionally, model–data mismatch in the northern North Atlantic alludes to the possibility of preferential contributions from “reactive” non-radiogenic detrital sediments. The new Nd isotope scheme forms an excellent tool for exploring global marine Nd cycling and the interplay between climatic and oceanographic conditions under both modern and palaeoceanographic contexts.

In 2021 JET exploited its unique capabilities to operate with T and D–T fuel with an ITER-like Be/W wall (JET-ILW). This second major JET D–T campaign (DTE2), after DTE1 in 1997, represented the culmination of a series of JET enhancements—new fusion diagnostics, new T injection capabilities, refurbishment of the T plant, increased auxiliary heating, in-vessel calibration of 14 MeV neutron yield monitors—as well as significant advances in plasma theory and modelling in the fusion community. DTE2 was complemented by a sequence of isotope physics campaigns encompassing operation in pure tritium at high T-NBI power. Carefully conducted for safe operation with tritium, the new T and D–T experiments used 1 kg of T (vs 100 g in DTE1), yielding the most fusion reactor relevant D–T plasmas to date and expanding our understanding of isotopes and D–T mixture physics. Furthermore, since the JET T and DTE2 campaigns occurred almost 25 years after the last major D–T tokamak experiment, it was also a strategic goal of the European fusion programme to refresh operational experience of a nuclear tokamak to prepare staff for ITER operation. The key physics results of the JET T and DTE2 experiments, carried out within the EUROfusion JET1 work package, are reported in this paper. Progress in the technological exploitation of JET D–T operations, development and validation of nuclear codes, neutronic tools and techniques for ITER operations carried out by EUROfusion (started within the Horizon 2020 Framework Programme and continuing under the Horizon Europe FP) are reported in (Litaudon et al Nucl. Fusion accepted), while JET experience on T and D–T operations is presented in (King et al Nucl. Fusion submitted).

JET, the world’s largest operating tokamak with unique Be/W wall and tritium handling capability, completed a Deuterium-Tritium (D-T) campaign in 2021 (Maggi et al 29th Fusion Energy Conf. ) following a decade of preparatory experiments, dedicated enhancements, technical rehearsals and training (Horton et al 2016 Fusion Eng. Des. 109–111 925). Operation with tritium raises significant technical, safety and scientific challenges not encountered in standard protium or deuterium operation. This contribution describes the tritium operational requirements, pulses and technical preparations, new operating procedures, lessons learned and details on the achieved operational availability and performance. The preparation and execution of the recent JET tritium experiments benefitted from the previous experience in 1991 (Preliminary Tritium Experiment), 1997 (DTE1 campaign) and 2003 (Trace Tritium Campaigns) and consisted of the following five phases: technical rehearsals and scenario preparation, tritium commissioning, 100% tritium campaign, D-T campaign (DTE2), tritium clean-up. Following the clean-up JET resumed normal operation and is currently undertaking a further D-T campaign (DTE3).

Objective To date, no biomarkers have been established to predict haematological complications and outcomes of extracorporeal membrane oxygenation (ECMO). The aim of this study was to investigate the expression of a panel of microRNAs (miRNAs), which are promising biomarkers in many clinical fields, in patients before and after initiating ECMO. Methods Serum miRNA levels from 14 patients hospitalized for acute respiratory failure and supported with ECMO in our medical intensive care unit were analysed before and 24 hours after ECMO. In total, 179 serum-enriched miRNAs were profiled by using a real-time PCR panel. For validation, differentially expressed miRNAs were individually quantified with conventional real-time quantitative PCR at 0, 24, and 72 hours. Results Under ECMO support, platelet count significantly decreased by 65 × 10 3 /µL (25th percentile = 154.3 × 10 3 /µL; 75th percentile = 33 × 10 3 /µL). Expression of the 179 miRNAs investigated in this study did not change significantly throughout the observational period. Conclusions According to our data, the expression of serum miRNAs was not altered by ECMO therapy itself. We conclude that ECMO does not limit the application of miRNAs as specific clinical biomarkers for the patients’ underlying disease.

