Case report of successful surgical treatment of giant cavernous hepatic hemangioma combined with generalized sarcoidosis is presented. Information about etiology, pathogenesis and management of these diseases, brief literature review are exhibited. The role of modern methods of diagnostic visualization, preoperative endovascular superselective embolization as well as features of blood-sparing extensive liver resection are discussed.

Data of 14 patients with chronic cystic pancreatitis, complicated by intracystic bleeding, were analyzed. Early diagnosis of a bleeding episode provides an in-time diagnostic and treatment procedures. Angiographic bleeding vessel occlusion is considered to be a method of choice. When the latter is impossible, a method of temporary intraoperative hemostasis is suggested, which decreases blood loss and permits an adequate surgical aid.

Results of examination of 233 patients with hepatic tumors provided new criteria of the patients' selection for extended hepatic resections. Correlation between the degree of affection of porto-hepatic hemodynamics and the risk of hepatic failure was found. It is also demonstrated that an increase in the diameter of hepatic tumor leads to a decrease in the main hemodynamic parameters in the portal vein. The level of proinflammatory cytokines may be regarded not only as an indicator of tissue alteration but also as a significant prognostic criterion.

The results of an efficiency study of the antinoceptive protection and of determination of an adaptation-reaction type in patients after total hip replacement with total intravenous or subarachnoid anesthesia and with spontaneous breathing are analyzed in the paper.

The vegetative regulation condition was investigated by the method of variation cardio-interval-graphy in patients who underwent the intravenous anesthesia in the outpatient setting. The experience of using, on the differential basis, the sympathomimetic and vagotonic anesthetics for different-type vegetative tonus was analyzed. An optimal state in the vegetative regulation of the cardiovascular system, a stable hemodynamics and an adequate post-narcosis rehabilitation in the anesthetic management of one-day surgery were ensured owing to the suggested method in choosing an anesthetic.

The authors review achievements in the field of prophylaxis and treatment of acute liver failure with donor's allo- and xenogenic isolated hepatocytes. Present-day clinical and experimental data on transplantation of hepatocytes evidence for their ability to produce a detoxicating action, to restore the lost functions of the affected part of the liver, to prevent death of hepatocytes caused by ischemia, to activate regeneration. This method is easy to conduct and obtained cells from one donor can by used for many recepients. Problems in transplantation of isolated hepatocytes are shown. Factors stimulating hepatocyte regeneration are mentioned with a special focus on hepatocyte growth factor having the most potent mitogenic effect.

The results of 226 extended surgeries for extensive cancer of the stomach, including 157 gastrectomies, 53 subtotal resections and 16 proximal resections, have been studied. Metastatic spreading into the lymph nodes along the left-side gastric artery was observed in 56.6%, in the hepatoduodenal ligament--25.2%, round the celiac trunk--29.2% and along the upper mesenteric artery--9.3%. Eighteen (7.9%) patients died after operation. The end results were analysed in 90 cases. Five-year survival after extended surgery was 72.7 (stage IIIa), 30.9 (stage IIIb), 33.3 (stage IVa) and 12.5% (stage IVb).

Data on 160 extensive surgical procedures for gastric cancer were analysed. Gastrectomy was performed in 109 cases of whom 12 (11%) died; subtotal resection--in 42 with 2 (4.8%) postoperative deaths whereas proximal resection--in 9 patients (no fatalities). End results were available for 58 cases of whom 29 (50%) survived over three years postsurgery. Radical surgery for locally advanced gastric cancer included removal of the greater and lesser omenta, bursa omentalis and retroperitoneal lymph nodes. Lymphadenectomy was confined to the following five fascial-fatty spaces: hepato-unciform, spleno-unciform, paraaortic, splenic and left reno-aortic.
