To evaluate the efficacy and safety of nintedanib plus docetaxel in patients with advanced adenocarcinoma non-small cell lung cancer (NSCLC) who progressed after chemotherapy and immune checkpoint inhibitor (ICI) therapy.VARGADO (NCT02392455) is an ongoing, prospective, non-interventional, real-world study of nintedanib plus docetaxel after first-line chemotherapy in the routine clinical treatment of patients with locally advanced, metastatic or locally recurrent adenocarcinoma NSCLC. Data were collected during routine visits. We report the results from cohort B (n = 80), who received third-line nintedanib plus docetaxel after first-line chemotherapy and second-line ICI therapy.The median duration of follow-up was 12.4 months. Median progression-free survival from initiation of third-line nintedanib plus docetaxel was 6.4 months (95% confidence interval 4.8, 7.3); median overall survival was 12.1 months (95% confidence interval 9.4, 13.5). The 1-year overall survival rate after initiation of third-line nintedanib plus docetaxel treatment (primary end point) was 52% (95% confidence interval 38.0%, 64.4%). Among 64 patients with a documented response, the objective response rate was 50% (n = 32; one complete response and 31 partial responses) and the disease control rate was 86% (n = 55). There were no new safety signals or unexpected toxicities. Among all treated patients, 74% (n = 59) experienced drug-related adverse events, most commonly (nintedanib-related/docetaxel-related) diarrhoea (34%/24%), a decreased white blood cell count (11%/19%) and nausea (13%/16%).Nintedanib plus docetaxel demonstrated a high response rate and disease stabilisation in the third-line setting after failure of prior chemotherapy and ICI treatment, with a manageable safety profile. These results suggest that nintedanib plus docetaxel represents an efficient treatment option after failure of prior ICIs. The ongoing VARGADO study provides valuable real-world data to inform clinical decision-making regarding treatment sequencing after chemotherapy and ICI failure in patients with adenocarcinoma NSCLC.

Bei der Untersuchung fielen ein Pleuraerguss rechts, Lymphödem linker Unterschenkel und Fuß und fleckige, landkartenartige Hautveränderungen auf. Bei der Pleurapunktion rechts konnte ein chylöser Erguss abgelassen werden. In der Ganzkörper-MRT war der Pleuraerguss rechts nachweisbar, eine Vermehrung der Lymphgefäße inguinal, parailiakal und paraaortal und prominente Chylusgefäße. Eine ossäre Beteiligung lag nicht vor. Der Ductus thoracicus abdominalis war bis auf 3 mm erweitert. Wir stellten die Diagnose einer generalisierten lymphatischen Anomalie (GLA) – Lymphangiomatose. Die GLA ist eine seltene Erkrankung unbekannter Ursache mit Fehlentwicklung des lymphatischen Systems. Thorakal fällt eine GLA oft durch die Entwicklung von chylösen Pleuraergüssen auf. Es können aber auch viele andere Organe betroffen sein. Die Therapie besteht meist in einer symptomatischen Behandlung und dem Versuch, den Progress der Erkrankung zu verlangsamen.

ZusammenfassungEine 39-jährige Patientin hat sich initial wegen Rückenschmerzen orthopädisch vorgestellt. Nachdem sich in der Magnetresonanztomografie der Lendenwirbelsäule der Verdacht auf eine tumoröse Erkrankung gestellt hatte, war in den weiterführenden Untersuchungen eine pulmonale Raumforderung, eine ausgedehnte mediastinale Lymphadenopathie und ein großer einseitiger Pleuraerguss nachweisbar. Histologisch zeigte sich sowohl in den Proben des Tumors im Wirbelkörper als auch in den Proben der transbronchialen Nadelaspiration eines mediastinalen Lymphknotens ein NUT-Karzinom. Das NUT-Karzinom, das früher als NUT-Mittellinienkarzinom bezeichnet wurde, ist eine sehr seltene, aggressive Tumorentität mit ungünstiger Prognose. Der Tumor wurde zunächst gehäuft in der Mittellinie der oberen Atemwege und des Mediastinums beschrieben, ist aber nicht darauf beschränkt. Das NUT-Karzinom trifft häufig junge Patienten. Aufgrund der Seltenheit existiert bisher keine etablierte Therapie.