Rain-fed peatlands are dominated by peat mosses (Sphagnum sp.), which for their growth depend on nutrients, water and CO2 uptake from the atmosphere. As the isotopic composition of carbon (12,13C) and oxygen (16,18O) of these Sphagnum mosses are affected by environmental conditions, Sphagnum tissue accumulated in peat constitutes a potential long-term archive that can be used for climate reconstruction. However, there is inadequate understanding of how isotope values are influenced by environmental conditions, which restricts their current use as environmental and palaeoenvironmental indicators. Here we tested (i) to what extent C and O isotopic variation in living tissue of Sphagnum is species-specific and associated with local hydrological gradients, climatic gradients (evapotranspiration, temperature, precipitation) and elevation; (ii) whether the C isotopic signature can be a proxy for net primary productivity (NPP) of Sphagnum; and (iii) to what extent Sphagnum tissue δ18O tracks the δ18O isotope signature of precipitation. In total, we analysed 337 samples from 93 sites across North America and Eurasia using two important peat-forming Sphagnum species (S. magellanicum, S. fuscum) common to the Holarctic realm. There were differences in δ13C values between species. For S. magellanicum δ13C decreased with increasing height above the water table (HWT, R2 = 17 %) and was positively correlated to productivity (R2 = 7 %). Together these two variables explained 46 % of the between-site variation in δ13C values. For S. fuscum, productivity was the only significant predictor of δ13C but had low explanatory power (total R2 = 6 %). For δ18O values, approximately 90 % of the variation was found between sites. Globally modelled annual δ18O values in precipitation explained 69 % of the between-site variation in tissue δ18O. S. magellanicum showed lower δ18O enrichment than S. fuscum (−0.83 ‰ lower). Elevation and climatic variables were weak predictors of tissue δ18O values after controlling for δ18O values of the precipitation. To summarize, our study provides evidence for (a) good predictability of tissue δ18O values from modelled annual δ18O values in precipitation, and (b) the possibility of relating tissue δ13C values to HWT and NPP, but this appears to be species-dependent. These results suggest that isotope composition can be used on a large scale for climatic reconstructions but that such models should be species-specific.

We apply tipping point analysis to a large record of ocean acoustic data to identify the main components of the acoustic dynamical system and study possible bifurcations and transitions of the system. The analysis is based on a statistical physics framework with stochastic modelling, where we represent the observed data as a composition of deterministic and stochastic components estimated from the data using time-series techniques. We analyse long-term and seasonal trends, system states and acoustic fluctuations to reconstruct a one-dimensional stochastic equation to approximate the acoustic dynamical system. We apply potential analysis to acoustic fluctuations and detect several changes in the system states in the past 14 years. These are most likely caused by climatic phenomena. We analyse trends in sound pressure level within different frequency bands and hypothesize a possible anthropogenic impact on the acoustic environment. The tipping point analysis framework provides insight into the structure of the acoustic data and helps identify its dynamic phenomena, correctly reproducing the probability distribution and scaling properties (power-law correlations) of the time series.

Nitrogen deposition was experimentally increased on a Scottish peatbog over a period of 13 years (2002–2015). Nitrogen was applied in three forms, NH3 gas, NH4Cl solution, and NaNO3 solution, at rates ranging from 8 (ambient) to 64 kg N ha−1 yr−1, and higher near the NH3 fumigation source. An automated system was used to apply the nitrogen, such that the deposition was realistic in terms of rates and high frequency of deposition events. We measured the response of nitrous oxide (N2O) flux to the increased nitrogen input. Prior expectations, based on the IPCC default emission factor, were that 1 % of the added nitrogen would be emitted as N2O. In the plots treated with NH4+ and NO3− solution, no response was seen, and there was a tendency for N2O fluxes to be reduced by additional nitrogen, though this was not significant. Areas subjected to high NH3 emitted more N2O than expected, up to 8.5 % of the added nitrogen. Differences in the response are related to the impact of the nitrogen treatments on the vegetation. In the NH4+ and NO3− treatments, all the additional nitrogen is effectively immobilised in the vegetation and top 10 cm of peat. In the NH3 treatment, much of the vegetation was killed off by high doses of NH3, and the nitrogen was presumably more available to denitrifying bacteria. The design of the wet and dry experimental treatments meant that they differed in statistical power, and we are less likely to detect an effect of the NH4+ and NO3− treatments, though they avoid issues of pseudo-replication.

