Abstract

Abstract We report a case of Mismatch Repair Deficiency (MMRD) caused by germline homozygous EPCAM deletion leading to tissue-specific loss of MSH2. Through the use of patient-derived cells and organoid technologies, we performed stepwise in vitro differentiation of colonic and brain organoids from reprogrammed EPCAMdel iPSC derived from patient fibroblasts. Differentiation of iPSC to epithelial-colonic organoids exhibited continuous increased EPCAM expression and hypermethylation of the MSH2 promoter. This was associated with loss of MSH2 expression, increased mutational burden, MMRD signatures and MS-indel accumulation, the hallmarks of MMRD. In contrast, maturation into brain organoids and examination of blood and fibroblasts failed to show similar processes, preserving MMR proficiency. The combined use of iPSC, organoid technologies and functional genomics analyses highlights the potential of cutting-edge cellular and molecular analysis techniques to define processes controlling tumorigenesis and uncovers a new paradigm of tissue-specific MMRD, which affects the clinical management of these patients.

Abstract

Background: Recurrence of colorectal cancer has been linked to the presence of methylated circulating tumour DNA (ctDNA) in patient plasma after surgery. The prognostic significance of ctDNA before treatment remains unclear. This study investigated the correlation between pretreatment ctDNA and current radiological [magnetic resonance imaging (MRI)] prognostic markers in patients with rectal cancer and its association with recurrence-free survival and overall survival (OS). Patients and methods: A total of 42 patients with rectal cancer were enrolled. All patients had staging MRI before treatment. Blood was taken at diagnosis for ctDNA analysis for the presence of either methylated branched chain amino acid transaminase 1 (BCAT1) or IKAROS family zinc finger 1 (IKZF1). The correlation of MRI prognostic indicators and ctDNA test results was assessed with chi-square tests. Univariable and multivariate Cox regression analyses were carried out to determine variables associated with recurrence-free survival and OS. Results: The mean age of patients was 64.4 years (standard deviation 12.5 years), and the majority were male (30/42, 71.4%). A total of 11, 13, 9 and 9 patients were in stages I, II, III and IV, respectively. Patients were followed up for a minimum of 36 months unless disease recurrence or death occurred earlier. A total of 36 (85.7%) patients received neoadjuvant chemoradiotherapy, and 30 (71.4%) underwent surgical resection. The 3-year survival rate was 64%. About 67% (28/42) of patients were positive for the methylated ctDNA at diagnosis. Further, 11 out of 12 patients with a positive circumferential resection margin (CRM+) were ctDNA positive; univariable analysis showed that prognostic indicators for OS were presence of extramural venous invasion [EMVI; hazard ratio (HR) 2.63, 95% confidence interval (CI) 0.95-7.31], CRM+ (HR 10.69, 95% CI 3.51-32.56), metastatic disease (HR 7.7, 95% CI 2.79-21.67) and ctDNA% methylation (HR 1.04, 95% CI 1.02-1.06). The presence of CRM+ and a positive ctDNA had an HR of 19.57 (95% CI 3.47-110.49). In the multivariate analysis, including adjustment for age and EMVI, only the CRM+/ctDNA+ variable was an independent predictor for poor survival (HR 19.57, 95% CI 3.47-110.49). Conclusions: In rectal cancer, almost all patients with CRM involvement have ctDNA, and these patients had the worst prognosis. Future studies with longitudinal ctDNA assessment before and after treatment may potentially inform prognosis and help tailor patients’ treatment.

5. Development of an online psychological intervention to prevent depression in patients with atrial fibrillation: lessons learned in a user-centered design study

Abstract

Abstract Background Approximately 30% of patients with atrial fibrillation suffer from depression. Depression in patients with atrial fibrillation is associated with poor health outcomes, reduced health-related quality of life, and elevated societal costs. Preventing depression in this population may therefore lead to better health outcomes for the individual patient and reduced burden on society. Online psychological interventions are innovative and evidence-based modes of healthcare delivery. The objective was—through a user-centered design—to develop a personalized online psychological intervention aiming at preventing depression in patients with atrial fibrillation. Methods Guided by the Intervention Mapping framework for the development of complex interventions, we used a qualitative design and conducted a series of collaborative workshops with patients (n = 8) with atrial fibrillation. Results Through intensive collaboration, we developed the HOPE-AF intervention targeting nine risk factors for depression that were meaningful to patients. It contains four basic modules that give effective tools to handle daily psychological struggles. Furthermore, patients will receive personal risk factors modules. The evidence-based psychological methodology was applied to all modules. Primary lessons learned were (1) engaging patients from the start to ensure the intervention becomes meaningful; (2) using positive phrasing in all patient-related materials to promote motivation; (3) incorporating patients’ choice of preference where possible to personalize the intervention. Conclusion Based on a user-centered design, the HOPE-AF intervention aiming to prevent depression in patients with AF, was developed. It was confirmed that it is crucial to engage end-users in the development of complex interventions to accommodate their needs and preferences. The Hope-AF intervention will now be tested in a feasibility study.