The present guideline provides a new and updated concept of the management of adult patients with community-acquired pneumonia. It replaces the previous guideline dating from 2016.The guideline was worked out and agreed on following the standards of methodology of a S3-guideline. This includes a systematic literature search and grading, a structured discussion of recommendations supported by the literature as well as the declaration and assessment of potential conflicts of interests.The guideline has a focus on specific clinical circumstances, an update on severity assessment, and includes recommendations for an individualized selection of antimicrobial treatment.The recommendations aim at the same time at a structured assessment of risk for adverse outcome as well as an early determination of treatment goals in order to reduce mortality in patients with curative treatment goal and to provide palliation for patients with treatment restrictions.Die vorliegende Leitlinie umfasst ein aktualisiertes Konzept der Behandlung und Prävention von erwachsenen Patienten mit ambulant erworbener Pneumonie und löst die bisherige Leitlinie aus dem Jahre 2016 ab.Sie wurde entsprechend den Maßgaben zur Methodologie einer S3-Leitlinie erarbeitet und verabschiedet. Hierzu gehören eine systematische Literaturrecherche und -bewertung, die strukturierte Diskussion der aus der Literatur begründbaren Empfehlungen sowie eine Offenlegung und Bewertung möglicher Interessenskonflikte.Die Leitlinie zeichnet sich aus durch eine Zentrierung auf definierte klinische Situationen, eine aktualisierte Maßgabe der Schweregradbestimmung sowie Empfehlungen zu einer individualisierten Auswahl der initialen antimikrobiellen Therapie.Die Empfehlungen zielen gleichzeitig auf eine strukturierte Risikoevaluation als auch auf eine frühzeitige Bestimmung des Therapieziels, um einerseits bei kurativem Therapieziel die Letalität der Erkrankung zu reduzieren, andererseits bei palliativem Therapieziel eine palliative Therapie zu eröffnen.

Thermal analyses of 284 repetitive single crystallization events were carried out. In particular, the isothermal liquid-to-crystal transformation kinetics of Poisson-distributed and non-Gaussian shaped exothermic DSC signals are calculated individually and by superimposition. It is found that modes of impingement for anisotropic growth, random and regular dispersions fail to accurately predict the growth exponent if a single crystal or a low number of crystals are growing. Non-stationary effects are evident at low and large crystal fractions which diminish when virtual signals of multiple crystallization events are superimposed. The results show that the precise analytic formulation to calculate the degree of transformation relies on an ensemble of crystals growing simultaneously. Where this is not the case, as in glass-forming liquids cooled from above liquidus temperature, the proper approach is to use a time-averaged signal of repetitive crystallization runs.

Granulated blast furnace slag (GBS) is a glassy by-product of the steel industry that is formed during quenching of the molten slag after the blast furnace in a water-jet. To elucidate the cooling history of GBS, calorimetric scanning in the glass transition range and viscometric experiments at temperatures above the liquidus were performed. The GBS studied was of industrial origin with a d50 = 700 μm and > 99% glassy. It is found that GBS glass is of high potential energy, showing a fictive temperature that is approx. 160 K higher than the glass transition temperature under standard cooling conditions. Using different viscosity models and the relationship between quench rate and shear viscosity at the fictive temperature it is calculated that GBS was formed at a cooling rate of approx. 2.6 × 105 K s−1 corresponding to a shear viscosity of 105.9 Pa s. Due to the high Tf and high amount of heat that is released during structural relaxation, GBS is assigned to the class of hyperquenched glasses.