Abstract In the present paper, we study the transmission of the two-dimensional photonic crystal (PC) superellipse ring resonator. The fast growing applications of optomechanical systems lead to strong demands in new sensing mechanism in order to design the sensing elements to nanometer scale. The photonic crystal based resonator has been investigated as promising solutions because the band gap structure and resonator characteristics are extremely sensitive to the deformation and position shift of rod / cavity in PC resonators. This structure opens a single channel filter. The study is extended for tuning of channel filter’s wavelength with a temperature of this structure. The transmission of the channel filter shows a red shift with temperature linearly. This wavelength shift of the channel filter is used for the sensor application. The sensitivity for the proposed structure is found to be 65.3 pm/°C. The outstanding sensing capability renders PC resonators as a promising optomechanical sensing element to be integrated into various transducers for temperature sensing applications.

Free Space Optical (FSO) communication is being realized as an effective solution for future accessing networks, offering light passed through air. The performance of FSO can be primarily degraded by various atmospheric attenuation namely, rain, fog, haze and snow. At present, hybridization of Dense Wavelength Division Multiplexing (DWDM) with Coarse Wavelength Division Multiplexing (CWDM) becomes necessary to scale the speed and high bandwidth of the services. In this paper, hybrid WDM system is proposed, designed and the network parameters such as Bit Error Rate (BER), Quality Factor and receiver sensitivity are analyzed with respect to link distance for various weather conditions. For investigation, 4 CWDM and 8 DWDM channels are considered whose corresponding channel spacing is 20nm and 0.8nm, respectively. From the simulation, it is investigated that the average link distance of proposed hybrid WDM-FSO system for DWDM and CWDM system at very clear condition are around 810km and 780km. The proposed hybrid WDM based FSO system is designed to handle the quality of transmission for 12 users, each at a data rate of 2.5Gbps.

A subset S of vertices in a graph G is called a geodetic set if every vertex not in S lies on a shortest path between two vertices from S. A subset D of vertices in G is called dominating set if every vertex not in D has at least one neighbor in D. A geodetic dominating set S is both a geodetic and a dominating set. The geodetic (domination, geodetic domination) number g(G)(γ(G),γ_g(G)) of G is the minimum cardinality among all geodetic (dominating, geodetic dominating) sets in G. In this paper, we show that if a triangle free graph G has minimum degree at least 2 and g(G) = 2, then γ_g(G) = γ(G). It is shown, for every nontrivial connected graph G with γ(G) = 2 and diam(G) > 3, that γ_g(G) > g(G). The lower bound for the geodetic domination number of Cartesian product graphs is proved. Geodetic domination number of product of cycles (paths) are determined. In this work, we also determine some bounds and exact values of the geodetic domination number of strong product of graphs.

Ecosystem functions such as biological pest control are mediated by the richness and abundance of service providers i.e., biological control agents (BCAs), relative contributions of individual taxa and community structure. This is especially relevant in the native range of agricultural herbivores, where a speciose community of co-evolved BCAs can prevent them from attaining pest status. Here, we use a powerful graphical approach to assess the functional structure of BCA communities of the fall armyworm (FAW) Spodoptera frugiperda (Lepidoptera: Noctuidae) on maize in the Neotropics. Drawing upon a curated database of all-time field and laboratory studies, we graphed patterns in the functional contribution, abundance and niche breadth for a respective 69, 53 and 3 taxa of resident parasitoids, predators and pathogens. Regardless of varying taxon coverage and rigor of the underlying studies, functional structure follows a saturating relationship in which the first three taxa account for 90–98% of aggregate biological control function. Abundance-functionality matrices prove critically incomplete, as more than 80% of invertebrate taxa miss empirically derived efficiency metrics while associated FAW infestation data are scarce. Despite its methodological shortfalls and data gaps, our work pinpoints Chelonus insularis, several taxa of egg parasitoids, Doru spp. and Orius spp. as taxa with outsized (average) functionality and conservation potential. This is also exemplified by the highly variable aggregate function across studies, with dispersion indices of 1.52 and 2.14 for invertebrate BCAs. Our work underlines the critical importance of functional ecology research, networked trials and standardized methodologies in advancing conservation biological control globally.