Abstract

Background and purpose: Despite advancements in genetic testing and expanded eligibility criteria, underutilisation of germline testing for pathogenic variants in BRCA1 and BRCA2 (BRCA) remains evident among breast cancer (BC) patients. This observational cohort study presents real-world data on BRCA testing within the context of clinical practice challenges, including incomplete family history and under-referral. Material and methods: From the Danish Breast Cancer Group (DBCG) clinical database, we included 65,117 females with unilateral stage I–III BC diagnosed in 2000–2017, of whom 9,125 (14%) were BRCA tested. Test results spanned from 1999 to 2021. We evaluated test rates overall and in three diagnosis periods. In logistic regression models, we examined the correlation between a BRCA test and patients’ age, residency region, receptor status, and diagnosis period. Results: Test rates rose most significantly among patients aged under 40 years, increasing from 47% (2000–2005) to 88% (2012–2017), albeit with regional discrepancies. Test timing shifted in recent years, with most results within 6 months of BC diagnosis, primarily among the youngest patients. BRCA test rates were higher for oestrogen receptor-negative/human epidermal growth factor receptor 2-negative BC (25% in 2000–2005 vs. 38% in 2012–2017), and these findings were confirmed in multivariate regression models. Interpretation: Our results indicate a critical need for an intensified focus on BRCA testing among BC patients older than 40, where a mainstreamed testing approach might overcome delayed or missed testing. Current DBCG guidelines recommend BRCA testing of all BC patients younger than 50 years, while a general recommendation for older patients is still missing.

Abstract

Although most attention has been focused on the drugs used to control asthma, it is increasingly recognised that effective delivery of these drugs to the lungs is just as important. The most effective drugs, beta2-agonists and corticosteroids, are given by inhalation so there has been a search for more efficient inhaler devices that are easier for patients to use. A symposium at the European Respiratory Society Annual Meeting in 2005 discussed some of the important issues in inhaler therapy in adults and children. This article summarises the major points of discussion that arose out of this symposium. New more effective inhaler devices are now becoming available and are likely to have an important impact on asthma management.

The study primarily endeavoured to comprehend the unique journeys of teachers as they navigated the multifaceted sphere of technology integration within their instructional landscapes. Understanding these experiences is crucial for developing enhancements in training and professional development programs, aiming to amplify the role of school principals in the successful implementation of instructional technology. The role of the principal, being of paramount importance in initiating and maintaining the momentum of technology implementation, served as a critical focus of this study. The inquiry probed into how principals act as catalysts in this process, as well as the strategies they employ to ensure the sustained progress of technology integration. The ultimate objective of this research was to identify leadership models and change frameworks that are most conducive to facilitating digital implementation in K-12 educational settings. Participants of the study highlighted that the complex relationship between instructional technology and leadership styles, casting light on the transformative potential of effective leadership in cultivating an environment conducive to technological advancement. Abstract

Abstract In studies of families with rare disease, it is common to screen for de novo mutations, as well as recessive or dominant variants that explain the phenotype. However, the filtering strategies and software used to prioritize high-confidence variants vary from study to study. In an effort to establish recommendations for rare disease research, we explore effective guidelines for variant (SNP and INDEL) filtering and report the expected number of candidates for de novo dominant, recessive, and autosomal dominant modes of inheritance. We derived these guidelines using two large family-based cohorts that underwent whole-genome sequencing, as well as two family cohorts with whole-exome sequencing. The filters are applied to common attributes, including genotype-quality, sequencing depth, allele balance, and population allele frequency. The resulting guidelines yield ~10 candidate SNP and INDEL variants per exome, and 18 per genome for recessive and de novo dominant modes of inheritance, with substantially more candidates for autosomal dominant inheritance. For family-based, whole-genome sequencing studies, this number includes an average of three de novo, ten compound heterozygous, one autosomal recessive, four X-linked variants, and roughly 100 candidate variants following autosomal dominant inheritance. The slivar software we developed to establish and rapidly apply these filters to VCF files is available at https://github.com/brentp/slivar under an MIT license, and includes documentation and recommendations for best practices for rare disease analysis.

