- Abstract
Worldwide, arsenic contamination has become a matter of extreme importance owing to its potential toxic, carcinogenic and mutagenic impact on human health and the environment. The magnetite-loaded biochar has received increasing attention for the removal of arsenic (As) in contaminated water and soil. The present study reports a facile synthesis, characterization and adsorption characteristics of a novel magnetite impregnated nitrogen-doped hybrid biochar (N/Fe3O4@BC) for efficient arsenate, As(V) and arsenite, As(III) removal from aqueous environment. The as-synthesized material (N/Fe3O4@BC) characterization via XRD, BET, FTIR, SEM/EDS clearly revealed magnetite (Fe3O4) impregnation onto biochar matrix. Furthermore, the adsorbent (N/Fe3O4@BC) selectivity results showed that such a combination plays an important role in targeted molecule removal from aqueous environments and compensates for the reduced surface area. The maximum monolayer adsorption (Qmax) of developed adsorbent (N/Fe3O4@BC) (18.15 mg/g and 9.87 mg/g) was significantly higher than that of pristine biochar (BC) (9.89 & 8.12 mg/g) and magnetite nano-particles (MNPs) [7.38 & 8.56 mg/g] for both As(III) and As(V), respectively. Isotherm and kinetic data were well fitted by Langmuir (R2 = 0.993) and Pseudo first order model (R2 = 0.992) thereby indicating physico-chemical sorption as a rate-limiting step. The co-anions (PO43-) effect was more significant for both As(III) and As (V) removal owing to similar outer electronic structure. Introduction La rhonchopathie represente la principale indication de realisation d’une polygraphie ventilatoire afin de confirmer ou d’exclure un SAOS associe. Methodes Une etude retrospective sur 3 ans (2014–2016) de 79 patients hospitalises pour rhonchopathie avec suspicion de SAOS chez qui nous avons realise une polygraphie ventilatoire ou une polysomnographie. Resultats Soixante-dix-neuf patients ont ete colliges, avec une predominance feminine (63 %), et un âge moyen de 43 ans avec des extremes allant de 15 a 80 ans. Le delai de consultation moyen est de 5 ans. L’indication principale est representee par la rhonchopathie isolee dans 53 % des cas (40 cas), suivie des services referents de cardiologie (17 cas), d’endocrinologie (11 cas), d’ORL (8 cas) et de neurologie (1 cas). L’antecedent d’hypertension arterielle est retrouve dans 39 % des cas, le diabete dans 16 %, un asthme ou BPCO dans 8 %, un tabagisme actif dans 8 %, et une depression dans 3 cas. Les symptomes diurnes etaient domines par la somnolence diurne et la fatigabilite matinale (67 %), les troubles cognitifs (35 %), les cephalees matinales (34 %), l’irritabilite (25 %) et les troubles de la libido (4 %). Pour les symptomes nocturnes, le ronflement est retrouve chez tous les patients, avec des pauses respiratoires dans 66 % des cas et des troubles mictionnels dans 39 % des cas. L’obesite est retrouvee dans 51 % des cas, avec une predominance feminine (60 % des femmes sont obeses versus 31 % des hommes). L’examen ORL note principalement une hypertrophie amygdalienne (6 cas), une deviation de la cloison nasale (6 cas), une macroglossie (5 cas), un voile du palais allonge (3 cas), un retrognathisme (1 cas) et une micromandibulie (1 cas). Une tetraplegie flasque a ete notee dans un cas d’atteinte par SLA. Le SAOS a ete confirme chez 63 cas, soit 80 % des patients ronfleurs avec legere predominance masculine (83 % des hommes versus 78 % des femmes). Un traitement hygieno-dietetique a ete indique dans tous les cas de SAOS leger a modere (40 cas). Un traitement par PPC a ete indique chez tous les cas de SAOS severe (23 cas). Un traitement chirurgical d’une pathologie ORL sous jacente a ete propose dans 7 cas. Les autres patients ronfleurs chez qui nous n’avons pas confirme le SAOS ont ete reorientes vers leur medecin traitant d’origine. Conclusion Il ressort de cette etude que le ronflement est un signe sensible mais non specifique du SAOS. Neanmoins ceci ne devrait pas freiner le depistage de cette pathologie qui demeure un veritable probleme de sante publique au Maroc.