A 33-year-old man presented to us with slowly progressive dyspnea. At the physical examination we found pleural effusion on the right side, lymph edema of the left lower limb and foot and blotchy map-like skin changes. The pleural puncture revealed a chylous effusion. In whole body MRI we saw the pleural effusion on the right, an increase in lymph vessels in the pelvis and paraaortic, and prominent chylous vessels. There was no bone involvement. The abdominal thoracic duct was enlarged by up to 3 mm. We diagnosed a general lymphatic anomaly (GLA) - lymphangiomatosis. GLA is a rare disease of unknown origin with dysplasia of the lymphatic system. The thoracic manifestation is often the development of chylous pleural effusions. However, many other organs can also be involved. Unsere zunehmende Abhängigkeit von Informationstechnik erhöht kontinuierlich die Safety- und Security-Anforderungen bei deren Einsatz. Ein zentrales Problem hierbei sind Schwachstellen von Hard- und Software. Marktkräfte konnten diese Situation bislang nicht grundsätzlich beheben. Eine Gegenstrategie sollte deshalb folgende Optionen erwägen: (1) private und staatliche Förderung offener und sicherer IT‑Produktion, (2) Verbesserung der souveränen Kontrolle bei der Produktion aller kritischen IT‑Komponenten innerhalb eines Wirtschaftsraumes sowie (3) verbesserte und durchgesetzte Regulierung. Dieser Beitrag analysiert Vor- und Nachteile dieser Optionen. Es wird vorgeschlagen, die Sicherheit der Schlüsselkomponenten einer Lieferkette durch weltweit verteilte, offene und ggf. mathematisch bewiesene Komponenten zu gewährleisten. Der beschriebene Ansatz erlaubt die Nutzung existierender und neuer proprietärer Komponenten. Increasing dependence on information technology calls for strengthening the requirements on their safety and security. Vulnerabilities that result from flaws in hardware and software are a core problem which market mechanisms have failed to eliminate. A strategy for resolving this issue should consider the following options: (1) private- and public-sector funding for open and secure production, (2) strengthening the sovereign control over the production of critical IT components within an economic zone, and (3) improving and enforcing regulation. This paper analyses the strengths and weaknesses of these options and proposes a globally distributed, secure supply chain based on open and mathematically proved components. The approach supports the integration of legacy and new proprietary components.

A 39-year-old woman presented with back pain at an orthopaedic clinic. Magnet resonance tomography revealed a spinal tumour. Further imaging also showed a pulmonary tumour, massive mediastinal lymphadenopathy as well as unilateral pleural effusion. Histology from biopsies from the spinal tumour and from a mediastinal lymph node revealed NUT carcinoma. The NUT carcinoma that earlier has been named NUT midline carcinoma is a very rare, aggressive tumour with a poor prognosis. Initially, the NUT carcinoma was thought to develop from organs in the midline such as upper airways or mediastinal cavity. However, the NUT carcinoma is not limited to structures in the midline. NUT carcinoma often affects young patients. Kurzfassung: Wie kann effektive Deutsche Cyberabschreckung aussehen? Dieser Artikel stellt die ersten Ergebnisse eines neuen Cyberabschreckungsmodells vor, genannt „Agile Cyberabschreckung“, das auf interdisziplinärer Forschung am ISPK basiert. Danach eröffnen Cyber-Operationen einen völlig neuen Weg in der Verteilung von Macht. Grundsätzlich dafür ist der spezielle virtuelle (technologische) Charakter der operativen Domäne „Cyber“. Die beschriebene Agile Cyberabschreckung adressiert die spezifischen Cyber-Verwundbarkeiten einer „Cyber-Mittel-Macht“ wie Deutschland und schlägt vier Abschreckungsoptionen vor.

Es besteht eine komplexe Beziehung zwischen der chronisch obstruktiven Lungenerkrankung (COPD) und Diabetes. In mehreren Studien wurde nachgewiesen, dass Diabetes die Progression und Prognose einer COPD verschlechtern kann. Dies kann Folge eines direkten Effekts einer Hyperglykamie auf die Lungenphysiologie, Inflammation oder Anfalligkeit fur bakterielle Infektionen sein. Auf der anderen Seite wurden ausgepragte Hinweise dafur gefunden, dass eine COPD das Risiko fur die Entwicklung eines Typ-2-Diabetes, als Folge inflammatorischer Prozesse oder als Nebenwirkung der Therapie mit hoher dosierten Kortikosteroiden, steigert. Obwohl es bedeutende Hinweise fur eine Verbindung zwischen COPD und Diabetes gibt, sind zusatzliche Studien erforderlich, um diese Interaktion und die daraus resultierenden Implikationen besser zu verstehen.