Abstract Visual validation is an important step to minimize false-positive predictions from structural variant (SV) detection. We present Samplot, a tool for creating images that display the read depth and sequence alignments necessary to adjudicate purported SVs across samples and sequencing technologies. These images can be rapidly reviewed to curate large SV call sets. Samplot is applicable to many biological problems such as SV prioritization in disease studies, analysis of inherited variation, or de novo SV review. Samplot includes a machine learning package that dramatically decreases the number of false positives without human review. Samplot is available at https://github.com/ryanlayer/samplot .

34. Suppression of very low frequency radio noise in transient electromagnetic data with semi-tapered gates

Abstract

The transient electromagnetic method (TEM) is widely used for mapping subsurface resistivity structures, but data are inevitably contaminated by noise from various sources. It is common practice to gate signals from TEM systems to reduce the amount of data and improve the signal-to-noise ratio (SNR). Gating acts as a filter, and optimum gating will pass the TEM signal un-attenuated while suppressing noise. In systems based on analog boxcar integrators, the gating corresponds to filtering with a square window. The frequency response of this window shape has large side lobes, which are often insufficient in attenuating noise, e.g., from radio signals in the very low frequency (VLF) 3–30 kHz band. Tapered gates have better side lobe suppression and attenuate noise better, but tapering with analog boxcar integrators is difficult. We propose using many short boxcar gates, denoted sub-gates, and combine the sub-gates into semi-tapered gates to improve noise rejection at late gates where low signal normally leads to poor SNR. The semi-tapering approach is analyzed and tested experimentally on data from a roving TEM system. We quantify the effect of semi-tapered gates by computing an improvement factor as the ratio between the standard error of data measured with boxcar gates and the standard error of data measured with semi-tapered gates. Data from a test survey in Gedved, Denmark, with 1825 measurements gave mean improvement factors between 1.04 and 2.22 for the 10 late-time gates centered between 78.7 and 978.1 µs. After inversion of the data, we find that semi-tapering increases the depth of investigation by about 20 % for this specific survey. We conclude that the semi-tapered approach is a viable path towards increasing SNR in TEM systems based on analog boxcar integrators.

Abstract

Abstract Background One in five patients with ischaemic heart disease (IHD) develop comorbid depression or anxiety. Depression is associated with risk of non-adherence to cardiac rehabilitation (CR) and dropout, inadequate risk factor management, poor quality of life (QoL), increased healthcare costs and premature death. In 2020, IHD and depression are expected to be among the top contributors to the disease-burden worldwide. Hence, it is paramount to treat both the underlying somatic disease as well as depression and anxiety. eMindYourHeart will evaluate the efficacy and cost-effectiveness of a therapist-assisted eHealth intervention targeting depression and anxiety in patients with IHD, which may help fill this gap in clinical care. Methods eMindYourHeart is a multi-center, two-armed, unblinded randomised controlled trial that will compare a therapist-assisted eHealth intervention to treatment as usual in 188 CR patients with IHD and comorbid depression or anxiety. The primary outcome of the trial is symptoms of depression, measured with the Hospital Anxiety and Depression Scale (HADS) at 3 months. Secondary outcomes evaluated at 3, 6, and 12 months include symptoms of depression and anxiety (HADS), perceived stress, health complaints, QoL (HeartQoL), trial dropout (number of patients dropped out in either arm at 3 months) and cost-effectiveness. Discussion To our knowledge, this is the first trial to evaluate both the efficacy and cost-effectiveness of a therapist-assisted eHealth intervention in patients with IHD and comorbid psychological distress as part of CR. Integrating screening for and treatment of depression and anxiety into standard CR may decrease dropout and facilitate better risk factor management, as it is presented as “one package” to patients, and they can access the eMindYourHeart program in their own time and at their own convenience. The trial holds a strong potential for improving the quality of care for an increasing population of patients with IHD and comorbid depression, anxiety or both, with likely benefits to patients, families, and society at large due to potential reductions in direct and indirect costs, if proven successful. Abstract