This study is the first to use an Unmanned Aerial Vehicle (UAV) for mapping moss beds in Antarctica. Mosses can be used as indicators for the regional effects of climate change. Mapping and monitoring their extent and health is therefore important. UAV aerial photography provides ultra-high resolution spatial data for this purpose. We developed a technique to extract an extremely dense 3D point cloud from overlapping UAV aerial photography based on structure from motion (SfM) algorithms. The combination of SfM and patch-based multi-view stereo image vision algorithms resulted in a 2 cm resolution digital terrain model (DTM). This detailed topographic information combined with vegetation indices derived from a 6-band multispectral sensor enabled the assessment of moss bed health. This novel UAV system has allowed us to map different environmental characteristics of the moss beds at ultra-high resolution providing us with a better understanding of these fragile Antarctic ecosystems. The paper provides details on the different UAV instruments and the image processing framework resulting in DEMs, vegetation indices, and terrain derivatives.

Proteomics, or the study of the set of proteins in an organism, tissue, or cell, is a rapidly growing field within bioanalytical chemistry that can be used to investigate many biological questions. However, proteomics is not fully integrated into the undergraduate chemistry curriculum because of a lack of access to expertise, the cost of required analytical instrumentation (liquid chromatography [LC] and mass spectrometry [MS]), and the time required for proteomics experiments. A virtual proteomics module was developed for flexible learning environments (i.e., in-person, hybrid, online) to introduce undergraduate Analytical Chemistry students to proteomics without the need for expensive instrumentation. This module teaches students about proteomics sample preparation and data acquisition, provides real LC-MS/MS data from unknown plasma samples for students to analyze, and provides experience with Mascot, a free online data analysis platform that can be used to identify the species of origin. The module was implemented in Introduction to Analytical Chemistry in two ways: (1) as a laboratory (online; ~120 min) and (2) as a lecture (hybrid; ~75 min). Students reported improved knowledge of proteomics after exposure to this module with average quiz scores increasing from 27% to 73%. In the lecture implementation, >90% of students successfully identified their unknowns, with almost 80% correctly justifying their conclusions. Additionally, students demonstrated increased interest in proteomics by formulating more complex research questions on the post-quiz (average of 40 words) than the pre-quiz (average of 18 words). This module was effective for students across majors and years and is a valuable educational tool that incorporates proteomics into the undergraduate chemistry curriculum across learning environments.

The purpose of this article is to explore suburban Black Christian girls' experiences of microaggressions in the public high school context. Through an ethnographic study, personal narratives were collected from six suburban Black Christian girls who all attended the same African Methodist Episcopal (AME) church where I served as the youth minister. These narratives offer insight into ways microaggression encounters in suburban schools can impact the personal identity, racial identity, and spiritual formation of Black adolescent girls. The Black sanctuary is the context where the girls in the study felt most accepted, represented, and safe. This implies re-imagining religious education practices with Black girls is important if pastors want to cultivate formative spaces that help Black youth thrive in an Oreo World.

Introduction. The aim of this study was to examine the effect of surgeon relocation on lymph node (LN) retrieval in colorectal cancer (CRC) resection. Methods. The study population was 213 consecutive patients undergoing CRC resection by a single surgeon, at two units: unit one 110 operations (2002–2005) and unit two 103 (2005–2009). Abstract Three-dimensional, organ-on-chip models that recapitulate kidney tissue are needed for drug screening and disease modeling. Here, we report a method for creating a perfusable 3D proximal tubule model composed of epithelial cells isolated from kidney organoids matured under static conditions. These organoid-derived proximal tubule epithelial cells (OPTECs) are seeded in cylindrical channels fully embedded within an extracellular matrix, where they form a confluent monolayer. A second perfusable channel is placed adjacent to each proximal tubule within these reusable multiplexed chips to mimic basolateral drug transport and uptake. Our 3D OPTEC-on-chip model exhibits significant upregulation of organic cation (OCT2) and organic anion (OAT1/3) transporters, which leads to improved drug uptake, compared to control chips based on immortalized proximal tubule epithelial cells. Hence, OPTEC tubules exhibit a higher normalized lactate dehydrogenase (LDH) release, when exposed to known nephrotoxins, cisplatin and aristolochic acid, which are diminished upon adding OCT2 and OAT1/3 transport inhibitors. Our integrated multifluidic platform paves the way for personalized kidney-on-chip models for drug screening and disease modeling.