6. Le pneumothorax spontané chez la femme : à propos de 50 cas

Introduction Le pneumothorax spontane se definit par l’entree d’air dans la cavite pleurale, en dehors de tout traumatisme ou manœuvre instrumentale. Bien que frequente en pneumologie, cette pathologie est peu frequente chez la femme avec une incidence qui varie entre 1,2–10 cas par 100 000 habitants Le but de ce travail est de souligner les aspects epidemiologiques, cliniques, radiologiques, etiologiques, therapeutiques et evolutifs du pneumothorax spontane chez la femme. Methodes Il s’agit d’une etude monocentrique retrospective realisee au service de pneumologie de l’hopital Ibn Sina de Rabat sur une periode de 7 ans. Resultats Il s’agit de 50 femmes sur un total de 479 cas de pneumothorax soit un peu plus de 10 %. La moyenne d’âge etait de 47 ans (19 et 90 ans), le tabagisme est retrouve dans 7 cas, un antecedent et une tuberculose pulmonaire active dans 13 cas, une notion d’exposition au feu de bois dans 2 cas, un contexte de pneumopathie interstitielle diffuse dans 6 cas (PHS, sarcoidose et fibrose) et une patiente presente une grossesse a terme. La symptomatologie etait dominee par une douleur thoracique associee a une dyspnee dans tous les cas. A la radiographie thoracique, l’atteinte etait unilaterale chez toutes nos patientes avec un pneumothorax total complet dans 47 cas et localise dans 3 cas. Quarante-sept patientes ont beneficie d’un drainage thoracique avec un repos strict et associe a une oxygenotherapie pour le reste. Le diagnostic de pneumothorax spontane idiopathique est retenu dans 24 cas. Les etiologies retenues etaient une BPCO (7 cas), des sequelles pulmonaires (9 cas), une tuberculose pulmonaire active (4 cas), deux cas sur sarcoidose, un cas sur PHS, deux cas sur fibrose et un cas de pneumothorax catamenial. L’evolution clinique etait bonne dans 46 cas avec retour du poumon a la paroi en moins de 10 jours, 3 patientes ont presente un emphyseme sous-cutane ayant necessite une duree de drainage plus longue avec une recidive dans un cas et nous deplorons un deces. Conclusion Le pneumothorax spontane chez la femme se caracterise par une faible incidence. La correlation avec la variation du cycle hormonal meriterait d’etre etudiee.

7. Profil épidémiologique, étiologique et bactériologique des pleurésies purulentes non tuberculeuses : à propos de 48 cas

Introduction La pleuresie purulente est definie par la presence dans l’espace pleural d’un liquide purulent. Elle s’observe dans des circonstances tres variees, souvent sur un terrain d’immunodepression, et constitue une urgence diagnostique et etiologique. Methodes Nous presentons une etude retrospective sur une serie de 48 cas de pleuresies purulentes non tuberculeuses colligees entre janvier 2012 et decembre 2016. Resultats La moyenne d’âge est de 45 ans, avec une nette predominance masculine (71 %). Le delai moyen de consultation est relativement long (31 j). Une comorbidite source d’immunodepression est associee chez 70 % des patients. Un tabagisme est releve dans la moitie des cas. La symptomatologie clinique est dominee par la douleur thoracique (80 %), la dyspnee (75 %), l’alteration de l’etat general (57 %), la fievre (54 %) et la toux (54 %). Sur la radiographie thoracique, l’epanchement est souvent de grande abondance (53 %) et de siege droit (59 %). La bacteriologie du liquide pleural est contributive dans 35,4 % des cas : Staphylocoque (25 %), Streptocoque (17 %), Escherichia coli (12 %), Klebsiella (10 %), enterobacteries (4 %), Pseudomonas (4 %), et Haemophilus para influenzae (2 %). Les etiologies sont representees essentiellement par les infections pulmonaires (52 %) dans le cadre d’epanchements parapneumoniques compliques. La pleuresie purulente a ete revelatrice d’une neoplasie dans 8 cas (17 %) : carcinome epidermoide bronchique (2 cas), adenocarcinome bronchique primitif (4 cas), carcinome a petites cellules (1 cas) et lymphome malin non hodgkinien type T (1 cas). Par ailleurs, on retrouve les causes iatrogenes (12 %) au decours de ponctions pleurales iteratives, les infections sous diaphragmatiques (8 %), le pneumothorax (1 cas) et la rupture d’abces pulmonaire (1 cas). La cause demeure inconnue dans 3 cas. La prise en charge therapeutique commune reside, outre le traitement etiologique, en l’instauration d’une antibiotherapie probabiliste intraveineuse a base d’amoxicilline-acide clavulanique et ciprofloxacine (81 %) ± metronidazole (25 %). L’evacuation pleurale consiste en la mise en place d’un drain thoracique dans 87 % des cas. La kinesitherapie respiratoire est prescrite dans 70 % des cas. Le recours a la chirurgie est note dans 7 cas. Nous deplorons deux deces (4 %) lie aux comorbidites et/ou neoplasie. Conclusion Il ressort de cette etude que l’evolution des pleuresies purulentes depend du contexte etiologique. Une prise en charge precoce et adequate ameliore le pronostic et evite les sequelles.