Abstract Background Type 2 diabetes (T2D) is associated with a significantly increased risk of cardiovascular disease (CVD). The DIAbetic CArdioVAscular Screening and intervention trial (DIACAVAS) was designed to clarify whether advanced imaging for subclinical atherosclerosis combined with medical treatment is an effective strategy to develop individualised treatment algorithms for Danish men and women with T2D aged 40–60. But in the DIACAVAS pilot study, the uptake was only 41%. Consequently, we explored how people experienced living with T2D to understand how to improve the uptake in initiatives targeting the prevention of CVD. Methods We used semi-structured interviews to obtain information on how the respondents experienced having T2D. For supplementary information, we used structured interviews on e.g. socioeconomic factors. From April to October 2019, 17 participants aged 40–60 years were recruited from general practices and diabetes outpatient clinics in Denmark. Several levels of analysis were involved consistent with inductive content analysis. Results The participants’ experiences of living with T2D fell along two continuums, from an emotional to a cognitive expression and from reactive to proactive disease management. This led to identification of four archetypal characteristics: (I) powerlessness, (II) empowerment, (III) health literacy, and (IV) self-efficacy. These characteristics indicated the importance of using different approaches to facilitate participation in cardiovascular preventive initiatives. Additionally, findings inspired us to develop a model for facilitating participation in future preventive initiatives. Conclusion Encouraging people with T2D to participate in cardiovascular preventive initiatives may necessitate a tailored invitation strategy. We propose a model for an invitational process that takes into consideration invitees’ characteristics, including powerlessness, empowerment, health literacy and self-efficacy. This model may enhance participation in such initiatives. However, participation is a general concern, not only in relation to cardiovascular prevention. Our proposed model may be applicable in preventive services for people with T2D in general.

Purpose: The medical trainee perspective regarding the prior authorization process has not been previously assessed. Here we evaluate the perceptions of radiation and medical oncology trainees regarding the prior authorization process and its effect on their training and patient care. Methods and Materials: A 12-question, nonincentivized, electronic national survey of radiation and medical oncology trainees at all Accreditation Council for Graduate Medical Education accredited oncology programs was conducted. Participation, perspectives, and experiences with the prior authorization process were assessed by Likert scale, free response, and multiple response selection. Results: Between January and March of 2019, the survey was distributed to 1505 trainees at 76 institutions with responses from 174/616 radiation (28.2%) and 139/889 medical oncology trainees (15.6%). The majority (69.2%) reported participating in the prior authorization process (radiation: 78.2% vs medical: 57.6%; P < .01). Most trainees (71%) reported concern for decline in the quality of patient care due to the prior authorization process. The majority of trainees (77.1%) reported decreased enthusiasm for work and choice of profession, with a higher incidence in medical oncology trainees (83.1% vs 73.7%, P = .04). The most commonly recommended modifications by trainees included that the insurance reviewer be in the same specialty as the ordering provider (87.7%), providers be compensated for participation (82.7%), and turnaround time be more rapid (74.3%). Conclusions: These data indicate that trainees in US oncology programs are active participants in the prior authorization process and report that prior authorization approvals negatively influence their medical training and the quality of patient care. Additional efforts to revise the insurance approval process are warranted.

45. What are the prevalence and predictors of psychosocial healthcare among patients with heart disease? A nationwide population-based cohort study

Objectives Psychosocial healthcare is recommended, but little is known about how patients perceive the level of care and whether subgroups of patients experience less psychosocial healthcare than others. We examined the prevalence of patient-reported psychosocial healthcare and factors predicting patient-reported lack of psychosocial healthcare among patients with heart disease.Design A cohort study.Setting Denmark, nationwide.Participants A registry-based random sample of 5000 patients with incident heart disease in 2013.Measures Patient-reported psychosocial healthcare was obtained from a survey and potential predictors before disease onset from registries. We used multivariable logistic regression analysis to determine predictors of patient-reported lack of care.Results We received responses from 56%; 40% reported lacking information on psychosocial aspects, 51% lacking psychosocial rehabilitation and support and 32% reported lacking both types of psychosocial healthcare. The type of heart disease was the strongest predictor of patient-reported lack of psychosocial healthcare, especially among patients with atrial fibrillation (OR: 3.11–3.98). Older age (OR: 1.48–2.05), female gender (OR: 1.27–1.53) and no contact with general practitioner (OR: 1.47–1.84) also predicted patient-reported lack of psychosocial healthcare. Patients outside the labour force (OR: 1.29) and living in the capital region (OR: 1.50) more frequently reported lacking psychosocial rehabilitation and support, and patients with recent (OR: 1.63) or past (OR: 1.33) anxiety or depression and severe comorbidities (OR: 1.34) more frequently reported lacking both types of psychosocial healthcare.Conclusions Many patients with heart disease reported lacking psychosocial healthcare. Importantly, patients who most need psychosocial healthcare are not those who report receiving it. Our results call for action to translate guidelines into clinical practice.