Nucleation kinetics of glass-ceramics is frequently determined using Tammann's double-stage heat-treatment. This method requires a complex deconvolution of the experimentally observed induction time (tind), i.e. the intercept of the linear part of the crystal number density curve with the nucleation time axis, into two components. In this paper, double-stage heat treatments were performed, with heating rates between the nucleation and development temperatures covering two orders of magnitude, in samples of a homogeneously nucleating glass-forming system, lithium disilicate. Our results show that tind increases with increasing heating rates with cubic root dependence. In accordance with the theory, tind was split into the intrinsic time required to establish a steady-state cluster size distribution, τ (time-lag) at the nucleation temperature and an incubation time (ti), which is a size, heating rate and development temperature (Td) dependent growth time. We demonstrate that the Collins-Kashchiev nucleation model performs poorly if ti is approximated by the time needed to experimentally detect the first crystal. In contrast, the Shneidman approach is consistent with theory. We found that at any given nucleation temperature, ti is a strong function of the heating rate, and is proportional to tind, whereas τ is a constant, as expected.

ZusammenfassungSchlafbezogene Atmungsstörungen (SBAS) als häufige und relevante Komorbidität erfordern sowohl im perioperativen Management als auch in der intensivmedizinischen Behandlung kritisch kranker Patienten besondere Berücksichtigung. Patienten mit SBAS benötigen aufgrund eines erhöhten perioperativen Risikos für kardiale, pneumologische und neurologische Komplikationen eine maßgeschneiderte perioperative Überwachung und Therapie, wobei die aktuellen Empfehlungen primär Patienten mit einer obstruktiven Schlafapnoe betrachten. Die intensivmedizinische Behandlung von kritisch kranken Patienten mit SBAS erfordert ein aufmerksames Management während der Beatmung, in der Weaning- und in der Postextubationsphase, um das Risiko einer prolongierten Beatmung oder eines Weaningversagens zu reduzieren.

In der Bronchoskopie gelang tatsächlich der Nachweis einer nicht tuberkulösen Mykobakteriose. Als Zufallsbefund zeigte sich im Mittellappenbronchus eine weißliche polypöse Schleimhautveränderung. Histologisch war ein Granularzelltumor (GZT) nachweisbar. Der GZT ist ein seltener Tumor, der an verschiedenen anatomischen Lokalisationen auftreten kann. In der Lunge kann er klinisch durch eine bronchiale Obstruktion manifest werden, bildgebend als Rundherd oder Infiltrat bzw. Atelektase oder als Zufallsbefund in einer Bronchoskopie auffallen. Therapie der 1. Wahl ist die chirurgische Entfernung.