Introduction L’inhalation de corps etrangers intrabronchiques chez l’adulte est une urgence diagnostique et therapeutique. Le but de ce travail est d’analyser les caracteristiques epidemiologiques, diagnostiques et cliniques de l’inhalation de corps etrangers intrabronchiques, de souligner leurs particularites dans notre contexte ainsi que de presenter leurs moyens de prise en charge therapeutique. Methodes Il s’agit d’une etude retrospective realisee au service de pneumologie de l’hopital Avicenne de Rabat entre janvier 2004 et decembre 2016. Resultats Durant cette periode, nous avons recense 68 cas d’inhalation de corps etranger dont 59 femmes (87 %), et 9 hommes (13 %). L’âge moyen est de 21 ans. Les epingles a foulard etaient retrouvees dans la majorite des cas (85 %). Les manifestations cliniques etaient dominees par le syndrome de penetration dans 58,8 % des cas, alors que 11,7 % des cas etaient asymptomatiques. La radiographie thoracique, realisee chez tous les patients, a localise le corps etranger dans 85,3 % des cas. La bronchoscopie souple a ete realisee chez tous nos patients, permettant de localiser le corps etranger dans 60 cas (88,2 %), et son extraction dans 23 cas (33,8 %). La bronchoscopie rigide a ete realisee chez 31 patients (45,6 %), permettant l’extraction du corps etranger dans 20 cas (29,4 %). L’indication operatoire a ete posee chez 21 patients (30,9 %). L’expulsion spontanee a ete notee dans 3 cas (4,4 %). L’extraction a ete faite par pince chirurgicale a travers l’orifice de tracheotomie chez un patient. L’evolution clinique etait favorable chez tous nos patients. Conclusion L’inhalation d’un corps etranger est un accident potentiellement grave. Sa nature, dans notre contexte, reste dominee par les epingles a foulard. L’extraction se fait le plus souvent par endoscopie, mais parfois le recours a la thoracotomie est le seul moyen therapeutique. Les mesures preventives restent le meilleur moyen pour eviter cet incident.

11. Simulation-based decision support tool for electrification of isolated areas using a network with multiple renewable sources

The Ecole Polytechnique Federale de Lausanne (EPFL), the Hassan II Institute of Agronomics and Veterinary Science (IAV) and Targa-Aid association (Rabat/Morocco) undertook a joint project to implement an electrification technique from different types of renewable sources, supplying small networks of isolated consumers. To help select the best solution among several production variants for satisfying the consumption, taking all of the constraints into account, the EPFL developed the OptElDec software, a simulation based decision support tool. This tool allows modeling consumers, energy production and storage elements. Furthermore, a priority system applied to consumers and power stations is introduced for acting on operation strategy of the production and storage elements. Thus, it gives an eloquent prediction of the performance of one production network variant. Once simulations are carried out, the decision support layer allows comparing several simulated variants regarding quantitative and qualitative criteria in order to find the optimum variant.

Competition leads value adding networks to increase their agility in order to acquire or maintain their market shares. To do so, managers must be able to propose and to assess several improvement scenarios in order to offer the best service level to the markets at minimum cost. As value adding networks often involve several actors evolving in a non-steady environment, simulation is one of the best approaches to catch the complexity of these systems. However, existing simulation tools are often criticized for their opacity and their limited reusability. As an answer to these limitations, we propose a new framework that lifts the veil on decision making in simulation by allowing the integration and the edition of decision making patterns with varying levels of complexity. The framework allows as well the separation and the visualization of both material, information and control flows.
16. Unraveling the Mystery of Desmoid Tumors: Insights From a Moroccan Tertiary Center.

- Abstract
Desmoid tumors (DTs) are rare, aggressive malignancies developing from clonal fibroblastic proliferation originating from soft tissues. Despite their low metastatic potential, their invasiveness towards neighboring organs and a high recurrence rate contribute significantly to morbidity and mortality, thereby impacting the quality of life of patients. Several therapeutic options are available, but standardized protocols are lacking. In this study, we reviewed 14 cases of DT retrospectively over a period of 15 years, from September 2008 to December 2023. The most prevalent tumor locations were in the extremities, and the majority of patients were female. We identified risk factors in two patients, those being surgical trauma and familial adenomatous polyposis (FAP). Half of the patients underwent surgery for DT, and two received salvage radiotherapy. Systemic therapy was used in the first and second lines and comprised of chemotherapy, endocrine therapy, and non-steroidal anti-inflammatory drugs (NSAI). Active surveillance was proposed in three patients. This is the first retrospective study to assess the characteristics of DT in Moroccan patients in a tertiary care setting. 17. An Unusual Cause of Spontaneous Pneumomediastinum: The Mounier-Kuhn Syndrome.