Abstract Introduction Resting state functional magnetic resonance imaging (rsfMRI) studies demonstrate that individuals with posttraumatic stress disorder (PTSD) exhibit atypical functional connectivity (FC) between the amygdala, involved in the generation of emotion, and regions responsible for emotional appraisal (e.g., insula, orbitofrontal cortex [OFC]) and regulation (prefrontal cortex [PFC], anterior cingulate cortex). Consequently, atypical amygdala FC within an emotional processing and regulation network may be a defining feature of PTSD, although altered FC does not seem constrained to one brain region. Instead, altered amygdala FC involves a large, distributed brain network in those with PTSD. The present study used a machine‐learning data‐driven approach, multi‐voxel pattern analysis (MVPA), to predict PTSD severity based on whole‐brain patterns of amygdala FC. Methods Trauma‐exposed adults (N = 90) completed the PTSD Checklist‐Civilian Version to assess symptoms and a 5‐min rsfMRI. Whole‐brain FC values to bilateral amygdala were extracted and used in a relevance vector regression analysis with a leave‐one‐out approach for cross‐validation with permutation testing (1,000) to obtain significance values. Results Results demonstrated that amygdala FC predicted PCL‐C scores with statistically significant accuracy (r = .46, p = .001; mean sum of squares = 130.46, p = .001; R2 = 0.21, p = .001). Prediction was based on whole‐brain amygdala FC, although regions that informed prediction (top 10%) included the OFC, amygdala, and dorsolateral PFC. Conclusion Findings demonstrate the utility of MVPA based on amygdala FC to predict individual severity of PTSD symptoms and that amygdala FC within a fear acquisition and regulation network contributed to accurate prediction.

Summary: Glutamine is a central nutrient for many cancers, contributing to the generation of building blocks and energy-promoting signaling necessary for neoplastic proliferation. In this study, we hypothesized that lowering systemic glutamine levels by exercise may starve tumors, thereby contributing to the inhibitory effect of exercise on tumor growth. We demonstrate that limiting glutamine availability, either pharmacologically or physiologically by voluntary wheel running, significantly attenuated the growth of two syngeneic murine tumor models of breast cancer and lung cancer, respectively, and decreased markers of atrophic signaling in muscles from tumor-bearing mice. In continuation, wheel running completely abolished tumor-induced loss of weight and lean body mass, independently of the effect of wheel running on tumor growth. Moreover, wheel running abolished tumor-induced upregulation of muscular glutamine transporters and myostatin signaling. 50. The role of personality, disability and physical activity in the development of medication-overuse headache: a prospective observational study

Abstract

Abstract Background Factors associated with development of medication-overuse headache (MOH) in migraine patients are not fully understood, but with respect to prevention, the ability to predict the onset of MOH is clinically important. The aims were to examine if personality characteristics, disability and physical activity level are associated with the onset of MOH in a group of migraine patients and explore to which extend these factors combined can predict the onset of MOH. Methods The study was a single-center prospective observational study of migraine patients. At inclusion, all patients completed questionnaires evaluating 1) personality (NEO Five-Factor Inventory), 2) disability (Migraine Disability Assessment), and 3) physical activity level (Physical Activity Scale 2.1). Diagnostic codes from patients’ electronic health records confirmed if they had developed MOH during the study period of 20 months. Analyses of associations were performed and to identify which of the variables predict onset MOH, a multivariable least absolute shrinkage and selection operator (LASSO) logistic regression model was fitted to predict presence or absence of MOH. Results Out of 131 participants, 12 % (n=16) developed MOH. Migraine disability score (OR=1.02, 95 % CI: 1.00 to 1.04), intensity of headache (OR=1.49, 95 % CI: 1.03 to 2.15) and headache frequency (OR=1.02, 95 % CI: 1.00 to 1.04) were associated with the onset of MOH adjusting for age and gender. To identify which of the variables predict onset MOH, we used a LASSO regression model, and evaluating the predictive performance of the LASSO-mode (containing the predictors MIDAS score, MIDAS-intensity and –frequency, neuroticism score, time with moderate physical activity, educational level, hours of sleep daily and number of contacts to the headache clinic) in terms of area under the curve (AUC) was weak (apparent AUC=0.62, 95% CI: 0.41-0.82). Conclusion Disability, headache intensity and frequency were associated with the onset of MOH whereas personality and the level of physical activity were not. The multivariable LASSO model based on personality, disability and physical activity is applicable despite moderate study size, however it can be considered as a weak classifier for discriminating between absence and presence of MOH.