Inherited mutations in MUTYH have been shown to predispose patients to colorectal cancers. Here, the authors show that MUTYH mutations lead to an increased somatic base substitution mutation rate in normal intestinal epithelial cells, which is the likely cause for the increased cancer risk.

It is unclear whether somatic mutation rates are elevated in Lynch Syndrome (LS), which is the most common cause of hereditary colorectal cancer. Here, the authors use whole-genome sequencing and organoid cultures to show that normal tissues in LS patients are genomically stable, while ancestor cells of neoplastic tissues undergo multiple cycles of clonal evolution.

Abstract Objectives We compared the proteomic signatures of the hippocampal lesion induced in three different animal models of mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE+HS): the systemic pilocarpine model (PILO), the intracerebroventricular kainic acid model (KA), and the perforant pathway stimulation model (PPS). Methods We used shotgun proteomics to analyze the proteomes and find enriched biological pathways of the dorsal and ventral dentate gyrus (DG) isolated from the hippocampi of the three animal models. We also compared the proteomes obtained in the animal models to that from the DG of patients with pharmacoresistant MTLE+HS. Results We found that each animal model presents specific profiles of proteomic changes. The PILO model showed responses predominantly related to neuronal excitatory imbalance. The KA model revealed alterations mainly in synaptic activity. The PPS model displayed abnormalities in metabolism and oxidative stress. We also identified common biological pathways enriched in all three models, such as inflammation and immune response, which were also observed in tissue from patients. However, none of the models could recapitulate the profile of molecular changes observed in tissue from patients. Significance Our results indicate that each model has its own set of biological responses leading to epilepsy. Thus, it seems that only using a combination of the three models may one replicate more closely the mechanisms underlying MTLE+HS as seen in patients.

Two Dimensional (2D) circular Photonic Crystal Ring Resonator (PCRR) based Add Drop Filter (ADF) is designed and the impacts of rod shape with its filling fraction is examined to evaluate the performance of the filter. The ADFs are devised separately using rods of circular, hexagonal and square shape in square lattice PC. For various values of rod’s cross sectional dimension and shape, the resonant wavelength, coupling efficiency, dropping efficiency, Q factor and passband width of the designed filters are investigated through simulation. The calculated filling fraction of a square, circular and hexagonal rods in the lattice structure is 11.9006 × 10-12 sq.m, 13.1134 × 10-12 sq.m and 15.16 × 10-12 sq.m and their respective resonant wavelength is 1494 nm, 1504 nm and 1520 nm. It is observed that there is 1 nm wavelength shift to longer wavelength while increasing the overall filling fraction by 0.1233 × 10-12 sq.m. From the simulated results, it is observed that the circular and hexagonal rods based ADF impart better performance than square rod based ADF.

Rationale: Deep brain stimulation (DBS) of the hippocampus is proposed for enhancement of memory impaired by injury or disease. Many pre-clinical DBS paradigms can be addressed in epilepsy patients undergoing intracranial monitoring for seizure localization, since they already have electrodes implanted in brain areas of interest. Even though epilepsy is usually not a memory disorder targeted by DBS, the studies can nevertheless model other memory-impacting disorders, such as Traumatic Brain Injury (TBI). Methods: Human patients undergoing Phase II invasive monitoring for intractable epilepsy were implanted with depth electrodes capable of recording neurophysiological signals. Subjects performed a delayed-match-to-sample (DMS) memory task while hippocampal ensembles from CA1 and CA3 cell layers were recorded to estimate a multi-input, multi-output (MIMO) model of CA3-to-CA1 neural encoding and a memory decoding model (MDM) to decode memory information from CA3 and CA1 neuronal signals. After model estimation, subjects again performed the DMS task while either MIMO-based or MDM-based patterned stimulation was delivered to CA1 electrode sites during the encoding phase of the DMS trials. Each subject was sorted (post hoc) by prior experience of repeated and/or mild-to-moderate brain injury (RMBI), TBI, or no history (control) and scored for percentage successful delayed recognition (DR) recall on stimulated vs. non-stimulated DMS trials. The subject’s medical history was unknown to the experimenters until after individual subject memory retention results were scored. Results: When examined compared to control subjects, both TBI and RMBI subjects showed increased memory retention in response to both MIMO and MDM-based hippocampal stimulation. Furthermore, effects of stimulation were also greater in subjects who were evaluated as having pre-existing mild-to-moderate memory impairment. Conclusion: These results show that hippocampal stimulation for memory facilitation was more beneficial for subjects who had previously suffered a brain injury (other than epilepsy), compared to control (epilepsy) subjects who had not suffered a brain injury. This study demonstrates that the epilepsy/intracranial recording model can be extended to test the ability of DBS to restore memory function in subjects who previously suffered a brain injury other than epilepsy, and support further investigation into the beneficial effect of DBS in TBI patients.