- Abstract
Mounier-Kuhn syndrome is a rare clinical and radiologic condition. It is characterized by tracheal and bronchial dilation. Diagnosis is made by computed tomography and bronchoscopy. An 81-year-old man presenting with an acute chest pain was referred to the pulmonology department. His chest computed tomographic scan showed a tracheobronchomegaly with an increase in the diameter of both the trachea and right and left main bronchi, associated with pneumomediastinum and fibrosis. Fiberoptic bronchoscopy revealed enlarged trachea and both main bronchi. These findings are consistent with a diagnosis of Mounier-Kuhn syndrome. Besides considering this long-neglected "orphan disease" when diagnosing spontaneous pneumomediastinum, clinicians should also be aware of an underlying Mounier-Kuhn syndrome in patients with recurrent respiratory infections, in order to avoid complications associated with the disease.

Primary cardiac sarcoma is a rare aggressive entity. It constitutes the second most common type of primary cardiac neoplasms. Its management has largely been guided by small retrospective series with a median survival of 6 months. Here, we discuss a unique case of 8-year survival cardiac leiomyosarcoma managed by surgical and adjuvant therapy.

- Abstract
Background: Triple-negative breast cancer (TNBC) is defined by the lack of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER-2) expression. This is an aggressive malignancy with a poor prognosis despite the high rates of response to chemotherapy. The aim of this study is to determine the clinicopathological, therapeutic features and outcomes associated with this type of breast cancer., Methods: This is a retrospective study of confirmed triple negative breast cancer females collected at the National institute of oncology of Rabat in Morocco, between January 2007 and December 2008. Epidemiological, clinical, histological, therapeutic and evolutive data were analyzed. OS and DFS rates were estimated by Kaplan-Meier analysis., Results: A total of one 152 patients with breast cancer, were identified as having triple-negative breast cancer (16,5%). The median age at diagnosis was 46 years. 130 patients (86%) had infiltrating ductal carcinoma and thirteen had medullar carcinoma (9%). 84 cases (55%) were grade III Scarff-Bloom-Richardson (SBR). 48 % had positive lymph nodes, and 5 % had distant metastases at diagnosis. According TNM staging, 12 patients (8%) had stage I, 90 patients (60%) had stage II and the 43(28%) had stage III. 145 patients received surgery. 41 (28%) had conservative surgery and 104 (72%) received radical mastectomy with axillary lymph nodes dissection. 14 patients with advanced tumors or inflammatory breast cancer have received neoadjuvant chemotherapy and four patients (28%) had complete pathologic response. From 131 patients how received adjuvant chemotherapy, 99 patients (75,5%) had Anthracycline based chemotherapy) and 27 patients (20,6%) had sequential Anthracycline and docetaxel,. Seven patients with metastatic disease received anthracycline-based regimen in the first line metastatic chemotherapy. The median follow-up time was 46 months (range 6,1 -60 months). Overall survival at 5 years for all patients was 76,5%., Conclusion: These results suggest that most TNBC characteristics in Moroccan patients are in accordance with literature data, especially concerning young age at diagnosis high grade tumors, advanced stage at diagnosis, and short time to relapse. Although the high response rate to chemotherapy, the overall prognosis of this subset of tumors remains poor.

21. Mantle cell lymphoma of the larynx: Primary case report.

- Abstract
Introduction: Primary laryngeal lymphomas are exceedingly rare. Only about a hundred cases have been reported. They consist mainly of non-Hodgkin lymphoma, especially of diffuse large B-cell lymphoma and mucosa-associated lymphoid tissue. We report the first case of a primary laryngeal mantle cell lymphoma., Case Presentation: We report a case of a primary mantle cell lymphoma of the larynx in a 70-year-old North African non-smoker male. We present a detailed report of his clinical and paraclinical data as well as treatment options., Conclusions: Mantle cell lymphoma is a very aggressive lymphoma subset associated with poor prognosis. Laryngeal mantle cell lymphoma is exceedingly rare. To the best of our knowledge, this is the first case to ever be reported.

- Abstract
Background: Germ cell tumours are uncommon in aged man. We present a rare case of metastatic seminoma of the testis associated with liver and renal insufficiencies in a 78-years-old man managed successfully with carboplatin based chemotherapy., Case Presentation: A 78 years old man admitted with signs and symptoms suggestive of a testicular cancer with alteration of health. Computed tomography of the pelvis and abdomen showed a large retroperitoneal tumour. The diagnosis of seminoma was established from the histological study of the left orchidectomy. At admission, the liver and renal check-up showed liver and renal insufficiencies., Conclusion: The patient received 4 of carboplatin based chemotherapy with significant improvement in symptoms, and complete radiological response.