The ability to disperse is considered essential for soil bacteria colonization and survival, yet very little is known about the dispersal ability of communities from different heterogeneous soil compartments. Important factors for dispersal are the thickness and connectivity of the liquid film between soil particles. The present results from a fractured clay till depth profile suggest that dispersal ability is common in various soil compartments and that most are dominated by a few dispersing taxa. Importantly, an increase in shared dispersers among the preferential flow paths of the clay till suggests that active dispersal plays a role in the successful colonization of these habitats., This study assessed the dispersal of five bacterial communities from contrasting compartments along a fractured clay till depth profile comprising plow layer soil, preferential flow paths (biopores and the tectonic fractures below), and matrix sediments, down to 350 cm below the surface. A recently developed expansion of the porous surface model (PSM) was used to capture bacterial communities dispersing under controlled hydration conditions on a soil-like surface. All five communities contained bacteria capable of active dispersal under relatively low hydration conditions (−3.1 kPa). Further testing of the plow layer community revealed active dispersal even at matric potentials of −6.3 to −8.4 kPa, previously thought to be too dry for dispersal on the PSM. Using 16S rRNA gene amplicon sequencing, the dispersing communities were found to be less diverse than their corresponding total communities. The dominant dispersers in most compartments belonged to the genus Pseudomonas and, in the plow layer soil, to Rahnella as well. An exception to this was the dispersing community in the matrix at 350 cm below the surface, which was dominated by Pantoea. Hydrologically connected compartments shared proportionally more dispersing than nondispersing amplicon sequence variants (ASVs), suggesting that active dispersal is important for colonizing these compartments. These results highlight the importance of including soil profile heterogeneity when assessing the role of active dispersal and contribute to discerning the importance of active dispersal in the soil environment. The gas composition of fire smoke depends on the material being burned. Plastics containing nitrogen, e. g. polyurethane, which are used in modern household furnishings release hydrogen cyanide (HCN) under the influence of heat. In order to evaluate the impact of HCN on the ability of victims to react and escape possibilities as well as on the cause of death, the concentration of HCN and other gaseous components of smoke were determined by burning materials containing polyurethane. Bench scale fire tests (smouldering fires) were performed using model sofa samples containing polyurethane. The concentrations of HCN, carbon monoxide (CO) and carbon dioxide (CO2) were measured using Fourier transformation infrared (FTIR) spectroscopy. Additionally, the oxygen (O2) consumption, transmission and optical smoke density were determined. The gaseous concentrations in smoke determined in the bench scale fire tests were compared with the results of an enclosed room fire test (developing fire to full fire). The average maximum concentrations measured were 42 ppm for HCN and 1100 ppm for CO. The measured values can be classified as toxic to humans but are not in the fatal range. In relation to the dominant CO concentration during smouldering fires, the HCN content in fire smoke can only be considered as having a low-grade influence on human poisoning; however, if material containing polyurethane is burned in a full fire the HCN concentration must be considered as at least a contributing factor to the death of fire victims.

A noticeable discrepancy between the observed lag times to crystal nucleation of continuous cooling experiments and the predictions of the classical nucleation theory (CNT) is evident. In particular, in these experiments nucleation at free surfaces and in contact with noble metal is delayed by many orders of magnitude with respect to CNT even if one neglects the heterogeneous character of the phase transition in the kinetic analysis. In this paper it is proposed that delayed nucleation is a consequence of a smaller Gibbs free energy of the evolving critical nucleus as compared to the growing macrocrystal. Considering lag times in scales of the reduced melting temperature T / T m from 0.5 to 0.92 a difference in the free energy of crystallization of 6.7 kJ mol − 1 and a melting point depression of 146 K is approximated. The results are in line with the description of heterogeneous systems as introduced by the generalized Gibbs approach but can be also a hint to metastable polymorphs at the nanoscale.