This article offers a thorough review of shape memory alloys' (SMAs') uses in the Space research field. The utility of SMAs in a variety of applications, including morphing wings (using both experimental and modelling methods), customising orientation and inlet shapes for various propulsion systems, implementing flexible chevrons to improve thrust while lowering noise, and reducing overall power consumption, is the main topic of this paper. The use of SMAs in applications in space is also covered in the paper, including how they may be used to create low-shock launchers, isolate microvibrations, and enable self-deployable solar sails. The essay also emphasises the novel structures and tools made possible by SMAs. One noteworthy method covered in the article is putting SMA wires in the laminate's midplane and embedding them into the fabric a layer of composite laminates. When compared to traditional composite constructions, the incorporation of SMAs into composite has shown better damage resistance and ductility. The reaction of a bright hybrid plastic composite plates to a very low-velocity impact is examined experimentally and numerically in this paper, which highlights the benefits of inserting SMA wires. Among these benefits are improved damage resistance, better ductility, higher composite hardness, and increased energy absorption before failure. Background:. Gender-affirming mastectomy has become one of the most frequently performed procedures for transgender and nonbinary patients. Although there are a variety of potential surgical approaches available, the impact of technique on outcomes remains unclear. Here we present our experience performing periareolar and double incision mastectomies, with a focus on comparing patient demographics, preoperative risk factors, and surgical outcomes and complication rates between techniques. Methods:. Retrospective review identified patients undergoing gender-affirming mastectomy by the senior author between 2017 and 2020. Patients were stratified according to surgical technique, with demographics and postoperative outcomes compared between groups. Results:. In total, 490 patients underwent gender-affirming mastectomy during the study period. An estimated 96 patients underwent periareolar mastectomy, whereas 390 underwent double incision mastectomy. Demographics were similar between groups, and there were no differences in rates of hematoma (3.1% versus 5.6%, respectively; P = 0.90), seroma (33.3% versus 36.4%; P = 0.52), or revision procedures (14.6% versus 15.8% P = 0.84) based on technique. Conclusions:. Our results demonstrate no difference in the rates of postoperative complications or revision procedures based on surgical technique. These results also suggest that with an experienced surgeon and proper patient selection, both techniques of gender-affirming mastectomy can be performed safely and with comparable outcomes.

Abstract Background Neck and low back pain represent dynamic conditions that change over time, often with an initial improvement after the onset of a new episode, followed by flare-ups or variations in intensity. Pain trajectories were previously defined based on longitudinal studies of temporal patterns and pain intensity of individuals with low back pain. In this study, we aimed to 1) investigate if the defined patterns and subgroups for low back pain were applicable to neck pain patients in chiropractic practice, 2) explore the robustness of the defined patterns, and 3) investigate if patients within the various patterns differ concerning characteristics and clinical findings. Methods Prospective cohort study including 1208 neck pain patients from chiropractic practice. Patients responded to weekly SMS-questions about pain intensity and frequency over 43 weeks. We categorized individual responses into four main patterns based on number of days with pain and variations in pain intensity, and subdivided each into four subgroups based on pain intensity, resulting in 16 trajectory subgroups. We compared baseline characteristics and clinical findings between patterns and between Persistent fluctuating and Episodic subgroups. Results All but two patients could be classified into one of the 16 subgroups, with 94% in the Persistent fluctuating or Episodic patterns. In the largest subgroup, “Mild Persistent fluctuating” (25%), mean (SD) pain intensity was 3.4 (0.6) and mean days with pain 130. Patients grouped as “Moderate Episodic” (24%) reported a mean pain intensity of 2.7 (0.6) and 39 days with pain. Eight of the 16 subgroups each contained less than 1% of the cohort. Patients in the Persistent fluctuating pattern scored higher than the other patterns in terms of reduced function and psychosocial factors. Conclusions The same subgroups seem to fit neck and low back pain patients, with pain that typically persists and varies in intensity or is episodic. Patients in a Persistent fluctuating pattern are more bothered by their pain than those in other patterns. The low back pain definitions can be used on patients with neck pain, but with the majority of patients classified into 8 subgroups, there seems to be a redundancy in the original model.

This present investigation is to enhance the hardness and tensile behavior of secondary particles reinforced with Al 7075. The secondary particles considered for this study are Si3N4 (2 wt%, 5 wt% and 8 wt%), HfC (0.5 wt%, 1.25 wt% and 2 wt%) and MoS2 (2 wt%, 3.5 wt% and 5 wt%). The hybrid metal matrix composites are fabricated using powder metallurgy technique. The fabricated samples were analyzed using scanning electron microscope and the presences of elements are confirmed by using energy dispersive spectroscopy. The two main mechanical properties are considered in this study such as tensile strength and hardness. Response surface methodology optimization technique is adopted to find the effective powder metallurgy process parameters and secondary particle optimal values to enhance the mechanical property. Analysis of variances results reveals that compacting pressure were the maximum influencing parameter for both the hardness and tensile strength. [ABSTRACT FROM AUTHOR]

Ecosystem services represent an important concept for assessing the sustainability of agricultural management. However, in practical applications, it can be difficult to find indicators suitable for specific services or specific spatial scales. In order to create a toolbox of indicators for assessing the actual or potential supply of ecosystem services in the context of agricultural land and soil management, we conducted a keyword-based literature review in Web of Science Core Collection and SCOPUS, using the terms ecosystem service AND indicator AND agricultur*. The search was performed in January 2019 and was restricted to journal articles written in English. After eliminating duplicates, we identified 180 articles, out of which 121 met our selection criteria. We extracted information on addressed ecosystem services and indicators which used a full-text review. Where studies used ecosystem service definitions other than the Common International Classification of Ecosystem Services (CICES V.5.1), indicators were assigned to the corresponding CICES class or classes. We used the information derived from the review to create factsheets for 37 ecosystem services. Each factsheet provides tables with available indicators applicable at multiple spatial scales that range from field to global, information on the type of input data required, and a reference to the article or articles that the indicator was taken from. The dataset provides a toolbox for researchers to find indicators that fit their respective research needs.

During the current pandemic, the vast majority of COVID-19 patients experienced mild symptoms, but some had a potentially fatal aberrant hyperinflammatory immune reaction characterized by high levels of IL-6 and other cytokines. Modulation of this immune reaction has proven to be the only method of reducing mortality in severe and critical COVID-19. The anti-inflammatory drug baricitinib (Olumiant) has recently been strongly recommended by the WHO for use in COVID-19 patients because it reduces the risk of progressive disease and death. It is a Janus Kinase (JAK) 1/2 inhibitor approved for rheumatoid arthritis which was suggested in early 2020 as a treatment for COVID-19. In this review the AI-assisted identification of baricitinib, its antiviral and anti-inflammatory properties, and efficacy in clinical trials are discussed and compared with those of other immune modulators including glucocorticoids, IL-6 and IL-1 receptor blockers and other JAK inhibitors. Baricitinib inhibits both virus infection and cytokine signalling and is not only important for COVID-19 management but is “non-immunological”, and so should remain effective if new SARS-CoV-2 variants escape immune control. The repurposing of baricitinib is an example of how advanced artificial intelligence (AI) can quickly identify new drug candidates that have clinical benefit in previously unsuspected therapeutic areas.