A 38 years old patient presented with a progressive reduction of his general condition and weight loss. Chest imaging revealed consolidations and cavities suggesting a mycobacterial infection. For further diagnosis, a bronchoscopy was performed. In fact, a nontuberculous mycobacterial infection was found. As an incidental finding, we saw a white polypoid tumor in the middle lobe bronchus. The histology of this tumor revealed a granular cell tumor (GCT). The GCT is a rare tumor entity which occurs at different anatomical locations. In the lungs, the GCT may become symptomatic as it can cause bronchial obstruction. In chest imaging, it can manifest as infiltration, atelectasis or nodule. Likewise, GCT can be found as an incidental finding in bronchoscopy. In this study, we developed a method that provides profiles of community-level surface dispersal from environmental samples under controlled hydration conditions and enables us to isolate and uncover the diversity of the fastest bacterial dispersers. The method expands on the porous surface model (PSM), previously used to monitor the dispersal of individual bacterial strains in liquid films at the surface of a porous ceramic disc. The novel procedure targets complex communities and captures the dispersed bacteria on a solid medium for growth and detection. The method was first validated by distinguishing motile Pseudomonas putida and Flavobacterium johnsoniae strains from their nonmotile mutants. Applying the method to soil and lake water bacterial communities showed that community-scale dispersal declined as conditions became drier. However, for both communities, dispersal was detected even under low-hydration conditions (matric potential, −3.1 kPa) previously proven too dry for P. putida strain KT2440 motility. We were then able to specifically recover and characterize the fastest dispersers from the inoculated communities. For both soil and lake samples, 16S rRNA gene amplicon sequencing revealed that the fastest dispersers were substantially less diverse than the total communities. The dispersing fraction of the soil microbial community was dominated by Pseudomonas species cells, which increased in abundance under low-hydration conditions, while the dispersing fraction of the lake community was dominated by Aeromonas species cells and, under wet conditions (−0.5 kPa), also by Exiguobacterium species cells. The results gained in this study bring us a step closer to assessing the dispersal ability within complex communities under environmentally relevant conditions. IMPORTANCE Dispersal is a key process of bacterial community assembly, and yet, very few attempts have been made to assess bacterial dispersal at the community level, as the focus has previously been on pure-culture studies. A crucial factor for dispersal in habitats where hydration conditions vary, such as soils, is the thickness of the liquid films surrounding solid surfaces, but little is known about how the ability to disperse in such films varies within bacterial communities. Therefore, we developed a method to profile community dispersal and identify fast dispersers on a rough surface resembling soil surfaces. Our results suggest that within the motile fraction of a bacterial community, only a minority of the bacterial types are able to disperse in the thinnest liquid films. During dry periods, these efficient dispersers can gain a significant fitness advantage through their ability to colonize new habitats ahead of the rest of the community.

Glass-ceramics are noted for their unusual combination of properties and manifold commercialized products for consumer and specialized markets. Evolution of novel glass and ceramic processing routes, a plethora of new compositions, and unique exotic nano- and microstructures over the past 60 years led us to review the definition of glass-ceramics. Well-established and emerging processing methods, such as co-firing, additive manufacturing, and laser patterning are analyzed concerning the core requirements of processing glass-ceramics and the performance of the final products. In this communication, we propose a revised, updated definition of glass-ceramics, which reads “Glass-ceramics are inorganic, non-metallic materials prepared by controlled crystallization of glasses via different processing methods. They contain at least one type of functional crystalline phase and a residual glass. The volume fraction crystallized may vary from ppm to almost 100%”.

A 65 year old female presented to the emergency department with dyspnea and progressive cough with very viscous elongated secretion plugs. She suffered from multiple cardiac comorbidities and chronic heart failure. The CT scan of the thorax demonstrated extensive pulmonary infiltrates, unspecific mediastinal lymphadenopathy and enlargement of pulmonary lymph vessels. Bronchoscopy was performed and showed extensive occlusive bronchial casts. We diagnosed a case of bronchitis plastica. Therapy with inhalative heparin led to clinical improvement.bronchitis plastica is a rare disease with formation of occlusive bronchial casts. They are often found in cardiac disease or lymphatic disease.Eine 65-jährige Patientin stellte sich mit Dyspnoe, zunehmendem Husten und Abhusten von sehr zähen, länglichen Sekretpropfen vor. Sie war multipel schwer kardial vorerkrankt und litt an einer chronischen Herzinsuffizienz. Im CT-Thorax zeigten sich ausgedehnte pulmonale Infiltrate beidseits, eine unspezifische mediastinale Lymphadenopathie und eine Erweiterung der pulmonalen Lymphgefäße. Bronchoskopisch fanden sich gallertige zähe Ausgusspräparate der Atemwege. Es wurde die Diagnose einer Bronchitis plastica gestellt. Unter einer Therapie mit inhalativem unfraktioniertem Heparin kam es zu einer Besserung. Fazit: Die Bronchitis plastica ist eine seltene Erkrankung, die durch die Bildung von Bronchialausgüssen charakterisiert ist. Man findet sie gehäuft bei kardialen Erkrankungen oder Lymphgefäßerkrankungen.
